BACKGROUNd Epithelial-myoepithelial carcinoma (EMC) is a rare, low-grade, malignant tumor that constitutes less than one percent of all salivary gland tumors. To date, only one other case report has described radiation-associated EMC in the English language medical *** SUMMARY In this report, we describe the case of a 56-year-old male patient who presented with a neck mass diagnosed as EMC of the left submandibular gland approximately 30 years after mantle field radiation and chemotherapy for hodgkin lymphoma. Treatment included resection, re-resection with nodal dissection, and adjuvant chemoradiotherapy. This patient was also diagnosed with 4 other secondary malignancies, including stage IV diffuse large B cell lymphoma in the abdomen with subsequent brain metastases, low-grade neuroendocrine carcinoma of the lung, hurthle cell adenoma, and small B cell lymphoma before the patient expired. This case provides important information regarding the pathology, clinical sequelae, and management of a patient diagnosed with radiation-associated EMC amidst four concurrent *** Further investigation is needed on the efficacy of adjuvant radiotherapy in EMC,especially atypical EMC.

Understanding the solid target dynamics resulting from the interaction with an ultrashort laser pulse is a challenging fundamental multi-physics problem involving atomic and solid-state physics,plasma physics,and laser *** of the initial interplay of the underlying processes is essential to many applications ranging from lowpower laser regimes like laser-induced ablation to high-power laser regimes like laser-driven ion *** the properties of the so-called pre-plasma formed as the laser pulse’s rising edge ionizes the target is complicated from the theoretical and experimental point of view,and many aspects of this laser-induced transition from solid to overdense plasma over picosecond timescales are still open *** the one hand,laser-driven ion acceleration requires precise control of the pre-plasma because the efficiency of the acceleration process crucially depends on the target properties at the arrival of the relativistic intensity peak of the *** the other hand,efficient laser ablation requires,for example,preventing the so-called“plasma shielding”.By capturing the dynamics of the initial stage of the interaction,we report on a detailed visualization of the pre-plasma formation and ***-thin diamond-like carbon foils are shown to transition from solid to plasma during the laser rising edge with intensitieshot near-infrared probe transmission measurements evidence sub-picosecond dynamics of an expanding plasma with densities above 10^(23)cm^(−3)(about 100 times the critical plasma density).The complementarity of a solid-state interaction model and kinetic plasma description provides deep insight into the interplay of initial ionization,collisions,and expansion.

We report on an experiment performed at the FLASh2 free-electron laser(FEL)aimed at producing warm dense matter via soft x-ray isochoric *** the experiment,we focus on study of the ions emitted during the soft x-ray ablation process using time-of-flight electron multipliers and a shifted Maxwell–Boltzmann velocity distribution *** find that most emitted ions are thermal,but that some impurities chemisorbed on the target surface,such as protons,are accelerated by the electrostatic field created in the plasma by escaped *** morphology of the complex crater structure indicates the presence of several ion groups with varying *** find that the ion sound velocity is controlled by the ion temperature and show how the ion yield depends on the FEL radiation attenuation length in different materials.

At the EAST tokamak, the ion temperature(T_(i)) is observed to be clamped around 1.25 keV in electron cyclotron resonance(ECR)-heated plasmas, even at core electron temperatures up to 10 keV(depending on the ECR heating power and the plasma density). This clamping results from the lack of direct ion heating and high levels of turbulence-driven transport. Turbulent transport analysis shows that trapped electron mode and electron temperature gradient-driven modes are the most unstable modes in the core of ECR-heated h-mode plasmas. Nevertheless, recently it was found that the T_(i)/T_(e)ratio can increase further with the fraction of the neutral beam injection(NBI) power, which leads to a higher core ion temperature(Ti0). In NBI heating-dominant h-mode plasmas, the ion temperature gradient-driven modes become the most unstable ***, a strong and broad internal transport barrier(ITB) can form at the plasma core in high-power NBI-heated h-mode plasmas when the T_(i)/T_(e)ratio approaches ~1, which results in steep core Teand Tiprofiles, as well as a peaked neprofile. Power balance analysis shows a weaker Teprofile stiffness after the formation of ITBs in the core plasma region, where Ticlamping is broken,and the core Tican increase further above 2 keV, which is 80% higher than the value of Ticlamping in ECR-heated plasmas. This finding proposes a possible solution to the problem of Ticlamping on EAST and demonstrates an advanced operational regime with the formation of a strong and broad ITB for future fusion plasmas dominated by electron heating.

4d imaging of void nucleation,growth,and coalescence from large and small inclusions in steel under tensile deformation

Samples of SA508 grade 3 nuclear pressure vessel ferritic steel were subjected to tensile straining whilst being simultaneously imaged in 3d in real time using high resolution,high frame rate time-lapse synchrotron computed tomography(CT).This enabled direct observation of void development from nucleation,through growth to coalescence and final failure validating many inferences made post-mortem or by theoretical models,as well as raising new *** sparse,large inclusions were found to nucleate voids at essentially zero plastic strain(consistent with zero interfacial strength);these became increasingly elongated with *** contrast,a high density of small spherical voids were found to nucleate from the sub-micron cementite particles at larger strains(>200%)only in the centre of the necked(high triaxiality)*** interfacial strength approaching 2100 MPa was inferred and soon after their nucleation,these small voids coalesce to form internal microcracks that lead to the final failure of the *** surprisingly,under these conditions of generally low triaxial constraint the large voids are simply cut across and appear to play no significant role in determining the final *** implications of these results are discussed in terms of ductile fracture behaviour and the Gurson model for ductile fracture.

Safety and efficacy of ultrasound ciliary plasty as a primary intervention in glaucoma patients

AIM: To evaluate the safety and the efficacy of the ultrasound ciliary plasty(UCP) on the intraocular pressure(IOP) control in glaucomatous eyes without previous glaucoma ***: A retrospective study included patients with primary and secondary glaucoma who underwent UCP in dar Al Shifa hospital, Kuwait between January 2017 to June 2018. high-intensity focused ultrasound procedures were performed under peribulbar anesthesia using the 2^(nd) generation probe with 8 s duration of each of the 6 shots. Complete ophthalmologic examinations were scheduled pre-treatment, and at 1 d, 1 wk, 1, 3, 6 and 12 mo posttreatment. Primary outcomes were the IOP reduction and success rates at 12 mo, while the secondary outcomes were the occurrence of vision threatening complications and visual ***: The records of 62 eyes of 62 patients were analyzed with mean age of 63.8 y(67.7% males). There was statistically significant reduction in the mean IOP from 35.2±8.3 mm hg before treatment to 20.6±8.7 mm hg at 12^(th) month(Ph a mean percentage IOP reduction of 42.3% with significant reduction in the mean number of antiglaucomatous drugs from 3.2±0.4 before treatment to 2.1±1.02 at 12 mo(Pd success was achieved in 77.4% of eyes at 12 mo. No major intra-or posttreatment complications were ***: Second-generation UCP prove to be effective in reducing IOP in naive glaucoma patients with lower success rates in cases of neovascular and uveitic glaucomas.
