Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors

Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. CA 19-9 < 37 U/mL, smaller tumor size, and R0 resection were found to be independent factors for long-term survival ≥ 5 years.

Depositional Environment, Petrophysical Evaluation and electrical Properties of Zeit Formation, Northwestern Shore of Gulf of Suez, Egypt

The Zeit sand reservoir is one of the most prolific formations at Northwestern side of the Gulf of Suez. In this research we will try to coordinate between electrical, petrophysical properties, depositional environment and facies discrimination in order to evaluate the hydrocarbon potentiality of studied Zeit Formation. The statistical parameters for potassium（K）, thorium（Th） and Th/U ratio contents have a general increase towards northwestern parts, whereas uranium（U） content has a general increase towards southeastern parts. The sandstone facies is distinguished from the other facies by its thorium content >4 ppm. U has high carbonate content（U ≥ 1 ppm）. Rocks' electrical properties vary greatly depending on a number of factors. electrical measurements were taken at frequencies range of(5 × 10-4Hz–100 kHz) for fully saturated samples（clayey sandstone） with NaCl（20 gm/L）. As salinity, clay content, and frequency increase, consequently does the electrical properties. The continental condition are present in northwestern part（back-sea） which is distinguished by high K percent, high Th, high Th/U ratio, and low U contents. Low K, Th, and Th/U ratio contents, with high U contents, characterize the marine depositional environment that existed around the east and southeastern parts（foresea coincide with the dipping of strata）. Furthermore, the studied Zeit Formation has good petrophysical properties that coincide with marine conditions. The middle and eastern parts（around ISS-94 and CSS-288） is a good reservoir（porosity 36%–39%, shale content <15%, hydrocarbon saturation 71%–92%, and net pay thickness 17–63 feet）.

Developing a biomaterial for wound healing applications is still a ***,dysprosium oxide(Dy_(2)O_(3)),aluminum oxide(Al_(2)O_(3)),and graphene oxide(GO)were embedded in cast films based on polycaprolactone(PCL)to be examined for wound dressing *** techniques were used to characterize the fabricated films including X-ray diffraction(XRD),Fourier transform infrared spectroscopy(FTIR),Raman,and scanning electron microscopy(SEM),besides their biological *** thermogravimetric analysis(TGA)exhibits high thermal stability as the scaffold weight decreases slightly to 98.6%after raising the temperature from room temperature to 280℃.The cell viability was investigated and it is shown that the viable cells grow up to approximately 93%at 75μg/***,the cell attachment shows excellent behavior as the cells attach on most of the surface of the modified PCL which shows high biocompatibility.

Eco-toxicity and health risk assessment of polycyclic aromatic hydrocarbons in surface sediments of Burullus Lake in Egypt

Polycyclic aromatic hydrocarbons(PAHs)are ubiquitous environmental contaminants of growing concern due to their potential ecological and human health *** study presents a comprehensive assessment of PAHs contamination in the surface sediments of Burullus Lake,a vital and second largest delta lake in *** aim was to evaluate the eco-toxicity and potential health risks associated with the presence of these *** seven sediment samples were collected from various drains in the southern part of Burullus *** extraction method was employed to extract PAHs(16PAHs)from the sediment ***,target compounds were located using *** results showed that samples contained PAHs levels ranging from 0.038×10^(-6)to 0.459×10^(-6),which is considered heavily polluted by the European standard for PAHs ***,there was no apparent source of PAHs in the elKhashah drain or the Brinbal Canal,as HPLC found none of the *** most prevalent compound in sediment samples along the study area was *** diagnostic indices in the present study indicated that the hydrocarbons in the region originated from pyrolytic and man-made sources along the drains of Burullus *** principal component analysis(PCA)and diagnostic ratios revealed that coal combustion and pyrolytic sources were responsible for the PAHs contamination in the surface *** non-carcinogenic risk(HI),which is the product of the HQs for the adult and child populations,respectively,was *** values under 1,therefore,demonstrated that they had no carcinogenic effects on human *** and MEQs in the sediments of Burullus Lake do not have a cancer-causing impact on *** the safety of nearby wildlife,aquatic life,and people,all activities that raise petroleum hydrocarbon levels in Burullus Lake must be adequately regulated and *** to the ecological risk assessment,there is little chance that PAHs will be found

Condition monitoring of the insulating system within power transformers has a massive importance according to the electrical *** gas analysis(DGA)is frequently used for this ***,DGA lacks the necessary level of accuracy to identify all equipment faults,particularly in their initial stages of ***,it does not have the capability for real-time monitoring and relies on manual sampling and laboratory testing,causing potential delays in fault ***,the interpretation of DGA data necessitates specialised expertise,which may pose difficulties for smaller entities that have limited access to ***,the contribution of this research is to use infrared spectroscopy measurements as a new effective technique substituting the DGA method for fault diagnosis in insulating *** inception faults that were considered in this study were the electrical fault(discharges of high energy)and the thermal fault(300°Cells were crafted especially for serving the simulation processes inside the laboratory for both types of inception ***,six samples of pure paraffinic mineral oil were taken to be degraded in the *** that,all of them besides another sample that were not subjected to any kind of faults were taken to be examined by Fourier transform infrared(FTIR)spectroscopy to obtain an overview of the oil's behaviour in each fault *** that,the FTIR analysis was initially verified utilising the DGA ***,for further affirmation,the dielectric dissipation factor(DDF)for all samples was *** the final analysis,the verification tests provide experimental evidence about the outperformance of this new optical technique in detecting the transformer's inception faults in addition to proving its potential for being a superior alternative to the well-known traditional diagnostic techniques.

Pyronaridine combined with diminazene aceturate inhibits Babesia in vitro and in vivo

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in ***:Bioinformatic analysis was performed using atom pair *** in vitro combination test was performed against Babesia bovis and Theileria ***,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory ***:Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular *** in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria *** addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post *** combination therapy also normalized the hematological parameters of infected ***:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.

Correlation between Wnt Signalling Inhibitors (Dickkopf-1 & Sclerostin) and the Intimal Medial Thickness in Children on Maintenance Hemodialysis

Background: Wnt signalling inhibitors (Dickkopf-1 and Sclerostin) signalling play a role in vascular development and may contribute to calcification. Aim: To investigate the association between Dickkopf-1 and sclerostin serum concentrations in children undergoing maintenance hemodialysis with intimal medial thickness and peak systolic velocity of the main arteries. Patients and Methods: A study was conducted on 40 children undergoing maintenance hemodialysis and controls of the same age and sex. The study measured the initial medial thickness (IMT) and peak systolic velocity (PSV) of the main vessels (carotid, ulnar, and femoral). Dickkopf-1 and sclerostin serum levels in both groups were assessed, and a routine investigation was performed. Results: The findings indicate that the levels of serum Dickkopf-1 and Sclerostin were significantly higher in the hemodialysis group 2540.65 (2215.4 - 2909.2 pg/ml) and 1.17 (0.85 - 2.03 ng/ml)respectively (P = 0.001), compared to their control group it was 1110.45 (885.45 - 1527.65 pg/ml) and 0.28 (0.25 - 0.32 ng/ml)) respectively P = 0.001. Additionally, there was a significant increase in intima-media thickness (IMT) with a decrease in peak systolic velocity (PSV) in the main blood vessels, including the carotid, ulnar, and femoral arteries. A significant correlation was also observed between Dickkopf-1 and sclerostin levels and IMT of the carotid, ulnar, and femoral arteries. Conclusion: Wnt signalling inhibitors (Dickkopf-1 and Sclerostin) exert effects beyond the bone and significantly contribute to early vascular calcification in pediatric patients undergoing maintenance hemodialysis.

Correlation between DKK-1 Level and Bone Density Status in Children on Maintenance Haemodialysis

Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and Methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DEXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BMD) with a Z-score of under -2.0. Eighteen of these patients had low BMD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BMD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-MBD.

Predictors of long term survival after hepatic resection for hilar cholangiocarcinoma:A retrospective study of 5-year survivors

AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.

Transient elastography and diffusion-weighted magnetic resonance imaging for assessment of liver fibrosis in children with chronic hepatitis C

BACKGROUND Chronic hepatitis C(CHC)is a health burden with consequent morbidity and *** biopsy is the gold standard for evaluating fibrosis and assessing disease severity and prognostic purposes *** altern-atives for liver biopsy such as transient elastography(TE)and diffusion-weighted magnetic resonance imaging(DW-MRI)are critical *** To evaluate TE and DW-MRI as noninvasive tools for predicting liver fibrosis in children with *** This prospective cross-sectional study initially recruited 100 children with CHC virus ***-four children completed the full set of investigations including liver stiffness measurement(LSM)using TE and measurement of apparent diffusion coefficient(ADC)of the liver and spleen using *** biopsies were evaluated for fibrosis using Ishak scoring *** and liver and spleen ADC were compared in different fibrosis stages and correlation analysis was performed with histopathological findings and other laboratory *** Most patients had moderate fibrosis(73.5%)while 26.5%had mild *** had severe fibrosis or *** majority(68.8%)had mild activity,while only 7.8%had moderate *** scores had a significant direct correlation with LSM(P=0.008)and were negatively correlated with both liver and spleen ADC but with no statistical significance(P=0.086 and P=0.145,respectively).Similarly,histopatho-logical activity correlated significantly with LSM(P=0.002)but not with liver or spleen ADC(P=0.84 and 0.98 respectively).LSM and liver ADC were able to significantly discriminate F3 from lower fibrosis stages(area under the curve=0.700 and 0.747,respectively)with a better performance of liver *** TE and liver ADC were helpful in predicting significant fibrosis in children with chronic hepatitis C virus infection with a better performance of liver ADC.
