Tetrakis hydroxymethyl phosphonium sulfate(THPS) was enhanced by a 14-mer Peptide A, with its core12-mer sequence mimicking part of Equinatoxin II protein, in the mitigation of sulfate reducing Desulfovibrio ferrophilus MIC(microbiologically influenced corrosion) of X80 carbon steel. Results proved that50 ppm(w/w) THPS was sufficient to mitigate the D. ferrophilus biofilm, and its very agressive MIC(19.7mg/cm^(2) in 7 days or 1.31 mm/a), but not 20 ppm THPS. To achieve effective mitigation at a low dosage of THPS, biofilm-dispersing Peptide A was added to 20 ppm THPS in the culture medium. Sessile cell counts were reduced by 2-log and 4-log after enhancement by 10 ppb and 100 ppb Peptide A, respectively. Enhancement efficiency(further reduction in corrosion rate) reached 69% for 10 ppb Peptide A and 83% for100 ppb Peptide A compared with 20 ppm THPS alone treatment, indicating that Peptide A was a good biocide enhancer for THPS.

In order to determine the effect of Natural Pozzolan （NP） content on the mechanical properties and durability characteristics on Engineered Cementitious Composites （ECC） as repair material. This study focused on the evaluation of the most factors influencing compatibility between the repair material and the base concrete including mechanicals properties such as, compressive and flexural strengths, elastic modulus, capillary absorption and drying shrinkage. The experimental results showed that natural pozzolan reduces the compressive strength and the flexuraI strength of ECC at all ages. The elastic modulus of ECC was remarkably lower than that of normal-strength concrete. This lower ~oung＇s modulus is desirable for repair concrete, because it prevents the stresses induced by restrained shrinkage. In addition, the incorporation of high-volume natural pozzolan decreases significantly the coefficient of capillary absorption at long term and increases the drying shrinkage. Generally, based on the results obtained in the present experimental investigation, ECC can be used effectively as an overlay material over existing parent concrete.

Efficacy of Intravenous versus Intrathecal Meperidine on Post-Spinal Shivering after Knee Arthroscopy: A Randomized Controlled Study

Background and Aims: Hypothermia is considered a common problem with anesthesia. Spinal anesthesia, affects the process of temperature regulation. The aim of this study was to compare the prophylactic effect of intravenous (IV) meperidine with intrathecal (IT) meperidine on the prevention of shivering during spinal anesthesia in patients underwent knee arthroscopy. Aim of the Study: The aim of this work is to study the effect of meperidine either intravenous (IV) vs intrathecal (IT) on controlling postoperative shriving after knee arthroscopy. Materials and Methods: This study will be conducted in accordance with the principles of the ethics committee Benha University hospitals. Full written informed consent will be obtained from all patients before inclusion in the study, and a prospective randomized double-blinded study was conducted in adult patients undergoing knee arthroscopy under spinal anesthesia. Cases are divided into three equal groups (20 patients in each). Group IT: will receive spinal anesthesia (12.5 mg heavy bupivacaine) with intrathecal meperidine (25 mg). Group IV: will receive spinal anesthesia (12.5 mg heavy bupivacaine) with intravenous meperidine (25 mg). Group C: will receive spinal anesthesia (12.5 mg heavy bupivacaine) only postoperatively. Incidence and degree of shivering (SS), sedation score (RSS), pain score (VAS), hemodynamics, and PONV will be measured at the following intervals: 0 min (once arrived at PACU), 20 min later, 40 min later, 80 min later. Results: Shivering was observed in 0%, 0%, and 10.5% of patients in Groups IT, IV, and C, respectively. There was a significant difference between Group IT and IV compared to Group C (P Conclusion: We concluded that IT meperidine and IV meperidine comparably can decrease intensity and incidence of shivering compared to control group as well as decrease the requirement for additional doses of meperidine for shivering control without any hemodynamic side effect. Introduction: Study compare spinal anesthesia using hyperbaric bupivacine (0.5% - 0.75%) between fixed dose and height adjusted dose during elective cesarean section. Methods: Hundred parturients who had given their consent and were scheduled for elective cesarean section under spinal anesthesia, were divided into four groups—first group received 0.5% hyperbaric bupivacine fixed dose, the second group received 0.5% hyperbaric bupivacine in a dose adjusted to the height, the third received 0.75% hyperbaric bupivacine fixed dose, the fourth group received 0.75% hyperbaric bupivacine in a dose adjusted to height. The anesthesia onset time, haemodynamic changes, side effects and fetal outcome observed. Results: Spinal block provide excellent surgical anesthesia in all patients. Anesthesia onset time is longer in adjusted than fixed groups, in 0.5% (5 ± 0.816 vs. 3.84 ± 0.746) (P Conclusion: Adjusting dose of hyperbaric bupivacine (0.5% - 0.75%) to patient’s height, decreases the dose of bupivacine in use, it also provide adequate anesthesia for elective cesarean section with decrease the use of vasopressors, the incidence and severity of maternal hypotention also markedly reduced.

AIM：To evaluate the efficacy of femtosecond laser assisted MyoRing intrastromal corneal implant and deep anterior lamellar keratoplasty （DALK） for management of moderate to advanced keratoconus regarding the degree of changes in visual acuity, refraction, corneal asphericity and ***：A prospective non comparative interventional case study was conducted in Ophthalmology Department,Ain Shams University Hospital in the period from January 2015 to February 2017. The study included 30 eyes of moderate to advanced keratoconus. MyoRing was implanted in one eye （Group Ⅰ） and DALK operation was performed in the contralateral eye of the same patient （Group Ⅱ）.Preoperative and 6mo post-operative uncorrected visual acuity（UCVA）, corrected distance visual acuity （CDVA）, spherical equivalent （SE）, corneal and refractive astigmatisms,keratometry and Q-value using topography images were acquired. Quality of vision was assessed in all eyes including total corneal, anterior corneal high order aberrations analysis at 5 mm pupil size and the Strehl ratio of point spread function （PSF） as an objective measure of ***：Mean postoperative UCVA, CDVA, SE, refractive astigmatism,keratometryreadingsandasphericity were statistically improved compared to preoperative parameters in both groups （P〈0.05）. Significant reduction of all corneal aberrations following both techniques（P〈0.05） was achieved except mean trefoil and mean PSF in Group Ⅰ （P〉0.05）. Postoperative corneal aberrations were significantly lower in Group Ⅱ compared to Group Ⅰ.A statistically significant negative correlation was found in Group Ⅰ between the mean change in CDVA （logMAR）and the mean preoperative and mean postoperative total corneal aberrations root mean square （RMS; r=-0.78,P=0.04）. 背景:据CA杂志估计,无论男性还是女性,结直肠癌发病率和死亡率均将位居恶性肿瘤的前三位。在中国,结直肠癌是最常见的恶性肿瘤之一,严重威胁人们的健康。研究表明,腹腔镜技术在结肠癌手术中具有创伤小、恢复快、对机体免疫系统影响小等优势,已经逐渐成为结直肠癌标准的手术治疗方法之一,获得了广泛的应用。腹腔镜左半结肠癌根治术在脾曲游离、淋巴结清扫、消化道重建等方面均存在困难,对手术技术要求较高。关于腹腔镜与开腹左半结肠癌根治术的对照研究鲜有报道。本研究旨在比较腹腔镜手术和开腹手术在左半结肠切除术中的应用,分析两种手术方式患者的临床相关资料,为左半结肠癌患者选择最优手术方式提供一定的理论依据。方法:回顾性分析2018年10月至2020年10月期间扬州大学附属苏北人民医院行左半结肠切除术患者的临床资料。根据术式不同分为腹腔镜组（Laparoscropic,LAP）或开放组（Open Surgery,OS）。比较两组患者一般资料（性别、年龄、BMI、肿瘤大小、肿瘤部位）、术中情况（手术时间、术中出血量、切口长度）、术后病理（TNM分期、病理类型）、术后并发症（肠梗阻、切口感染、肺部感染、切口疝、吻合口瘘、术后1年死亡率、局部复发率、远处转移率）、术后恢复状况（首次排气、进食、下床活动时间,术后拔除引流管时间,炎性指标变化）及患者满意度。结果:在我们的研究中,对167例患者的资料进行分析。分别有89例（53.3%）患者纳入LAP组接受了腹腔镜左半结肠切除术,有78例（46.7%）患者纳入OS组接受了开腹左半结肠切除术。OS组平均年龄62.60±11.378岁,LAP组平均年龄61.97±11.414岁,两组之间年龄差异无统计学意义（P=0.719）。OS组BMI为24.47±3.32 kg/m2,LAP组为24.46±3.74 kg/m2,两组之间差异无统计学意义（P=0.983）。两组性别之间差异也无统计学意义（P=0.923）。肿瘤部位中有91例患者位于降结肠乙状结肠交界部,其中LAP组和OS组分别为48例（52.7%）和41例（47.3%）,52例患者病灶位于降结肠,其中LAP组和OS组分别为30例（57.7%）和22例（42.3%）,19例患者病灶位于结肠脾区,其中LAP组和OS组分别为10例（52.6%）和9例（47.4%）,5例患者肿瘤部位位于横结肠左1/3部,其中LAP组和OS组分别为3例（60.0%）和2例（40.0%）,两组之间在肿瘤位置间差异无统计学意义（P=0.792）。根据TNM分期,两组Ⅲ期患者数量比较多,OS组和LAP组分别为58例和71例,两组之间差异无统计学意义（P=0.705）;同样,对于根据肿瘤深度（T1、T2、T3和T4）,两组之间差异无统计学意义（P=0.666）。两组患者淋巴结阳性数目,差异无统计学意义（P=0.973）。肿瘤病理特征显示,两组中大多数肿瘤为溃疡型,OS组为61.53%,LAP组为69.66%,两组之间差异无统计学意义（P=0.17）。比较两组手术时间,OS组为125.21±36.12 min,LAP组为129.43±33.06 min,两组之间差异无统计学意义（P=0.432）。两组中OS组失血量为109.23±94.22 ml,LAP组为43.17±26.85 ml,差异有统计学意义（P＜0.001）。手术切口上,OS组患者的手术平均切口长度为16.23 ±3.567 cm,LAP组患者切口长度为5.57±1.71 cm,差异具有统计学意义（P＜0.001）。术后肛门首次排气时间,LAP组平均为3.29±1.99天,OS组为5.00±1.84天,差异有统计学意义（P＜0.001）。LAP组患者术后首次进食时间为4.29±1.99天低于OS组患者的5.99±1.84天,两组之间差异有统计学意义（P＜0.001）。LAP组患者术后下床活动时间为2.11±0.76天低于OS组患者的2.56±0.73天,两组之间差异有统计学意义（P＜0.001）。两组炎性指标中OS组术后第1天的白细胞计数为11.70±4.22*10^9/L,LAP组为10.38±3.18*10^9/L,两组之间差异有统计学意义（P=0.023）。术后第3天,OS组的白细胞计数为7.30±2.25*10^9/L,LAP组为6.25±2.05*10^9/L,两组之间差异有统计学差异（P=0.003）。OS组术后第3天NE%检测率为73.78±8.78%,LAP组为67.91±14.25%,两组之间差异有统计学差异（P=0.002）。OS组术后第1 天 CRP 为 78.89±14.11 mg/L,LAP 组为 73.41±14.89 mg/L,两组之间差异有统计学差异（P=0.016）。OS组术后第3天CRP为43.19±17.
