The paper proposes a scalable fuzzy approach for mapping the status of the environment integrating several distinct models exploiting geo big data. The process is structured into two phases: the first one can exploit products yielded by distinct models of remote sensing image interpretation defined in the scientific literature, and knowledge of domain experts, possibly ill-defined, for computing partial evidence of a phenomenon. The second phase integrates the partial evidence maps through a learning mechanism exploiting ground truth to compute a synthetic Environmental Status Indicator (ESI) map. The proposal resembles an ensemble approach with the difference that the aggregation is not necessarily consensual but can model a distinct decision attitude in between pessimistic and optimistic. It is scalable and can be implemented in a distributed processing framework, so as to make feasible ESI mapping in near real time to support land monitoring. It is exemplified to map the presence of standing water areas, indicator of water resources, agro-practices or natural hazard from remote sensing by considering different models.

AIM To investigate matrix metalloproteinase-11(MMP-11) expression in adipose tissue dysfunction, using in vitro and in vivo models of insulin *** Culture of mouse 3T3-L1 preadipocytes were induced to differentiation into mature 3T3-L1 adipocytes. Cellular insulin resistance was induced by treating differentiated cultured adipocytes with hypoxia and/or tumor necrosis factor(TNF)-α, and transcriptional changes were analyzed in each condition thereafter. For the in vivo studies, MMP-11 expression levels were measured in white adipose tissue(WAT) from C57BL/6J mice that underwent low fat diet or high-fat feeding in order to induce obesity and obesity-related insulin resistance. Statistical analysis was carried out with GraphP ad Prism *** MMP-11 m RNA expression levels were significantly higher in insulin resistant 3T3-L1 adipocytes compared to control cells(1.46±0.49vs0.83±0.21, respectively;P<0.00036). The increase in MMP-11 expression was observed even in the presence of TNF-α alone(3.79±1.11vs1±0.17, P<0.01) or hypoxia alone(1.79±0.7vs0.88±0.1, P<0.00023). The results obtained in in vitro experiments were confirmed in the in vivo model of insulin resistance. In particular, MMP-11 m RNA was upregulated in WAT from obese mice compared to lean mice(5.5±2.8vs1.1±0.7, respectively; P<3.72E-08). The increase in MMP-11 levels in obese mice was accompanied by the increase in typical markers of fibrosis, such as collagen type Ⅵ alpha 3(Col6_α3), and fibroblast-specific protein *** Our results indicate that dysregulation of MMP-11 expression is an early process in the adipose tissue dysfunction, which leads to obesity and obesity-related insulin resistance.

Bleeding and coagulopathy are critical issues complicating pediatric liver transplantation and contributing to morbidity and mortality in the cirrhotic child. The complexity of coagulopathy in the pediatric patient is illustrated by the interaction between three basic models. The first model, "developmental hemostasis", demonstrates how a different balance between pro- and anticoagulation factors leads to a normal hemostatic capacity in the pediatric patient at various ages. The second, the "cell based model of coagulation", takes into account the interaction between plasma proteins and cells. In the last, the concept of "rebalanced coagulation" highlights how the reduction of both pro- and anticoagulation factors leads to a normal, although unstable, coagulation profile. This new concept has led to the development of novel techniques used to analyze the coagulation capacity of whole blood for all patients. For example, viscoelastic methodologies are increasingly used on adult patients to test hemostatic capacity and to guide transfusion protocols. However, results are often confounding or have limited impact on morbidity and mortality. Moreover, data from pediatric patients remain inadequate. In addition, several interventions have been proposed to limit blood loss during transplantation, including the use of antifibrinolytic drugs and surgical techniques, such as the piggyback and lowering the central venous pressure during the hepatic dissection phase. The rationale for the use of these interventions is quite solid and has led to their incorporation into clinical practice; yet few of them have been rigorously tested in adults, let alone in children. Finally, the postoperative period in pediatric cohorts of patients has been characterized by an enhanced risk of hepatic vessel thrombosis. Thrombosis in fact remains the primary cause of early graft failure and re-transplantation within the first 30 d following surgery, and it occurs despite prolongation of standard

AIM: To identify suitable biomarkers of response to bevacizumab(BV)- it remains an open question. The measurement of serum vascular endothelial growth factor(VEGF) has been proposed as a predictive factor for this drug, even if literature data are contradictory. METHODS: We prospectively evaluated the role of BV, total and not BV-bound VEGF and angiopoietin-2(Ang-2) serum levels as potential predictive factors of response for BV in combination with an oxaliplatinbased chemotherapy. BV, Ang-2, total and not BVbound VEGF levels were measured at baseline, before 2^(nd) and 5^(th) cycle of oxaliplatin-based chemotherapy in 20 consecutive metastatic colorectal cancer patients. RESULTS: Results were correlated to response to treatment. Variability in BV levels have been found, with decreased level in less responding patients. In particular, the concentration of BV increased of 3.96 ± 0.69 folds in serum of responsive patients after 3 more cycles of therapy compared to those with stable or progressive disease with a 0.72 ± 0.25 and 2.10 ± 0.13 fold increase, respectively. The determination of free and total VEGF demonstrated that the ratio between the two values, evaluated immediately before the 2^(nd) and the 5^(th) cycle of therapy, decreased from 26.65% ± 1.33% to 15.50% ± 3.47% in responsive patients and from 53.41% ± 4.75 to 34.95% ± 2.88% in those with stable disease. Conversely, in those with progression of disease, the ratio showed the opposite behavior coming up from 25.99% ± 5.23% to 51.71% ± 5.28%. The Ang-2 levels did not show any relationship. CONCLUSION: Our data show that the ratio of not BV-bound VEGF to total VEGF serum and BV plasma concentrations for predicting the response to BV plus oxaliplatin-based chemotherapy could be a promising biomarker of response to BV.

Mesopores in porous solids can produce a pronounced sorption hysteresis at moderate and high reduced vapor pressures of the ambient gas that is condensed in the pores. Unlike to other conventional porous materials, cement pastes often behave exceptionally. The water sorption hysteresis frequently persists at very low humidity. This hysteresis is reflected in a corresponding hysteresis loop of the solid skeleton volume. We discuss a theoretical model based on the strong compression force exerted by a condensate on the walls of narrow slit pores embedded in an elastic solid. This compression force is shown to be capable of shifting walls of narrow slit pores. Humidity-dependent closing and reopening of slit pores can produce hysteresis loops even at low humidity.

This paper describes a particular stack performance realized in a building （school） at real scale by computation of the wind induced ventilation and a comparison of the stack performance （airflow rate extracted and wind speed） respect to other systems （wind catcher, wind jetter and wind turbine） are also showed. The realization of the system, actually working, shows the synergy between a plant design and installation using traditional energy sources with innovative engineering techniques providing for the use of integrative energy. In this case, the wind action plays an important role for the conditioning of the school, integrating and giving a significant energetic contribution to the air cooling system. The school building （a nursery） has been built in Modena and is actually working.

The management of acutely dehydrated children with relapsed nephrotic syndrome (NS) can be challenging when edema and intravascular volume depletion are present simultaneously. In that condition the excess in body fluid, typically associated with NS, may be inappropriately low, with regard to albumin level, and the excess in fluid is not promptly shifted into the vascular bed because of the low plasma protein concentration. This peculiar condition exposes the child to a greater risk of acute kidney injury (AKI) particularly if albumin infusion is provided. Here in a case of dehydration in ongoing NS is described in order to highlight the danger of infusing albumin when dehydration coexists. The present case report provides a framework for discussing an unusual and preventable pathophysiological mechanism of AKI related to the inappropriate administration of albumin infusion.

Celiac disease, an immune-mediated enteropathy induced in genetically susceptible individuals by the ingestion of gluten, is the most frequent disorder associated with splenic hypofunction or atrophy. Defective splenic function affects more than one-third of adult patients with celiac disease, and it may predispose to a higher risk of infections by encapsulated bacteria and thromboembolic and autoimmune complications, particularly when celiac patients have concomitant pre-malignant and malignant complications (refractory celiac disease, ulcerative jejunoileitis and enteropathy-associated T-cell lymphoma). However, the clinical management of patients with celiac disease does not take into account the evaluation of splenic function, and in patients with high degree of hyposplenism or splenic atrophy the prophylactic immunization with specific vaccines against the polysaccharide antigens of encapsulated bacteria is not currently recommended. We critically re-evaluate clinical and diagnostic aspects of spleen dysfunction in celiac disease, and highlight new perspectives in the prophylactic management of infections in this condition.
