Rational. and Objectives: Cystic l.ng disease may be accuratel. diagnosed by imaging interpretation of special.st radiol.gists, without other information. We hypothesized that with minimal.training non-special.sts coul. perform simil.rl. to special.st physicians in the diagnosis of cystic l.ng disease. Methods: 72 cystic l.ng disease cases and 25 cystic l.ng disease mimics were obtained from three sources: 1) a prospective acquired diffuse l.ng disease registry, 2) a retrospective search of medical.records and 3) teaching fil.s. Cases were anonymized, randomized and interpreted by 7 diffuse l.ng disease special.sts and 15 non-special.st radiol.gists and pul.onol.gists. Cl.nical.information other than age and sex was not provided. Prior to interpretation, non-special.sts viewed a short PDF training document expl.ining cystic l.ng disease interpretation. Resul.s: Correct first choice diagnosis of 85%-88% may be achieved by high-performing special.st readers and 71%-80% by non-special.sts and l.wer-performing special.sts, with mean accuracies in the diagnosis of l.M (91%, p Concl.sion: With specific but l.mited training, non-special.st physicians can diagnose cystic l.ng diseases from CT appearance al.ne with simil.r accuracy to special.sts, correctl. identifying approximatel. 75% of cases.

Autotrophic Biofil.ers for Oxidation of Nitric Oxide

Carbon foam—a kind of new engineering material.as packing material.was adopted in three biofil.ers with different pore dimensions and adapted autotrophic nitrite nitrobacteria to investigate the purification of nitric oxide (NO) in a gas stream. The biofil. was devel.ped on the surface of carbon foams using nitrite as its onl. nitric source. The moisture in the fil.er was maintained by ul.rasonic aerosol.equipment which can minimize the thickness of the l.quid fil.. The l.quid phase nitrification test was conducted to determine the variabil.ty and the potential.of performance among the three carbon foam biofil.ers. The investigation showed that during the NO2^-—N inl.t concentration of 200 g·l.-1·min^-1 to 800 g·l.-1·min^-1, the 24PPC (pores per centimeter) carbon foam biofil.er had the greatest potential. achieving the NO2^-—N removal.efficiency of 94% to 98%. The 8PPC and 18PPC carbon foam biofil.ers achieved the NO2^-—N removal.efficiency of 15% to 21% and of 30% to 40%, respectivel.. The potential.for this system to remove NO from a gas stream was shown on the basis of a steady removal.efficiency of 41% to 50% which was attained for the 24PPC carbon foam biofil.er at specified NO inl.t concentration of 66.97 mg·m^-3 to 267.86mg·m^-3 and an empty-bed residence time of 3.5 min.

采用一种新的工程材料多孔碳作为填料 ,在 3个5 0 8mm填充着不同孔径多孔碳和驯化培养好的自养型亚硝酸盐硝化菌的生物过滤器中 ,进行了净化气体中NO的探索性研究 .实验采用亚硝酸盐为唯一氮源 ,在多孔碳的表面培养生物膜 ,采用超声波气溶胶发生器来维持过滤器内的湿度并保持多孔碳表面液膜的厚度较小 .液相硝化试验用来比较 3种不同孔径的多孔碳之间硝化性能的潜力和差异 .结果表明 ,在亚氮 (NO- 2 N)进口负荷约2 0 0～ 80 0g/(l.min)的范围内 ,2 4孔 /cm(PoresPerCentimeter,PPC)多孔碳过滤器的硝化速率最大 ,达到 94%～98%;8PPC和 1 8PPC多孔碳过滤器的硝化速率分别达到 1 5 %～ 2 1 %和 3 0 %～ 40 %.气相试验表明 ,在NO空床停留时间 3 5min、进口浓度 66 97～ 2 67 86mg/m3的范围内 ,2 4PPC多孔碳过滤器对NO的去除效率为 41 %～5 2 %左右。

背景：临床上常常可以看到血清促甲状腺素(thyroid—stimul.ting hormone，TSH)水平超出正常范围而游离甲状腺素(free thyroxine，FT4)和三碘甲状腺氨酸(triidothyromine，T3)水平处于正常范围的患者。对这些患者是否需要进一步检查，可以采用何种治疗以及治疗的急迫性尚无定论。目的：明确亚临床甲状腺疾病的定义，复习其流行病学，推荐适当的评估方式，探讨治疗的危险和获益以及不予治疗的后果，确定是否应进行人群普查。资料来源：我们检索了MEDl.NE、EMBASE、Biosis、卫生保健研究及医疗质量机构、国家临床指南交流库、系统综述和对照试验登记处Cochrane数据库以及若干国家健康服务(英国)数据库，查寻了1995—2002年发表的有关亚临床甲状腺疾病的文章。1995年以前发表的文章由专家顾问推荐。研究选择和资料提取：总共复习195篇英语文章或译文。述评、个案研究、病例少于10人的研究和非系统性综述排除在外。提取有关作者身份、发表年代、病例数量、研究设计和结果的信息，这是证据报告的基础。该报告由相应专题图表和摘要组成。资料综合：我们评价了未予治疗与临床症状和不良临床结局相关的证据的强度，并为临床实践提出了建议。有关亚临床甲状腺功能低下可发展为显性疾病的资料其证据分级强度良好；但是有关治疗防止疾病进展的资料，对确认治疗的获益而言，证据不足。有关血清TSH水平高于10mIU／l.血清胆固醇增高关系的资料，强度分级一般；但是有关治疗是否获益的资料则分级为不充分。所有其他有关症状与治疗获益关系的资料其分级均为不充分或是缺乏。有关血清TSH浓度低于0．1mIU／l.心房纤颤的关系或发展成为显性甲状腺功能亢进的资料分级为良好，但是没有资料支持治疗能阻止这些结果的产生。有关TSH水平恢复正常与骨矿物质密度改善关系的资料分级一般。其他阐述亚临床甲状腺疾病与不良后果或治疗获益关系的资料强度分级均为不充分或缺乏。怀孕期间的亚临床甲状腺疾病是一种特殊情况，对孕妇采取积极诊断和治疗是恰当的。结论：几乎没有资料支持亚临床甲状腺疾病与症状、不良临床预后、治疗获益有关。亚临床甲状腺疾病(血清TSH 0．1～0．45mIU／l.是4．5～10．0mIU／l.的后果微不足道，我们不建议对TSH水平处于这些范围的病人采用常规治疗。没有充分的证据支持有必要对普通人群进行普查。对于怀孕、年龄大于60岁的妇女和其他有甲状腺功能异常的高危病人应予积极检查。
