Objective:Early treatment of acute ischemia of the heart by performing immediate percutaneous coronary intervention(PCI)to restore blood fl ow in patients with the clinical presentation of an acute coronary syndrome and more specifi cally with ST-elevation myocardial infarction(STEMI)can save *** study aims to identify the mean time(door to balloon time and fi rst contact to balloon time)to primary PCI for STEMI patients and to assess the percentage of primary PCI and its success rate in ***:A registry study of patients presenting to cardiac centers in Egypt was designed,where patients’basic characteristics,the treatment strategy,and the door to balloon time and the fi rst contact to balloon time were ***:One thousand six hundred fi fty STEMI patients with a mean age of 57 years were included in the *** transfer for primary PCI was the most used treatment strategy,representing 74.6%of all treatment strategies *** door to balloon time and the fi rst contact to balloon time were 50 and 60 minutes,respectively,with a primary PCI success rate of 65.1%.Conclusion:The registry study results showed a marked improvement by implementation of the best treatment strategy with respect to the time factor to achieve a better outcome for STEMI patients in Egypt.

GIS-Mapping of Soil Available Plant Nutrients (Potentiality, Gradient, Anisotropy)

The research was elaborated in Kafr El-Dawar area (Egypt northern region) to study the availability of the soil plant nutrients. The research introduced three parameters to comprehensively and carefully describe the availability of the soil plant nutrients: potentiality, gradient and anisotropy. Potentiality defines the categories of soil ability to supply plant nutrients;meanwhile gradient expresses the increasing rate of the availability of the soil plant nutrients. The gradient anisotropy refers to the directions or orientation of the increasing rate of the availability of the soil plant nutrients. The introduced parameters enabled to spatially study the availability of the soil plant nutrients. Analytical data, of soil available phosphorus (P), indicated that P ranged from 0.2 ppm to 11.4 ppm to locate all studied soil samples into the low class of the soil nutritional P ability. This was not the case of available potassium (K), where the soil samples were distributed into three available K soil categories: medium, high, and very high. GIS map of soil P nutritional potentiality for plant (potato), displayed the soil studied area in one category, as low P soil nutritional potentiality to coincide with the analytical data classification. Contrary, the K map classified the soil studied area into three categories of soil K nutritional potentiality: medium, high and excessive. This obviously referred that the individual determination of soil K nutritional potentiality is misleading for interpretation of soil tests because it does care of the spatial distribution of soil available K. Nearly, all soil samples had high available micronutrients that they were located in the high category in both classification of analytical data and GIS maps. In this paper, we have presented the numerical investigation of the geometric phase and field entropy squeezing for a two-level system interacting with coherent field under decoherence effect during the time evolution. The effects of the initial state setting and atomic dissipation damping parameter on the evolution of the geometric phase and entropy squeezing have been examined. We have reported some new results related to the periodicity and regularity of geometric phase and entropy squeezing.

Controllable synthesis of Na P zeolite and its application in calcium adsorption

NaP zeolite particles were prepared through a template-free hydrothermal condition at 100oC. Parameters like Na2O/Si O2 and Si O2/Al2O3 molar ratios, gel aging time and aging temperature were investigated for regulating the crystallization of the final product. The samples at different synthesis stages were characterized with N2 adsorption at-196oC, field emission scanning electron microscopy(FE-SEM), temperature programmed desorption of ammonia adsorption, X-ray diffraction, thermogravimetric and differential scanning calorimetry analysis and Fourier transform infrared spectroscopy. The experimental results evidenced that a high ordering crystallinity of Na P zeolite was obtained at Na2O/Si O2 and Si O2/Al2O3 molar ratios of 1.10 and 1.40, respectively, and a temperature of 100°C for 4 days of aging. FE-SEM image exhibited polycrystalline aggregates of Na P zeolite with crystallite sizes ranging up to 5.04 μm with cactus/cabbage like structures. The quantitative analysis of the total number of acid sites was found to increase as Na2 O increases. The Na P zeolite sample with a Na2O/Si O2 molar ratio of 1.13 exhibited a large fraction of the acid sites on its external surface, contrary to that synthesized with a relevant ratio of 1.10. The optimized Na P zeolite could be successfully bestowed with excellent sorption properties of Ca(II) in aqueous solution. The kinetic parameters for Ca(II) adsorption were thoroughly investigated.

The Cost Functional and Its Gradient in Optimal Boundary Control Problem for Parabolic Systems

The problems of optimal control (OCPs) related to PDEs are a very active area of research. These problems deal with the processes of mechanical engineering, heat aeronautics, physics, hydro and gas dynamics, the physics of plasma and other real life problems. In this paper, we deal with a class of the constrained OCP for parabolic systems. It is converted to new unconstrained OCP by adding a penalty function to the cost functional. The existence solution of the considering system of parabolic optimal control problem (POCP) is introduced. In this way, the uniqueness theorem for the solving POCP is introduced. Therefore, a theorem for the sufficient differentiability conditions has been proved.

Major Hepatic Resection in Hepatic Hydatidosis

Background: Ecchinococcal disease is still a serious problem in certain parts of the world, with liver as the most affected organ. Surgery remains the mainstay of treatment of hydatidosis, but the optimal surgical procedure remains unsettled. Objective: Safety and efficacy of major hepatic resection in multiple and giant hepatic hydatidosis. Patients & Methods: 63 patients had hepatic hydatidosis associated with spleen, lung, and suprarenal hydatidosis were managed by major hepatic resection between April 2005 to April 2011. 43 (68%) males and 20 (32%) females, age range 8 - 56 years. Cysts were found in the liver only in 51 (81%) patients, liver with spleen in 5 (8%), liver, spleen with lung in 2 (3.1%), liver and lung in 4 (6.3%), liver and suprarenal in one patient (1.6%). Results: Multiple cysts were found in 38 (60%) and solitary cyst in 25 (40%) with Cysts diameter ≤5 cmin 22 (35%), 5 - 10 cm in 16 (25%), and 10 - 38 in 25 (40%). Right hepatectomy in 24 (38%), right trisectionectomy in 2 (3.2%), right hepatectomy with Segment III in 4 (6.3%), right hepatectomy with Segment I in 2 (3.2%), left hepatectomy in 12 (19%), left lateral sectionectomy in 6 (9.5%), left hepatectomy with Segment VI in 3 (4.8%), left lateral sectionectomy with right posterior sectionectomy in 2 (3.2%), right hepatectomy with splenectomy in 7 (11%) and right hepatectomy with right suprarenal in one (1.6%) patient were performed. Hospital stay was 4.2 (3 - 13 days) There was one mortality and 12 (19%) morbidities. No recurrence on follow up period (8 - 60 months) was observed. Conclusion: Radical procedure is safe and effective option for hepatic hydatidosis and should be performed when the entire lobe is diffusely involved by huge or multiple hydatidosis with little healthy liver tissue.

Association between IL-37 and VEGF Gene Expression in Psoriasis Pathogenesis in Egyptian Population

Background: Psoriasis is considered a common skin disease, marked by the production and elevation of inflammatory plaques that regularly shed scales resulting from extensive skin epithelial cell proliferation. T lymphocytes, neutrophils, and other leukocytes enter the irritated skin, resulting in epidermal keratinocyte hyperplasia, vascular hyperplasia, ectasia, and T lymphocyte, neutrophil other forms of leukocyte infiltration. Aim of the Work: the study aimed to investigate the role of IL-37 and VEGF gene expression in the pathogenesis of psoriasis in Egyptian patients. Methodology: Polymerase chain reaction techniques (PCR) were applied to detect VEGF in the skin homogenates of psoriasis patients. In addition, the ELIZA technique was applied to investigate IL-37 in skin homogenates. One hundred cases have been divided into two groups: 50 healthy volunteers as the group I healthy control;50 psoriasis patients as group II. PCR real-time technology was assessed through extracted DNA samples for VEGF amplification in skin homogenate. Result: Our results revealed that psoriasis patients significantly had a substantial reduction in IL-37 compared to the control group (p Conclusion: There is a significant inverse association between IL-37 and VEGF in psoriasis patients. Study findings revealed that IL-37 gene expression decreases while VEGF gene expression increases in psoriatic individuals. Such a measurement may be beneficial in determining the severity of the condition, as well as taking into consideration of the disease’s diagnosis.

Role of Vascular Endothelial Growth Factor-C during Stem Cell Therapy Using Autologous Bone Marrow Mononuclear Cells in Patients with Lower Limb Lymphedema

Introduction: Vascular endothelial growth factor-C (VEGF-C) is the primary lymphangiogenic factor that stimulates lymphangiogenesis by signaling via specific receptor, vascular endothelial growth factor receptor 3 (VEGFR3). This study was conducted to evaluate the change in the level of VEGF-C before and after autologous bone marrow mononuclear cell transplantation for treatment of Lower limb lymphedema. Patient and methods: Forty patients with lower limb lymphedema were divided into two groups. Group I included 20 patients with chronic lower limb lymphedema who underwent autologous bone marrow mononuclear cell transplantation. Group II included 20 patients with chronic lower limb lymphedema who were exposed only to compression therapy as a control group. VEGF-C level in the diseased limbs was measured in both groups at the beginning of the study then 3 and 6 months respectively. Results: Group I included 20 patients, 8 patients were male (40%) and 12 patients were females (60%) with mean age 29.5 ± 12.15 while group II included 20, 10 patients were male (50%) and 10 patients were females (50%) with mean age 39.5 ± 11.5. In group I, the specimens were taken at 3 and 6 months after transplantation showed a marked decrease in the VEGF-C level with statistically significant p value, 0.02 and 0.001 respectively. In group II the level of VEGF-C after compression therapy alone at 3 and 6 months interval showed fluctuation with statistically non-significant p value, 0.64 and 0.55 respectively. Conclusion: VEGF-C is essential for regulation of lymphangiogenesis. The level of VEGF-C was found elevated in patients with lymphedema and decrease after autologous mononuclear bone marrow cells, however these results were statically non-significant.

Renal transplantation remains the most effective treatment of End Stage Renal Failure (ESRF). In this cross sectional study we explore the reasons behind refusal of renal transplantation among adults’ Sudanese haemodialysis patients. The subjects of the study are ESRF adults’ patients on regular haemodialysis treatment in 15 haemdoialysis centres in Khartoum/Sudan. All patients who are on regular haemodialysis were interviewed by questionnaire to explore the reasons of refusal of renal transplantation. A total of 1583 ESRF adults’ patients on regular haemodialysis have been participated in the study, 381 (24.1%) patients refused kidney transplantation. The mean age of patients refusing kidney transplantation was (58.5 + 15.1 years);77.4% of them were ≥50 years old, 59.2% were males and 88.1% were unemployed, patients older than 50 years old and unemployed are tend to refuse renal transplantation

Bullying Among School Students in the UAE Society

The purpose of this study is to research the bullying phenomenon among school students in the UAE society. This is done through showing the extent of prevalence of bullying, the rate of recurrence of bullying incidences, the most widespread forms of bullying among school children in the Emirati society, and finally, the variation with regards to the prevalence and forms of bullying as related to the student’s gender. Therefore, this study aims to probe the prevalence of this phenomenon in schools, and the frequency of bullying cases as well as its forms. For this purpose, a questionnaire was designed and conducted on a sample size of 1,309 students of both genders. The data were later analyzed using descriptive statistical and analytical metrics that are appropriate for the variables’ measurement level, and which achieve the objectives of the study. The study found that a third of the students (33.3%) were involved in bullying incidents. Furthermore, it was found that 14.2% were the party causing the bullying incident, while 19.1% were the party upon which bullying was inflicted. The study also revealed that within school premises the places where bullying was most likely to occur are corridors, and the places which students felt were the least safe are the closed spaces. As for the forms of bullying students are subjected to, offensive name calling or insults came in first place, followed by cyber/online bullying. The young age and smaller size of a student were among the most important motivators for students to bully him/her. It was also found that 32.8% of students who are exposed to bullying respond in a similar manner. The study showed that most of the bullied students (78.4%) know the person doing the bullying, the females being more cognizant of the perpetrator bullying them. Moreover, 40.7% of the students believe that the teachers and other employees are aware of the bullying taking place, female students to a greater extent than males in this regard. In t
