Superior mesenteric Artery Syndrome, a Rare Complication of Severe malnutrition: Two Cases from a Subsaharian Endocrinology Department

Introduction: Superior mesenteric artery syndrome (SmAS), a rare diagnosis due to compression of the third duodenum between the superior mesenteric artery (SmA) and the aorta resulting in bowel obstruction, may lead to severe malnutrition. We report two cases of patients hospitalised in the Internal medicine, Endocrinology, Diabetology, and Nutrition Department of the National Hospital Center (NHC) of Pikine. Observations: Patient 1: A 35-year-old female was referred for an aetiological diagnosis due to a rapid weight loss of 15 kilograms in one month, accompanied by persistent vomiting, following an appendectomy performed a month before admission. Upon clinical examination, she presented severe malnutrition (Buzby index of 76%), early post-prandial chronic vomiting, and a poor general condition. An abdominal CT scan revealed aortomesenteric clamp syndrome (AmCS) with an angulation between the aorta and the SmA of 13˚. The underlying cause in this patient was severe malnutrition. Fortunately, her condition improved with medical treatment. Patient 2: We report the case of a 30-year-old female hospitalized due to unusual weight-bearing post-prandial epigastric pain and intermittent vomiting over the past six months. Upon physical examination at admission, she exhibited severe malnutrition with a body mass index (BmI) of 14 kg/m2, a Buzby index of 71%, trophic disorders, and a stage IV general condition assessment according to the World Health Organization (WHO). An abdominal CT scan revealed AmCS with an angle between the aorta and the SmA of 22˚ and an aortomesenteric space of 4 mm. The outcome was poor with medical treatment failure and, unfortunately, the patient died before surgery. Conclusion: SmAS is rarely evoked in clinical practice despite the presence of contributing factors and suggestive clinical signs. The prognosis depends on management time.

Additionally, it examines the creation of a predictive model empowered by tailored artificial intelligence techniques. Rigorous simulations are conducted to extract valuable insights, facilitating method evaluation and performance assessment, all within the dynamic Hadoop environment, thereby reaffirming the precision of the proposed approach. The results and analysis section reveals compelling findings derived from comprehensive simulations within the Hadoop environment. These outcomes demonstrate the efficacy of the Sport AI model (SAIm) framework in enhancing the accuracy of sports-related outcome predictions. Through meticulous mathematical analyses and performance assessments, integrating AI with big data emerges as a powerful tool for optimizing decision-making in sports. The discussion section extends the implications of these results, highlighting the potential for SAIm to revolutionize sports forecasting, strategic planning, and performance optimization for players and coaches. The combination of big data, cloud computing, and AI offers a promising avenue for future advancements in sports analytics. This research underscores the synergy between these technologies and paves the way for innovative approaches to sports-related decision-making and performance enhancement.

A novel coordinated control strategy considering power smoothing for a hybrid photovoltaic/battery energy storage system

This work presents a novel coordinated control strategy of a hybrid photovoltaic/battery energy storage(PV/BES) system. Different controller operation modes are simulated considering normal, high fluctuation and emergency conditions. When the system is grid-connected, BES regulates the fluctuated power output which ensures smooth net injected power from the PV/BES system. In islanded operation, BES system is transferred to single master operation during which the frequency and voltage of the islanded microgrid are regulated at the desired level. PSCAD/EmTDC simulation validates the proposed method and obtained favorable results on power set-point tracking strategies with very small deviations of net output power compared to the power set-point. The state-of-charge regulation scheme also very effective with SOC has been regulated between 32% and 79% range.

magnesium and aluminum alloys continually attract interest as lightweight structural materials for transport applications. However, joining these dissimilar alloys is very challenging. The main obstacle that hinders progress in dissimilar mg-Al joining is the formation of brittle intermetallic compounds(ImCs). As a solid-state joining technique, FSW is an excellent candidate to attenuate the deleterious ImC effects in dissimilar Al-mg joining due to the inherent low heat inputs involved in the process. However, the ImCs, namely Al_(3)mg_(2) and Al_(12)mg_(17) phases, have also been reported to form during Al-mg dissimilar FSW;their amount and thickness depend on the heat input involved;thus,the weld parameters used. Since the heat dissipated in the material during the welding process significantly affects the amount of ImCs,the heat input during FSW should be kept as low as possible to control and reduce the amount of ImCs. This review aims to critically discuss and evaluate the studies conducted in the dissimilar Al/mg FSW through a scientometric analysis and also with a focus on the strategies recently applied to enhance joint quality. The scientometric analysis showed that the main research directions in mg/Al FSW are the technological weldability of aluminum and magnesium during FSW, structural morphology, and mechanical properties of dissimilar welded joints. Considering the scope of application of the aforementioned joints, the low share of articles dealing with environmental degradation and operational cracking is surprising. This might be attributed to the need for well-developed strategies for obtaining high-quality and sustainable joints for applications. Thus, the second part of this review is conventional, focusing mainly on the new strategies for obtaining high-quality mg/Al joints. Research Background: The high prevalence of diabetes in Sudan, estimated at 16%, highlights the importance of effective health education in diabetes management. Diabetes self-management education has been identified as a crucial tool in enhancing the knowledge, attitudes, and abilities necessary for self-management among individuals with diabetes. Aim: To assess the impact of diabetes self-management education on medication adherence and glycemic control in Sudanese adults with type 2 diabetes before and 3 months after the DSmE intervention. method: The study was conducted in Sudan between September 2022 and march 2023, it was an interventional, one-group, pre- and post-test study that aimed to assess the impact of diabetes self-management education (DSmE) on medication adherence and diabetes control in Sudanese adults with type 2 diabetes. The research was conducted in primary health care centers in six cities in Sudan and involved 244 participants. The data entry and statistical analysis were conducted using the Statistical Package for Social Sciences version 27.0. A paired t test was used for analysis. Results: The study included 244 participants, 67% of whom were males. The age mean ± SD was 48.6 ± 9.3 years, and 85.3% of participants were married. Age at onset of diabetes mean ± SD was 40.60 ± 7.81 years;44.6% had diabetes for less than 5 years;and 84.1% had a positive family history of diabetes mellitus. The levels of poor, low, and partial adherence to medication decreased by 8.2%, 4%, and 20.6%, respectively, after the intervention. The levels of good and high medication regime adherence increased by 13% and 19.8%, respectively;BmI decreased by 1.1 ± 0.73 kg/m2 (p = 0.005). The fasting blood sugar decreased by 69 ± 32.9 mg/dl (p = 0.049), and the glycated hemoglobin decreased by 1.21 ± 0.28% (p = 0.001). A practical application of photogrammetry to performing rib characterization measurements in an underground coal mine using a DSLR camera

Understanding coal mine rib behavior is important for inferring pillar loading conditions as well as ensuring the safety of miners who are regularly exposed to ribs. Due to the variability in the geometry of underground openings and ground behavior, point measurements often fail to capture the true movement of mine workings. Photogrammetry is a potentially fast, cheap, and precise supplemental measurement tool in comparison to extensometers, tape measures, or laser range meters, but its application in underground coal has been limited. The practical use of photogrammetry was tested at the Safety Research Coal mine, National Institute for Occupational Safety and Health(NIOSH). A commercially available, digital single-lens reflex(DSLR) camera was used to perform the photogrammetric surveys for the experiment. Several experiments were performed using different lighting conditions, distances to subject,camera settings, and photograph overlaps, with results summarized as follows: the lighting method was found to be insignificant if the scene was appropriately illuminated. It was found that the distance to the subject has a minimal impact on result accuracy, and that camera settings have a significant impact on the photogrammetric quality of images. An increasing photograph resolution was preferable when measuring plane orientations; otherwise a high point cloud density would likely be excessive. Focal ratio(F-stop) changes affect the depth of field and image quality in situations where multiple angles are necessary to survey cleat orientations. Photograph overlap is very important to proper three-dimensional reconstruction, and at least 60% overlap between photograph pairs is ideal to avoid unnecessary post-processing. The suggestions and guidelines proposed are designed to increase the quality of photogrammetry inputs and outputs as well as minimize processing time, and serve as a starting point for an underground coal photogrammetry study.

m2BPGi for assessing liver fibrosis in patients with hepatitis C treated with direct-acting antivirals

Development of new potentiometric sensors for the determination of proguanil hydrochloride in serum and urine

Potentiometric electrodes were developed for the rapid determination of proguanil hydrochloride in pure samples, pharmaceutical preparations and spiked serum and urine samples using PVC membrane,screen printed（SPE）, coated wired（CWE）, carbon paste（CPE） and modified carbon paste（mCPE）electrodes based on the ion-exchanger of proguanil with phosphotungestic acid（Pr-PT） as a chemical modifier. The prepared electrodes showed Nernestian slopes of 59.7, 58.1, 58.5, 58.5 and 57.0 for the PVC,SPE, CWE, CPE and mCPE for the proguanil ions in a wide concentration range of 1.0 ＊ 10^-5–1.0 ＊ 10^-2mol L^-1 at 25°C with detection limits of 7.94 ＊ 10^-6, 1.0 ＊ 10^-5, 1.0 ＊ 10^-6, 7.07 ＊ 10^-6 and 2.5 ＊ 10^-6 mol L^-1, respectively. The prepared electrodes exhibited high proguanil selectivity in relation to several inorganic ions and sugars and they could be successfully utilized for its determination in pure solutions, pharmaceutical preparations and serum and urine samples using the direct potentiometry and standard addition methods with very good recovery values.

There are several challenges that hospitals are facing according to the emergency department(ED).Themain two issues are department capacity and lead ***,the lack of consensus on performance criteria to evaluateEDincreases the complication of this ***,this study aims to evaluate the efficiency of the emergency department in 20 Egyptian hospitals(12 private and 8 general hospitals)based on 13 performance *** research suggests an integrated evaluation model assess ED under a framework of plithogenic *** proposed framework addressed uncertainty and ambiguity in information with an efficient manner via presenting the evaluation expression by plithogenic *** Envelopment Analysis(DEA)technique is used in order to measure the efficiency of the emergency department of 20 hospitals according to the number of treated patients and effect on patient’s life quality based on 11 *** the Analytic Hierarchy Process(AHP),the weight of efficiency factors will be measured based on neutrosophic linguistic scale pairwise *** operations provide more accurate aggregation result according to contradiction degree between criteria *** results show that ten of the hospitals are providing efficient service in their emergency department,while the other ten are less *** analysis of the results shows that 58%of private hospitals emergency department is operating efficiently,while the efficient general hospitals represent 38%.
