Management of Acute Idiopthic Intestinal Invaginal in Infants at Conakry University Hospital

Introduction: Acute intestinal intussusception (AII) has been known for a long time but its diagnosis sometimes remains difficult and its etiologies, particularly the idiopathic forms, poorly elucidated. The objective of this study was to describe the clinical and therapeutic aspects of acute intestinal intussusception in order to contribute to the improvement of the management of this condition in the pediatric surgery department of the Donka National Hospital. Materials and Methods: This is a retrospective descriptive study of patients taken in charge of acute idiopathic intestinal intussusception over a period of 7 years (January 2017 to December 31, 2023). Results: We collected 72 patients operated on for IIA, i.e. a hospital frequency of 1.1% and an incidence of 10.3 cases per year. The mean age was 16.58 months with extremes of 3 months and 28 months. The sex ratio was 12.5 in favor of the boy. The triad of ombredanne dominated the clinical picture and paroxysmal abdominal pain with period of calm represented the main reason for consultation (72 cases) i.e. 100%, followed by vomiting (62 cases) and rectal bleeding (50 cases). Ultrasound was performed in 56 patients visualizing the intussusception sausage in 54 patients or 75%. The most common anatomoclinical variety of intussusception was the ileo-caeco-colic form with 35 cases (or 38%) followed by the ileocolic form 24 cases (33%). The surgical procedure performed was a manual reduction in 54 patients or 75% and an anastomosis resection in 18 patients;the appendectomy of principle was done in 62.25%. The postoperative course was simple in 64 patients or 88.88%. The mortality rate was 6.94 %. Conclusion: AII is a common abdominal surgical emergency in infants. Recognition of the Ombredanne triad especially abdominal pain with a period of calm in children and infants in particular is an important element for vital prognosis. The paper examines the role of shared spaces in divided cities in promoting future sustainable communities and spaces described as inclusive to all. It addresses the current challenges that prevent such inclusiveness and suggests future trends of its development to be of benefit to the wider city community. It explains how spaces in divided cities are carved up into perceived ownerships and territorialized areas, which increases tension on the shared space between territories; the control of which can often lead to inter-community disputes. The paper reports that common shared space in-between conflicting communities takes on increased importance since the nature of the conflict places emphasis on communities＇ confidence, politically and socially, while also highlighting the necessity for confidence in inclusion and feeling secure in the public domain. In order to achieve sustainable environments, strategies to promote shared spaces require further focus on the significance of everyday dynamics as essential aspects for future integration and conflict resolution.

Management of Hepatocellular Carcinoma: Updated Review

Hepatocellular carcinoma (HCC) represents one of the most challenging potentially curable tumors with high incidence, prevalence and mortality rates. For proper assessment, prognosis estimation and treatment decisions, at least seven important guidelines and staging systems were designated. Proper treatment needs the interaction of multidisciplinary HCC clinic to choose the most appropriate line of treatment. The different modalities of management include resection (surgery or transplantation), local ablation, chemoembolization, radioembolization and molecular targeted therapies with a wide range of investigational drugs that developed after the FDA approved sorafenib. Downstaging and bridging are two important strategies to manage HCC patients who will undergo liver transplantation to improve their postoperative survival. Finally, survival and prognosis depends on several prognostic factors that are either patient related or tumor related. In our study, we aim to provide an updated comprehensive review of the different aspects of liver cancer management starting from staging systems to the different applied treatment modalities.

Efficient Fast Multiplication Free Integer Transformation for the 1-D DCT of the H.265 Standard

In this paper, efficient one-dimensional (1-D) fast integer transform algorithms of the DCT matrix for the H.265 stan-dard is proposed. Based on the symmetric property of the integer transform matrix and the matrix operations, which denote the row/column permutations and the matrix decompositions, along with using the dyadic symmetry modification on the standard matrix, the efficient fast 1-D integer transform algorithms are developed. Therefore, the computational complexities of the proposed fast integer transform are smaller than those of the direct method. In addition to computational complexity reduction one of the proposed algorithms provides transformation quality improvement, while the other provides more computational complexity reduction while maintaining almost the same transformation quality. With lower complexity and better transformation quality, the first proposed fast algorithm is suitable to accelerate the quality-demanding video coding computations. On the other hand, with the significant lower complexity, the second proposed fast algorithm is suitable to accelerate the video coding computations.

On the Accuracy Assessment of the Latest Releases of GOCE Satellite-Based Geopotential Models with EGM2008 and Terrestrial GPS/Levelling and Gravity Data over Egypt

The Global Geopotential Models (GGMs) of GOCE (Gravity Recovery and steady- state Ocean Circulation Explorer) differ globally as well as regionally in their accuracy and resolution based on the maximum degree and order (d/o) of the fully normalized spherical harmonic (SH) coefficients, which express each GGM. The main idea of this study is to compare the free-air gravity anomalies and quasi geoid heights determined from several recent GOCE-based GGMs with the corresponding ones from the Earth Gravitational Model 2008 (EGM2008) over Egypt on the one hand and with ground-based measurements on the other hand. The results regarding to the comparison of GOCE-based GGMs with terrestrial gravity and GPS/levelling data provide better improvement with respect to EGM2008. The 4th release GOCE-based GGM developed with the use of space-wise solution strategy (SPW_R4) approximates the gravity field well over the Egyptian region. The SPW_R4 model is accordingly suggested as a reference model for recovering the long wavelength (up to SH d/o 200) components of quasi geoid heights when modelling the gravimetric quasi-geoid over the Egypt. Finally, three types of transformation models: Four-, Five- and Seven-parameter transformations have been applied to reduce the data biases and to provide a better fitting of quasi geoid heights obtained from the studied GOCE-based GGMs to those from GPS/levelling data. These models reveal that the standard deviation of vertical datum over Egypt is at the level of about 32 cm.

The aim of this study was to report a case of multi-visceral sarcoidosis at the Mother-Child Hospital Center (CHME) “Le Luxembourg” in Bamako, Mali. Observation: This is a patient aged 62 at the time of consultation, a housewife, residing in the Banconi district, who was referred to us for thoracic-abdominopelvic imaging for chronic liver disease. After several diagnostic errors, the thoracic-abdominopelvic CT scan and liver MRI performed in our center showed, at the thoracoabdominal level, bilateral diffuse pulmonary micronodules and bilateral mediastinal-hilar lymphadenopathy;on the abdominal level, a dysmorphic liver with plaques of steatosis and a granular appearance of the liver parenchyma without periportal fibrosis. These imaging data combined with those from the liver nodule biopsy and biology confirmed the diagnosis of sarcoidosis type II. Treatment with corticosteroids gave satisfactory results and the patient recovered after 18 months. Clinical and CT monitoring 2 years from the start of the disease and 2 months from the end of treatment showed complete resolution of the lesions. Conclusion: The multi-visceral location of sarcoidosis is an entity whose diagnosis remains difficult;diagnostic and interventional imaging has an important place in its management.

The Salvadora persica(***)*** stick,usually known as miswak,is still being em-ployed as an oral hygiene agent for plaque and gingivitis *** study aims to assess the antibacterial,antibiofilm,antioxidant,and phytochemical profile of *** *** ***-sica was purchased from a local market,grinded and extracted with petroleum *** disk dif-fusion,microdilution,and micro-plate assays were performed to evaluate the antibacterial and an-tibiofilm activities of the prepared extract at different concentrations against𝛽-lactam resistance Streptococcus *** radical scavenging 2,2-diphenyl-1-picrylhydrazyl(DPPH)and stable radical cationic chromophore,2,2-azinobis-(3-ethylbenzothiazoline-6-sulfonate)(ABTS)methods were used to determine their antioxidant *** and spectrometric analyses were performed using gas chromatography-mass(GC-MS)*** minimum inhibitory concentration(MIC)of *** extract against𝛽-lactam resistance Streptococcus species ranged from 6.25 to 12.5 mg/*** maximum suppression of biofilm formation by *** extract was observed at MIC with a percentage of 68.66%,against Streptococcus *** *** ex-tract exhibited antioxidant activity with IC 50 of 20μg/mL and 35μg/mL from DPPH and ABTS,*** phytochemical characterization showed the presence of 22 compounds with ma-jor compounds;benzyl isothiocyanate(36.21%)and n-hexadecanoic acid(27.62%).The *** extract exhibited antibacterial activity against𝛽-lactam resistant Streptococcus species,showing a promising natural alternative that could be a treatment option.

Experimental and Numerical Investigation of Desalination Plant Outfalls in Limited Disposal Areas

In this study, experimental and numerical investigations of the dense brine jets are conducted for disposal areas of limited extent. First, a new experimental model representing a section of sea floor with a single port brine outfall is built to study different characteristics of dense jets. Second, a number of numerical experiments have been conducted via Fluent CFD package to compare the numerical results with its corresponding physical observations and measurements. Experimental observations are made for both the terminal height of rise of dense jets discharged vertically from circular outlets into calm and homogeneous environment and for concentration profiles along the dense jet trajectory. Various combinations of port diameters and concentration of effluent salinities are investigated to cover a wide range of conditions. The results from the carried out experiments are compared to different available experimental and field observations from the literature. A new model for the terminal height of rise of dense jets has been derived. The experimental observations of concentrations along the dense jet trajectory are analyzed to quantify the mixing patterns for a given operating condition from the source point to the terminal height of rise. The numerical model has been used to identify the penetration depth and also to get the temporal variation of the brine breakthrough curves at different locations above the disposal port. The numerical model has shown the existence of multipeak breakthrough curves for the farest points from the port (but the closest to the water free surface).
