Tailored pancreatic reconstruction after pancreaticoduodenectomy: a single-center experience of 892 cases

BACKGROUND: Pancreatic reconstruction following pancreaticoduodenectomy(PD) is still debatable even for pancreatic surgeons. Ideally, pancreatic reconstruction after PD should reduce the risk of postoperative pancreatic fistula(POPF) and its severity if developed with preservation of both exocrine and endocrine pancreatic functions. It must be tailored to control the morbidity linked to the type of *** study was to show the best type of pancreatic reconstruction according to the characters of pancreatic stump. METHODS: We studied all patients who underwent PD in our center from January 1993 to December 2015. Patients were categorized into three groups depending on the presence of risk factors of postoperative complications: low-risk group(absent risk factor), moderate-risk group(presence of one risk factor) and high-risk group(presence of two or more risk factors). RESULTS: A total of 892 patients underwent PD for resection of periampullary tumor. BMI >25 kg/m~2, cirrhotic liver, soft pancreas, pancreatic duct diameter elopment of postoperative complications. POPF developed in 128(14.3%) patients. Delayed gastric emptying occurred in 164(18.4%) patients, biliary leakage developed in 65(7.3%) and pancreatitis presented in 20(2.2%). POPF in low-, moderate-and high-risk groups were 26(8.3%), 65(15.7%) and 37(22.7%) patients, respectively. Postoperative morbidity and mortality were significantly lower with pancreaticogastrostomy(PG) in high-risk group, while pancreaticojejunostomy(PJ) decreases incidence of postoperative steatorrhea in all groups. CONCLUSIONS: Selection of proper pancreatic reconstruction according to the risk factors of patients may reduce POPF and postoperative complications and mortality. PG is superior to PJ as regards short-term outcomes in high-risk group but PJ provides better pancreatic function in all groups and therefore, PJ is superior in l

3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GSH) content made other melanoma cells resistant to 3BP. Concurrent use of a GSH depletor with 3BP killed resistant melanoma cells. Survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. Adjuvant Pelvic Radiotherapy *** Chemoradiotherapy for High-Risk StageⅠ-ⅡEndometrial Carcinoma

Objective To explore if the addition of adjuvant chemotherapy with paclitaxel and carboplatin to radiotherapy confers an advantage for overall survival （OAS）, and progression free survival （PFS）; to assess the incidence of relapses over standard pelvic radiotherapy; and to evaluate the related toxicity in high-risk stage I-II endometrial carcinoma Methods Medical records were reviewed to identify high-risk stage I-1I endometrial carcinoma cases treated in the Clinical Oncology and Nuclear Medicine department between 2002 and 2008 with adjuvant radiotherapy alone （arm Ⅰ）（57 patients） or with sequential carboplatin （AUCS-6） and paclitaxel （135-175 mg/m^2） with radiotherapy （arm Ⅱ）（51 patients）. Radiotherapy was performed through the four-field box technique at doses of 45-50 Gy （1.8 Gy/day × 5 days/week）. Results The toxicity was manageable and predominantly hematologic with a grade 3 neutropenia and thrombocytopenia in 9.8% and 6% of the patients in arm Ⅰ and arm Ⅱ, respectively, without febrile neutropenia. All patients experienced hair loss. Chernoradiotherapy arm was associated with a lower incidence rate of relapse （9.8% vs. 22.7%）. After a median follow-up period of 48 months, the 5-year OAS and PFS rates for chemoradiotherapy-treated patients were significantly more favorable than those who did not receive chemotherapy （P=0.02 and 0.03, respectively）. In arm I, the OAS and PFS rates were 73.7% and 66.7% compared with those in arm II, whose rates were 90.2% and 84.3%. Conclusions Adjuvant chemoradiation with paclitaxel and carboplatin improved the survival rates and decreased the recurrence rates in patients with high-risk stage Ⅰ-Ⅱ endometrial carcinoma. Chemotherapy was associated with an acceptable rate of toxicity. However, a prospective study with a larger number of patients is needed to define a standard adjuvant treatment for high-risk stage Ⅰ-Ⅱ endometrial carcinoma.

Effect of melatonin and/or cysteamine on development and vitrification of buffalo embryos

Objective:To assess the effects of melatonin and/or cysteamineonin vitro maturation, culturing and post-warming of buffalo ***: Buffalo oocytes were classified into control, cysteamine (50 μM), melatonin (10 ng/mL) and cysteamine (50 μM) + melatonin (10 ng/mL) treatment groups. In experiment 1, previous treatments were added duringin vitromaturation and culturing of buffalo ***:Cleavage and blastocyst rates were significantly (Pelatonin treated group (70.5±0.9 and 12.8±1.0, respectively). However this effect was potentiated when combined with cysteamine (74.0±1.7 and 14.8±1.7, respectively). In experiment 2, the treatements were added in maturtaion, culturing as well as post-warming culture media. Embryos at 7 d were *** assessement directly after warming showed significant increase (Pelatonin and their combination groups (76.8±2.8, 80.0±2.1 and 83.3±1.7, respectively) than control (65.8±2.4);but the viability after 24 h post-warming was the best in cysteamine + melatonin combination group (61.4±2.1).Conculsions: Enriching maturation, culturing and post-warming media of buffalo oocytes and embryos with melatonin and/or cysteamine have significantly beneficial effects on oocyte developmental competence as well as embryos vitrification procedure outcomes which in turn resulting in enhancement of commercial buffalo embryo production.

Current investigation has shown that human exposure to aflatoxins is not limited to the administration of contaminated cereals,but water is another possible *** study was aimed to design easily applicable method to eliminate aflatoxin B2(AFB2)from contaminated drinking *** has been used for preparation of probiotic-coated polyvinyl alcohol(PVA)and cellulose acetate(CA)*** of these hybrid nanofibers were studied by scanning electron microscopy(SEM)and Fourier-transformed infrared spectroscopy(FT-IR).SEM showed the proper coating of probiotic strains(Kluyveromyces lactis CBS 2359 and Saccharomyces cerevisiae ATCC 9763)on both nanofiber *** areas(1-5 cm^(2))of the probiotic-nanofiber hybrid were used to enhance the removal of 20 ng/ml of aflatoxin B2(AFB2)from prepared AFB2-contaminated water over *** revealed that a 5 cm^(2) area of probiotic-coated PVA nanofibers can eliminate 97.5% of AFB2 as compared to 87.5%,90.5%,93.5%,and 95.5%,for 1 cm2,2 cm^(2),3 cm^(2),and 4 cm^(2),respectively,while probiotic-coated CA nanofibers were slightly less ***,the cytotoxicity of probiotics-CA treated water on cultured human fibroblasts was almost 10 times lower than the cytotoxicity recorded in probiotics-PVA treated ***,results of the current research suggest that probiotics-polymer nanofiber membranes can be used as an extra stage in the water purification system for the treatment of AFB2-contaminated water.

The ability of any steganography system to correctly retrieve the secret message is the primary criterion for measuring its ***,researchers have tried to generate a new natural image driven from only the secret message bits rather than using a cover to embed the secret message within it;this is called the stego *** paper proposes a new secured coverless steganography system using a generative mathematical model based on semi Quick Response(QR)code and maze game image *** system consists of two *** first component contains two processes,encryption process,and hiding *** encryption process encrypts secret message bits in the form of a semi-QR code image whereas the hiding process conceals the pregenerated semi-QR code in the generated maze game *** the other hand,the second component contains two processes,extraction and decryption,which are responsible for extracting the semi-QR code from the maze game image and then retrieving the original secret message from the extracted semi-QR code image,*** results were obtained using the bit error rate(BER)*** results confirmed that the system achieved high hiding capacity,good performance,and a high level of robustness against attackers compared with other coverless steganography methods.

Molecular and Probiotic Characterizations of <i>Lactobacillus reuteri</i>DSM 12246 and Impact of pH on Biomass and Metabolic Profile in Batch-Culture

Lactobacillus reuteri is a powerful probiotic and adjunct functional culture candidate received a lot of scientific attention as it is one of the few endogenous “Lactobacillus” species found in the gastrointestinal tract of vertebrates, including humans, rats, pigs and chicken. The organism has been widely utilized as a probiotic in humans and animals for many years. In the present work L. reuteri DSM 12243;a high reuteri producer strain was molecularly characterized by 16SrRNA and RAPD analyses further, the probiotic properties including acid resistance, bile tolerance, adhesion to epithelial gastric cells, and antibiotic susceptibility were also assessed. Furthermore, the effect of pH on biomass production and metabolic profile of L. reuteri DSM 12246 in batch-culture was studied. The L. reuteri DSM 12246 showed a high similarity with L. reuteri strain I49 KR 36477 (100%) and type strain ATCC 55730 (99% of identity). The strain adhered well to CaCO2 cells and showed to be a highly resistance to acid juice (pH 3.0), with 0.7 log10 cfu/ml reduction in count after 60 min exposition. There is no significant change in the cell count after exposure to bile salts. In batch-cultures, at low pH values both glucose consumption and metabolites were low while the production of lactic acid was noticeable. Maximum biomass was reached at pH 5.5, with growth rate of μ = 0.641/h. The switch in pH values from 3.7 to 6.7 resulted in raising of glucose depletion as well as in the yield of acetate and ethanol. It is concluded that L. reuteri DSM 12246 was deemed as a successful candidate to be used as potential probiotic.

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Soybean lecithin-based extender improves Damascus goat sperm cryopreservationand fertilizing potential following artificial insemination

Objective: To study the effect of adding different concentrations of soybean lecithin in Tris based extender and to compare the results of Tris soybean lecithin extender with two commercial diluents (egg yolk-based: BullXcell and plant-based: OptiXcell) for Damascus goat sperm cryopreservation. Methods: The ejaculates from 4 mature male Damascus goats were obtained by using an artificial vagina. Semen samples were pooled and diluted in Tris extender supplemented with soybean lecithin at different concentrations of 1.5%, 3.0%, 6.0% and 10.0% to get better concentration to be used for further experiments, with a final concentration of 240×106 spermatozoa/mL. Semen samples were packed in straws (0.25 mL), frozen by using an automated system and stored in liquid nitrogen at -196 ℃ for 48 h. After thawing (37 ℃/30 s), the samples were evaluated for sperm quality parameters, including sperm motility, membrane integrity and acrosome integrity. Malondialdehyde concentration was estimated as a marker for lipid peroxidation. Based on the previous investigations, only Tris extender supplemented with 3.0% soybean lecithin (based on its positive results) was used versus BullXcell and OptiXcell for sperm ultrastructure evaluation and artificial insemination by using electron microscope and artificial insemination of the synchronized does. Results: There was no significant difference between Tris-soybean lecithin at 3.0% and BullXcell/OptiXcell diluents in post-thaw sperm parameters and fertility following artificial insemination;meanwhile, the other concentrations of soybean lecithin (1.5%, 6.0% and 10%) showed lower sperm parameters following cryopreservation. Conclusions: Using of Tris-soybean lecithin based extender at the level of 3.0% can be an appropriate alternative to either BullXcell or OptiXcell for Damascus goat sperm cryopreservation.
