We present the ca.e of a.34-yea.-old woma. who presented to our depa.tment four yea.s a.ter her second cesa.ea. section with a.history of pa.n a.d blood discha.ge from a.previous Pfa.nenstiel incision, during menstrua.ion, with a. a.sence of va.ina. menstrua. flow. Despite a.prior surgica. repa.r opera.ion, her symptoms persisted. a.pelvic MRI wa. done to confirm the dia.nosis of utero-cuta.eous fistula. a.d surgica. ma.a.ement wa. pursued. This ca.e report a.ms to contribute to the existing litera.ure on utero-cuta.eous fistula.a.d provide insights into the dia.nostic considera.ions a.d ma.a.ement stra.egies for this ra.e complica.ion.

In this editoria.,we respond to a.review a.ticle by Na.i et a.,in which the a.thors discussed ga.troesopha.ea. reflux(GER)following perora. endoscopic myotomy(POEM).POEM is presently the prima.y thera.eutic option for a.ha.a.ia.which is both sa.e a.d effective.a.few a.verse effects were documented a.ter POEM,including *** dia.nostic criteria.were not clea. enough beca.se a.proxima.ely 60%of pa.ients ha.e a.long a.id exposure time,while only 10%experience reflux *** predictors of high disea.e incidence ha.e been identified,including old a.e,fema.e sex,obesity,a.d a.ba.eline lower esopha.ea. sphincter pressure of less tha. 45 *** technica. steps during the procedure,such a. a.lengthy or full-thickness myotomy,ma. further enha.ce the *** pump inhibitors a.e currently the first line of *** voices a.e increa.ingly a.voca.ing for the routine combining of POEM with a. endoscopic fundoplica.ion method,such a. perora. endoscopic fundoplica.ion or tra.sora. incisionless ***,more resea.ch is necessa.y to determine the sa.ety a.d effectiveness of these procedures in the long term for pa.ients who ha.e undergone them.
