Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy

AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver ***:We studied retrospectively all patients who underwent PD in our centre between January 2002and December *** A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic *** cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were *** primary outcome measure was hospital mortality *** outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival ***:Only 67/442 patients(15.2%)had cirrhotic *** blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic *** stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group *** hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with ***:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver *** is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension.

Once a column in building is removed due to gas explosion,vehicle impact,terrorist attack,earthquake or any natural disaster,the loading supported by removed column transfers to neighboring structural *** these elements are unable to resist the supplementary loading,they continue to fail,which leads to progressive collapse of *** this paper,an efficient strategy to model and simulate the progressive collapse of multi-story reinforced concrete structure under sudden column removal is *** strategy is subdivided into several connected steps including failure mechanism creation,MBS dynamic analysis and dynamic contact simulation,the latter is solved by using conserving/decaying scheme to handle the stiff nonlinear dynamic *** effect of gravity loads,structure-ground contact,and structure-structure contact are accounted for as *** main novelty in this study consists in the introduction of failure function,and the proper manner to control the mechanism creation of a frame until its total ***,this contribution pertains to a very thorough investigation of progressive collapse of the structure under sudden column *** proposed methodology is applied to a six-story frame,and many different progressive collapse scenarios are *** results ilustrate the efficiency of the proposed strategy.

Objective:To study the antioxidant and anti-inflammatory activity of Butia capitata(***)leaf extracts along with phytochemical analysis of the proposed bioactive ***:Different successive extracts of *** *** were prepared with selective organic solvents and screened for their anti-inflammatory activities in tested animals and invitro antioxidant *** extensive phytochemical investigation of the bioactive extracts through paper chromatography,thin layer chromatography,column chromatography,gasliquid chromatography(GLC),high pressure liquid chromatography and spectral ***,ultraviolet,hydrogen and carbon nuclear magnetic resonance,electron ionization-mass spectrometry,heteronuclear multiple bond correlation and heteronuclear multiple quantum correlation were carried out Results:Results showed that different extracts possess promising antioxidant effect and significant anti-inflammatory activity with variable *** results of the phytochemical investigation of the bioactive extracts revealed the presence of volatile substances,lipoidal matter,α-tocopherol,free sugars,polysaccharides and flavonoidal ***:*** leaf extracts were shown to possess variable antioxidant effect,the most promising was methanol extract Both polar and non polar extracts were proved to have anti-inflammatory activity,the non polar extract was superior in this respect The bioactivity of the extracts was ascribed to the presence of flavonoids,sterols andα-tocopherol.

Living donor liver transplantation for high model for endstage liver disease score:What have we learned?

AIM: To assess the impact of model for end-stage liver disease(MelD) score on patient survival and morbidity post living donor liver transplantation(LDLT). METHODS: A retrospective study was performed on 80 adult patients who had LDLT from 2011-2013. Nine patients were excluded and 71 patients were divided into two groups; Group 1 included 38 patients with a MelD score elD score > 20. Comparison between both groups was done regarding operative time, intra-operative blood requirement, intensive care unit(ICU) and hospital stay, infection, and patient ***: eleven patients died(15.5%); 3/38(7.9%)patients in Group 1 and 8/33(24.2%) in Group 2 with significant difference(P = 0.02). Mean operative time, duration of hospital stay, and ICU stay were similar in both groups. Mean volume of blood transfusion and cell saver re-transfusion were 8 ± 4 units and 1668 ± 202 m L, respectively, in Group 1 in comparison to 10 ± 6 units and 1910 ± 679 m L, respectively, in Group 2 with no significant difference(P = 0.09 and 0.167, respectively). The rates of infection and systemic complications(renal, respiratory, cardiovascular and neurological complications) were similar in both groups. CONCLUSION: A MelD score > 20 may predict mortality after LDLT.

The Nutritive Value of Leaves and Fruits of Three Grewia Species under Semi-Arid Environment

This study was carried out to assess the leaves and fruits of Grewia mollis, Grewia tenax and Grewia villosa for their potentials to produce high nutritive value fodder under semi-arid conditions. The experiment was conducted at the farm of the College of Natural Resources and Environmental Studies, University of Juba-Khartoum-Sudan. Randomized Complete Block design with three replications was used. Proximate analysis was carried to estimate nutritive value, and mineral contents of leaves, seeds and fruit pulp of the three species. Results indicated significant difference （P ≤ 0.05） in nutritive value of the leaves among the three species; dry matter, ash, and acid detergent factor （ADF） kg/ha, while no significant difference was observed among the species in crude protein （CP） and nutrient detergent factor （NDF） kg/ha. Mineral contents of the leaves of the three species showed variations among the species and seasons. Seeds and fruit pulp were found to have considerable levels of nutrients and mineral contents. The nutritive value of the three species was higher in the rainy season compared to the dry season. The results of this study indicate that the three species can be introduced as a source of fodder in animal production farms and silvopastural systems.

Different Growth Characteristics of Grewia mollis, Grewia tenax and Grewia villosa Under Nursery and Field Conditions

This study was carried out to assess the growth characteristics of Grewia moll＆, Grewia tenax and Grewia villosa under the nursery and field conditions. Two experiments were conducted at the farm of the College of Natural Resources and Environmental Studies, University of Juba, Khartoum, Sudan. Randomized complete block design with three replications was used. Morphological and physiological factors were measured. Seedlings height, number of leaves, number of branches and sub-branches were different （P 〈 0.05） among the three species at the nursery stage and under field conditions. Collar diameter showed significant difference among the species under field conditions. Physiological factors exhibited more significant variations in the field than at the nursery stage. Variations in growth characteristics were attributed to genetics differences and different growth habit, while variations in physiological factors （photosynthesis and transpiration rate） were attributed to differences in leaf structure, size and number of stomatal pores.

Implications of Financial and Non-financial Measures on the Refinement of the Assessments of Audit Risk： An Empirical Investigation

Identifying and assessing audit risk is a key part of the audit process. Prior research documented that auditors primarily look at financial data, information, and measures when assessing the audit risk for an audit engagement. However, professional standard setters, regulators, and academic researchers have discussed the potential for non-financial data, information, and measures to provide a powerful and independent benchmark for evaluating the validity of the numbers of financial statements of an audit client. A field study was conducted in Egypt during the years 2013 and 2014 to explore how auditors perceive and assess audit risk for an audit engagement in the period following the Egyptian revolution of January 25, 2011. The results of the field study indicated that auditors appear not to give sufficient attention to non-financial data, information, and measures when assessing the audit risk during an audit engagement. Auditors seem to rely on financial data, information, and measures when assessing the audit risk of an audit engagement. Furthermore, auditors do not seem to consider the inconsistencies between financial and non-financial data, information, and measures of an audit client as an indicator of the existence of fraud or material misstatements in the financial statements of an audit client.

The olive species(Olea europaea L.)is an ancient traditional crop grown under rainfed conditions in the Mediterranean *** response to the growing national and international demand for olive oil,the olive cultivars are introduced into highly arid new bioclimatic ***,the morpho-physiology and phytochemistry of olive trees are potentially changing among cultivar types and geographical *** the present work,we have undertaken an assessment on the impacts of geographical location and cultivar types on the leaf morpho-physiology and phytochemistry of olive ***,leaves of the two most cultivated olive tree varieties,Chemlal and Sigoise,were collected from three geographical regions(Setif,Batna,and eloued)with increasing aridity in *** samples from the geographical regions were analyzed using the standard physiological experiment,colorimetric method,and a chromatography *** of both cultivars exhibited a significant variance in terms of the leaf shape index but not for the leaf tissue density,specific leaf weight,and specific leaf *** pigment contents were affected by both cultivar type and geographical location,with the lowest pigment content recorded in the Sigoise cultivar from the Setif *** with the Setif and Batna regions,dried leaves of both cultivars from the eloued region showed the higher levels of the total polyphenol,total flavonoid,and total tannin,as well as a better antioxidant *** chromatography-mass spectrometry analysis of all leaf extracts identified the following phenolic acids as major compounds:oleuropein,naringin,apigenin-7-O-glucoside,kaempferol,quercetin,quercitrin,luteolin-7-O-naringenin,and quinic *** contents were found for p-Coumaric acid,trans-Ferulic acid,hyperoside,rutin,apigenin,caffeic acid,protocatechuic acid,o-Coumaric acid,and gallic ***,epicatechin and catechin+were not found in the leaf extracts of the Sigoise *** leaf organ
