Neuroanatomical Basis of Postoperative Pain and Assessment of Its Management in a Series of Patients Undergoing Caesarean Sections

Introduction: In recent decades, the cost of postoperative pain has been the subject of many studies based on protocols developed by scientific societies for its assessment and optimization. At the Regional Hospital of Saint-Louis (Senegal), several protocols have been developed for pain management, but no study has focused on the assessment of postoperative pain management specifically. We therefore initiated this work, the objectives of which were to remind the neuroanatomical and neurophysiological bases of postoperative pain, and to analyze the assessment and management of this pain in patients who have undergone a caesarean section. Materials and methods: This was a prospective and descriptive study, which took place in the gynecology-obstetrics department, over a period from January 2019 to July 2020. All patients who gave birth by cesarean section were included. The data was collected from a survey sheet written for this purpose. For each of the patients, the information was taken every day throughout the duration of postoperative hospitalization. Results: It appears from our work that after a cesarean section, the pain felt evolves on the first postoperative days with a peak during the second day. As in the data reported in the literature, there does not seem to be a difference in terms of pain intensity and analgesia dosage between scheduled and emergency caesarean sections. However, young age and female gender—for other types of surgeries—are risk factors associated with high postoperative pain scores. This trend is probably related to the low pain experience of tested patients. Our initial hypothesis was that acute post-operative pain after caesarean sections could be linked to defects in the perception and processing of pain by caregivers. Prevalence and Associated Factors of Diabetic Kidney Disease in Senegalese Patients: A Cross-Sectional Study in Saint-Louis

Introduction: Diabetes is a leading cause of chronic kidney disease in the world. During the next decade, its burden is expected to increase in Africa with potential complications such as chronic kidney disease. However, epidemiology and risk factors of diabetic kidney disease are poorly described at population level. This study aimed to determine prevalence of diabetic kidney disease (DKD) in adult diabetics living in Saint-Louis, northern Senegal. Methods: A cross-sectional study including diabetic patients followed-up aged ≥18 years during a five-year period (2013-2018) in Saint-Louis. Clinical and biological parameters were collected during annual community-based mass screening. Diabetes was defined as fasting blood glucose ≥ 1.26 g/L confirmed by a second lab dosage. DKD was defined as persistence of albuminuria ≥ 30 mg/24h and/or estimated glomerular filtration rate (eGFR) 2. Data were analyzed with Stata 12.0. Results: We included a total of 1310 diabetic patients among whom 3.7% (95% CI = 1.4% - 9.8%) presented DKD. Their mean age was 46.2 ± 11.8 years and sex-ratio was 0.7. Micro-albuminuria and macro-albuminuria were present respectively in 59.2% and 18.4% of patients with DKD and half of them had a normal eGFR. Before the survey 89.8% of patients with DKD were not aware of their renal disease and only four of them had seen a nephrologist. After multivariate analysis, age (OR = 1.5;95% CI = 1.1 - 3.4), duration of diabetes (OR = 1.2;95% CI = 1.6 - 4.4) and hypertension (OR = 2.5;95% CI = 1.4 - 4.6) were associated with the presence of DKD in diabetic patients while no significant association was not found with gender, blood glucose level, smoking and familial history. Conclusion: DKD is a frequent complication in diabetic adult population living in Saint-Louis. Early detection and management should be promoted in order to prevent progression to end-stage renal disease.

Emergency Drainage of Upper Urinary Tract with a Double Catheter in the Hospital General de Grand Yoff of Dakar (HOGGY)

Goals: The goals are to assess the use of Double Jendo-prostheses in urinary obstructions in the Hospital General de Grand Yoff of Dakar (HOGGY). Patients and methods: This is a 4-year retrospective study (1st January 2009-31st December 2012). The study included all patients with obstruction of the upper urinary tract, and with a reversing type Double Jendo-prostheses. Findings: A total of 82 patients were chosen for this study, including 41 males. Patient’s average age was 45 years. Kidney failure accounted for 11.5% of the causes of consultations. Lithiasic obstacles (34.2%) and tumours (21.5%) were the most recurrent. Catheter was successfully placed in 78% of cases, and highly contributed to improve renal function in 69% of cases. The frequently complications found in patients with Double J Catheters were back pain and urinary tract infections. Four cases of catheters calcifications were reported. Catheter was replaced in 62.2% of patients within an average period of 6.9 months. Conclusion: Double J Catheters remain important in the preservation of renal function in case of obstruction. Its use requires a mastery of these indications and a rigorous follow-up.

Transient Diabetes Induced by L-Asparaginase

Background: Although hyperglycaemia is one of the known side effects of L-asparaginase, its contribution to the development of diabetic ketoacidosis (DKA) is less well known in the literature. Asparaginase is an essential component of treatment protocols for acute lymphoblastic leukaemia (ALL) in combination with other chemotherapeutic drugs. On further evaluation, she was found to have high anion gap metabolic acidosis, hyperglycaemia and ketonuria. In recent decades, the use of these chemotherapeutic agents has led to a decrease in mortality and disease-free survival in ALL. L-asparaginase is one of the chemotherapy protocols used in the treatment of acute lymphoblastic leukaemia (ALL) and can induce hyperglycaemia which is aggravated by the concomitant use of corticosteroids. We report the observation of a 14-year-old girl treated with chemotherapy (GFA LAL protocol) who developed transient diabetes following the use of L-asparaginase. She was treated at the Donka paediatric haemato-oncology unit by a multidisciplinary team including a paediatric oncologist, a psychologist and a diabetologist. The aim of this study was to highlight blood glucose monitoring before and after the use of asparaginase in acute lymphoblastic leukaemia. Conclusion: We conclude that the occurrence of ketoacidosis following the use of asparaginase is a rare event. We recommend close monitoring of blood glucose levels for hyperglycaemia in patients with ALL receiving L-asparaginase.

PI and RST Control Design and Comparison for Matrix Converters Using Venturini Modulation Strategy

This paper presents a thorough design and comparative study of two popular control techniques, i.e., classical Proportional Integral (PI) and RST, for Matrix Converters (MCs) in terms of tracking the reference and robustness. The output signal of MCs is directly affected by unbalanced grid voltage. Some research works have attempted to overcome this problem with PI control. However, this technique is known to offer lower performance when it is used in complex and nonlinear systems. On the other hand, RST control offers better performance, even in case of highly nonlinear systems. Therefore, the RST can achieve better performance to overcome the limitation of PI control of nonlinear systems. In this paper, a RST control method is proposed as output current controller to improve the performance of the MC powered by unbalanced grid voltage. The overall operating principle, Venturini modulation strategy of MC, PI control and characteristics of RST are presented.

An unusual cause of gastric outlet obstruction in a young girl: Trichobezoar

Trichobezoars are an infrequent form of bezoars formed from ingested hair. They are more common in adolescent females with history of trichotillomania. Exceptionally, it can occur in young children making the diagnosis difficult. We report the case of a 6-year-old girl with a history of abdominal pain, distension, weight loss, and attacks of vomiting. Upper gastrointestinal endoscopy revealed a trichobezoar occupying almost the whole gastric cavity. The gastric bezoar was removed by surgery. Gastric trichobezoar is exceptional in young children and can lead to stunting and gastric outlet obstruction. After definitive surgical or endoscopic treatment, pediatric psychiatric consultation should be opted for any mental disorder and for prevention of further recurrence.

Papilloma is a rare benign tumor. Laryngeal location is the most frequent. The bronchial involvement remains unusual. It represents 0.38% of pulmonary tumors. We report the case of a 39-year-old, smoker man, who consulted for a cough following during six months with persistent radiological opacity despite antibiotic treatment. The physical examination was without particularities. Flexible bronchoscopy revealed a swollen endobronchial lesion completely obstructing the orifice of the intermediate trunk. Bronchial biopsies at this level showed inflammatory mucosa. The chest CT scan objectified an endobronchial tissular mass in the intermediate trunk. Pulmonary neoplasia was suspected. Diagnostic and therapeutic surgery was performed. Histopathological examination concluded to an endobronchial squamous papilloma.

Production and Characterization of Pozzolan with Raw Clay from Burkina Faso

Four raw deposits clayey materials in Burkina Faso have been characterized using X-ray diffraction, infrared spectroscopy and ICP-AES to evaluate their suitability to be used as pozzolan in cement. The samples have been activated by thermal treatment at 730°C during 5 hours. The obtained metakaolin was investigated by means of complementary techniques for their use as pozzolan in building materials. Fineness, surface area and amorphousness of metakaolins were the dominant factors which influenced their pozzolan reactivity. The compressive strengths of blended specimens were slightly increased with metakaolins named MK1 and MK2 than the reference for the substitution of 20 - 25 wt%. This was due to the metakaolins’ pozzolan reactivity and their filler effects which promoted CSH formation and reduced pores in the specimens. MK1 and MK2 were suitable for replacement of Portland cement in the field of building materials.

Côte d’Ivoire is currently experiencing strong growth in the mining sector. Identifying the formations present in our subsoil is therefore essential for mining recovery. It is in this context that we conducted studies on the formations present in the locality of Guintéguéla. It is located in the northwest of Côte d’Ivoire in the bafing region. The aim of this work was to determine the petrographic and structural characteristics of the formations of the area. The methodology began with documentation and then followed petrography and structural analysis work on the macroscopic and microscopic levels. We observed six groups of rocks: granitoids, amphibolites, orthogneiss, quartzites (poor and rich in magnetites), volcano-sediments and filonian rocks. Metamorphism is of amphibolite to granulite facies. However, volcano-sediments must be associated with the green schist facies. With regard to the structural, structures and microstructures such as foliation;fractures;sigmoidal figures reveal that the studied area was affected by ductile and also brittle tectonics whose main directions are oriented along the shear corridor, so N-S to NNW-SSE.

Cerebral Malaria: Epidemiological, Clinical and Prognosis Aspects in the Anesthesia-Resuscitation Department CHU Ignace Deen

Objective: To describe the epidemiological, clinical, paraclinical and prognostic aspects of cerebral malaria received in the anesthesia-resuscitation department. Methodology: This was a prospective descriptive type study carried out in the anesthesia-resuscitation department over a period of three (03) months (June 01 to August 31, 2022). All patients admitted for cerebral malaria were included. Sociodemographic, clinical and prognostic parameters were studied. Results: We collected 25 cases out of 105 admitted patients (23.8%). The average age was 27.6 ± 9.5 years with extremes of 11 and 50 years. The sex ratio was 0.7 (M/F). Students and housewives were in the majority, i.e. 52% and 24%. Neurological disorders were found on admission in all patients, dominated by impaired consciousness at 100% followed by convulsion (60%), prostration (44%), confusion (36%) associated with deep coma in (68%). Gross hemoglobinuria was present in (84%) of cases. On the blood count, anemia was present in (70%) of the patients followed by thrombocytopenia in more than half of the cases (60%) and transfusion was necessary in 19 cases. P Falciparum malaria was found in all patients (100%), the average parasite density was 60342.8 ± 30425.6 trophozoites/μl with extremes of 9000 to 100000 trophozoites/μl. All our patients were treated with intravenous injectable artesunate. Transfusion was performed in 76% of our patients. Eighty percent of the patients had benefited from dialysis. High oxygen therapy was performed in (92%) of cases. The average duration of hospitalization was 5.74 ± days with extremes of 1 to 17 days. Mortality was 48%. Conclusion: Cerebral malaria can take different clinical forms, the most important of which is cerebral involvement. Prompt initiation of appropriate resuscitation can reduce mortality.
