End-stage renal disease is associated with increased post endoscopic retrograde cholangiopancreatography adverse events in hospitalized patients

AIm To determine if end-stage renal disease (ESRD) is a risk factor for post endoscopic retrograde cholangio-pancreatography (ERCP) adverse events (AEs). mETHODS We performed a retrospective cohort study using the Nationwide Inpatient Sample (NIS) 2011-2013. We identified adult patients who underwent ERCP using the International Classification of Diseases 9^(th) Revision (ICD-9-Cm). Included patients were divided into three groups: ESRD, chronic kidney disease (CKD), and control. The primary outcome was post-ERCP AEs including pancreatitis, bleeding, and perforation determined based on specific ICD-9-Cm codes. Secondary outcomes were length of hospital stay, in-hospital mortality, and admission cost. AEs and mortality were compared using multivariate logistic regression *** There were 492175 discharges that underwent ERCP during the 3 years. The ESRD and CKD groups contained 7347 and 39403 hospitalizations respectively, whereas the control group had 445424 hospitalizations. Post-ERCP pancreatitis (PEP) was significantly higher in the ESRD group (8.3%) compared to the control group (4.6%) with adjusted odd ratio (aOR) = 1.7 (95% CI: 1.4-2.1, ~aP mpared to the control group 1.5% (aOR = 1.86, 95%CI: 1.4-2.4, ~aP mortality 7.1% vs 1.15% in the control OR = 6.6 (95%CI: 5.3-8.2, ~aP mean difference (amD) = 5.9 d (95% CI: 5.0-6.7 d, ~aP mD = $+82064 (95%CI: $68221-$95906, ~aP mortality. Careful selection and close monitoring is warranted to improve outcomes.

Instant evaluation of contrast enhanced endoscopic ultrasound helps to differentiate various solid pancreatic lesions in daily routine

BACKGROUND Contrast enhanced harmonic endoscopic ultrasound(CEH-EUS) is a spreading technique; some studies have shown its value in the diagnosis of pancreatic adenocarcinoma using quantitative *** To examine the value of CEH-EUS for differentiating various pancreatic lesions in everyday routine with qualitative and quantitative *** Data of 55 patients with pancreatic lesions who underwent CEH-EUS were analysed retrospectively. Perfusion characteristics were classified by the investigator qualitatively immediately upon investigation, quantitative analysis was performed later on. Samples from fine needle aspiration(EUS-FNA) or surgical specimen served as gold *** CEH-EUS showed 39 hypoenhanced lesions, 3 non-enhanced and 13 hyperenhanced lesions. Concordance of the investigators qualitative classification of peak contrast enhancement with quantitative analysis later on was 100%, while other parameters such as arrival time, time to peak or area under the curve did not show additional value. 34 of 39 hypoenhanced lesions were pancreatic adenocarcinoma; of the hyperenhanced lesions 4 were inflammatory, 3 neuroendocrine carcinomas, 1 lymphoma, 1 insulinoma and 4 metastases(2 of renal cell carcinoma, 2 of lung cancer). Non-enhanced lesions showed up as necroses. Sensitivity for the detection of pancreatic adenocarcinoma was 100%,specificity 87.2% for hypoenhancement alone; in otherwise healthy pancreatic tissue all hypoenhanced lesions were pancreatic adenocarcinoma(sensitivity and specificity 100%, PPV and NPV for adenocarcinoma 100%).CONCLUSION This study again shows the excellent value of CEH-EUS in everyday routine for diagnostics of various focal pancreatic lesions suggesting that qualitatively assessed hypoenhancement is highly predictive for adenocarcinoma. Effect of processing and accelerated storage on the volatile composition and sensory profile of a tomato soup

The volatile and sensory profile of pasteurized(95℃,18 min)and sterilized(120℃,14 min)tomato soup and its changes at accelerated storage conditions(37℃and 42℃)were identified,characterized,and correlated by predictive *** led to more profound changes over storage compared to pasteurisation,showing enhanced levels of‘dark'odour,‘burnt'flavour,and‘complexity',while pasteurized soup had more pronounced‘green'odour,‘vegetables'flavour,‘harmony',and‘freshness'.Over time,‘dark'and‘burnt'significantly increased for sterilized soups,whereas‘green'decreased significantly only for sterilized soup stored at 42℃.The changes of‘dark',‘green',‘off-flavour,‘burnt',‘har mony',and‘freshness'were associated with formation of furans(furfural and 2-acetyl-5-methylfuran),and decline of unsaturated aldehydes like E-2-octenal and E,E-2,*** compounds were identified to represent potential shelf-life markers,yet more studies on their odour activity in the tomato soup matrix are necessary to understand their contribution to the sensory *** construction of partial least squares models allowed the prediction of‘green',‘off-flavour',‘burnt',and‘freshness'based on selected *** models will be important tools in the process of predicting the end of shelf life,but need to be supplemented with consumer acceptability levels.

Can fecal microbiota transplantation cure irritable bowel syndrome?

To verify the utility of treatment with fecal microbiota transplantation (FmT) in patients with irritable bowel syndrome (IBS).mETHODSWe searched EmBASE, Cochrane Library and Pubmed in march, 2017. The reviewed literature was based on two systematic searches in each of the databases. The meSH terms used were IBS and fecal microbiota transplantation and the abbreviations IBS and FmT. Reference lists from the articles were reviewed to identify additional pertinent *** total of six conference abstracts, one case report, one letter to the editor, and one clinical review were included. In the final analysis, treatment of 48 patients was evaluated. Treatment revealed an improvement in 58% of cases. The varying structure of the nine included studies must be taken into *** on FmT and IBS are too limited to draw sufficient conclusions. Standardized double blinded randomized clinical trials need to be carried out to evaluate the effect of FmT on IBS.

Signal or cue: the role of structural colors in flower pollination

Angle dependent colors, such as iridescenee, are produced by structures present on flower petals changing their visual appearanee. These colors have been proposed to act as signals for plantinsect communication. However, there is a paucity of behavioral data to allow for interpretations of how to classify these colors either as a signal or a cue when con sidering the n atural con ditions under which pollination occurs. We sampled flowers from 6 plant species across various viewpoints looking for changes in the visual appearanee of the petals. Spectral characteristics were measured with different instruments to simulate both the spectral and spatial characteristics of honeybee's vision. We show the presenee of color patches produced by angle dependent effects on the petals and the calyx of various species;however, the appearance of the angle dependent color patches significantly varies with viewpoint and would only be resolved by the insect eye at close distances. Behavior experiments with honeybees revealed that pollinators did not use angle dependent colors to drive behavior when presented with novel flower presentations. Results show that angle dependent colors do not comply with the requirements of a signal for plant-pollinator communication since the information transmitted by these colors would be unreliable for potential, free-flying pollination vectors. We thus classify angle dependent colors produced by micro- and ultra-structures as being a cue (a feature which has not evolved for communication), and observe no evidence supporting claims of these angle dependent colors having evolved as visual signal.

Establishment of PEEK-Associated Drug Delivery Systems—Limits and Perspectives

In order to enhance promising properties of polyethertherketone (PEEK) for biomedical application, we evaluate two different strategies for the provision of PEEK with a drug delivery system at the example of the model drug chlorhexidine diacetate. On one hand we discuss the deposition of a biodegradable polymeric drug-release coating, based on well-established poly(L-lactide) (PLLA), on PEEK and on the other hand the modification of PEEK via sulfonation and its subsequent drug loading. Results are compared to pure PLLA-associated drug delivery systems and limits and perspectives are shown.

Initial joint bleed volume in a delayed on-demand treatment setup correlates with subsequent synovial changes in hemophilic mice

Background: Hemophilic arthropathy is a debilitating morbidity of hemophilia caused by recurrent joint bleeds. We investigated if the joint bleed volume, before initiation of treatment, was linked to the subsequent degree of histopathological changes and the development of bone pathology in a mouse model of hemophilic ***: FVIII knock-out(F8-KO) mice were dosed with a micro-CT blood pool agent prior to induction of hemarthrosis. Eight hours after induction, the bleed volume was quantified with micro computed tomography(micro-CT) and recombinant FVIII treatment initiated. On Day 8, inflammation in the knees was characterized by fluorescence molecular tomography. On Day 14, knee pathology was characterized by micro-CT and histopathology. In a second study, contrast agent was injected into the knee of wild-type(WT) mice, followed by histopathological evaluation on Day ***: The average joint bleed volume before treatment was 3.9 mm3. The inflammation-related fluorescent intensities in the injured knees were significantly increased on Day 8. The injured knees had significantly increased synovitis scores, vessel counts, and areas of hemosiderin compared to un-injured knees. However, no cartilage-or bone pathology was observed. The bleed volume before initiation of treatment correlated with the degree of synovitis and was associated with high fluorescent intensity on Day 8. In F8-KO and WT mice, persistence of contrast agent in the joint elicited morphological ***: When applying a delayed on-demand treatment regimen to hemophilic mice subjected to an induced knee hemarthrosis, the degree of histopathological changes on Day 14 reflected the bleed volume prior to initiation of treatment.
