Acute Fevers in the Medical Unit of the Medical-Surgical Emergency Department of the Donka National Hospital

Introduction: Fever is a high core temperature ≥ 37.5°C in the morning and 37.8°C in the evening. It is acute when it evolves from 0 to 20 days. Very common in clinical practice, the etiological diagnosis, particularly in developing and tropical countries, is often a challenge for clinicians due to their diversity and the limited availability of diagnostic tools. There is a wide spectrum of etiological diagnoses including infectious causes and non-infectious causes. The aim of this study was to investigate the etiology of fevers acute at the medical unit in the medico-surgical emergency department of the Donka National Hospital. Methods: This was a descriptive cross-sectional study lasting 03 months (January 01, 2022 to March 31, 2022). We included in this study all patients seen in the medical unit, whose age ≥ 18 years, without distinction of sex, from any origin, with an axillary temperature ≥ 37.5°C in the morning and 37.8°C in the evening, evolving from 0 to 20 days, hospitalized or followed on an outpatient basis, and having given verbal consent. Results: Of a total of 1087 patients seen, 466 had an acute fever. The mean age was 40.04 ± 18.91 years (18 and 96 years). The female sex (58.15%) was predominant with a sex ratio of 0.72. Malaria (50.86%) was the main diagnosis. The treatment consisted of compressed paracetamol (59.01%), arthemether + lumefantrine (50.85%). Conclusion: The incidence of acute fevers is high in the medical unit of the medico-surgical emergency department of the Donka National Hospital. Malaria was the main pathology. Treatment was etiological and symptomatic. This high incidence could be explained by the fact that Guinea is an endemic malaria zone. A study taking into account other etiological factors would be of great interest.

Protective Effect of Ketamine against Acetic Acid-Induced Ulcerative Colitis in Rats

Objective: Inflammatory bowel diseases (IBD), including Crohn’s disease and ulcerative colitis (UC), are chronic and recurrent disorders of the gastrointestinal tract with unknown etiology. Considering the adverse effects and incomplete efficacy of currently administered drugs, it is crucial to explore new drugs with more desirable therapeutic profiles. As non-competitive N-methyl-D-aspartate (NMDA) receptor antagonists have shown analgesic and anti-inflammatory properties in vitro and in vivo, this study aims to investigate the role of ketamine, a noncompetitive NMDA receptor antagonist, in acetic acid-induced rat colitis. Methods: Ketamine (10, 50 mg/kg), and dexamethasone (1 mg/kg) were given intraperitoneally 30 min before induction of colitis which was done by instillation of 2 mL of 4% acetic acid (vol/vol). At the 4th day of colitis induction, animals were sacrificed and distal colons were assessed macroscopically and microscopically. Furthermore, the mucosal contents of lipid peroxidation (LPO), reduced glutathione (GSH), nitric oxide (NO) and tumor necrosis factor-α (TNF-α) were assessed. Results: Ketamine (50 mg/kg) and dexamethasone significantly (p α and elevated GSH levels. Conclusion: Our data suggest that ketamine has valuable protective effects in acetic acid colitis and it may be a new therapy target in ulcerative colitis patients, possibly by regulating antioxidants and inflammatory mediators.

12 Years Analysis of Bacterial Isolates and Their Antibiotics Sensitivity in a Tertiary Care Burns Unit

The aim of this study was to review the changes in distribution of bacterial populations and their antibiotic sensitivity over 12 years in a tertiary care burn unit. Understanding the periodic variation of isolated microorganisms and their antibiotic sensitivity helps in selecting the appropriate antimicrobial therapy before culture and sensitivity are reported. It also aids the design of antibiotics protocols. The study was retrospective. The data were obtained from the computerised hospital medical record system and the burn unit records. Overall, Pseudomonas aeruginosa was the most commonly isolated microorganism followed by Staphylococcus aureus, Meticillin-resistant Staphylococcus aureus (MRSA), and the genus Acinetobacter. Acinetobacter isolation rose rapidly and became more prevalent than P. aeruginosa over the last three years. Other organisms became isolated more frequently, such as Klebsiella pneumoniae, but their overall prevalence was low. Pseudomonas species frequency of isolation declined. P. aeruginosa, MRSA, and other microorganisms showed increasing sensitivity to a number of antibiotics. MRSA remained highly sensitive to vancomycin, and Acinetobacter showed high resistance to all antibiotics tested except colistin. K. pneumoniae was highly resistant to most of the antibiotics tested except the carbapenems, but the resistance to carbapenems increased over time.

Effectiveness of MRA on embolized intracranial aneurysms: a comparison of DSA, CE-MRA, and TOF-MRA

Purpose: The endovascular treatment of intracranial aneurysms was proven safe and effective compared to the alternative method of surgical clipping, despite the high recurrence rate. Follow-up of embolized intracranial aneurysms is mandatory for the early detection of recurrence and improved outcomes. DSA is used as the reference standard for this assessment. To determine the effectiveness of MRA in follow-up evaluations of intracranial aneurysms after embolization by comparing DSA, CE-MRA, and TOF-MRA. Materials and Methods: Sixty-eight consecutive patients undergoing DSA, TOF-MRA, and CE-MRA during an interval of els, and artifacts. The modified Raymond-Roy occlusion classification and Aneurysm Embolization Grades were used to assess the occlusion status and initial DSA images for detection of recurrence in two filtered study phases with optimized selection criteria. Seventeen observers(phase I: 9, phase II: 8) independently interpreted the double-blinded images. Agreement was expressed with a Fleiss kappa value; p ely; Phase II: k = 0.503, p ≤ 0.001; k = 0.303, p ≤ 0.001; k = 0.115, p = 0.038, respectively). TOF-MRA was as effective as DSA(TOF: k = 0.335, p ≤ 0.001; DSA: k = 0.323, p ≤ 0.001) for recurrence detection. Conclusion: We suggest TOF-MRA as a first-line follow-up tool to detect aneurysm recurrence, and DSA to quantify the filling space to make a definite decision on re-embolization.

Concentration of Copper, Zinc, Lead, Cadmium, and Nickel in the Incinerator Bottom Ash in Five County Hospitals in Kenya

Healthcare wastes contain potentially harmful microorganisms, inorganic and organic compounds that pose a risk to human health and the environment. Incineration is a common method employed in healthcare waste management to reduce volume, quantity, toxicity as well as elimination of microorganisms. However, some of the substances remain unchanged during incineration and become part of bottom ash, such as heavy metals and persistent organic pollutants. Monitoring of pollution by heavy metals is important since their concentrations in the environment affect public health. The goal of this study was to determine the levels of Copper (Cu), Zinc (Zn) Lead (Pb), Cadmium (Cd) and Nickel (Ni) in the incinerator bottom ash in five selected County hospitals in Kenya. Bottom ash samples were collected over a period of six months. Sample preparation and treatment were done using standard methods. Analysis of the heavy metals were done using atomic absorption spectrophotometer, model AA-6200. One-Way Analysis of Variance (ANOVA) was performed to determine whether there were significant differences on the mean levels of Cu, Zn, Pd, Cd and Ni in incinerator bottom ash from the five sampling locations. A post-hoc Tukey’s Test (HSD) was used to determine if there were significant differences between and within samples. The significant differences were accepted at p ≤ 0.05. To standardize the results, overall mean of each metal from each site was calculated. The metal mean concentration values were compared with existing permissible levels set by the WHO. The concentrations (mg/kg) were in the range of 102.27 - 192.53 for Cu, Zn (131.68 - 2840.85), Pb (41.06 - 303.96), Cd (1.92 - 20.49) whereas Ni was (13.83 - 38.27) with a mean of 150.76 ± 77.88 for Copper, 131.66 ± 1598.95 for Zinc, 234.60 ± 262.76 for Lead, 12.256 ± 10.86 for Cadmium and 29.45 ± 18.24 for Nickel across the five sampling locations. Cancellation Causes of elective Surgical Procedures in a Major Pediatric Surgery Department

Introduction: Cancellation of surgical operation is a surgical operation registered in the official schedule the day before or added to the list after and not carried out on the operating day. The purpose of this work was to determine the causes of cancellation of elective surgical operations in a major pediatric surgery department in Senegal. Patients and methods: It was a prospective and descriptive study of 278 patients scheduled during a period of 13 weeks. The study took place between April 3rd, 2017, and January 31st, 2018. Mean age was 2.9 years with extremes of 3 days and 15 years. The age group of 29 days to 30 months was the most represented (62.2%). Sex ratio was 1.41. Causes of cancellation were categorized into administrative and organizational causes, patient-related causes and staff-related causes. Results: Cancellation rate was 29.4%. Patient-related causes were most common (51.2%). Upper Respiratory tract infection (URTI) was commonest reason within this category (57.5%). Organizational causes (28.1%) came second and were mainly represented by the unavailability of the operating room (60.8%) related to breakdowns of anesthesia equipment. Finally, staff-related causes (20.7%) were due for most to the unavailability of the anesthesiologist (12 cases/17). Conclusion: Majority of causes that led to cancellation of elective surgical operations in our Pediatric surgery department are related to intercurrent illnesses affecting the patient, in particular URTI.

A Wearable Dual-Band Dielectric Patch Antenna for LTE and WLAN

In this paper, a dual-band microstrip patch array antenna for both the Multiple Input Multiple Output (MIMO) 4G Long-Term Evolution (LTE) and the Wireless Local Area Network (WLAN) systems is developed. Design simulation and optimization processes are carried out with the aid of the Advanced Design System (ADS) electromagnetic simula-tor that uses the full–wave Method of Moment (MoM) numerical technique [1]. Rectangular microstrip patch antenna constructed from Multi-Walled Carbon Nanotubes (MWCNT) ink with electrical conductivity of 2.2 × 104 S/m and relative permittivity of 5-j1 is used as the conductor patch [2]. The patch is deposited on Rogers substrate RT-Durid 5880 single substrate with (εr = 2.2) and thickness of 62 mil. U-shape slot is used to provide the dual-band. The pro-posed antenna operates at 3.5 GHz for LTE and 5 GHz for WLAN. The proposed antenna parameters are compared with published result of copper patch [3,4] for the same application. Great enhancement in antenna size, matching, and frequency band width are achieved for the proposed antenna at the expense of gain and antenna efficiency compared with copper one. The proposed MWCNT dielectric antenna size is 18.4 mm × 27 mm compared with size of 27 mm × 37 mm for copper one respectively. The gain is better than 7dBi with antenna efficiency of 87.29% at 3.5GHz for cop-per patch antenna and is better than 1.692 dBi with antenna efficiency of 27.05% at 3.5GHz for MWCNT respectively.

Parameters Influencing Power Generation in Eco-friendly Microbial Fuel Cells

Pulp and papermaking industries generate high volumes of carbohydrate-rich effluents. Microbial fuel cell(MFC) technology is based on organic materials' consumption and efficient power production. Using a classical two-chamber lab-scale MFC design with an external resistance of2000 W, we investigated the effects of anode chamber biofilm adaptation(ACBA) and cathode chamber redox solutions(CCRS) on the operation efficiency of MFC when treating wastewater. In ACBA studies, biofilm growth activation showed an increase in the power density to 20.48, 35.18, and36.98 mW/m^2 when the acetate feeding concentrations were 3, 6, and 12 g/L,respectively. Improvement by biofilm adhesion on granular activated carbon(GAC) was examined by scanning electron microscopy(SEM). The obtained power density increased to 25.47, 33.42, and 40.39 mW/m^2 when the GAC particles concentrations were 0, 50, and 100 g/L, respectively. The generated power densities were 51.26 and 40.39 mW/m^2 as well as the obtained voltages were 0.41 and 0.72 V when the electrode area increased from 16 to 64 cm^2,respectively. Using the MFC optimized parameters, CCRS studies carried out using five different cathodic redox solutions. The results revealed that the use of manganese dioxide dissolved in hydrochloric acid generated the maximum power density of 112.6 mW/m^2, current density of 0.094 A/m^2, and voltage of1.20 V with a successful organic removal efficiency of 86.0% after 264 h of operation.

Multipath Cognitive Routing in Integrated Multicast Satellite-HAP System Structures
