In Mali, cholesteatoma surgery remains a challenge due to the limited number of otological practitioners, and the low socio-economic level of patients reflecting the difficulties in performing CT or MRI for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatoma. Materials and Method: This was a retrospective and prospective, descriptive study that took place in the eNT and head and neck surgery department of the Gabriel Touré University Hospital over 36 months from November 2020 to October 2023. These were patients admitted to the department for chronic cholestematous otitis media. Result: In total, we collected 34 files of patients admitted for cholesteatoma. This represented 9% of cases compared to all otological surgeries carried out during the same period, i.e. 362 cases. The average age of our patients was 35.31 years, with extremes ranging from 7 years to 80 years, there were 24 men and 10 women, i.e. a M/F ratio of 2.4. The average time to diagnosis was 7 years. The main functional signs were dominated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of meningeal complication and three cases of cerebral empyema were reported. Clinically, a postero-superior and marginal tympanic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-marginal tympanic perforation in 32%. Pure-tone audiometry performed preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a mini box. An ossiculoplasty was performed immediately with fragments of cartilage. The postoperative course found the disappearance of otorrhea (88.23%), the persistence of otorrhea in one case, two cases of facial paralysis, and one case of tinnitus. We did not identify

The feasibility of using N,N,N＇,N＇-tetra（2-ethylhexyl）diglycolamide （TeHDGA） in 75 vol.% n-dodecane-25 vol.% n-octanol as agents for the extraction and separation ofNd（III）, Sm（III）, Dy（llI）, Fe（III）, Ni（II）, and Cs（I） from concentrated chlo- ride solution was investigated. Different extraction behaviors were obtained towards rare earth elements （Ree） studied and Fe（IIl）, Ni（II） and Cs（I）. efficient separation of Nd（III）, Sm（III） and Dy（III） from Fe（III）, Ni（II）, and Cs（I） was achieved by TeHDGA, depending on the HCI, HNO3 or H2SO4 concentration. A systematic investigation was carried out on the detailed extraction prop- erties of Nd（III）, Sm（III）, and Dy（III） with TeHDGA from chloride media. The IR spectra of the extracted species were investi- gated.

The possibility of utilizing thorium as a fuel in a pressurized water reactor(PWR)has been proven from the neutronic perspective in our previously published work without assessing the thermal hydraulic(TH)and solid structure ***,the TH and solid structure performances must be studied to confirm these results and ensure the possibility of using a thorium-based fuel as an excellent accident-tolerant *** TH and solid structure performances of thorium-based fuels were investigated and compared with those of UO_(2).The radial and axial power peaking factors(PPFs)for UO_(2),(^(232)Th,^(235)U)O_(2),and(^(232)Th,^(233)U)O_(2)were examined with a PWR assembly to determine the total PPF of each *** Gd_(2)O_(3)and er_(2)O_(3)were tested as burnable absorbers(BAs)to manage the excess reactivity at the beginning of the fuel cycle(BOC)and reduce the total ***_(2)O_(3)resulted in a more significant reduction to the total PPF and,therefore,a greater reduction to the temperature distribution compared to Gd_(2)O_(3).Given these results,we analyzed the effects of adding er_(2)O_(3)to thorium-based fuels on their TH and solid structure performances.

Protective effects of Balanites aegyptiaca extract, melatonin and ursodeoxycholic acid against hepatotoxicity induced by methotrexate in male rats

Objective: To compare the degree of ameliorative effects of Melatonin(MeL), Ursodeoxycholic acid(UDCA) and Balanites aegyptiaca(BA) against hepatotoxicity induced by MTX for one month. Methods: eighty adult male rats(Sprague Dawely) weighing(190±10g), were randomly divided into eight equal groups: Control, MTX, MeL, BA, UDCA, MTX+MeL, MTX+BA, MTX+UDCA. Liver function biomarker enzymes, liver tissue oxidative stress parameters, together with total antioxidant capacity and tumor necrosis factor(TNF-α) were determined. Histopathological and immunohistochemistry examinations for TNF-α were also done. Results: MTX showed significant increase in alanine transaminase(ALT), aspartate transaminase(AST), alkaline phosphatase(ALP), gamma glutamyl transferase(GGT), total and direct bilirubin, as well as TNF-α levels, oxidized glutathione(GSSG), malodialdehyde(MDA) and nitric oxide(NO). whereas, total protein, albumin, total antioxidant capacity, reduced glutathione(GSH), glutathione peroxidase(GPx), glutathione reductase(GR), glutathione S-transferase(GST), superoxide dismutase(SOD) and catalase(CAT) levels were significantly decreased in MTX treated group. These alterations were improved by MeL and BA treatment, whereas no improvement was noticed in UDCA treatment. Conclusions: BA may be as promising as MeL in the hepatoprotection against MTX toxicity through their antioxidant and radical scavenging activities. In addition, it is not recommended to co-administer UDCA with MTX as it enhanced inflammation and damage to the liver.

Introduction and Significance: Burn injury (BI) is a considerable health issue which is responsible for around 300,000 deaths and affecting about 11 million people every year worldwide. In Saudi Arabia, the prevalence of BIs array from 112 to 518 per 100,000 per year. The appropriate awareness of performing first aid could facilitate to improve the outcomes of burns. Purpose and Objectives: To appraise the community that acknowledges burns, first aid, and associated factors among the community population in Jazan City, Saudi Arabia. The paper aims to identify limitations to encourage additional research and persuade legislators to develop improved burn-injury care recommendations and training programs. Materials and Methods: An observational-based sample survey was conducted among the people who live in Jazan City aging 13 years or more, during April 5 to May 5, 2023. Data collection was done by a validated online self-administrated questionnaire sent randomly to community members in different parts of Jazan City via social media platforms. Collected data were coded and cleaned by an excel program, and finally exported on SPSS 26.0 software. The variables were analyzed using descriptive statistics like frequencies and percentages. Also, the Chi-square test was used to investigate the relation between different variables, with a significance value of P Results: This study included 243 participants (about 62%) among them were mostly male participants (151) having a university degree. The majority of participants 75% did not take any form of BFA training in the past. This study shows that 69.9% of the participants have inadequate awareness, despite 72% having a constructive attitude towards burn first aid. Previous burn-related first aid training was significantly associated with participants’ knowledge of BFA at a p-value less than 0.05. Bone marrow-derived mesenchymal stem cells ameliorate sodium nitrite-induced hypoxic brain injury in a rat model

Sodium nitrite（Na NO2） is an inorganic salt used broadly in chemical industry. Na NO2 is highly reactive with hemoglobin causing hypoxia. Mesenchymal stem cells（MSCs） are capable of differentiating into a variety of tissue specific cells and MSC therapy is a potential method for improving brain functions. This work aims to investigate the possible therapeutic role of bone marrow-derived MSCs against Na NO2 induced hypoxic brain injury. Rats were divided into control group（treated for 3 or 6 weeks）, hypoxic（HP） group（subcutaneous injection of 35 mg/kg Na NO2 for 3 weeks to induce hypoxic brain injury）, HP recovery groups N-2 w R and N-3 w R（treated with the same dose of Na NO2 for 2 and 3 weeks respectively, followed by 4-week or 3-week self-recovery respectively）, and MSCs treated groups N-2 w SC and N-3 w SC（treated with the same dose of Na NO2 for 2 and 3 weeks respectively, followed by one injection of 2 × 106 MSCs via the tail vein in combination with 4 week self-recovery or intravenous injection of Na NO2 for 1 week in combination with 3 week self-recovery）. The levels of neurotransmitters（norepinephrine, dopamine, serotonin）, energy substances（adenosine monophosphate, adenosine diphosphate, adenosine triphosphate）, and oxidative stress markers（malondialdehyde, nitric oxide, 8-hydroxy-2′-deoxyguanosine, glutathione reduced form, and oxidized glutathione） in the frontal cortex and midbrain were measured using high performance liquid chromatography. At the same time, hematoxylin-eosin staining was performed to observe the pathological change of the injured brain tissue. Compared with HP group, pathological change of brain tissue was milder, the levels of malondialdehyde, nitric oxide, oxidized glutathione, 8-hydroxy-2′-deoxyguanosine, norepinephrine, serotonin, glutathione reduced form, and adenosine triphosphate in the frontal cortex and midbrain were significantly decreased, and glutathione reduced form/oxidized glutathione and adenosine monophosphate/adenosine

The garnet muscovite granitic pegmatite of Um Solimate,in southern egypt,represents a promising asset for strategic and economic metals,especially Bi-Ni-Ag-Nb-Ta as well as U and *** ore bodies occur as large masses,pockets and/or veins of very coarse-grained pegmatites,which consist mainly of K-feldspar,quartz and albite with subordinate muscovite,garnet,and *** data revealed that e U-and e Th-contents of the pegmatites reach up to 39 ppm and 82 ppm,*** studied pegmatites are enriched in primary U and Th minerals(uraninite,coffinite,thorianite and uranothorite)as well as Hf-rich zircon and monazite,which give rise to anomalous radioactive ***-tantalium-bearing minerals(***,microlite and uranopyrochlore),xenotime,barite,galena,fluorite,and apatite are ubiquitous,and,consequently,the studied pegmatites belong tothe Niobium-Yttrium-Fluorine-type(NYF)*** noble metal mineralization includes argentite(Ag_(2)S),native Ni and Bi as well as bismite and *** addition,beryl and tourmaline are observed in pegmatites near the contact with metasediments and ultramafic *** observed compositional variations of Ta/(Ta+Nb)and Mn/(Mn+Fe)ratios in columbite(0.08-0.45 and 0.11-0.57,respectively)and Hf contents in zircon(3.54-6.46 wt%)may reflectan extreme degree of magmatic fractionation leading to formation of the pegmatite orebody.

Accidents during Tanker Accident in Confined Spaces

One of the major hazards when working onboard Tankers is working in confined spaces, improving the procedures in working in such spaces is obvious, but developing the equipments used in rescue operation is rare to happen, that＇s why this paper is focusing on differentiating between the manual ＆ more developed equipments used specially in rescuing the crew in such an adequate time, to save the workers＇ life. The manual way is called ＂MUCKY CRANe＂ which is used for rescue purposes onboard tankers, in any of the confined spaces, should be replaced by excel crane which is air or hydraulic driven machine, to achieve better results. As safety precautions measures taken in such tasks are not enough for the required objective achievement. Such safety procedures have been discussed and critical situations have been pointed out.

Taxpayers Fraudulent Behavior Modeling The Use of Datamining in Fiscal Fraud Detecting Moroccan Case

The fraudulent behavior of taxpayers impacts negatively the resources available to finance public services. It creates distortions of competition and inequality, harming honest taxpayers. Such behavior requires the government intervention to bring order and establish a fiscal justice. This study emphasizes the determination of the interactions linking taxpayers with tax authorities. We try to see how fiscal audit can influence taxpayers’ fraudulent behavior. First of all, we present a theoretical study of a model pre established by other authors. We have released some conditions of this model and we have introduced a new parameter reflecting the efficiency of tax control;we found that the efficiency of a fiscal control have an important effect on these interactions. Basing on the fact that the detection of fraudulent taxpayers is the most difficult step in fiscal control, We established a new approach using DATA MINING process in order to improve fiscal control efficiency. We found results that reflect fairly the conduct of taxpayers that we have tested based on actual statistics. The results are reliable.

Validation of prognostic indices in egyptian Budd-Chiari syndrome patients: A single-center study

AIMTo compare predictive ability of Budd-Chiari syndrome (BCS) prognostic indices (PIs) for one-year survival and Transjugular intrahepatic portosystemic shunt (TIPS) *** retrospective study enrolled 194 egyptian patients with primary BCS who presented to the Budd-Chiari Study Group of Ain Shams University Hospital. Calculation of the available PIs was performed using Child-Pugh and model for end-stage liver disease scores, BCS-specific PIs (Clichy, New Clichy and Rotterdam) for all patients, and BCS-TIPS PI only for patients who underwent TIPS. The overall one-year survival rate and the one-year shunt patency rate for TIPS were *** overall one-year survival rate was 69.6%, and the New Clichy PI revealed the best validity for its prediction at a cut-off value of 3.75, with sensitivity and specificity of 78% and 73.3%, respectively [area under receiver operating characteristic curve (AUC) = 0.806]. The one-year survival rate post-TIPS was 89.7%, and the BCS-TIPS score demonstrated validity for its prediction at a cut-off value of 3.92 (sensitivity and specificity were 71.4% and 64.5%, respectively) (AUC = 0.715). Logistic regression analysis revealed that the New Clichy PI (P = 0.030), high serum total bilirubin (P = 0.047) and low albumin (P ere independent factors for predicting mortality within one year. The one-year shunt patency rate in TIPS was 80.2%, and none of the PIs exhibited significant validity for its *** New Clichy score could independently predict the one-year survival in egyptian BCS patients.
