Impact of renal transplantation on cardiac morphological and functional characteristics in children and adults

AIM To compare the effects of renal transplantation on cardiac functions in children and *** One hundred and ten patients attending the nephrology outpatient clinic were enrolled in this study and were divided into six groups. The first two groups consisted each of 30 renal transplant patients who had a successful renal transplantation more than six months, but less than one year. Group Ⅰ were less than 18 years and group Ⅱ were more than 18 years. The third and fourth groups, each were 20 chronic renal failure patients on regular hemodialysis. Again, group Ⅲ were less than 18 years and group Ⅳ were more than 18 years. Group Ⅴ and Ⅵ(The control Groups) consisted each of 5 subjects below and above 18 years of age, respectively with normal kidney functions. All patients were subjected to history and examination. The kidney functions and the hemoglobin were analyzed. Afterobtaining informed consent, echocardiography was done to all *** There was a statistically significant improvement(P < 0.0001) in all cardiac parameters. A regression in left ventricular end diastolic volume(LVED) both in children(4.7 ± 0.8 to 4.2 ± 0.5) and in adults(5.9 ± 0.7 to 4.9 ± 0.6) were found. There was a regression in left ventricular end systolic volume(LVES) both in children(3.1 ± 0.6 to 2.4 ± 0.4) and in adults(4.1 ± 0.9 to 3.1 ± 0.5). Fractional shortening improves both in children(32.6 ± 5.3 to 41.7 ± 7.6) and in adults(29.0 ± 6.6 to 36.5 ± 4.1). The improvement in ejection fraction(EF) was higher in children(59.7 ± 7.0 to 71.9 ± 6.1) than in adults(52.0 ± 12.5 to 64.8 ± 5.9). However, this degree of improvement(in children: 12.2 ± 5.1) did not show statistical difference(P-value 0.8), when compared to adults(12.7 ± 9.8). CONCLUSION After renal transplantation cardiac functions and morphology(EF/LVED/LVES) do improve markedly and rapidly in both children and adults.

Background: In developed countries, colon cancer is the second most prevalent cancer, only exceeded by prostate cancer in men and breast cancer in women. After Hepatocellular carcinoma, breast cancer, bladder cancer, lung cancer, Non-Hodgkin Lymphoma and brain tumors, colon cancer is the 7th most common cancer in Egypt, in both sexes, representing 3.47% and 3%, in both male and female cancers, respectively. Aim of the Work: The aim of this study was to evaluate the prognostic and predictive significance of pretreatment Neutrophil/lymphocytes ratio (NLR), in terms of disease-free survival (DFS) and recurrence, in high-risk stage II and stage III Colorectal cancer patients who underwent curative resection. Patients and Methods: We retrospectively evaluated 103 patients, who were submitted to upfront surgery as first therapeutic option in curative intent, between January 2017 and December 2018. Pretreatment Neutrophil/lymphocytes ratio (NLR), as well as demographics, clinical, histopathologic, and laboratory data were analyzed. Univariate and multivariate analyses were conducted to identify prognostic factors associated with disease free survival (DFS) and recurrence. Results: The cutoff point of Neutrophils/lymphocytes ratio (NLR) was calculated with Kaplan-Meier curves and log-rank test to 3. This study revealed that neutrophils/lymphocytes ratio (NLR) was significantly associated with disease free survival (p as no difference in efficacy between both chemotherapy regimens FOLFOX and XELOX in both high-risk stage II and stage III colon cancer regarding disease free survival & the toxicity profile associated with each regimen and its grades between patients. Conclusion: Our study suggests that preoperative Neutrophils/lymphocytes ratio (NLR) more than 3 may be an independent prognostic marker for TTR (time to recurrence) in high-risk stage II and stage III colon cancer patients.

In the present study, we investigated the biochemical alterations and gene expression of carbohydrate and lipid metabolism after oral administration of vitamin A and E for 2 months in diet-induced obese Wistar rats. Vitamin A and E administration reduced significantly the increase in body weight and food intake and normalized the alterations in lipid profiles in obese rats compared to normal rats. Moreover, both vitamins decreased the fat accumulation in liver tissues of obese rats. Finally, they up-regulated mRNA expression of Pyruvate Kinase (PK) and Glucose Transporter-2 (GLUT-2), and increased lipolysis and cholesterol metabolism through up-regulation of lipoprotein lipase (LPL), Sterol Responsible Element Binding Protein-1a (STREBP-1a) and STREBP-1c mRNA expression. In conclusion, vitamin A and E regulate gene expression of carbohydrate and lipid metabolism, and also ameliorate changes associated with obesity induced by high fat diet in Wistar rats.

Prevalence and Correlates of Overweight and Obesity among School Children in Sohag, Egypt

Background: Continually rising prevalence of obesity and overweight in children and adolescents is a major public health concern. This is due to its various and serious health hazards on one side and its preventable nature on the other side. This study aims at identification of prevalence of overweight and obesity and its risk factors among children between 6 - 14 years of age in Sohag, Egypt. Method: A cross sectional study was conducted on 711 school children between 6 - 14 years in four months’ period from February to May 2016. Weight and height of the studied children were measured to calculate BMI, which was compared with standard Egyptian growth charts for determining overweight and obesity. Then a questionnaire containing data about socio demographic, parental obesity, early life and dietary risk factors was completed by the children’s parents. Result: Out of 711 studied children, 117 (16.5%) were overweight and 104 (14.6%) were obese. Residing in urban area, having an obese parent or both of them, low level of maternal education, being on formula feeding during early life and lack of fresh vegetables and fruits in diets were significant risk factors for overweight and obesity in the studied children (P value < 0.05). Conclusion: Improperly educated mothers together with sedentary life style manifested in obesity among the parents and lack of sport practice among children constituted the majors risk factors for childhood obesity. This emphasizes the need for increased health awareness of the community about the importance of proper nutrition and physical activity.

Outcome of Management of Pseudomyxoma Peritonei: National Cancer Institute Experience

Background: The current treatment for pseudomyxoma peritonei (PMP) consists of radical cytoreductive surgery (CRS) followed by hyperthermic intra-peritoneal chemotherapy (HIPEC). Aim: To assess PMP patients regarding the clinical and pathological characteristics, the treatment including surgery (CRS) and chemotherapy either HIPEC type or post-operative systemic chemotherapy aiming to evaluate end results regarding recurrence and survival. Patients and Methods: This retrospective study included 39 patients with PMP who were diagnosed, treated and followed-up from 2009-2014 at National Cancer Institute, Cairo, Egypt. Results: High grade mucinous adenocarcinoma was found in 23.1% of patients. Patients with low grade tumor showed higher survival rate compared with patients with high grade disease. The mean operative PCI score (peritoneal cancer index) that was done to all patients who were explored was 15.81. Our study reported success to achieve complete cytoreduction that was combined with HIPEC in 44% of patients who were planned for this modality. Treatment related postoperative grade (3 - 5) complications mainly surgery related developed in 17.3 of patients. Operative mortality was 22.2%. The follow up period in our study was quite short (mean 22.9). However the overall survival at the end of the follow up in our study was 48.7%, 1 year survival was 82%, and 2 year survival was 41%. The overall survival in patients treated with CRS and HIPEC was 66.6%, with 1 year and 2 year survival of 91% and 66.6% respectively. Only 2 patients developed recurrent disease during the follow up period. Conclusions: The outcome of PMP treatment process is extremely variable. Combined CRS and HIPEC is considered the best therapeutic approach for patients with PMP. Surgical experience combined with proper patient selection have to be built up together to improve the outcome. That could only be achieved through more centralization of patients’ treatment in specialized units or center.

Assessment of plant diversity of endemic species of the Saharo-Arabian region in Egypt

Savanna, semi-deserts, and hot deserts characterize the Saharo-Arabian region, which includes Morocco, Mauretania, Algeria, Tunisia, Libya, Egypt, Palestine, Kuwait, Saudi Arabia, Qatar, Bahrain, the United Arab Emirates, Oman, Yemen, southern Jordan, Syria, Iraq, Iran, Afghanistan, Pakistan, and northern India. Its neighboring regions, the Sudano-Zambezian region belonging to the Paleotropical Kingdom and the Mediterranean and Irano-Turanian regions included in the Holarctic Kingdom, share a large portion of their flora with the Saharo-Arabian region. Despite the widespread acknowledgment of the region's global importance for plant diversity, an up to date list of the Saharo-Arabian endemics is still unavailable. The available data are frequently insufficient or out of date at both the whole global and the national scales. Therefore, the present study aims at screening and verifying the Saharo-Arabian endemic plants and determining the phytogeographical distribution of these taxa in the Egyptian flora. Hence, a preliminary list of 429 Saharo-Arabian endemic plants in Egypt was compiled from the available literature. Indeed, by excluding the species that were recorded in any countries or regions outside the Saharo-Arabian region based on different literature, database reviews, and websites, the present study has reduced this number to 126 taxa belonging to 87 genera and 37 families. Regarding the national geographic distribution, South Sinai is the richest region with 83 endemic species compared with other eight phytogeographic regions in Egypt, followed by the Isthmic Desert(the middle of Sinai Peninsula, 53 taxa). Sahara regional subzone(SS1) distributes all the 126 endemic species, Arabian regional subzone(SS2) owns 79 taxa, and Nubo-Sindian subzone(SS3) distributes only 14 endemics. A Wearable Dual-Band Dielectric Patch Antenna for LTE and WLAN

In this paper, a dual-band microstrip patch array antenna for both the Multiple Input Multiple Output (MIMO) 4G Long-Term Evolution (LTE) and the Wireless Local Area Network (WLAN) systems is developed. Design simulation and optimization processes are carried out with the aid of the Advanced Design System (ADS) electromagnetic simula-tor that uses the full–wave Method of Moment (MoM) numerical technique [1]. Rectangular microstrip patch antenna constructed from Multi-Walled Carbon Nanotubes (MWCNT) ink with electrical conductivity of 2.2 × 104 S/m and relative permittivity of 5-j1 is used as the conductor patch [2]. The patch is deposited on Rogers substrate RT-Durid 5880 single substrate with (εr = 2.2) and thickness of 62 mil. U-shape slot is used to provide the dual-band. The pro-posed antenna operates at 3.5 GHz for LTE and 5 GHz for WLAN. The proposed antenna parameters are compared with published result of copper patch [3,4] for the same application. Great enhancement in antenna size, matching, and frequency band width are achieved for the proposed antenna at the expense of gain and antenna efficiency compared with copper one. The proposed MWCNT dielectric antenna size is 18.4 mm × 27 mm compared with size of 27 mm × 37 mm for copper one respectively. The gain is better than 7dBi with antenna efficiency of 87.29% at 3.5GHz for cop-per patch antenna and is better than 1.692 dBi with antenna efficiency of 27.05% at 3.5GHz for MWCNT respectively.

Over the past few decades,the shortage in the kidney donor pool as compared to the increasing number of candidates on the kidney transplant waitlist led to loosening of kidney donors’acceptance *** and obesity represent risk factors for chronic kidney disease,both in native kidneys and those in kidney transplant *** great progress has been made in kidney transplantation from living donors to benefit the recipient survival and quality of life,progress has been slow to fully risk-characterize the *** review critically reassesses the current state of understanding regarding the risk of endstage kidney disease in those donors with obesity,hypertension or *** risk assessment tools need to be developed urgently to fully understand the risk glomerular filtration rate compensation failure in the remaining kidney of the donors.
