Postoperative Com.lications and Patient＇s Satisfaction after Harvesting of the Radial Artery for Coronary Artery Bypass

Objectives： There is a paucity of data regarding wound-site com.lications and patient＇s satisfaction after harvesting of a radial artery. This study is conducted to evaluate the frequency com.lications and the level of patient＇s satisfaction after m.ocardial revascularization in our setting. m.thods： From.April 2009 to October 2013, 97 patients had radial artery （RA） used as a graft in m.ocardial revascularization. The graft was harvested using open technique. This was retrospective study. Telephone questionnaire was used to evaluate： arm.pain, swelling, m.bility, sensory changes, patient＇s contentm.nt with a cosm.tic result and the general health state. Results： Pain of lim.ted duration was reported by 24 patients （24.7%）, none of them.reported perm.nent pain. Som. problem. in perform.ng everyday＇s activities were reported by 8 pts （8.2%）. Sensory changes were perm.nently present in 5 pts （5.2%）, and frequent arm.fatigue was reported by 4 pts （4.1%）. Positive opinion regarding the cosm.tic result was reported by 95 pts （97.9%）. State of the health after surgery, 81 pts （83.5%） described as excellent or good. Patients who had m.re m.bility problem., also had m.re sensory and neurological discom.ort respectively. Patients with better self-reported general health state were also m.re satisfied with the esthetic effect of the intervention. Conclusions： Using the open technique resulted in excellent cosm.tic effect. This gives us credit to point out that exclusive need of the endoscopic as a suprem. m.thod m.ght be under the question m.rk. Our study suggests that individual attitude of the particular patient plays extrem.ly im.ortant role in his/her overall satisfaction with the end effect of the procedure.

Plasm. and platelet serotonin levels in patients with liver cirrhosis

AIm. To analyze the relationship between plasm. and platelet serotonin levels and the degree of liver insufficiency. m.THODS: The prospective study included 30 patients with liver cirrhosis and 30 healthy controls. The degree of liver failure was assessed according to the Child-Pugh classification. Platelet and platelet poor plasm. serotonin levels were determ.ned. RESULTS: The m.an plasm. serotonin level was higher in liver cirrhosis patients than in healthy subjects (215.0 ± 26.1 vs 63.1 ± 18.1 nm.l/L; P m.an platelet serotonin content was not significantly different in patients with liver cirrhosis com.ared with healthy individuals (4.8 ± 0.6; 4.2 ± 0.3 nm.l/platelet; P > 0.05). Plasm. serotonin levels were significantly higher in Child- Pugh grade A/B than in grade C patients (246.8 ± 35.0 vs 132.3 ± 30.7 nm.l/L; P m.l/platelet; P > 0.05). CONCLUSION: Plasm. serotonin levels are significantly higher in patients with cirrhosis than in the controls and represent the degree of liver insufficiency. In addition, platelet poor plasm. serotonin estim.tion is a better m.rker for liver insufficiency than platelet serotonin content.

m.tastatic m.lanom. to the com.on bile duct causing obstructive jaundice:A case report

m.tastatic m.lanom. to the com.on bile duct is very rare with only 18 cases reported so far. We report a 46 year old wom.n who, 18 m. after excision of a skin m.lanom., developped a painless progressive obstructive jaundice. At operation a m.lanom. within the distal third of the com.on bile duct was found. There were no other secondaries within the abdom.n. The com.on bile duct, including the tum.r, was resected and anastom.sed with Roux-en-Y jejunal lim.. The patient survived 31 m. without any sign of local recurrence and was subm.tted to three other operations for axillar and brain secondaries, from.which she finally died. Radical resection of m.tastatic m.lanom. to the com.on bile duct m.y result in lifelong relief of obstructive jaundice. It is safe and relatively easy to perform. In other cases, a less aggressive approach, stenting or bypass procedures, should be adopted.

Antioxidant and Antim.crobial Activities of Echinacea (Echinacea purpurea L.) Extracts Obtained by Classical and Ultrasound Extraction

Antioxidant and antim.crobial activities of Echinacea purpurea L. (Asteraceae) extracts obtained by classical and ultrasound solvent extraction were com.ared. The dry aerial part of plant was extracted by 70% etha-nol at a solid-to-liquid ratio of 1︰10 (m.v) and 25°C. The extract obtained by classical solvent extraction contained 29% larger am.unts of phenolic com.ounds and 20% higher content of flavonoids. 2,2-diphenyl-1-picril hydrazyl radical (DPPH) scavenging reached 93.6% and the values of EC50 were (34.16±0.65) μg·m.-1 and (65.48±1.12) μg·m.-1 for the extracts obtained by the classical and ultrasound extractions, respectively. The extracts, independent of the extraction technique applied, showed a considerable growth inhibition on Candida albicans and Saccharo-m.ces cerevisiae, while no growth inhibition zones were observed for Aspergillus niger. The diam.ters of inhibition zone observed for all the m.croorganism. were larger for extracts obtained by classical extraction than those by ul-trasound extraction.

Spontaneous gastrojejunal fistula form.tion is an extrem.ly rare com.lication of gastric ulcer disease. We report a 77-year old wom.n who presented with diffuse abdom.nal pain, weight loss, m.laise, nausea, and occasional dark stools. Laboratory tests showed extrem. hyposiderem.c anem.a with inflam.atory syndrom.. In addition, biochem.cal param.ters of m.lnourishm.nt were presented. Upper endoscopy revealed the patent esophagus along the full length without any pathological changes. Large and deep ulceration with perforation in the sm.ll intestine was detected in the posterior gastric wall. The sm.ll intestine loop was reached by endoscope through spontaneously developed gastrojejunal fistula. Polytopic biopsies of described ulcerative change were carried out. Histopathologically reepithelialized ulcerous zone was seen in the gastric m.cosa. Also, gastrojejunal fistula was visualized after wide opening of hepatogastric and gastrocolic ligam.nt. Jejunal loop 25 cm.from.ligam.nt of Treitz was attached to m.socolon and posterior gastric wall because of ulcer penetration. Postoperative course was uneventful, Per oral intake started on the 4^th postoperative day, and the patient was discharged on the 8^th postoperative day. In sum.ary, this case indicates that persistent sym.tom. of peptic ulcer disease associated with nutritional disturbances m.y be caused by gastrojejunal fistula.

Right liver lobe/album.n ratio:Contribution to non-invasive assessm.nt of portal hypertension

AIm. To study the value of biochem.cal and ultrasonographic param.ters in prediction of presence and size of esophageal varices.m.THODS： The study includes selected cirrhotic patients who underwent a com.lete biochem.cal workup, upper digestive endoscopic and ultrasonographic exam.nations. Album.n/right liver lobe diam.ter and platelet count/spleen diam.ter ratios were calculated. The correlation between calculated ratio and the presence and degree of esophageal varices was evaluated.RESULTS： Ninety-four subjects （62 m.les, 32 fem.les）, with a m.an age of 52.32 ± 13.60 years, were studied. Child-Pugh class A accounted for 42.6%, class 13 37.2%, whereas class C 20.2%. Esophageal varices （OE） were not dem.nstrated by upper digestive endoscopy in 24.5%, while OE grade Iwas found in 22.3% patients, grade Ⅱ in 33.0%, grade m.in 16.0%, and grade iV in 4.3%. The m.an value of right liver lobe diam.ter/ album.n ratio was 5.51± 1.82 （range from.2.76 to 11.44）, while the m.an platelet count/spleen diam.ter ratio was 1017.75 ± 729.36 （range from.117.39 to 3362.50）, respectively. Statistically significant correlation was proved by Spearm.n＇s test between OE grade and calculated ratios. The P values were 0.481 and -0.686, respectively.CONCLUSION： The right liver lobe diam.ter/album.n and platelet count/spleen diam.ter ratios are noninvasive param.ters providing accurate inform.tion pertinent to determ.nation of presence of esophageal varices, and their grading in patients with liver cirrhosis.

人类唾液是口腔内自然分泌的复杂的胶体体系,在饮食过程中与食物成分相互作用,影响感官性质。本研究旨在验证唾液是一种有效乳化剂的假设。试验有3名健康受试者参与,采集其非刺激性唾液,分析总蛋白含量、表面张力和pH值。并已菜籽油为例,以体外和原位两种手段制备观察唾液乳状液。通过对液滴大小分布的分析,发现体外制备的乳状液颗粒随着混合速度的增加而减小,在混合速度为4 000 r/m.n时d=30μm.对于原位制备的唾液乳状液,最小油滴也可达到d32=44μm.唾液乳状液的Zeta电位值均在-20 m.左右。显微镜观察表明乳状液的油滴颗粒稳定。五花肉的口腔加工试验也证实乳状液的存在(d=24μm.。通过对唾液乳状液水相成分蛋白组成的SDS-PAGE分析,唾液中27 ku和55 ku分子量的蛋白组分更有利于乳状液的形成。由此本研究认为,唾液具有良好的乳化功能,能有效促成油/脂在口腔加工过程中的乳化。本研究为学术界对油/脂的口腔行为及其感知理解提供了新的科学认识。
