Transient elastography and diffusion-weighted magnetic resonance imaging for assessment of liver fibrosis in children with chronic hepatitis C

BACKGROUND Chronic hepatitis C(CHC)is a health burden with consequent morbidity and *** biopsy is the gold standard for evaluating fibrosis and assessing disease severity and prognostic purposes *** altern-atives for liver biopsy such as transient elastography(TE)and diffusion-weighted magnetic resonance imaging(DW-MRI)are critical *** To evaluate TE and DW-MRI as noninvasive tools for predicting liver fibrosis in children with *** This prospective cross-sectional study initially recruited 100 children with CHC virus ***-four children completed the full set of investigations including liver stiffness measurement(LSM)using TE and measurement of apparent diffusion coefficient(ADC)of the liver and spleen using *** biopsies were evaluated for fibrosis using Ishak scoring *** and liver and spleen ADC were compared in different fibrosis stages and correlation analysis was performed with histopathological findings and other laboratory *** Most patients had moderate fibrosis(73.5%)while 26.5%had mild *** had severe fibrosis or *** majority(68.8%)had mild activity,while only 7.8%had moderate *** scores had a significant direct correlation with LSM(P=0.008)and were negatively correlated with both liver and spleen ADC but with no statistical significance(P=0.086 and P=0.145,respectively).Similarly,histopatho-logical activity correlated significantly with LSM(P=0.002)but not with liver or spleen ADC(P=0.84 and 0.98 respectively).LSM and liver ADC were able to significantly discriminate F3 from lower fibrosis stages(area under the curve=0.700 and 0.747,respectively)with a better performance of liver *** TE and liver ADC were helpful in predicting significant fibrosis in children with chronic hepatitis C virus infection with a better performance of liver ADC.

Optimal searching for a Helix target motion

This paper discusses a search problem for a Helix target motion in which any information of the target position is not available to the searchers. There exist three searchers start searching for the target from the origin. The purpose of this paper is to formulate a search model and finds the conditions under which the expected value of the first meeting time between one of the searchers and the target is finite. Also, the existence of the optimal search plan that minimizes the expected value of the first meeting time is shown. Furthermore,this optimal search plan is found. The effectiveness of this method is illustrated by using an example with numerical results.

A mathematical model for preventing HIV virus from proliferating inside CD4 T brownian cell using Gaussian jump nanorobot

We present a statistical distribution of a nanorobot motion inside the *** distribution is like the distribution of A and B particles in continuous time random walk scheme inside the fAuid reactive anomalous transport with stochastic waiting time depending on the Gaussian distribution and a Gaussian jump length which is detailed in Zhang and Li[***,Published Online with doi:10.1007/s10955-018-2185-8,2018].Rather than estimating the length parameter of the jumping distance of the nanorobot,we normalize the Probability Density Function(PDF)and present some reliability properties for this *** addition,we discuss the truncated version of this distribution and its statistical properties,and estimate its length *** use the estimated distance to study the conditions that give a finite expected value of the first meeting time between this nanorobot in the case of nonlinear flow with independent d-dimensional Gaussian jumps and an independent d-dimensional CD4 T Brownian cell in the blood(d-space)to prevent the HIV virus from proliferating within this cell.

Impact of COVID-19 infection during pregnancy on maternal and fetal outcomes

Objectives:The objective of this study is to assess the impact of coronavirus disease 2019(COVID-19)infection during pregnancy on maternal and fetal outcome in relation to gestational ***:This retrospective study was conducted between May 2020 and July ***-five women infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and hospitalized at a quarantine hospital were ***-four women received prenatal care at the hospital until delivery,while 31 dropped out from the study due to COVID-19 recovery and discharge before ***:COVID-19 was found in 83%of the pregnant women after 20 weeks of *** most common symptoms were cough and *** complications included severe pneumonia and *** maternal mortality rate was 9.2%.Premature rupture of membranes and severe hypertension during labor affected nearly 9%of the pregnant women who completed prenatal care until delivery at the *** labor occurred at a rate of 38.2%.There were seven(20.8%)stillbirths and two cases of infant ***:Hospitalized pregnant women who have coronavirus infection,lymphopenia,and a high C-reactive protein level were at a higher risk of developing severe illness,which can lead to maternal and neonatal complications.

Transient elastography compared to liver biopsy and morphometry for predicting fibrosis in pediatric chronic liver disease: Does etiology matter?

AIM: To evaluate transient elastography (TE) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver ***: A total of 90 children [50 with chronic hepatitis C virus (HCV), 20 with autoimmune hepatitis (AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement (LSM) using TE. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction (FAF) using digital image analysis (morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was also performed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiver-operating characteristic curve. Cut-off values with optimal clinical performance (optimal sensitivity and specificity simultaneously) were ***: The majority of HCV group had minimal activity (80%) and no/mild fibrosis (72%). On the other hand, the majority of AIH group had mild to moderate activity (70%) and moderate to severe fibrosis (95%) and all Wilson disease group had mild to moderate activity (100%) and moderate to severe fibrosis (100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter (r = 0.839 vs 0.879, P < 0.0001 for both). LSM discriminated individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH (16.15 ± 7.23 kPa) compared to Wilson disease (8.30 ± 0.84 kPa) and HCV groups (7.43 ± 1.73 kPa). Expanding the Tanh-Function Method for Solving Nonlinear Equations

In this paper, using the tanh-function method, we introduce a new approach to solitary wave solutions for solving nonlinear PDEs. The proposed method is based on adding integration constants to the resulting nonlinear ODEs from the nonlinear PDEs using the wave transformation. Also, we use a transformation related to those integration constants. Some examples are considered to find their exact solutions such as KdV- Burgers class and Fisher, Boussinesq and Klein-Gordon equations. Moreover, we discuss the geometric interpretations of the resulting exact solutions.

Presepsin and Procalcitonin as Potential Biomarkers for Early Diagnosis and Prognosis of Sepsis in Critically Ill Patients

Background: Sepsis has a poor prognosis for critically ill patients, even with intensive management. Early diagnosis of sepsis and detection of patients with worsening prognosis are important for immediate intervention to improve the clinical outcome. Objective: To investigate serum presepsin (PS) and procalcitonin (PCT) as early diagnostic and prognostic biomarkers for sepsis in critically ill patients. Methods: 60 critically ill patients with sepsis were subdivided into three groups of sepsis, severe sepsis and septic shock according to Acute Physiology and Chronic Health Evaluation II (APACHEII) and quick Sequential Organ Failure Assessment (qSOFA) scores. Patients were compared with 20 age and sex matched controls. Serum PS and PCT were measured by enzyme linked immunosorbent assay (ELISA). Results: Serum PS and PCT levels were significantly increased in septic patients than controls, and their increase was positively correlated with progression of sepsis severity till reached the highest levels in septic shock. Receiver operating characteristic (ROC) curve for predicting sepsis revealed that PS has the highest area under curve (AUC) (0.967) with 97.5% sensitivity, 85% specificity and cut-off of >635.5

Physiological Role of Humic Acid and Nicotinamide on Improving Plant Growth, Yield, and Mineral Nutrient of Wheat (<i>Triticum durum</i>) Grown under Newly Reclaimed Sandy Soil

Humic acid is not a fertilizer as it does not directly provide nutrients to plants, but is a compliment to fertilizer. Nicotinamide (Vitamin pp) is a stress-associated compound that can induce and regulate secondary metabolic accumulation and/or the manifestation of defense metabolism in plants. A field experiment was conducted at the experimental station of National Research center at El-Nubaria region, Egypt, to study the role of foliar application of humic acid (as soil conditioner 13 cm/l) and/or priming grains in nicotinamide (vitamin pp 5 mg/l) in saving irrigation water, decreasing fertilizer dose of NPK and at the same time increasing durum wheat (Triticum durum) cultivars’ (Beni Sweif-1 and Beni Sweif-3) productivity grown under newly reclaimed sandy soil exposed to drought for three weeks continuously. The results showed that plant treated with humic acid or nicotinamide increased significantly all morphological criteria (plant height, leaves number, fresh and dry weights of shoots), metabolism (photosynthetic pigment, total soluble sugar, total carbohydrates, total amino acids and proline), mineral contents (N, P, K, Ca and Mg) and yield (grain, straw and biology) of both cultivars amended with either recommended or half recommended doses of NPK. Foliar application of humic acid to plant priming in Vitamin pp induced significant increases in all studied parameters (morphology, chemical and yield) of plants amended with recommended or half recommended doses of NPK as compared with control plants. The maximum yields of grain, straw and biology of both cultivars were obtained in response to triple treatment (humic acid + nicotinamide + recommended dose of NPK) or (humic acid + nicotinamide + half recommended dose of NPK) respectively.

With the prevalence of the Internet of Things(IoT)systems,smart cities comprise complex networks,including sensors,actuators,appliances,and cyber *** complexity and heterogeneity of smart cities have become vulnerable to sophisticated cyber-attacks,especially privacy-related attacks such as inference and data poisoning *** Learning(FL)has been regarded as a hopeful method to enable distributed learning with privacypreserved intelligence in IoT *** though the significance of developing privacy-preserving FL has drawn as a great research interest,the current research only concentrates on FL with independent identically distributed(i.i.d)data and few studies have addressed the non-i.i.d *** is known to be vulnerable to Generative Adversarial Network(GAN)attacks,where an adversary can presume to act as a contributor participating in the training process to acquire the private data of other *** paper proposes an innovative Privacy Protection-based Federated Deep Learning(PP-FDL)framework,which accomplishes data protection against privacy-related GAN attacks,along with high classification rates from non-i.i.d ***-FDL is designed to enable fog nodes to cooperate to train the FDL model in a way that ensures contributors have no access to the data of each other,where class probabilities are protected utilizing a private identifier generated for each *** PP-FDL framework is evaluated for image classification using simple convolutional networks which are trained using MNIST and CIFAR-10 *** empirical results have revealed that PF-DFL can achieve data protection and the framework outperforms the other three state-of-the-art models with 3%–8%as accuracy improvements.
