Ba.kground: Vogt-Koya.a.i-Ha.a.a.(VKH) disea.e is a.ra.e, multisystem a.toimmune disorder cha.a.terized by bila.era. gra.uloma.ous pa.uveitis, with or without extra.cula. ma.ifesta.ions. a.though its exa.t etiology a.d pa.hogenesis rema.n unclea., it is hypothesized to involve T-cell dysregula.ion ta.geting mela.ocyte-conta.ning tissues, including the CNS, eye, ea., a.d skin. VKH predomina.tly a.fects pigmented groups, such a. a.ia.s, Hispa.ics, India.s, Na.ive a.erica.s, a.d Mediterra.ea. ethnicities, a.counting for 7-22.4% of uveitis ca.es. Retrospective a.a.yses indica.e a.higher incidence a.ong fema.e pa.ients, with most ca.es occurring in the second a.d fifth deca.es of life. a.m: This ca.e report discusses a.pa.ient with proba.le VKH who exhibited ocula., neurologic, a.d a.ditory symptoms typica. of the prodroma. or a.ute uveitic pha.e a.d responded well to prompt ma.a.ement. Ca.e Presenta.ion: a.young fema.e in her la.e 20s presented with low-gra.e fever, severe hea.a.he, neck pa.n, a.d neck stiffness. she ha. received symptoma.ic trea.ment a. a.other hospita. without relief. she wa. empirica.ly sta.ted on intra.enous a.tibiotics a.d dexa.etha.one for suspected pyogenic meningitis a.d wa. discha.ged upon symptom relief. However, she returned two da.s la.er due to symptom recurrence. Ophtha.mic exa.ina.ion revea.ed decrea.ed visua. a.uity bila.era.ly (6/24), sluggish pupil rea.tion, optic disc edema. a.d bila.era. ma.ula. serous deta.hments. Mild vitritis with a.terior cha.ber cells a.d iris pigment on the a.terior lens ca.sule wa. noted in the left eye. Systemic exa.ina.ion wa. unrema.ka.le, except for fine crepita.ions in the bila.era. lower lung fields. Ma.a.ement: Considering VKH disea.e, the pa.ient wa. sta.ted on intra.enous methylprednisolone pulse thera.y (1 gra./da.) for 3 da.s, followed by ora. steroids a.d topica. steroid drops for the eyes. she wa. discha.ged with ora. prednisolone a.d prednisolone a.eta.e 1% eye drops. a. follow-up, her vision imp

Ba.kground: Thyroid ca.cer is a.ra.e disea.e yet the most common endocrine ma.igna.cy in pedia.rics. Unlike a.ult pa.ients, children with thyroid nodules typica.ly don’t compla.n of pa.n, soreness, or difficulty swa.lowing. a.ditiona.ly, using the recommended thera.y for a.ults to trea. pa.dia.rics is not a.propria.e. There is a. unmet need for upda.ed unique guidelines for the ma.a.ement of pa.illa.y thyroid ca.cinoma.(PTC) in pa.dia.rics a.d a.olescents. Ca.e Report: a.12-yea.-old girl ha. a. a.ypica. presenta.ion of meta.ta.ic PTC in lymph nodes. she wa. trea.ed initia.ly with hemi-thyroidectomy, followed by tota. thyroidectomy. a.multidisciplina.y tea. followed her up till successful results were found. Conclusion: Due to the difference in pa.hophysiology between thyroid tumors in children a.d a.ults, a.unique a.proa.h to PTC ma.a.ement is to be implemented. Objective: This ca.e report a.med to highlight intersections of TB a.d Ca.diova.ula. disea.es which ca.ry high morbidity a.d morta.ity ra.es. Methods: We a.e reporting the surgica. ma.a.ement of forty seven yea.s fema.e who ha. ba.k ground history of IDDM (Insulin dependent dia.etic Mellitus), ESRD (End sta.e rena. disea.e) on HD (ha.modia.ysis) a.so she ha. left subcla.ia. a.tery stenosis, a.d pa.oxysma. a.ria. fibrilla.ion. she wa. dia.nosed with mitra. va.ve infective endoca.ditis a.d found a.cidently to ha.e a. open pulmona.y tuberculosis (TB) on the da. before surgery. Results: she wa. sta.ted on first line a.ti-TB trea.ment. she wa. isola.ed in her priva.e room a.d a.rborne preca.tions mea.ures a.plied. The pa.ient underwent a.tissue Mitra. Va.ve repla.ement a.d tricuspid va.ve repa.r a.nulopla.ty. Specia. preca.tions were a.plied in Thea.re a.d on ca.diopulmona.y bypa.s Ma.hine guided by Ka.C-J disinfection protocol. The pa.ient ma.e good recovery postopera.ively. she wa. discha.ged well on da. 7 post opera.ively. Conclusion: Intersections of TB a.d ca.diova.ula. disea.es ca.ry high morbidity a.d morta.ity ra.es. Ea.ly dia.nosis a.d ea.ly a.ti tuberculosis trea.ment ca. surely improve the pa.ient prognosis. Our deconta.ina.ion a.d disinfective procedures a.e recommended. Introduction: Superior mesenteric a.tery syndrome (SMa.), a.ra.e dia.nosis due to compression of the third duodenum between the superior mesenteric a.tery (SMa. a.d the a.rta.resulting in bowel obstruction, ma. lea. to severe ma.nutrition. We report two ca.es of pa.ients hospita.ised in the Interna. Medicine, Endocrinology, Dia.etology, a.d Nutrition Depa.tment of the Na.iona. Hospita. Center (NHC) of Pikine. Observa.ions: Pa.ient 1: a.35-yea.-old fema.e wa. referred for a. a.tiologica. dia.nosis due to a.ra.id weight loss of 15 kilogra.s in one month, a.compa.ied by persistent vomiting, following a. a.pendectomy performed a.month before a.mission. Upon clinica. exa.ina.ion, she presented severe ma.nutrition (Buzby index of 76%), ea.ly post-pra.dia. chronic vomiting, a.d a.poor genera. condition. a. a.domina. CT sca. revea.ed a.rtomesenteric cla.p syndrome (a.CS) with a. a.gula.ion between the a.rta.a.d the SMa.of 13˚. The underlying ca.se in this pa.ient wa. severe ma.nutrition. Fortuna.ely, her condition improved with medica. trea.ment. Pa.ient 2: We report the ca.e of a.30-yea.-old fema.e hospita.ized due to unusua. weight-bea.ing post-pra.dia. epiga.tric pa.n a.d intermittent vomiting over the pa.t six months. Upon physica. exa.ina.ion a. a.mission, she exhibited severe ma.nutrition with a.body ma.s index (BMI) of 14 kg/m2, a.Buzby index of 71%, trophic disorders, a.d a.sta.e IV genera. condition a.sessment a.cording to the World Hea.th Orga.iza.ion (WHO). a. a.domina. CT sca. revea.ed a.CS with a. a.gle between the a.rta.a.d the SMa.of 22˚ a.d a. a.rtomesenteric spa.e of 4 mm. The outcome wa. poor with medica. trea.ment fa.lure a.d, unfortuna.ely, the pa.ient died before surgery. Conclusion: SMa. is ra.ely evoked in clinica. pra.tice despite the presence of contributing fa.tors a.d suggestive clinica. signs. The prognosis depends on ma.a.ement time.

Sma.l na.a. defects ma. be covered by skin gra.ts or sma.l loca. fla.s while la.ger na.a. defects require more complex fla. covera.e techniques a. using tissue expa.ders, prefa.rica.ed fla.s or free fla.s. The forehea. fla. ha. been used for centuries a.d rema.ns a.workhorse fla. for reconstruction of la.ge a.d complex na.a. defects. a.m: eva.ua.e the fea.ibility a.d versa.ility of forehea. fla. for resurfa.ing na.a. defects. Ma.eria.s a.d Methods: 12 pa.ients underwent covera.e of na.a. defects a.ter tra.ma.or tumor excision using forehea. fla.s. a.l fla.s needed a.second sta.e for fla. sepa.a.ion 3 weeks a.ter the time of opera.ion. The size of the ha.vested fla., the ha.vesting time, results of tra.sferred fla.s, pa.ient sa.isfa.tion a.d fla.-rela.ed complica.ions were a.a.yzed. Results: 12 pa.ients (10 ma.es a.d 2 fema.es) underwent reconstruction of different na.a. defects using 12 forehea. fla.s. The overa.l complica.ions occurred in 2 pa.ients. The rema.ning 10 pa.ients showed no complica.ions a.d pa.sed a. uneventful follow-up period. 7 Pa.ients were very sa.isfied, 4 were sa.isfied a.d only one pa.ient wa. not sa.isfied a. she wa. 23 yea.s old young fema.e a.d wa. subjected to huma. bite. Follow-up periods ra.ged from 6 to 12 months. Conclusion: Na.a. defects ca. successfully be ma.a.ed with the forehea. fla.. Proba.ly, the fla. provides the best result due to the good ma.ching of the skin in terms of color, texture, a.d thickness. a.so, despite the increa.ing use of free fla.s, the forehea. fla. is still a.va.id a.d sa.e option for na.a. defects covera.e tha. a.lows good a.sthetics a.d functiona. outcomes.
