Paraoxonase 2 Gene (Cys<sup>311</sup>-Ser) Polymorphism and the Risk of Coronary Artery Disease

Background: Human paraoxonase-2(PON2) which is exclusively intracellular possesses unique properities that distinguish it from PON1 and PON3. Recently, it was demonstrated that PON2 protects against atherosclerosis by preventing LDL oxidation. Emerging evidences have proposed that genetic variations in the PON2 gene may be associated with coronary artery disease (CAD). Objectives: To investigate the relationship between a common PON2 gene (Cys311-Ser) polymorphism and the presence and extent of CAD. Methods: The study comprised 112 patients recruited from those undergoing coronary angiography for suspected CAD, who were divided according to the presence or absence of CAD into 2 groups Group I including 62 patients with CAD and Group II including 50 patients proved to have normal coronaries. All the subjects included in the study were genotyped for the (Cys311-Ser) polymorphism of PON2 gene using RCR-RFLP. Results: The frequency of Cys allele was significantly higher in group I compared to Group II (77.4% vs. 56% respectively, P < 0.01). Patients with vessel score 3 had significantly higher severity score and higher Cys allele frequency than patients with vessel score 2, the latter group had also significantly higher severity score and Cys allele frequency than patients with vessel score 1. In multivariate logistic regression analysis of different variables for prediction of CAD, age [OR 3.79, CI (1.33 - 12.7), P < 0.01], smoking [OR 0.71, CI (0.23 - 7.81), P 311 Cys allele [OR 5.67, CI (1.99 - 14.77), P < 0.001] were significantly independent predictors of CAD. Conclusion: Cys allele of PON2 311 gene polymorphism is an independent risk factor for CAD and it is associated not only with the presence of CAD but also with its extent and severity.

Diagnostic Accuracy of Strain Imaging of the Left Ventricle in Detection of Obstructive Coronary Artery Disease

Background: Speckle tracking echocardiography using average global strain and strain rate (SR) parameters for early detection of high risk patients with coronary artery disease (CAD) has gained a substantial clinical interest. Objective: Assessment of the diagnostic accuracy of strain imaging of the left ventricle in detection of obstructive coronary artery disease. Methods: One hundred patients were enrolled. They were divided into 3 groups: Group I (n = 40 patients) presented with ST segment elevation myocardial infarction (STEMI), Group II (n = 40 patients) presented with non-ST segment elevation myocardial infarction (NSTEMI) or unstable angina (UA) and Group III (n = 20 patients) with normal coronary angiography served as a control group. All patients were subjected to 2D speckle tracking echocardiography (2D STE) to assess LV longitudinal strain and strain rate (SR). Sensitivity, specificity and diagnostic accuracy of 2D STE in prediction of CAD and its severity using Gensini score were assessed. Results: The mean age for Group I, II and III was 52.20 ± 11.83, 51.97 ± 14.53 and 52.75 ± 10.75 respectively. Paraoxonase 1 gene (Gln<sup>192</sup>-Arg) polymorphism and the risk of coronary artery disease in type 2 diabetes mellitus

Background: Paraoxonase 1 (PON1) is reported to have an antioxidant and cardioprotective properties. Recently, an association of glutamine (Gln) or (type A)/arginine (Arg) or (type B) polymorphism at position 192 of PON1 gene has been suggested with coronary artery disease (CAD) among patients with diabetes mellitus (DM). However, conflicting results have also been reported. Objectives: To investigate the relationship between PON1 gene (Gln192-Arg) poly-morphism and the presence, extent and severity of CAD in type 2 DM. Methods: The study comprised 180 patients recruited from those undergoing coronary angiography for suspected CAD, who were divided according to the presence or absence of CAD and DM into 4 groups;Group I (n = 40 patients) nondiabetic subjects without CAD, Group II (n = 45 patients) diabetic patients without CAD, Group III (n = 47 patients) non diabetic patients with CAD and Group IV (n = 48 patients) diabetic patients with CAD. PON1 (Gln192-Arg) genotype was assessed using polymerase chain reaction (PCR) followed by AlwI digestion. Results: The frequency of Gln allele (Type A) was significantly higher in group I and group II compared to group III and group IV (62.5%, 60% vs 38.3%, 31.25% respectively, p 100 mg/dL [OR 4.31, CI (1.25 - 12.5), P < 0.001], high density lipoprotein (HDL) cholesterol <40 mg/dL [OR 5.11, CI (1.79 - 16.33), P < 0.001] and PON1 192 Arg allele [OR 4.62, CI (1.67 - 13.57), P < 0.001] were significantly independent predictors of CAD. Conclusion: Arg allele of PON1 192 gene polymorphism is an independent risk factor for CAD and it is associated not only with the presence of CAD but also with its extent and severity and its impact is clearly more pronounced in diabetic patients.

Diagnostic value of dobutamine stress Doppler tissue imaging in diabetic patients with suspected coronary artery disease

Background: Coronary artery disease (CAD) is often silent in diabetic patients, and it is typically in ad-vanced stages of development by the time it manifests. Various forms of stress testing have been investigated to detect obstructive CAD in diabetes mellitus. Ob-jectives: To assess the diagnostic value of dobutamine stress pulsed-wave Doppler tissue imaging (DTI) compared with standard wall motion analysis in de-tection of myocardial ischemia in diabetic patients with suspected CAD. Methods: The study comprised 46 diabetic patients with suspected CAD underwent dobutamine stress echocardiography (DSE) with DTI within 4 weeks before coronary angiography(CA). Dobutamine infusion started at 5 μ/kg/min and in-creased up to 40 μ/kg/min with additional atropine during submaximal heart rate responses. In addition to wall-motion score index (WMSI) analysis, pulsed- wave DTI examination of basal and mid segments of posteroseptal, lateral, anterior, inferior and antero- septal walls was performed. Myocardial velocities were measured at rest in the apical 4, 3 and 2-cham- ber views. The measurements were repeated at low dose (10 - 15 μ/kg/min) and at peak stress (40 μ/kg/min). DTI measurements included peak systolic velocity (S), peak early diastolic velocity (E) and peak late diastolic velocity (A) and the results were com-pared to WMSI analysis. Patients were classified into two groups according to CA results;group (I) di-abetics with positive CA (n = 27) and group (II) di-abetics with negative CA (n = 19). Results: There was no significant difference between the two groups in duration of diabetes, global WMSI at rest or the ? changes (stress-rest/rest) of WMSI (P > 0.05). Global S and global E were significantly lower in group I compared to group II at peak stress (11.3 ± 3.7 cm/sec vs. 14.5 ± 2.2 cm/sec, p < 0.01) and (11.3 ± 1.6 cm/sec vs. 13.1 ± 2.1 cm/sec, p < 0.01) respectively. Penile prosthetic implantation represents a cornerstone for patients with organic erectile dysfunction(ED)that is refractory,unsatisfactory,or contra-indicated for other approved medical or mechanical *** this study,we introduce the“Ghattas technique,”wherein we constructed a polypropylene mesh sheath that surrounds and is fixed to a 13-mm malleable prosthesis cylinder,which can increase the cylinder diameter for cases that need a larger *** patients underwent preoperative evaluation and completed the five-item International Index of Erectile Function questionnaire(IIEF-5).Postoperative outcomes were evaluated by IIEF-5 and Erectile Dysfunction Inventory of Treatment Satisfaction(EDITS)questionnaires at final *** mean age of the 23 included patients was 57.9(standard deviation[s.d.]:11.4)years and the mean duration of ED was 8.5(s.d.:7.9)*** improvement was determined by comparing mean preoperative and postoperative IIEF-5 scores(8.3[s.d.:3.9]vs 24.6[s.d.:0.6],P<0.001).High treatment satisfaction was determined according to a mean EDITS score of 94.9(s.d.:9.9).The proposed Ghattas technique was safe and effective in our patients,and provides opportunity for cases that need a diameter larger than 13 *** studies are needed to confirm these results.

A New BEM for Fractional Nonlinear Generalized Porothermoelastic Wave Propagation Problems

The main purpose of the current article is to develop a novel boundary element model for solving fractional-order nonlinear generalized porothermoelastic wave propagation problems in the context of temperaturedependent functionally graded anisotropic(FGA)*** system of governing equations of the considered problem is extremely very difficult or impossible to solve analytically due to nonlinearity,fractional order diffusion and strongly anisotropic mechanical and physical properties of considered porous ***,an efficient boundary element method(BEM)has been proposed to overcome this difficulty,where,the nonlinear terms were treated using the Kirchhoff transformation and the domain integrals were treated using the Cartesian transformation method(CTM).The generalized modified shift-splitting(GMSS)iteration method was used to solve the linear systems resulting from BEM,also,GMSS reduces the iterations number and CPU execution time of *** numerical findings show the effects of fractional order parameter,anisotropy and functionally graded material on the nonlinear porothermoelastic stress *** numerical outcomes are in very good agreement with those from existing literature and demonstrate the validity and reliability of the proposed methodology.

Impact of COVID-19 infection during pregnancy on maternal and fetal outcomes

Objectives:The objective of this study is to assess the impact of coronavirus disease 2019(COVID-19)infection during pregnancy on maternal and fetal outcome in relation to gestational ***:This retrospective study was conducted between May 2020 and July ***-five women infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and hospitalized at a quarantine hospital were ***-four women received prenatal care at the hospital until delivery,while 31 dropped out from the study due to COVID-19 recovery and discharge before ***:COVID-19 was found in 83%of the pregnant women after 20 weeks of *** most common symptoms were cough and *** complications included severe pneumonia and *** maternal mortality rate was 9.2%.Premature rupture of membranes and severe hypertension during labor affected nearly 9%of the pregnant women who completed prenatal care until delivery at the *** labor occurred at a rate of 38.2%.There were seven(20.8%)stillbirths and two cases of infant ***:Hospitalized pregnant women who have coronavirus infection,lymphopenia,and a high C-reactive protein level were at a higher risk of developing severe illness,which can lead to maternal and neonatal complications.

A numerical model based on the dual-reciprocity boundary element method(DRBEM)for studying the transient magneto-thermo-viscoelastic waves in a rotating nonhomogeneous anisotropic solid is *** formulation is tested through its application to the problem of an initially stressed solid placed in a constant primary magnetic field acting in the direction of the z-axis and rotating about this axis with a constant angular *** the case of plane deformation,a numerical scheme for the implementation of the method is presented and the numerical computations are carried out for the temperature,displacement components and thermal stress *** validity of DRBEM is examined by considering a magneto-thermo-viscoelastic solid occupies a rectangular region and good agreement is obtained with existent *** results obtained are presented graphically to show the influence of initial stress on the displacement components and thermal stress components.
