Clinical usefulness of ursodeoxycholic acid for Japanese patients with autoimmune hepatitis

AIM To evaluate the therapeutic effects of ursodeoxycholic acid(UDCA) on autoimmune hepatitis(AIH).METHODS A total 136 patients who were diagnosed with AIH were included in our study. All of the patients underwent a liver biopsy, and had at least a probable diagnosis on the basis of either the revised scoring system or the simplified scores. Initial treatment included UDCA monotherapy(Group U, n = 48) and prednisolone(PSL) monotherapy(Group P, n = 88). Group U was further classified into two subgroups according to the effect of UDCA: Patients who had achieved remission induction with UDCA monotherapy and showed no sign of relapse(Subgroup U1, n = 34) and patients who additionally received PSL during follow-up(Subgroup U2, n = 14). We compared the clinical and histological findings between each groups, and investigated factorscontributing to the response to UDCA *** In Group U, 34 patients(71%) achieved and maintained remission over 49(range: 8-90) mo(Subgroup U1) and 14 patients(29%) additionally received PSL(Subgroup U2) during follow-up. Two patients in Subgroup U2 achieved remission induction once but additionally required PSL administration because of relapse(15 and 35 mo after the start of treatment). The remaining 12 patients in Subgroup U2 failed to achieve remission induction during follow-up, and PSL was added during 7(range: 2-18) mo. Compared with Subgroup U2, Subgroup U1 had significantly lower alanine aminotransferase(ALT) levels at onset(124 IU/L vs 262 IU/L, P = 0.023) and a significantly higher proportion of patients with mild inflammation(A1) on histological examination(70.6% vs 35.7%, P = 0.025). When multivariate analysis was performed to identify factors contributing to the response to UDCA monotherapy, only a serum ALT level of 200 IU/L or lower was found to be associated with a significant difference(P = 0.013).CONCLUSION To prevent adverse events related to corticosteroids, UDCA monotherapy for AIH needs to be considered in pat

Potential Power of the Pyramidal Structure V: Seasonal Changes in the Periodicity of Diurnal Variation of Biosensors Caused by Entanglement Due to Pyramid Effects

Since October 2007, we have been conducting rigorous scientific experiments to elucidate the so-called “pyramid power”. The experiments use a pyramidal structure (PS). In order to detect the PS effects, a biosensor is made from cucumber fruit sections and the released volatile components are analyzed as gas concentration. We reported the phenomenon of entanglement between biosensors in part IV of the paper series, Potential Power of the Pyramidal Structure. The entanglement affected the gas concentration of the biosensors 8 m away, but not the biosensors at the PS apex. In this paper, we report another characteristic of the entanglement. The results and our conclusion are as follows. Result 1: The periodicity of diurnal variation in gas concentration changed with entanglement. Result 2: As a result of analyzing the gas concentration data separately for the four seasons of winter, spring, summer, and autumn, the seasonal dependence of the periodicity of diurnal variation due to entanglement was clarified. Conclusion: We reaffirmed the existence of the entanglement between biosensors due to the pyramid effects by a phenomenon different from Part IV of the paper series, Potential Power of the Pyramidal Structure. We expect that our research results will be widely accepted in the future and will become the foundation for a new research field in science, with a wide range of applications.

＜正＞A sharable software repository for Japanese LVCSR （Large Vocabulary Continuous Speech Recognition） is introduced. It is designed as a baseline platform for research and developed by researchers of different academic institutes under a governmental support. The repository consists of a recognition engine （Julius）, Japanese acoustic models and statistical language models us well as Japanese morphological analysis tools. These modules can be easily integrated and replaced under a plug-and-play framework, which makes it possible to fairly evaluate components and to develop specific application systems. Assessment of these modules and systems in a 20000-word dictation task is reported. The software repository is freely available to the public.

Feasibility of Outpatient Chemotherapy with S-1 and Cisplatin for Gastric Cancer

Salary fairness is the key to the reform of income distribution mechanisms in China. This research examines salary fairness under the background of marketization, using evidence from Chinese pharmaceutical listed companies. Based on the Rucker coefficient and related theories, a value added statement analysis is conducted. The research results indicate that salary inequity exists in China's pharmaceutical industry. It also suggests that non-state owned enterprises that located in regions with lower marketization level face a more serious problem of salary fairness. Most significantly, it reveals that the shareholders and government claim a larger portion of the value produced by the enterprises than other stakeholders. This is especially true concerning the shareholders who claim the largest share.

Objective: To evaluate the feasibility of S-1 and high-dose cisplatin short hydration regimens for outpatients with unresectable metastatic gastric cancer. Methods: Data for individual outpatients treated in our institution were retrospectively pooled to assess the feasibility of an S-1 and highdose cisplatin short hydration regimen (S-1: 80 to 120 mg on Days 1 to 21;cisplatin: 60 mg/m2?on Day 8, every 5 weeks), which included 2250 ml of intravenous fluids and 1000 ml oral hydration. Ten consecutive patients were treated with S-1 and high-dose cisplatin short hydration for unresectable metastatic gastric cancer from July 2011 to May 2012 and were included in the analysis. Results: With a median of 3.5 medication cycles, unscheduled admission occurred in two patients for 5 days each due to paralytic ileus and cerebral infarction. Four patients required dose reduction, in both S-1 and cisplatin in two patients, and in S-1 alone and cisplatin alone in one patient each. Renal function transiently declined after administration of cisplatin, but serum creatinine level and estimated glomerular filtration rate were both improved by the time of the next administration. Conclusion: This study suggests that an S-1 and high-dose cisplatin short hydration strategy for outpatients with unresectable metastatic gastric cancer might be feasible.

An Experimental Investigation on the Quasi-Static Flotation Transport of a Glass Substrate Using Vortex Bearing Elements

In this paper, we build an air conveyor with newly developed vortex bearing elements, and study the flotation precision of the front-end of the substrate in quasi-static flotation transport. We experimentally discuss the three influential factors: air supply pressure, thickness of the substrates and installing direction of the vortex bearing element. We find that during the process of transport the movement of the substrate leads to the variation of flotation height. The amplitude of variation (e.g. flotation precision) is dependent upon the bearing stiffness and the suction force of the vortex bearing elements. Increasing air supply pressure properly can improve the flotation precision, but an excess pressure can cause over-suction due to high negative pressure and result in a poor flotation precision. We also know that the flotation precision of thin and light substrates are easily affected by the suction force of vortex flow because they float with a high flotation height and are more susceptible to deformation. Finally, we investigate four installing directions of the vortex bearing element. Different installing direction can lead to different variation of flotation height.

The effect of far-infrared (FIR) irradiation pasteurization on fungi was quantitatively evaluated and compared with the effect of thermal conductive heating. After the bulk temperature of the sterile saline irradiated by FIR reached a steady given temperature, yeast cells (Candida albicans NBRC 1950 and Saccharomyces cerevisiae NBRC 1067) or fungal spores (Aspergillus niger NBRC 4781) were inoculated and FIR heating was conducted. A mullite cylinder FIR heater, with a main wavelength of 4 - 7 μm, was used for FIR heating. Death of fungi by FIR heating and by thermal conductive heating both followed first-order reaction kinetics, and the apparent death rate constants under different temperature conditions were obtained. For the same bulk temperatures, pasteurization by FIR heating was more effective than thermal conductive heating. The activation energy for the death of fungi by FIR irradiation was slightly lower than thermal conductive heating, indicating differences in the mechanism of action.

Clinical Evaluation of New Biomarkers including IFN-<i>γ</i>for the Diagnosis of Active Tuberculosis Disease

Background: The aim of this study was to evaluate the clinical potential of new biomarkers such as IL-2, TNF-α, MCP-1, MIP-1β including IFN-γ for the diagnosis of active tuberculosis (TB) disease. Materials and Methods: The subjects consisted of 31 patients with active TB disease and 62 patients with non-TB disease. We measured IFN-γ using QuantiFERON-TB Gold In Tube, and IL-2, TNF-α, MCP-1, and MIP-1β using the supernatant from whole blood stimulated with MTB (Mycobacterium tuberculosis)-specific antigens. Results: In the patient group with active TB disease, while the positive response rate of IFN-γ was 74%, that of IL-2 using the supernatant was 61%, TNF-α was 71%, MCP-1 was 81% and MIP-1β was 81%. In the patient group with non-TB disease, while the positive response rate of IFN-γ was 11%, that of IL-2using the supernatant was 32%, TNF-α was 26%, MCP-1 was 23% and MIP-1β was 19%. All biomarker levels of the patients with active TB disease using the supernatant were significantly higher than those of the patients with non-TB disease. While MIP-1β was the most sensitive of all the biomarkers (sensitivity: 80.6%), IFN-γ was the best in terms of specificity (specificity: 87.0%). Conclusions: Several biomarkers, apart from IL-2, showed similar results compared to IFN-γ. The combination of IFN-γ and other new biomarkers may increase the diagnostic accuracy of active TB.
