Effect of Bilateral Ultrasound-Guided Quadratus Lumborum Block versus Lumbar Epidural Block on Postoperative Analgesia following Major Lower Abdominal Cancer Surgery

Background: Poor postoperative pain control leads to longer postoperative care, longer hospital stay and decreased patient overall satisfaction. Aim: To compare the efficacy and safety of bilateral ultrasound-guided quadratus lumborum block versus lumbar epidural block on the management of postoperative pain following major lower abdominal cancer surgery. Methods: The study was a double-blinded, and randomized study, conducted in South Egypt Cancer Institute, Assiut University, Egypt. It included cancer patients scheduled for major lower abdominal cancer surgery in the period from 2019 to 2020. They were divided into two groups: Group Ι received pre-emptive ultrasound-guided Quadratus Lumborum Block (QLB) with 25 mL of 0.25% bupivacaine on each side of the abdominal wall before induction of General Anesthesia (GA), and Group II received pre-emptive lumbar epidural block with 15 mL of 0.25% bupivacaine before induction of GA. VAS score, and time of the first analgesic request and postoperative total analgesic consumption were evaluated. Results: Sixty patients were included in our study. VAS score at rest was comparable between both studied groups in the first 6 h. At 8 and 10 h, Group II had a significantly higher VAS score at rest (P ely). Meanwhile, at 12 h, patients in Group I had a significantly higher VAS score (P = 0.026). Mean time of the first request for rescue analgesia was significantly prolonged in Group I (13.27 ± 2.38 hrs.) compared to Group II (10.20 ± 1.42 hrs.) Comparative study between three carbonaceous nanoblades and nanodarts for antimicrobial applications

The design of nanostructured materials occupies a privileged position in the development and management of affordable and effective technology in the antibacterial ***,we discuss the antimicrobial properties of three carbonaceous nanoblades and nanodarts materials of graphene oxide(GO),reduced graphene oxide(RGO),and single-wall carbon nanotubes(SWCNTs)that have a mechano-bactericidal effect,and the ability to piercing or slicing bacterial *** demonstrate the significance of size,morphology and composition on the antibacterial activity mechanism,the designed nanomaterials have been *** minimum inhibitory concentration(MIC),standard agar well diffusion,and transmission electron microscopy were utilized to evaluate the antibacterial activity of GO,RGO,and *** on the evidence obtained,the three carbonaceous materials exhibit activity against all microbial strains tested by completely encapsulating bacterial cells and causing morphological disruption by degrading the microbial cell membrane in the order of RGO>GO>*** of the external cell wall structure and outer membrane proteins,the synthesized carbonaceous nanomaterials exhibited higher antibacterial activity against Gram-positive bacterial strains than Gram-negative and fungal *** had the lowest MIC values(0.062,0.125,and 0.25 mg/mL against ***,***,and ***,respectively),as well as minimum fungal concentrations(0.5 mg/mL for both *** and ***).At 12 hr,the cell viability values against tested microbial strains were completely *** lysis and death occurred as a result of severe membrane damage caused by microorganisms perched on RGO *** work gives an insight into the design of effective graphene-based antimicrobial materials for water treatment and remediation.

Standardization of meibomian gland dysfunction in an Egyptian population sample using a non-contact meibography technique

Industry 4.0, or the Fourth Industrial Revolution, is based on digitized the manufacturing process and makes use of all digital tools so its combination of various digital technologies computers, ERP software, IoT, machine learning and AI techniques, Manufacturing Execution Systems (MES), and big data analytics to create a new, fully digitized manufacturing system. The Critical Success Factors (CSFs) of MES adoption are both a quantitative and qualitative measurement. We use the case of ready-made garments to improve each of the three Overall Equipment Efficiency (OEE) factors: Availability, Performance, and Quality. In this study, we adopt real-time management of production activities on the shop floor from order receipt to finished products, then measure the improvement.

The impact of foot reflexology on fatigue and sleep quality in school-aged children undergoing hemodialysis

Transient elastography and diffusion-weighted magnetic resonance imaging for assessment of liver fibrosis in children with chronic hepatitis C

BACKGROUND Chronic hepatitis C(CHC)is a health burden with consequent morbidity and *** biopsy is the gold standard for evaluating fibrosis and assessing disease severity and prognostic purposes *** altern-atives for liver biopsy such as transient elastography(TE)and diffusion-weighted magnetic resonance imaging(DW-MRI)are critical *** To evaluate TE and DW-MRI as noninvasive tools for predicting liver fibrosis in children with *** This prospective cross-sectional study initially recruited 100 children with CHC virus ***-four children completed the full set of investigations including liver stiffness measurement(LSM)using TE and measurement of apparent diffusion coefficient(ADC)of the liver and spleen using *** biopsies were evaluated for fibrosis using Ishak scoring *** and liver and spleen ADC were compared in different fibrosis stages and correlation analysis was performed with histopathological findings and other laboratory *** Most patients had moderate fibrosis(73.5%)while 26.5%had mild *** had severe fibrosis or *** majority(68.8%)had mild activity,while only 7.8%had moderate *** scores had a significant direct correlation with LSM(P=0.008)and were negatively correlated with both liver and spleen ADC but with no statistical significance(P=0.086 and P=0.145,respectively).Similarly,histopatho-logical activity correlated significantly with LSM(P=0.002)but not with liver or spleen ADC(P=0.84 and 0.98 respectively).LSM and liver ADC were able to significantly discriminate F3 from lower fibrosis stages(area under the curve=0.700 and 0.747,respectively)with a better performance of liver *** TE and liver ADC were helpful in predicting significant fibrosis in children with chronic hepatitis C virus infection with a better performance of liver ADC.

Prediction of COVID-19 Transmission in the United States Using Google Search Trends

Accurate forecasting of emerging infectious diseases can guide public health officials in making appropriate decisions related to the allocation of public health *** to the exponential spread of the COVID-19 infection worldwide,several computational models for forecasting the transmission and mortality rates of COVID-19 have been proposed in the *** accelerate scientific and public health insights into the spread and impact of COVID-19,Google released the Google COVID-19 search trends symptoms open-access *** objective is to develop 7 and 14-day-ahead forecasting models of COVID-19 transmission and mortality in the US using the Google search trends for COVID-19 related ***,we propose a stacked long short-term memory(SLSTM)architecture for predicting COVID-19 confirmed and death cases using historical time series data combined with auxiliary time series data from the Google COVID-19 search trends symptoms *** the SLSTM networks trained using historical data only as the base models,our base models for 7 and 14-day-ahead forecasting of COVID cases had the mean absolute percentage error(MAPE)values of 6.6%and 8.8%,*** the other side,our proposed models had improved MAPE values of 3.2%and 5.6%,*** 7 and 14-day-ahead forecasting of COVID-19 deaths,the MAPE values of the base models were 4.8%and 11.4%,while the improved MAPE values of our proposed models were 4.7%and 7.8%,*** found that the Google search trends for“pneumonia,”“shortness of breath,”and“fever”are the most informative search trends for predicting COVID-19 *** also found that the search trends for“hypoxia”and“fever”were the most informative trends for forecasting COVID-19 mortality.
