Vascular calcifications are commonly observed in patients with chronic kidney disease （CKD） and contri-bute to the excessive cardiovascular morbidity and mortality rates observed in these patients populations. Although the pathogenetic mechanisms are not yet fully elucidated, recent evidence suggests a link between bone metabolism and the development and progression of vascular calcifications. Moreover, accumulating data indicate that receptor activator of nuclear factor κB ligand/osteoprotegerin axis which plays essential roles in the regulation of bone metabolism is also involved in extra-osseous bone formation. Further studies are required to establish the prognostic significance of the above biomarkers as predictors of the presence and severity of vascular calcifications in CKD patients and of cardiovascular morbidity and mortality. Moreover, randomized clinical trials are needed to clarify whether inhibition of osteoclast activity will protect from vascular calcifcations.

Detection of Angioectasias and Haemorrhages Incorporated into a Multi-Class Classification Tool for the GI Tract Anomalies by Using Binary CNNs

The proposed deep learning algorithm will be integrated as a binary classifier under the umbrella of a multi-class classification tool to facilitate the automated detection of non-healthy deformities, anatomical landmarks, pathological findings, other anomalies and normal cases, by examining medical endoscopic images of GI tract. Each binary classifier is trained to detect one specific non-healthy condition. The algorithm analyzed in the present work expands the ability of detection of this tool by classifying GI tract image snapshots into two classes, depicting haemorrhage and non-haemorrhage state. The proposed algorithm is the result of the collaboration between interdisciplinary specialists on AI and Data Analysis, Computer Vision, Gastroenterologists of four University Gastroenterology Departments of Greek Medical Schools. The data used are 195 videos (177 from non-healthy cases and 18 from healthy cases) videos captured from the PillCam(R) Medronics device, originated from 195 patients, all diagnosed with different forms of angioectasia, haemorrhages and other diseases from different sites of the gastrointestinal (GI), mainly including difficult cases of diagnosis. Our AI algorithm is based on convolutional neural network (CNN) trained on annotated images at image level, using a semantic tag indicating whether the image contains angioectasia and haemorrhage traces or not. At least 22 CNN architectures were created and evaluated some of which pre-trained applying transfer learning on ImageNet data. All the CNN variations were introduced, trained to a prevalence dataset of 50%, and evaluated of unseen data. On test data, the best results were obtained from our CNN architectures which do not utilize backbone of transfer learning. Across a balanced dataset from no-healthy images and healthy images from 39 videos from different patients, identified correct diagnosis with sensitivity 90%, specificity 92%, precision 91.8%, FPR 8%, FNR 10%. The aim of this work is the study of the magnetohydrodynamic (MHD) unsteady free convective flow of water near 4℃ past an infinitely vertical plate moving with constant velocity. The influence of constant uniform suction was also considered. The partial differential equations (PDEs) and their initial and boundary conditions, describing the problem under consideration, are dimensionalized and the numerical solution is obtained by using the finite volume discretization methodology which is suitable for Fluid Mechanics applications. The numerical results for the velocity and temperature fields are shown in figures for different dimensionless parameters entering in the problem under consideration, such as the magnetic parameter, M and the Grashof number, Gr. This study predicts the effects of a constant magnetic field and uniform suction on the free convective flow of water near 4℃, when the water is electrically conductive. Analysis of the results showed that the velocity and temperature profiles are noticeably influenced by these parameters.

AIM： To determine the serum levels of c-reactive protein （CRP）, transferrin （TRF）, a2-macroglobulin （A2M）, ceruloplasmin （CER）, al-acid glycoprotein （AAG）, prealbumin （P-ALB） and retinol-binding protein （RBP） in gastric carcinoma patients and to explore their possible correlation with underlying Helicobacter pylori （H pylon） infection. METHODS： We measured the serum levels of CRP, TRF, A2M, CER, AAG, P-ALB, and RBP in 153 preoperative patients （93 males; mean age： 63.1±11.3 years） with non-cardia gastric adenocarcinoma and 19 healthy subjects. RESULTS： The levels of CRP, CER, RBP, and AAG in cancer patients were significantly higher than those in healthy controls （P〈0.0001）, while no difference was found regarding the TRF, P-ALB, and A2M levels. Cancer patients with H py/ori infection had significantly lower RBP values compared to non-infected ones （P〈0.0001） and also higher values of CRP and AAG （P = 0.09 andP = 0.08, respectively）. CONCLUSION： High serum levels of CRP, CER and AAG in cancer patients do not seem to be related to H pylori infection. Retinol-binding protein seems to discriminate between infected and non-infected patients with gastric carcinoma. Further studies are needed to explore if it is directly involved in the pathogenesis of the disease or is merely an epiphenomenon.

This paper presents a comprehensive study of spatial and temporal patterns of water chemistry (1995-2008) in the Yesilirmak River catchment in Northern Turkey. Biological oxygen demand (BOD), dissolved oxygen (DO) and nutrient concentrations (nitrogen and phosphorus) are variable across the catchment because the upland areas are relatively undisturbed, and the lower catchment is dominated by urban, industrial and agricultural inputs. Seasonally, high nutrient concentrations occur in winter possibly due to flushing from the soil zone. Low summer flow and reduced dilution lead to high orthophosphate concentrations. However, denitrification seems to be more significant than dilution processes and this generates low nitrate concentrations in summer. Nutrient levels since 1995 do not show a significant upward trend. The current water quality status indicates that the river system is in poor condition. The majority of sites fall in the Turkish water classification class II-III and more than half fail the EU standards because of high nutrient concentrations. In order to improve the status of water quality to achieve good chemical and ecological status, there is clearly a need to improve pollution control within the river system by installing waste water treatment plants, while keeping the agricultural pollution to a minimum in the system.

AIM: To determine the prevalence of steatosis and steatohepatitis in a series of autopsies in Northwestern Greece. METHODS: Liver biopsy material from a total of 600 autopsies performed over a period of 2 years (2006-2008) to def ine the cause of death was subjected to histological examination. Patient demographic data were also collected. Tissue sections were stained with different dyes for the evaluation of liver architecture, degree of fibrosis and other pathological conditions when necessary. RESULTS: Satisfactory tissue samples for histological evaluation were available in 498 cases (341 male, 157 female) with a mean age of 64.51 ± 17.78 years. In total, 144 (28.9%) had normal liver histology, 156 (31.3%) had evidence of steatosis, and 198 (39.8%) had typical histological findings of steatohepatitis. The most common causes of death were ischemic heart disease with or without myocardial infarction (43.4%), and traffic accidents (13.4%). CONCLUSION: A high prevalence of steatosis and steatohepatitis was detected in postmortem biopsies from Northwestern Greece. Since both diseases can have serious clinical consequences, they should be considered as an important threat to the health of the general population in Greece.

Incidence and economic impact of ophthalmological occupational accidents in Greece

Scope: This article seeks to assess the eco-nomic impact of industrial accidents with ref-erence to the incidence of opthalmological in-dustrial injuries occurring in the Thriacion Plain industrial zone in Greece during the period 1991-2000. Material and Methods: Data provided by 53 industrial units have been collected and classified. During the study period, 2,011 adults and juveniles out of the 15,600 people employed in the area (a 13% aggregate) suffered opthal-mological injuries occurring at the workplace. Results: The highest percentage of injuries were due to trauma (61% of adults, 67% of juveniles), followed by chemical burns. In the majority of cases (75% of adults and 50% of juveniles) treatment was provided at the local IKA (Social Security Fund) health care unit, in the town of Elefsis. It was estimated that a total of 10,192 working days were lost, with compensation costs (benefit payments) alone amounting to approximately € 118,000.00, not allowing for in-direct employer costs such as loss of produc-tivity, substitution and replacement costs. Con-clusions: The high incidence of industrial acci-dents in combination with the high percentage of child labor in the area of Thriacion Industrial Plain were striking and contributed significantly to high- er rates of work-related injuries of ocular interest in Greece. The latter were shown to result in a sig- nificant deterioration of the employees’ health status, with the majority of the accidents being due to the lack of safety precautions at the work-place at a considerable financial burden on the social security system in direct benefit payments. The social security system needs to press for tighter work safety regulations and their proper implementation if to minimize the burden these accidents impose on its budget.

The present work proposes a methodological approach for modeling decisions regarding energy reduction in an elevator. This is achieved with the integration of existing as well as acquired knowledge, in a decision module implemented in the electronics of an elevator. So far, elevators do not exploit information regarding their recent usage. In the developed system decisions are driven based on information arising from monitoring the use of the elevator. Monitoring provides various records of usage which consequently are used to predict elevator’s future usage and to adapt accordingly its functioning. Till now, there are only elevators that encompass in their electronics algorithms with if then rules in order to control elevator’s functioning. However, these if then rules are based only on good practice knowledge of similar elevators installed in similar buildings. Even this knowledge which unavoidably is associated with uncertainty is not encoded in a mathematically consisted way in the algorithms. The design, the implementation and a first pilot evaluation study of an elevator’s intelligent decision module are presented. The study concludes that the presented application sufficiently reduces energy consumption through properly controlled functioning.
