The Impact of Single Low Dose IV Magnesium Sulphate Adjuvant to Ultrasound Guided Transverses Abdominis Plain Block for Control of Postcesarean Pain

Objective: The aim of this study is to determine the role of preoperative low dose intravenous MgSO4 when given adjuvant to ultrasound guided transversus abdominis plane (TAP) block in augmenting postcesarean analgesic effects and reducing opioid requirements during the first 24 hours. Subjects and Methods: In this prospective, randomized double blind study, a total of sixty full term pregnant women were recruited for the study underwent caesarean section. Thirty patients were assigned to MgSO4 group (A) and another thirty to placebo group (B). Participants in group (A) received 50 mg/kg MgSO4 in 100 ml isotonic saline intravenous (IV) over 20 minutes prior to induction of general anesthesia by 30 minutes while participants in group (B) received 100 ml isotonic saline (placebo) by the same route and over the same duration as control. Results: Visual Analogue Scale (VAS) was analysed within 24 hours postoperatively. The mean pain score at 6 and 12 hours postoperatively was significantly lower in MgSO4 group compared to control group (40.4 ± 5.12 vs 53.6 ± 4.92;26.1 ± 3.01 vs 35.5 ± 3.98 respectively, p = 0.012, 0.005). Comparing both groups regarding the mean time interval of first rescue analgesia (morphine sulphate) requested by the patients, it was longer in MgSO4 group compared to control group. The total dose of rescue analgesia consumed during 24 hours was analysed and it was significantly higher in control group compared to MgSO4 group (10.1 ± 0.95 vs 6.2 ± 0.87, p = 0.001). Conclusion: We concluded that preoperative low doses (50 mg/Kg) of MgSO4 with general anesthesia combined with ultrasound guided TAP block offer longer postoperative pain free periods thus reducing total opioid consumption. In addition to the safety of the drug to the mother and fetus so we recommend IV MgSO4 as an adjuvant therapy with TAP block.

Changes in Maternal Serum Leptin Levels during Pregnancy and after Labor in Preeclampsia, and Its Correlation to Neonatal Cord Leptin

Objective: The aim of this study is to determine changes in maternal serum leptin level during pregnancy and after labor in preeclamptic patients compared to healthy pregnant women. Furthermore, to investigate whether maternal serum leptin levels are correlated to the clinical characteristics and laboratory parameters of the study participants, and the possible correlation between maternal and neonatal leptin levels. Subjects and Methods: In this case control study, a total number of fifty five pregnant women in third trimester of pregnancy (≥28 weeks) were recruited. All of them were of the same age, body mass index, and gestational age. After a detailed obstetrical and medical history, they were divided into 2 groups. Group (A) 30 pregnant women with preeclampsia, and Group (B) 25 normotensive pregnant women. Results: During pregnancy, maternal serum leptin levels were significantly higher 41.0 ± 9.78 ng/ml in preeclamptic group compared to control group 24.6 ± 3.64 ng/ml (p = 0.007). After labor, it decreased significantly in both groups to 15.3 ± 3.19, and 11.2 ± 2.68 ng/ml respectively (p = 0.001, 0.002). In group (A) there were significant positive correlations between maternal serum leptin and diastolic blood pressure (r = 0.419, p = 0.021), total cholesterol (r = 0.383, p = 0.026), and uric acid (r = 0.424, p = 0.012) compared to controls, and no significant correlations were found between maternal serum leptin and body mass index, neonatal birth weight or cord leptin level in both groups. Conclusion: Maternal serum leptin is significantly increased in preeclamptic patients compared with normal pregnant women independent of body mass index. There is strong evidence that placenta, rather than maternal adipose tissue is responsible for that. In addition, maternal serum leptin levels were found to correlate positively with diastolic blood pressure, uric acid, and total cholesterol, but not correlated with body mass index, cord blood leptin and birth weight.

Prevalence of Maternal Anemia in Pregnancy: The Effect of Maternal Hemoglobin Level on Pregnancy and Neonatal Outcome

Objective: The aim of our study is to assess the prevalence of anemia in pregnant women, and to evaluate the effect of severity of anemia on maternal and perinatal outcome. Methods: This retrospective cohort study was conducted at the Department of Obstetrics and Gynecology from hospital records. Study population of all pregnant women who had delivered in our hospital after twenty weeks gestation between July 2014 and December 2016. Results: A total of 2654 pregnant women fulfilled the inclusion criteria, 42% were anemic, 83.3% mild anemia group (I) which represents the majority of patients and 16.7% moderate to severe anemia group (II). The majority of cases were due to iron deficiency anemia 92.8%, while 7.2% were due to sickle cell trait, B-thalassemia intermedia, and other causes. The incidence of postpartum hemorrhage, cesarean delivery, and infections was significantly higher in group II compared to group I (5.4%, 40.3%, 3.8% Vs, 2.9%, 31.0%, 2.0%;p = 0.007, 0.041, 0.043 respectively). Low Apgar score, preterm labor, and low birth weight babies were significantly higher in group II compared to group I (11.8%, 12.9%, 11.3% Vs 8.7%, 9.0%, 7.4%;p = 0.034, 0.046, 0.032). Conclusions: This study clarified that anemia is prevalent among pregnant women particularly mild anemia. Early diagnosis and treatment from first trimester has an essential role in managing maternal anemia and it reflects directly on the perinatal outcome. Prematurity, low birth weight infants, and postpartum hemorrhage are the commonest maternal and neonatal complications.

The Impact of Third Trimester Maternal Serum Vitamin B12 and Folate Status on Fetal Birth Weight. Is Maternal Serum Homocysteine a Predictor of Low Birth Weight Infants?

Objective: The aim of this study is to evaluate vitamin B12, folate, and homocysteine status in pregnant women in the third trimester of pregnancy and their relationship to fetal birth weight and their correlation to corresponding neonatal cord blood levels, and in addition, to evaluate the possibility of maternal serum homocysteine level as a predictor of low birth weight infants. Subjects and Methods: In this cross-sectional study, a total of two hundred pregnant women in third trimester (≥28 weeks) were recruited. After a detailed obstetrical and medical history, and clinical assessment, participants were subdivided into two groups: Group (A)—pregnant women who delivered average birth weight (ABW) infants and Group (B) for those who delivered low birth weight (LBW) infants between completed 37 and 42 weeks. Results: Vitamin B12 deficiency was observed in 24.1% of the total cohort. The mean vitamin B12 level was significantly lower in group (B) compared to group (A) (195.2 ± 38.9 vs. 225.9 ± 66.59 respectively P = 0.008). The mean level of homocysteine for women in group (B) was significantly higher than those determined from women in group (A) (9.10 ± 5.9 vs. 7.6 ± 3.83 respectively, P = 0.042). On the other hand, the mean folate levels showed statistically insignificant differences between both groups. The mean cord vitamin B12 level was significantly lower in LBW infants in comparison to ABW infants (277 ± 61.93 vs. 312.03 ± 81.87 respectively, P = 0.015), while the mean level of cord homocysteine for LBW infants was significantly higher than those levels determined from ABW infants (7.9 ± 3.79 vs. 6.6 ± 2.09 respectively P = 0.0049). Conclusion: Maternal micronutrients particularly cobalamin deficiency could be significant risk for LBW infants. Hyperhomocysteinemia has been shown to be a predictor for adverse pregnancy outcomes particularly LBW.

Textile dyes are dramatic sources of pollution and non-aesthetic disturbance of aquatic life and therefore represent a potential risk of bioaccumulation that can affect living *** is imperative to reduce or eliminate these dyes from liquid effluents with innovative biomaterials and ***,this research aims to highlight the performance of Capparis spinosa L waste-activated carbon(CSLW-AC)adsorbent to remove crystal violet(CV)from an aqueous *** mechanism of CV adsorption on CSLW-AC was evaluated based on the coupling of experimental data and different characterization *** efficiency of the CSLW-AC material reflected by the equilibrium adsorption capacity of CV could reach more than 195.671 mg·g^(–1) when 0.5 g·L^(–1) of CSLW-AC(Particle size≤250μm)is introduced into the CV of initial concentration of 100 mg·L^(–1) at pH 6 and temperature 65℃ and in the presence of potassium ions after 60 min of contact time according to the one parameter at a time *** adsorption behavior of CV on CSLW-AC was found to be consistent with the pseudo-second-order kinetic model and Frumkin's linear isothermal *** thermodynamic aspects indicate that the process is physical,spontaneous,and *** optimization of the process by the Box Behnken design of experiments resulted in an adsorption capacity approaching 183.544 mg·g^(–1)([CV]=100 mg·L^(–1) and[CSLW-AC]=0.5 g·L^(–1) at 35 min).The results of the Lactuca sativa seeds germination in treated CV(70%),adsorbent solvent and thermal regeneration(more than 5 cycles),and process cost analysis(1.0484 USD·L^(–1))tests are encouraging and promising for future exploitations of the CSLW-AC material in different industrial fields.

A 56-day feeding period was performed to investigate the possible impacts of dietary nano curcumin(0,50,100,150,and 200 ppm)on the growth,nutrient utilization,non-specific immune parameters,antioxidants in Nile tilapia under chronic low temperature(21.02±0.11◦C).Fishes(n=225;Initial weight=4.39±0.08 g/fish)were randomly stocked at 15 fish/tank for five experimental groups in *** low-temperature circumstances,dietary curcumin in nano form showed no notable alteration in growth variable,nutrient efficiency,digestive enzymes efficiency,biometric indices,survival rates,and hematological ***,the serum of fishes with nano curcumin diets under low-temperature stress displayed higher total protein as well as lower glucose,cortisol,and total cholesterol compared with the control ***,fish fed nano curcumin diets displayed higher lysozyme and bactericidal activities compared to the control group and the best performance was found at dietary nano curcumin level of≥100 ***,groups fed the basal diet demonstrated the poorest antioxidant capacity,and the best superoxide dismutase(SOD)and glutathione peroxidase(GPx)existed in fish with nano curcumin diets while the best catalase(CAT)efficiency occurred at higher nano curcumin levels≥100 *** addition,higher counts of intestinal microbiota in terms of total bacterial count(TBC),total yeast and molds count(TYMC),and coliform were noticed in fish consumed the basal diet compared to groups fed on nano curcumin *** conclusion,incorporating nano curcumin at a level of≥100 mg/kg diet(particularly at 150 mg/kg)improved a non-specific immune response,antioxidant,and healthier gastrointestinal microbiota in Nile tilapia under chronic low-temperature stress.

Adaptive Fuzzy Sliding Mode Power System Stabilizer Using Nussbaum Gain

Power system stability is enhanced through a novel stabilizer developed around an adaptive fuzzy sliding mode approach which applies the Nussbaum gain to a nonlinear model of a single-machine infinite-bus （SMIB） and multi-machine power system stabilizer subjected to a three phase fault. The Nussbaum gain is used to avoid the positive sign constraint and the problem of controllability of the system. A comparative simulation study is presented to evaluate the achieved performance.
