The Malaysian cataract surgery registry:incidence and risk factors of postoperative infectious endophthalmitis over a 7-year period

AIM：To report the incidence,risk factors and visual outcomes for postoperative endophthalmitis（POE）based on 7-year data from the Malaysian Ministry of Health Cataract Surgery Registry（MOH CSR）.METHODS：Data was collected from the web-based MOH *** consecutive cataract surgery patients from 1st June 2008 to 31st December 2014 were *** criteria were traumatic cataract or previous ocular *** data,ocular co-morbidities,intraoperative details and postoperative visual acuity（VA）at final ophthalmological follow-up were *** eyes were taken for *** with POE were compared against subjects with no POE for risk factor assessment using multiple logistic ***：A total of 163 503 subjects were *** incidence of POE was 0.08%（131/163 503）.Demographic POE risk factors included male gender（OR：2.121,95%CI：1.464-3.015）and renal disease（OR：2.867,95%CI：1.503-5.467）.POE risk increased with secondary causes of cataract（OR：3.562,95%CI：1.740-7.288）,uveitis（OR：11.663,95%CI：4.292-31.693）and diabetic retinopathy（OR：1.720,95%CI：1.078-2.744）.Intraoperative factors reducing POE were shorter surgical time（OR：2.114,95%CI：1.473-3.032）,topical or intracameral anaesthesia（OR：1.823,95%CI：1.278-2.602）,posterior chamber intraocular lens（PCIOL;OR：4.992,95%CI：2.689-9.266）and foldable IOL（OR：2.276,95%CI：1.498-3.457）.POE risk increased with posterior capsule rupture（OR：3.773,95%CI：1.915-7.432）and vitreous loss（OR：3.907,95%CI：1.720-8.873）.Postoperative VA of 6/12 or better was achieved in 15.27%（20/131）subjects with ***：This study concurs with other studies regarding POE risk *** strengthening of MOH CSR data collection process will enable deeper analysis and optimization of POE treatment.

患者,男,37岁,以"右眼肿块迅速增大,无痛感5mo"就诊于我科。行眼科常规检查,发现一发白带蒂肿块位于右眼睑裂区角膜缘颞侧。遂行肿块切除术,手术成功。对切除肿块进行病理组织学检查,显示为分化良好的鳞状细胞癌。通过对此例居住在热带地区的亚裔患者发生的结膜鳞状细胞癌的诊治,认为长期的阳光照射是导致鳞状细胞癌发生的一个主要危险因素,提示我们对于任何眼表面病变都必须进行仔细的评估,进而正确诊断和治疗。

For the treatment of MRSA-infected wounds,the spatiotemporally sequential delivery of antibacterial and anti-inflammatory drugs is a promising *** this study,ROS-responsive HA-PBA/PVA(HPA)hydrogel was prepared by phenylborate ester bond cross-linking between hyaluronic acid-grafted 3-amino phenylboronic acid(HA-PBA)and polyvinyl alcohol(PVA)to achieve spatiotemporally controlled release of two kinds of drug to treat MRSA-infected *** hydrophilic antibiotic moxifloxacin(M)was directly loaded in the *** hydrophobic curcumin(Cur)with anti-inflammatory function was first mixed with Pluronic F127(PF)to form Cur-encapsulated PF micelles(Cur-PF),and then loaded into the HPA *** to the different hydrophilic and hydrophobic nature of moxifloxacin and Cur and their different existing forms in the HPA hydrogel,the final HPA/M&Cur-PF hydrogel can achieve different spatiotemporally sequential delivery of the two *** addition,the swelling,degradation,self-healing,antibacterial,anti-inflammatory,antioxidant property,and biocompatibility of hydrogels were ***,in the MRSA-infected mouse skin wound,the hydrogel-treated group showed faster wound closure,less inflammation and more collagen *** experiments further confirmed that the hydrogel promoted better repair by reducing inflammation(TNF-a)and promoting vascular(VEGF)*** conclusion,this HPA/M&CurPF hydrogel that can spatiotemporally sequential deliver antibacterial and anti-inflammatory drugs showed great potential for the repair of MRSA-infected skin wounds.

Blade Wrap Angle Impact on Centrifugal Pump Performance:Entropy Generation and Fluid-Structure Interaction Analysis

The centrifugal pump is a prevalent power equipment widely used in different engineering patterns,and the impeller blade wrap angle significantly impacts its performance.A numerical investigation was conducted to analyze the influence of the blade wrap angle on flow characteristics and energy distribution of a centrifugal pump evaluated as a low specific speed with a value of *** study investigates six impellermodels that possess varying blade wrap angles(95°,105°,115°,125°,135°,and 145°)that were created while maintaining the same volute and other geometrical *** investigation of energy loss was conducted to evaluate the values of total and entropy generation rates(TEG,EGR).The fluid-structure interaction was considered numerically using the software tools ANSYS Fluent and *** elastic structural dynamic equation was used to estimate the structural response,while the shear stress transport k–ωturbulence model was utilized for the fluid domain *** findings suggest that the blade wrap angle has a significant influence on the efficiency of the *** impeller featuring a blade wrap angle of 145°exhibits higher efficiency,with a notable increase of 3.76%relative to the original *** in the blade wrap angle impact the energy loss,shaft power,and pump *** model with a 145°angle exhibited a maximum equivalent stress of 14.8MPa and a total deformation of 0.084 *** results provide valuable insights into the intricate flow mechanism of the centrifugal pump,particularly when considering various blade wrap angles.

Sleep Posture Classification Using RGB and Thermal Cameras Based on Deep Learning Model

Sleep posture surveillance is crucial for patient comfort,yet current systems face difficulties in providing compre-hensive studies due to the obstruction caused by *** posture assessment remains challenging because of the complex nature of the human body and variations in sleep ***,this study introduces an innovative method utilizing RGB and thermal cameras for comprehensive posture classification,thereby enhancing the analysis of body position and *** method begins by capturing a dataset of sleep postures in the form of videos using RGB and thermal cameras,which depict six commonly adopted postures:supine,left log,right log,prone head,prone left,and prone *** study involves 10 participants under two conditions:with and without ***,the database is normalized into a video *** subsequent step entails training a fine-tuned,pretrained Visual Geometry Group(VGG16)and ResNet50 *** the third phase,the extracted features are utilized for *** fourth step of the proposed approach employs a serial fusion technique based on the normal distribution to merge the vectors derived from both the RGB and thermal ***,the fused vectors are passed to machine learning classifiers for final *** dataset,which includes human sleep postures used in this study’s experiments,achieved a 96.7%accuracy rate using the Quadratic Support Vector Machine(QSVM)without the ***,the Linear SVM,when utilized with a blanket,attained an accuracy of 96%.When normal distribution serial fusion was applied to the blanket features,it resulted in a remarkable average accuracy of 99%.

Failure characterization of fully grouted rock bolts under triaxial testing

Confining stresses serve as a pivotal determinant in shaping the behavior of grouted rock ***,prior investigations have oversimplified the three-dimensional stress state,primarily assuming hydrostatic stress *** these conditions,it is assumed that the intermediate principal stress(σ_(2))equals the minimum principal stress(σ_(3)).This assumption overlooks the potential variations in magnitudes of in situ stress conditions along all three directions near an underground opening where a rock bolt is *** this study,a series of push tests was meticulously conducted under triaxial *** tests involved applying non-uniform confining stresses(σ_(2)≠σ_(3))to cubic specimens,aiming to unveil the previously overlooked influence of intermediate principal stresses on the strength properties of rock *** results show that as the confining stresses increase from zero to higher levels,the pre-failure behavior changes from linear to nonlinear forms,resulting in an increase in initial stiffness from 2.08 kN/mm to 32.51 kN/*** load-displacement curves further illuminate distinct post-failure behavior at elevated levels of confining stresses,characterized by enhanced ***,the peak load capacity ranged from 27.9 kN to 46.5 kN as confining stresses advanced from σ_(2)=σ_(3)=0 to σ_(2)=20 MPa and σ_(3)=10 ***,the outcomes highlight an influence of confining stress on the lateral deformation of *** levels of confinement prompt overall dilation in lateral deformation,while higher confinements maintain a state of ***,diverse failure modes have been identified,intricately tied to the arrangement of confining *** confinements tend to induce a splitting mode of failure,whereas higher loads bring about a shift towards a pure interfacial shear-off and shear-crushed failure mechanism.
