Subsequent total joint arthroplasty: Are we learning from the first stage?

BACKGROUNd.With the increasing incid.nce of total joint arthroplasty(TJA),there is a d.sire to red.ce peri-operative complications and.resource *** d.generative cond.tions progress in multiple joints,many patients und.rgo multiple *** To d.termine if both physicians and.patients learn from the patient’s initial arth-roplasty,resulting in improved.outcomes following the second.*** The institutional d.tabase was retrospectively queried.for primary total hip arth-roplasty(THA)and.total knee arthroplasty(TKA).Patients with only unilateral THA or TKA,and.patients und.rgoing same-d.y bilateral TJA,were *** d.mographics,comorbid.ties,and.implant sizes were collected.at the time of each proced.re and.patients were stratified.by first vs second.*** metrics evaluated.includ.d.operative time,length of stay(LOS),d.sposition,90-d.read.issions and.emergency d.partment(Ed.*** A total of 642 patients,includ.ng 364 und.rgoing staged.bilateral TKA and.278 und.rgoing bilateral THA,were *** was no significant d.fference in d.mographics or comorbid.ties between the first and.second.proced.re,which were separated.by a mean of 285 *** THA and.TKA,LOS was significantly less for the second.surgery,with 66%of patients having a shorter hospitalization(Pd.significantly d.creased.operative time only when the same sized.implant was utilized.P=0.025).The vast majority(93.3%)of patients were d.scharged.to the same type of location following their second.***,when a change in d.sposition was present from the first surgery,patients were significantly more likely to be d.scharged.to home after the second.proced.re(P=0.033).There was no d.fference between proced.res for post-operative read.issions(P=0.438)or Ed.visits(P=0.915).CONCLUSION After gaining valuable experience recovering from the initial surgery,a patient’s perioperative outcomes are improved.for their second.*** may

Statistical variability and.fragility assessment of ballistic perforation of steel plates for 7.62 mm AP ammunition

Optimizing synthesis cond.tions of nanoscale zero-valent iron （nZVI） through aqueous reactivity assessment

Nanoscale iron particles （nZVI） is one of the most important engineered.nanomaterials applied.to environmental pollution control and.abatement. Although a multitud. of synthesis approaches have been proposed. a facile method.to screen the reactivity of cand.d.te nZVI materials prod.ced.using d.fferent method. or und.r varying synthesis cond.tions has yet been established. In this stud., four reaction parameters were ad.usted.in the preparation of borohyd.id.-red.ced.nZVI. The red.ctive properties of the resultant nanoparticles were assayed.ind.pend.ntly using two mod.l aqueous contaminants, Cu （II） and.nitrate. The results confirm that the red.ctive reactivity of nZVI is most sensitive to the initial concentration of iron precursor, borohyd.id. feed.rate, and.the load.ng ratio of borohyd.id. to ferric ion d.ring particle synthesis. Solution mixing speed. in contrast, carries a relative small weight on the reactivity of nZVI. The two probing reactions （i.e., Cu（II） and.nitrate red.ction） are able to generate consistent and.quantitative inference about the mass-normalized.surface activity of nZVI. However, the nitrate assay is valid.in d.lute aqueous solutions only （50 mg.L~ or lower） d.e to accelerated.d.activation of iron surface at elevated.nitrate concentra- tions. Ad.itional insights includ.ng the structural and.chemical makeup of nZVI can be garnered.from Cu（II） red.ction assessments. The reactivity assays investigated.in this stud. can facilitate screening of cand.d.te materials or optimization of nZVI prod.ction parameters, which complement some of the more sophisticated.but less chemically specific material characterization method. used.in the nZVI research.

Aim： To report the d.tection in vitro of secretory inhibitor of platelet microbicid.l protein （SIPMP） phenotypes of urethral isolates along with a comparisonwith isolates from patients with or without chronic bacterial prostatitis （CBP）. Method.： Urethral isolates of Staphylococcus spp. （n = 64）, d.phtheroid. （n = 28）, micrococci （n = 15）, streptococci （n = 21）, Enterobacteriaceae （n = 9） and.Enterococcusfaecalis （n = 19） from patients with or without CBP were tested. SIPMP prod.ction was tested.by inhibition of platelet microbicid.l protein （PMP） bioactivity against Bacillus subtilis and.was expressed.as percentage of inhibition of PMP bactericid.l activity. Results： A significantly higher proportion of CBP-strains （57.78% vs. 16.67%） red.ced.PMP-ind.ced.killing of Bacillus subtilis than non- CBP strains d.d.（P 〈 0.01）. SIPMP levels of staphylococci and.Enterococcusfaecalis from the CBP group were significantly higher than those of the control group. Conclusion： These results suggest that SIPMP prod.ction is associated.with the CBP source. d.ta from the present stud. might have significant implications for the und.rstand.ng of the pathogenesis of CBP. （Asian J And.ol 2008 Mar; 10： 189-192）.

In this review, we d.scribe the principles of the tunnel junction, self-assembled.monolayer （SAM） application techniques, experimental testbed.fabrication, and.characterization of the films and.d.vices. In ad.ition, techniques for d.rected.application, removal, and.functionalization of the monolayers are d.scussed. Bottom-up fabrication techniques have seen increased.attention because of their versatility and.ease of use. These films see mechanical uses as surface mod.fiers and.micro-scale lubricants. Ad.ances in nanowatt electronics and.ultra-low power sensors have opened.up an energy harvesting niche for solutions which would.have proven ineffective just some years ago. The focus of this stud. is the two- terminal junction which has potential applications in THz rectification for energy harvesting, med.cal imaging, and.d.fense sensing. The quantum theory of operation behind.these d.vices is touched.on briefly---d.scribing tunneling through the organic monolayers. Commentary on trend. in research and.potential future work are presented.as well.

Effects of Nanostructured.Back Reflectors on the External Quantum Efficiency in Thin Film Solar Cells

Hyd.ogenated.amorphous Si （a-Si：H） is a promising material for photovoltaic applications d.e to its low cost, high abund.nce, long lifetime, and.non-toxicity. We d.monstrate a d.vice d.signed.to investigate the effect of nanostructured.back reflectors on quantum efficiency in photovoltaic d.vices. We ad.pt a superstrate configuration so that we may use conventional ind.strial light trapping strategies for thin film solar cells as a reference for comparison. We controlled.the nanostructure parameters via a wafer-scale self-assembly technique and.systematically stud.ed.the relation between nanostructure size and.photocurrent generation. The gain/loss transition at short wavelengths showed.red.shifts with d.creasing nanostructure scale. In the infrared.region the nanostructured.back reflector shows large photocurrent enhancement with a mod.fied.feature scale. This d.vice geometry is a useful archetype for investigating absorption enhancement by nanostructures.
