Splenectomy with chemotherapy vs surgery alone as initial treatment for splenic marginal zone lymphoma

AIM： To evaluate the clinical characteristics of splenic marginal-zone lymphoma （SMZL） following antigen expression and the influence of therapeutic approaches on clinical outcome and overall survival （OS）.METHODS： A total of 30 patients with typical histological and immunohistochemical SMZL patterns were examined. Splenectomy plus chemotherapy was applied in 20 patients, while splenectomy as a single treatment-option was performed in 10 patients. Prognostic factor and overall survival rate were analyzed.RESULTS： Complete remission （CR） was achieved in 20 （66.7%）, partial remission （PR） in seven （23.3%）, and lethal outcome due to disease progression occurred in three （10.0%） patients. Median survival of patients with a splenectomy was 93.0 mo and for patients with splenectomy plus chemotherapy it was 207.5 mo （Log rank = 0.056, P 〉 0.05）. Time from onset of first symptoms to the beginning of the treatment （mean 9.4 too） was influenced by spleen dimensions, as measured by computerized tomography and ultra-sound （t = 2.558, P = 0.018）. Strong positivity （＋＋＋） of CD20 antigen expression in splenic tissue had a positive influence on OS （Log rank = 5.244, P 〈 0.05）. The analysis of factors interfering with survival （by the Kaplan-Meier method） revealed that gender, general symptoms, clinical stage, and spleen infiltration type （nodular vs diffuse） had no significant （P 〉 0.05） effects on the OS. The expression of other antigens （immunohistochemistry） also had no effect on survival-rate, as measured by a χ^2 test （P 〉 0.05）.CONCLUSION： Initial splenectomy combined with chemotherapy has been shown to be beneficial due to its advanced remission rate/duration; however, a larger controlled clinical study is required to confirm our findings.

By using the standard PMNS (Pontecorvo-Maki-Nakagawa-Sakata) mixing matrix and applying the rule for the sum of the oscillation probabilities of three neutrinos, the equations of motion were derived in which the Dirac CP violating phase appeared as an unknown quantity. The equations of motion were separately derived for each of the three possible transitions for flavor-neutrino oscillations. Two roots of those equations were obtained in the form of two formulas for the Dirac CP violating phase with opposite signs. In the mathematical sense, the connection between those formulas was established in order to maintain the continuous process of oscillation of three neutrinos. This made it possible to calculate the numerical value for the Dirac CP violating phase, the Jarlskog invariant and to write the general form of the PMNS mixing matrix in the final form in which all its elements are defined with explicit numerical values.

Gastrin and antral G cells in course of Helicobacter pylori eradication: Six months follow up study

AIM： To assess long-term effects of Helicobacter pylori （H pylon} eradication on antral G cell morphology and function in patients with and without duodenal ulcer （DU）.METHODS： Consecutive dyspeptic patients referred to the endoscopy entered the study. Out of 39 Hpylori positive patients, 8 had DU （Hpylori ＋DU） and 31 gastritis （Hpylori＋G）. Control groups consisted of 11 uninfected dyspeptic patients （CG1） and 7 healthy volunteers （CG2）. Basal plasma gastrin （PGL）, antral tissue gastrin concentrations （ATGC）, immunohistochemical and electron microscopic characteristics of G cells were determined, prior to and 6 mo after ***： We-demonstrated elevated PGL in infected patients compared to uninfected controls prior to *** PGL were registered in all Hpylori＋patients （Hpylori＋DU： 106.78＋22.72 pg/mL, Hpylori＋G： 74.95＋15.63,CGI： 68.59＋17.97, CG2：39.24＋5.59 pg/mL, P〈0.01）.Successful eradication （e） therapy in Hpylori＋patients lead to significant decrease in PGL （Hpylori＋DU： 59.93＋9.40 and Hpylori＋Ge： 42.36＋10.28 pg/mL, P〈0.001）. ATGC at the beginning of the study were similar in infected and uninfected patients and eradication therapy lead to significant decrease in ATGC in Hpylori＋gastritis, but not in DU patients. In the Hpylori＋DU patients, the mean number of antral G cells was significantly lower in comparison with all other groups （P〈0.01）, but after successful eradication was close to normal values found in controls. By contrast, G cell number and volume density were significantly decreased （P〈0.01） in Hpylori＋Ge group after successful eradication therapy （294＋32 and 0.31＋0.02,respectively）, in comparison to values before eradication （416~40 and 0.48~0.09）. No significant change of the G cell/total endocrine cell ratio was observed during the 6 mo of follow up in any of the groups. Three Neutrinos and the Formula for the Dirac CP Violation Phase

Based on the derived equations of three neutrinos, especially for motion through a physical vacuum and for space with a constant density of matter, the same formula for Dirac’s CP-violating phase was obtained. The main property of this formula is that it does not depend on mixing angles θ12, θ13, θ23 and remains unchanged for the spaces through which the neutrino beam moves. Using that formula, the final form for the Jarlskog invariant formula was formed. Knowing the Dirac CPV phase would have the following consequences: 1) By obtaining an explicit mathematical formula for the Dirac CPV phase, it would no longer be necessary to perform computer simulations to draw areas where it could be found. 2) At the same time, the Dirac CPV phase does not depend on the mixing angles θ12, θ13, θ23 that make up the elements of the PMNS matrix, but depends only on the ratio of the corresponding differences of the squares of the neutrino masses.

Testing the Results of Measurements of Neutrino Parameters Using the Dirac CPV Phase Formula

Essentially the main intention of this paper was to test the formula for the Dirac CPV phase and see if it can reflect the results of experimental measurements of neutrino parameters. By knowing the mathematical formula for the Dirac CPV phase, a connection was established with some of the residual symmetry groups, which made it possible to develop a procedure for directly determining the range in which the numerical value for the Dirac CPV phase could be found. In this sense, two different sources of information containing measured data for neutrinos were used for the corresponding calculations, and then a comparative overview of the calculated results was presented. It is particularly emphasized that the formula for the Dirac CPV phase does not depend on the mixing angles that are incorporated into the PMNS matrix, but only on the ratio between the corresponding squares of the neutrino mass difference. All the numerous results obtained from the corresponding calculations for the Dirac CPV phase point to the justified introduction of the theory that is related to three neutrinos, and thus the agreement of our results with the STEREO experiment is justified, so that the hypothesis of the possible existence of a sterile neutrino in nature should be excluded.

This paper aims at solving several open questions in current neutrino physics: the neutrino mass hierarchy, the Dirac CP violating phase, the absolute mass of neutrinos, the nature of neutrinos (Dirac or Majorana), the Majorana matrix and the absolute value of the effective Majorana neutrino mass. In the research presented in this paper, we have shown that the precise definition of the mass splittings between neutrino mass eigenstates, done in the latest analysis of experimental data, can be of crucial importance for defining the nature of neutrino mass hierarchy. The Standard Model has three generations of fundamental matter particles. Three generations of the charged lepton mass show a hierarchical structure: mτ > mμ > me. Owing to that, there is a belief and it is considered that neutrinos may follow such hierarchical structure. In our calculations, we have also included the latest data obtained, based on the processing of measurement results, which showed that even with such data, obtained results favor the normal neutrino mass hierarchy. As for the individual neutrino mass calculated in this paper, in today’s neutrino physics it is only known that neutrino mass scale is bounded only from above, and both the Dirac and the Majorana character of neutrinos are compatible with all observations. Among some of the questions resolved in this paper, which are related to the properties of neutrinos, a positive answer was also given to the question of whether light neutrinos are self-conjugate particles or not.

AIM： To investigate the expression of different cytokeratins （CKs） in gastric epithelium of adult patients with chronic gastritis infected with Helicobacter pylori （H pylon） cagA ＋ strains. METHODS： The expression of CK 7, 8, 18, 19 and 20 was studied immunohistochemically in antral gastric biopsies of 84 patients. All the CKs were immunostained in cagA＋Hpylori gastritis （57 cases）, non-Hpylori gastritis （17 cases） and normal gastric mucosa （10 cases）. RESULTS： In cagA＋ H pylori gastritis, CK8 was expressed comparably to the normal antral mucosa from surface epithelium to deep glands. Distribution of CK18 and CK 19 was unchanged, i.e. transmucosal, but intensity of the expression was different in foveolar region in comparison to normal gastric mucosa. Cytokeratin 18 immunoreactivity was significantly higher in the foveolar epithelium of H pylori-positive gastritis compared to both Hpylori-negative gastritis and controls. On the contrary, decrease in CK19 immunoreactivity occurred in foveolar epithelium of H pylori-positive gastritis. In both normal and inflamed antral mucosa without Hpyloriinfection, CK20 was expressed stronglyl moderately and homogenously in surface epithelium and upper foveolar region, but in H pylod -induced gastritis significant decrease of expression in foveolar region was noted. Generally, in both normal antral mucosa and H pylori-negative gastritis, expression of CK7 was not observed, while in about half cagA＋ H pylori-infected patients, moderate focal CK7 immunoreactivity of the neck and coiled gland areas was registered, especially in areas with more severe inflammatory infiltrate. CONCLUSION： Alterations in expression of CK 7, 18, 19 and 20 together with normal expression of CK8 occur in antral mucosa of H pylori-associated chronic gastritis in adult patients infected with cagA＋ strains. SNUC （undifferentiated sinonasal carcinoma） is a rare tumor. The exact cause of this cancer is not known. Patients usually have signs and symptoms of bleeding from the nose, runny nose, duplicate photos, symptoms of chronic sinus infections and nose, nasal obstruction, or facial pain. We reported a case of seventy year old patient with sinonasal undifferentiated carcinoma in the right maxillary sinus. Six months ago, the patient was conducted operations polyp in the right nasal hall. He had pain in the right side of his face and swelling in the mouth in the area of the upper jaw on the right side. The patient was operated tumor of the right maxillary sinus, with expansion in the ethmoid sinus, nasal cavity and infratemporal space. The neck dissection was done on the same side. Postoperatively, he received radiation therapy and chemotherapy. SNUC is very rare aggressive cancer that requires aggressive therapy. This cancer is considered in need of modified maxillary sinuses, as well as for modified sinus in polyps or after the operation of the same. Despite extensive surgery, radio therapy and implemented chemotherapy, the prognosis of these tumors is very poor
