Mechanical Complications of Osteosynthesis: Frequency and Management at the Orthopedics-Traumatology Department of CHU Ignace DEEN

Introduction: Mechanical complications after osteosynthesis are spontaneous and harmful modifications of the joint compromising the consolidation process. The aim of this study is to report on the frequency of these complications and their management in the Orthopaedic-Traumatology Department of CHU Ignace Deen. Patients and Methods: we conducted a retrospective descriptive and analytical cross-sectional study from January 2017 to December 2022. It focused on the records of patients hospitalized and treated in the department for a mechanical complication after osteosynthesis. Results: The frequency of mechanical complications was 1.2%, with an average age of 44.2 years and a sex ratio of 3.2 in favor of men. Non-compliance with postoperative instructions, non-compliance with surgical technique, postoperative infection and early loading were the main contributing factors. Disassembly of the screw-plate was the most common cause in 6 cases (35.5%), with a mean delay of 4.1 months. Revision osteosynthesis was carried out using screw plates in 8 cases (47.1%). Conclusion: Mechanical complications of osteosynthesis are less frequent traumatic conditions in our department. Several factors contribute to their occurrence.

AIM:To investigate the prevalence of visual impairment(VI)and provide an estimation of uncorrected refractive errors in school-aged children,conducted by optometry students as a community ***:The study was *** 3343 participants were included in the *** initial examination involved assessing the uncorrected distance visual acuity(UDVA)and visual acuity(VA)while using a+2.00 D *** inclusion criteria for a subsequent comprehensive cycloplegic eye examination,performed by an optometrist,were as follows:a UDVAl(0.20 logMAR)and/or a VA with+2.00 D≥0.8 decimal(0.96 logMAR).RESUlTS:The sample had a mean age of 10.92±2.13y(range 4 to 17y),and 51.3%of the children were female(n=1715).The majority of the children(89.7%)fell within the age range of 8 to *** the ethnic groups,the highest representation was from the luhya group(60.6%)followed by luo(20.4%).Mean logMAR UDVA choosing the best eye for each student was 0.29±0.17(range 1.70 to 0.22).Out of the total,246 participants(7.4%)had a full eye *** estimated prevalence of myopia(defined as spherical equivalent≤-0.5 D)was found to be 1.45%of the total *** around 0.18%of the total sample had hyperopia value exceeding+1.75 *** astigmatism(cillence was 1.26%of the total *** our cases of VI,76.2%could be attributed to uncorrected refractive *** was detected in 0.66%(22/3343)of the screened *** was no statistically significant correlation observed between age or gender and refractive ***:The primary cause of VI is determined to be uncorrected refractive errors,with myopia being the most prevalent refractive error *** findings underscore the significance of early identification and correction of refractive errors in school-aged children as a means to alleviate the impact of VI.

Health-care waste contains potentially harmful microorganisms and compounds which can infect and affect hospital patients, healthcare workers, the general public and environment. Therefore, management of health care waste requires safe handling, treatment and disposal procedures. While incineration reduces the volume and quantity of waste for final disposal, it leads to the production of fly and bottom ashes laden with toxic incomplete combustion products such as Polycyclic Aromatic Hydrocarbons (PAHs), dioxins, furans and heavy metals. This exposes workers who handle and dispose the bottom ashes, hospital patients, the general public and environment. The goal of this study was to determine the total and individual levels of 16 most prevalent and toxic PAHs. Bottom ash samples were collected from incinerators in five county hospitals in Kenya, namely;Moi-Voi, Narok, Kitale, Makindu and Isiolo. Bottom ash samples were collected over a period of six months from the five hospitals. The samples were then sieved, homogenised and stored at 4°C in amber coloured glass containers. The PAHs were extracted using 30 ml of a hexane-acetone solvent (1:1) mixture by ultrasonication at room temperature (23°C) for 45 minutes. The PAHs were then analyzed with a GC-MS spectrophotometer model (Shimadzu GCMS-QP2010 SE) connected to a computer work station was used for the PAHs analysis. The GC-MS was equipped with an SGE BPX5 GC capillary column (30 m × 0.25 mm × 0.25 μm) for the separation of compounds. Helium was used as the carrier gas at a flow rate of 15.5 ml/minute and 14.5 psi. 1 μl of the sample was injected at 280°C, split mode (10:1). The oven programming was set for a total runtime of 40 minutes, which included: 100°C (2-minute hold);10°C /min rise to 200°C;7°C /min rise to 249°C;3°C /min rise to 300°C (2-minute hold). This study aims to provide electricity to a remote village in the Union of Comoros that has been affected by energy problems for over 40 years. The study uses a 50 kW diesel generator, a 10 kW wind turbine, 1500 kW photovoltaic solar panels, a converter, and storage batteries as the proposed sources. The main objective of this study is to conduct a detailed analysis and optimization of a hybrid diesel and renewable energy system to meet the electricity demand of a remote area village of 800 to 1500 inhabitants located in the north of Ngazidja Island in Comoros. The study uses the Hybrid Optimization Model for Electric Renewable (HOMER) Pro to conduct simulations and optimize the analysis using meteorological data from Comoros. The results show that hybrid combination is more profitable in terms of margin on economic cost with a less expensive investment. With a diesel cost of $1/l, an average wind speed of 5.09 m/s and a solar irradiation value of 6.14 kWh/m2/day, the system works well with a proportion of renewable energy production of 99.44% with an emission quantity of 1311.407 kg/year. 99.2% of the production comes from renewable sources with an estimated energy surplus of 2,125,344 kWh/year with the cost of electricity (COE) estimated at $0.18/kWh, presenting a cost-effective alternative compared to current market rates. These results present better optimization of the used hybrid energy system, satisfying energy demand and reducing the environmental impact.

Chronic subdural hematoma represents 25% of traumatic subdural collections;common in elderly subjects with a clear male predominance. It occurs over the age of 50 in more than 90% of cases and 50% of patients have no history of head trauma even if the latter remains the main risk factor with others such as coagulopathy, anticoagulant treatment, chronic alcoholic poisoning. Its discovery is rarely fortuitous and has an important clinical polymorphism including an intracranial hypertension syndrome (complete or incomplete). We report a clinical case of a left hemispheric subacute subdural hematoma with post-traumatic falcorial involvement in a 70-year-old patient admitted with a picture of impaired consciousness and weakness of the left hemibody. The objective of this work is to draw the attention of neurosurgeons and neurologists to the paradoxical existence of the neurological deficit on the same side as the causal brain lesion.

The shiny cowbird Molothrus bonariensis parasitizes many species with different life-history traits and has a detrimental effect on the survival of the progeny of the *** response,hosts have developed numerous antiparasitic ***,we examined the effects of brood parasitism by shiny cowbird on the clutch and brood sizes(83 nests)in a small host,the black-backed water tyrant Fluvicola *** also studied whether the death of parasite nestlings was related to the care of the foster parents and whether the host had any antiparasitic defense against the shiny *** results indicate that brood parasitism significantly decreased the host hatching and fledging *** majority of nest failures(57%)were caused by brood *** cowbird parasitism occurred in 52%of nests and the intensity of parasitism was 1.23±0.53 eggs per parasitized *** the total host eggs,54%were *** the incubation stage,20 nests(47%)were abandoned because of egg punctures by shiny cowbirds *** two parasitic fledglings were recorded,while the remaining nestlings either died from starvation(n=12)or predation(n=3).Foster parents abandoned parasitic nestlings between 5 and 10 days *** findings demonstrate that the shiny cowbird has very low rates of fledging success when parasitizing black-backed water ***,parasitism had a high reproductive cost in the black-backed water tyrant because a very low proportion(7%)of the parasitized nests(n=43)were successful.
