AIM:To compare results of liver stiffness measurements by transient elastography(TE) obtained in our patients population with that used in a recently published ***:This was a single center cross-sectional *** patients with chronic viral hepatitis scheduled for liver biopsy at the outpatient ward of our Infectious Diseases Department were *** was carried out by using FibroScan(Echosens,Paris,France).Liver biopsy was performed on the same day as TE,as day case *** was staged according to the Metavir scoring *** diagnostic performance of TE was assessed by using receiver operating characteristic(ROC) curves and the area under the ROC curve ***:Two hundred and fifty-two patients met the inclusion ***(2%) patients were excluded due to unreliable TE ***,246(171 men and 75 women) patients were *** hundred and ninety-five(79.3%) patients had chronic hepatitis C,41(16.7%) had chronic hepatitis B,and 10(4.0%) were coinfected with human immunodeficiency *** curve analysis identified optimal cut-off value of TE as high as 6.9 kPa forF ≥ 2;7.9 kPa forF ≥ 3;9.6 kPa for F = 4 in all patients(n = 246),and as high as 6.9 kPa for F ≥ 2;7.3 kPa for F ≥ 3;9.3 kPa for F = 4 in patients with hepatitis C(n = 195).Cut-off values of TE obtained by maximizing only the specificity were as high as 6.9 kPa for F ≥ 2;9.6 kPa for F ≥ 3;12.2 kPa for F = 4 in all patients(n = 246),and as high as 7.0 kPa forF ≥ 2;9.3 kPa forF ≥ 3;12.3 kPa forF = 4 in patients with hepatitis C(n = 195).CONCLUSION:The cut-off values of TE obtained in this single center study are comparable to that obtained in a recently published meta-analysis that included up to 40 studies.

Liver transplantation for recurrent hepatic adenoma

Hepatic adenoma (HA) is a rare indication for liver transplantation (LTx). So far 20 cases of LTx for HA are reported in PubMed. In rare cases HA presents as multiple hepatic adenomas or recurrent adenoma after initial liver resection and in such cases LTx is the only potential cure and prevents the risk of bleeding or cancer transformation into hepatocellular carcinoma. We report the case of a 56 years old lady who underwent a left hepatectomy for giant adenoma in 2005 and resection of segment Ⅴ-Ⅵ for recurrence of liver adenoma in 2007. She developed a second recurrence of HA with 3 new lesions in the right liver in 2008. The patient underwent LTx. After 3 years the patient is alive with no evidence of disease. LTx is indicated in patients with HA in which resection is not technically feasible.

Recent advances in the molecular genetics of type 2 diabetes mellitus

Type 2 diabetes mellitus(T2DM) is a complex disease in which both genetic and environmental factors interact in determining impaired β-cell insulin secretion and peripheral insulin resistance. Insulin resistance in muscle, liver and fat is a prominent feature of most patients with T2DM and obesity, resulting in a reduced response of these tissues to insulin. Considerable evidence has been accumulated to indicate that heredity is a major determinant of insulin resistance and T2DM. It is believed that, among individuals destined to develop T2DM, hyperinsulinemia is the mechanism by which the pancreatic β-cell initially compensates for deteriorating peripheral insulin sensitivity, thus ensuring normal glucose tolerance. Most of these people will develop T2DM when β-cells fail to compensate. Despite the progress achieved in this field in recent years, the genetic causes of insulin resistance and T2DM remain *** gene association, linkage and genome-wide association studies have highlighted the role of genetic factors in the development of T2DM. Using these strategies, a large number of variants have been identified in many of these genes, most of which may influence both hepatic and peripheral insulin resistance, adipogenesis and β-cell mass and function. Recently, a new gene has been identified by our research group, the HMGA1 gene, whose loss of function can greatly raise the risk of developing T2DM in humans and mice. Functional genetic variants of the HMGA1 gene have been associated with insulin resistance syndromes among white Europeans, Chinese individuals and Americans of Hispanic ancestry. These findings may represent new ways to improve or even prevent T2DM.

青普扬州瘦西湖文化行馆紧邻风景秀丽的瘦西湖,是一家拥有20间客房的精品度假酒店。小湖泊和现有建筑散布在场地上,为设计带来了特殊的限制。业主期望对基地原有的部分老建筑进行适用性改造,为之赋予新的功能,同时增建新的建筑以满足酒店的容量需求。为了统一项目中分散的元素,如恩采用围墙和通廊组成的平面网格规划系统。这样的组织形式创造了多个围合庭院,也是对中国传统四合院的现代诠释。和传统的四合院一样,院落的形式赋予了空间层次感,将天空与地面的景致框入,让建筑融于自然,创造出内部与外部的重叠。

目的评估前列腺术前多参数MRI(mp-MRI)在预测根治性前列腺切除术(RP)和扩大盆腔淋巴结清扫术(ePLND)后病人淋巴结转移中的作用。方法回顾性分析101例在本研究所接受前列腺癌术前mp-MRI和RP与ePLND的病人。记录每例病人的完整术前临床资料和肿瘤特征。最终的组织病理学分级为参考标准。进行单因素和多因素logistic回归分析。结果 23/101(22.8%)例病人发现淋巴结转移。在单因素分析中,所有临床和放射学参数均与淋巴结侵犯相关(均P<0.03);MRI上肿瘤的体积(mrV)、MRI上肿瘤的ADC值和肿瘤T分期(mr T)是最准确的预测因子(分别为AUC=0.93、0.86和0.84)。包括前列腺特异性抗原(PSA)水平、原发性Gleason分级、mrT和mrV的多变量模型显示出高预测准确性(AUC=0.956)。在mrT2期和mrV<1 mL(1.8%)的肿瘤中可见淋巴结转移的患病率非常低。结论前列腺术前mp-MRI可以预测前列腺癌病人的淋巴结转移,可能允许更好地选择ePLND。

扬州青普瘦西湖文化行馆位于扬州风景秀丽的西湖附近。由于场地各处散布着小湖泊和一些现有的建筑,这家包含20间客房的精品度假酒店对如恩来说是一个颇有挑战的项目。业主希望对基地原有的部分老建筑进行适应性再利用,为之赋予新的功能,同时增加新的建筑以满足酒店的容量需求。为将这些分散元素统一起来,如恩采用了网格的平面规划,框定出围墙和通廊的布局,从而将各个功能整合在一起,形成一个多院落的围场。设计的灵感源自中国四合院的建筑类型。和传统的庭院一样,院落的形式为空间赋予了层次,将天空与地面的景观框架其中,让景观融入建筑,创造出内部与外部的重叠。

Human intravenous immune globulin(IVIg),a purified IgG fraction composed of~60%IgG1 and obtained from the pooled plasma of thousands of donors,is clinically used for a wide range of *** biological actions of IVIg are incompletely understood and have been attributed both to the polyclonal antibodies therein and also to their IgG(IgG)Fc ***,we demonstrated that multiple therapeutic human IgG1 antibodies suppress angiogenesis in a target-independent manner via FcγRI,a high-affinity receptor for *** we show that IVIg possesses similar anti-angiogenic activity and inhibited blood vessel growth in five different mouse models of prevalent human diseases,namely,neovascular age-related macular degeneration,corneal neovascularization,colorectal cancer,fibrosarcoma and peripheral arterial ischemic *** was mediated by the Fc region of IVIg,required FcγRI and had similar potency in transgenic mice expressing human Fcγ***,IVIg therapy administered to humans for the treatment of inflammatory or autoimmune diseases reduced kidney and muscle blood vessel *** data place IVIg,an agent approved by the US Food and Drug Administration,as a novel angioinhibitory drug in doses that are currently administered in the clinical *** addition,they raise the possibility of an unintended effect of IVIg on blood vessels.
