Correlation between DKK-1 Level and Bone Density Status in Children on Maintenance Haemodialysis

Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and Methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DEXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BMD) with a Z-score of under -2.0. Eighteen of these patients had low BMD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BMD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-MBD.

Numerical Modeling and Technico-Economic Analysis of a Hybrid Energy Production System for Self-Consumption: Case of Rural Area in the Comoros

This study aims to provide electricity to a remote village in the Union of Comoros that has been affected by energy problems for over 40 years. The study uses a 50 kW diesel generator, a 10 kW wind turbine, 1500 kW photovoltaic solar panels, a converter, and storage batteries as the proposed sources. The main objective of this study is to conduct a detailed analysis and optimization of a hybrid diesel and renewable energy system to meet the electricity demand of a remote area village of 800 to 1500 inhabitants located in the north of Ngazidja Island in Comoros. The study uses the Hybrid Optimization Model for electric Renewable (HOMER) Pro to conduct simulations and optimize the analysis using meteorological data from Comoros. The results show that hybrid combination is more profitable in terms of margin on economic cost with a less expensive investment. With a diesel cost of $1/L, an average wind speed of 5.09 m/s and a solar irradiation value of 6.14 kWh/m2/day, the system works well with a proportion of renewable energy production of 99.44% with an emission quantity of 1311.407 kg/year. 99.2% of the production comes from renewable sources with an estimated energy surplus of 2,125,344 kWh/year with the cost of electricity (COE) estimated at $0.18/kWh, presenting a cost-effective alternative compared to current market rates. These results present better optimization of the used hybrid energy system, satisfying energy demand and reducing the environmental impact.

Particle Swarm Optimization-Based Hyperparameters Tuning of Machine Learning Models for Big COVID-19 Data Analysis

Analyzing big data, especially medical data, helps to provide good health care to patients and face the risks of death. The COVID-19 pandemic has had a significant impact on public health worldwide, emphasizing the need for effective risk prediction models. Machine learning (ML) techniques have shown promise in analyzing complex data patterns and predicting disease outcomes. The accuracy of these techniques is greatly affected by changing their parameters. Hyperparameter optimization plays a crucial role in improving model performance. In this work, the Particle Swarm Optimization (PSO) algorithm was used to effectively search the hyperparameter space and improve the predictive power of the machine learning models by identifying the optimal hyperparameters that can provide the highest accuracy. A dataset with a variety of clinical and epidemiological characteristics linked to COVID-19 cases was used in this study. Various machine learning models, including Random Forests, Decision Trees, Support Vector Machines, and Neural Networks, were utilized to capture the complex relationships present in the data. To evaluate the predictive performance of the models, the accuracy metric was employed. The experimental findings showed that the suggested method of estimating COVID-19 risk is effective. When compared to baseline models, the optimized machine learning models performed better and produced better results.

Condition monitoring of the insulating system within power transformers has a massive importance according to the electrical *** gas analysis(DGA)is frequently used for this ***,DGA lacks the necessary level of accuracy to identify all equipment faults,particularly in their initial stages of ***,it does not have the capability for real-time monitoring and relies on manual sampling and laboratory testing,causing potential delays in fault ***,the interpretation of DGA data necessitates specialised expertise,which may pose difficulties for smaller entities that have limited access to ***,the contribution of this research is to use infrared spectroscopy measurements as a new effective technique substituting the DGA method for fault diagnosis in insulating *** inception faults that were considered in this study were the electrical fault(discharges of high energy)and the thermal fault(300°Cells were crafted especially for serving the simulation processes inside the laboratory for both types of inception ***,six samples of pure paraffinic mineral oil were taken to be degraded in the *** that,all of them besides another sample that were not subjected to any kind of faults were taken to be examined by Fourier transform infrared(FTIR)spectroscopy to obtain an overview of the oil's behaviour in each fault *** that,the FTIR analysis was initially verified utilising the DGA ***,for further affirmation,the dielectric dissipation factor(DDF)for all samples was *** the final analysis,the verification tests provide experimental evidence about the outperformance of this new optical technique in detecting the transformer's inception faults in addition to proving its potential for being a superior alternative to the well-known traditional diagnostic techniques.

A Typical Case of Classic Potter’s Syndrome: A Case Report

Potter syndrome is a rare congenital malformation that primarily affects male fetuses;it is characterized by pulmonary hypoplasia, skeletal malformation, and kidney abnormalities. The pressure of the uterine wall due to oligohydramnios leads to an unusual facial appearance, abnormal limbs in abnormal positions, or contractures. The fetus generally dies soon after birth due to respiratory insufficiency. The baby was a live preterm male, born to a 30-year-old multigravida, out of a non-consanguineous marriage via cesarean section. There was no liquor at the time of delivery. The baby did not cry immediately after birth and required resuscitation, followed by mechanical ventilation. Multiple congenital anomalies suggestive of Potter’s syndrome were noted including facial features, flattened nose, low protruding ear, retrognathism, and epicanthal folds with unilateral atresia of the choana. Chest X-ray showed small volume lung fields suggestive of pulmonary hypoplasia, and we had on ultrasonography bilateral polycystic kidney disease on ultrasonography. At 42 hours of life, the baby developed tachypnea and severe chest retractions and died due to respiratory insufficiency. Our case highlights the importance of regular prenatal checks and examinations in each pregnancy, which helps to collect suspected cases and improve knowledge of this syndrome for better management.

Enhancing Data Analysis and Automation: Integrating Python with Microsoft Excel for Non-Programmers

Microsoft Excel is essential for the End-User Approach (EUA), offering versatility in data organization, analysis, and visualization, as well as widespread accessibility. It fosters collaboration and informed decision-making across diverse domains. Conversely, Python is indispensable for professional programming due to its versatility, readability, extensive libraries, and robust community support. It enables efficient development, advanced data analysis, data mining, and automation, catering to diverse industries and applications. However, one primary issue when using Microsoft Excel with Python libraries is compatibility and interoperability. While Excel is a widely used tool for data storage and analysis, it may not seamlessly integrate with Python libraries, leading to challenges in reading and writing data, especially in complex or large datasets. Additionally, manipulating Excel files with Python may not always preserve formatting or formulas accurately, potentially affecting data integrity. Moreover, dependency on Excel’s graphical user interface (GUI) for automation can limit scalability and reproducibility compared to Python’s scripting capabilities. This paper covers the integration solution of empowering non-programmers to leverage Python’s capabilities within the familiar Excel environment. This enables users to perform advanced data analysis and automation tasks without requiring extensive programming knowledge. Based on Soliciting feedback from non-programmers who have tested the integration solution, the case study shows how the solution evaluates the ease of implementation, performance, and compatibility of Python with Excel versions.

Efficiency of Bio-Fertilizing as One of the Natural Alternatives to Improve the Growth of Khaya senegalensis and Swietenia mahagoni Trees and for Sustainability

A field experiment was carried out at Ismailia Research Station, Ismailia Governorate from 2020-2022 to improve the growth of Khaya senegalensis and Swietenia mahagoni by using a combination of mineral fertilizer (NPK) and biological fertilizer (Azotobacter chroococcum, Bacillus megatherium, and Bacillus circulant) as recommended dose under new sandy soils conditions. Split plot designed with four treatments (Control, (50% Mineral fertilizer (M.) + 50% Biological fertilizer (Bio.)), 100% M. and 100% Bio.) of each species. Vegetative growth, leaf area, tree biomass, stored carbon, basal area, tree volume, and in the soil both of microbial account and mineral content were determined. The experimental results showed no significant differences between studied species among the most studied parameters except for Khaya senegalensis which gave the highest significant difference in root biomass and below-stored carbon than Swietenia mahagoni. Evidently, the highest significant growth parameters were 100% mineral fertilizer followed by (50% M. + 50% Bio.) as compared with control. No significant difference between 100% M. and (50% M. + 50% Bio.) of shoot dry biomass (15.19 and 12.02 kg, respectively) and above-stored carbon (0.28 and 0.22 Mt, respectively). Microbial account and mineral content in soil were improved after cultivation of tree species compared to before planting and control, especially with 50% mineral fertilizer and 50% bio-fertilizer treatment. In conclusion, a treatment containing 50% mineral fertilizer and 50% bio-fertilizer has led to the ideal Khaya senegalensis and Swietenia mahagoni growth in sandy soil for cheaper and sustainable.

A Comparative Analysis on Surgical Outcomes and Complications of Endoscopic and Open Vein Harvesting Techniques

Objective: The great saphenous vein (GSV) is commonly used as a conduit for grafting during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of patients who underwent Coronary artery bypass graft (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed for both groups. Intraoperative variables such as the number of grafts, cardiopulmonary bypass time, X clamp time, and type of procedure were analyzed. Postoperative variables, including infection and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graft (CABG) with need of great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of age, while a significant difference in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of grafts compared to OVH. Conversion to an open technique occurred in a portion of the EVH cases, and infection rates did not significantly differ between the EVH and OVH groups. However, the incidence of postoperative bleeding was significantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of reduced infection rates compared to OVH. However, the higher incidence of postoperative bleeding associated with EVH raises concerns about potential risks.

Identification of Secondary Metabolites in Tunisian Tilia platyphyllos *** MALDI-TOF and GC-MS

This study is the first to evaluate the phytochemical content and biological properties of Tunisian *** Scop.A total of 23 compounds of essential oils were identified by gas chromatography-mass spectrometry(GC-MS)analysis of bracts and fruit *** results show that oxygenated monoterpenes were the dominant class of essential *** phenolic composition was investigated by matrix-assisted laser desorption/ionizationtime of flight(MALDI-TOF).The analysis showed that the chemical profiles of the ethanolic extracts of bracts and fruits are substantially *** highest polyphenol content was found in the ethanolic extracts of the fruits(7.65 mg gallic acid equivalents(GAE).g−1 on dry weight(DW).As for the antioxidant capacity,it was recorded in the essential oils of the fruits(IC_(50)=0.91 ***^(−1)).The results obtained showed that the antioxidant activity of the fruit essential oil was higher than that of the ethanol *** fruit essential oil was also found to have the highest reducing power(IC_(50)=0.67 ***^(−1)).In terms of iron-chelating power,fruit essential oil has the highest chelating power(IC_(50)=2.03 ***^(−1)).Compared to the ethanolic extract,the essential oil had the maximum antioxidant *** enzymatic activity of acetylcholinesterase(AChE)inhibition of the essential oil from *** bracts had the most potent inhibitory effect(IC_(50)=0.77 ***^(−1)),followed by the essential oil from the fruits(IC_(50)=0.95 ***^(−1)).The results suggest that *** can be used as a potential source of naturally occurring bioactive compounds and antioxidants.
