Evaluation of Bilateral Mental Nerve Block as an Alternative to General Anesthesia for Resection of Lower Lip Tumors

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Objectives: To assess the impact of heart failure diseases on stroke severity and short term (1 month mortality). Materials and Methods: Totally, 200 patients admitted to Ain Shams university specialized hospital, were diagnosed clinically to have acute ischemic stroke within 3 days. History taking about previous heart disease was taken, and full general and neurological examinations were done. Full metabolic profile, cardiac examination and investigations were done. Carotid duplex, MRI stroke protocol and lastly clinical reevaluation using NIH Stroke Scale (NIHSS) after 1 month from stroke onset were done. Results: Patients with systolic dysfunction (EF ≤ 40%) had lacunar infarction in 18.75% of cases, partial anterior circulation infarction in 62.5% of cases, posterior circulation infarction in 6.25% of cases and total anterior circulation infarction in 12.5% of cases. Leucoaraiosis was present in 87% of cases and significant intracranial vessel stenosis was showed in 87.5% who had done MRI. Their median NIHSS score at admission was 10.5 with IQR of 5 - 21 and at follow-up it was 5 with IQR of 2.5 - 10.5. At follow-up 22.7% (5 patients) were dead. Conclusion: Systolic dysfunction (EF < 40) i.e. heart failure has no significant effect on stroke severity and 1 month follow up prognosis.

Chenopodium album is an invasive weed in wheat fields in Egypt. The present study indicated the potentials for utilization of MILAE (Mangifera indica leaves aqueous extract) at different concentrations (5%, 10%, 15% and 25%) to suppress the germination and growth of C. album in Petri dish experiment. Results indicated that predictable degrees of inhibition in C. album germination and growth rate of seedlings were accompanied with the increase in the concentration of MILAE. PL (Plumule) and RL (radicle) lengths of C. album were negatively affected as the concentration of MILAE increased and the effect was prominent at 25% extract concentration. This study recommended the use of MILAE as an alternative bioherbicides, since its leaves extract possess strong phytotoxic potential allelochemicals.

The present study is based on the investigation of performance of C,N-bipyrazole receptor grafted onto silica surface (SG2P) of adsorption Arsenic (AS) from aqueous solutions. The effects of operating parameters that include pH, contact time, concentration of As and dosage of adsorbent on adsorption were accomplished. The results clearly showed that the removal efficiency of As was decreased with an increasing of As concentration, pH, and temperature, while it was continuously increasing with time and adsorbent dose. Moreover, the removal efficiency of Cr (VI) adsorption was 75% corresponding to pH;temperature (°C), initial concentration (ppm) and weight of dose (g) were 6, 25, and 0.04 respectively at 24 hours. The adsorption capacity of the synthesized sorbent (SG2P) for arsenic at pH Escherichia coli (ATCC25922) and Staphylococcus aureus (ATCC25932) were as a reference strains, while, the SG2P was able to inhibit growth only at high concentration (MIC = 1.5625 mg/ml).

Formation and Characterization of Zinc Phosphate Coating on Reinforcing Steel Rebars

Zinc phosphate coating has been formed on reinforcing steel rebars by immersion in a zinc phosphate bath to enhance the corrosion resistance. The morphology and the coating composition on the electrode surface were analyzed by scanning electron microscopy (SEM), energy dispersive spectroscopy (EDX) and X-ray diffraction. The phosphate films formed are mainly composed of hopeite (Zn3(PO4)2·4H2O) and phosphophyllite (Zn2Fe(PO4)2·4H2O). A mechanism is proposed to explain the formation of these phases. In addition the corrosion protection of the coating was studied by weight loss and electrochemical measurements. The results showed that, the coating layer on the rebar surface can effectively improve the corrosion resistance of steel in marine environment.

This paper represents comparative study on two control techniques applied to H-bridges multilevel inverter. The considered techniques are standard PWM and harmonic elimination control. Their performances are compared in terms of THD and commutation’s losses so that the choice of power switches is optimized. Simulation results indicated that the harmonic elimination technique exhibits better results than the standard PWM technique, which is sustained by the reduce of the commutation losses, and a power rise for the switches ensuring the use of GTOs, IGCTs.

The following dissertation aims at investigating the multilevel topology basis through the analyses of the harmonic elimination control modeling methodology of the asymmetrical inverter. Thus the contribution of this paper can be summarized mainly by the fact that the pre-calculated PWM control used to control the symmetrical structure, as applied to the asymmetrical structure with a reduced number of cells, provides the same performance level resolution and signal strength to the load terminals, which guarantees a constant voltage frequency report. This observation is similar for the three cells, which allows the use of different drivers such as MOSFET, IGBT and GTO. These results have a good performance from the intrinsic proprieties of each semiconductor for each cell.
