Effect of itraconazole on the cornea in a murine suture model and penetrating keratoplasty model

AIM： To investigate the anti-（lymph）angiogenic and/or anti-inflammatory effect of itraconazole in a corneal suture model and penetrating keratoplasty（PK） model. METHODS： Graft survival, corneal neovascularization, and corneal lymphangiogenesis were compared among itraconazole, amphotericin B, dexamethasone, phosphate buffered saline（PBS） and surgery-only groups following subconjunctival injection in mice that underwent PK and corneal suture. Immunohistochemical staining and analysis were performed in each group. Real-time polymerase chain reaction（RT-PCR） was performed to quantify the expression of inflammatory cytokines（TNF-alpha, IL-6） and vascular endothelial growth factor（VEGF）-A, VEGF-C, VEGFR-2, and ***： In the suture model, the itraconazole group showed less angiogenesis, less lymphangiogenesis, and less inflammatory infiltration than the PBS group（all P〈0.05）. The itraconazole group showed reduced expression of VEGF-A, VEGFR-2, TNF-alpha, IL-6 than the PBS group（all P〈0.05）. In PK model, the two-month graft survival rate was 28.57% in itraconazole group, 62.50% in dexamethasone group, 12.50% in PBS group, 0 in amphotericin B group and 0 in surgery-only group. Graft survival in the itraconazole group was higher than that in the amphotericin, PBS and surgery-only group（P=0.057, 0.096, 0.012, respectively）. The itraconazole group showed less total angiogenesis and lymphangiogenesis than PBS group（all P〈0.05）.CONCLUSION： Itraconazole decrease neovascularization, lymphangiogenesis, and inflammation in both a corneal suture model and PK model. Itraconazole has anti-（lymph）-angiogenic and anti-inflammatory effects in addition to its intrinsic antifungal effect and is therefore an alternative treatment option in cases where steroids cannot be used.

Effect of sorafenib in a murine high risk penetrating keratoplasty model

AIM： To evaluate the effect of sorafenib in murine high risk keratoplasty model. ~ METHODS： Graft survival, corneal neovascularization, and corneal lymphangiogenesis were compared among the sorafenib, dexamethasone, dimethyl sulfoxide （DMSO）, and phosphate buffered saline （PBS） groups following subconjunctival injection in mice that underwent high risk penetrating keratoplasty （HRPK）. Real-time polymerase chain reaction was performed to quantify the expression of inflammatory cytokines and vascular endothelial growth factor （VEGF）-A, VEGF-C, vascular endothelial growth factor receptor （VEGFR）-2, VEGFR-3. RESULTS： The two-month graft survival rate for HRPK was 42.86% in sorafenib group, 37.50% in dexamethasone group, 0 in DMSO group, and 0 in PBS group. Sorafenib significantly increased graft survival compared to the DMSO and PBS group （P〈0.05）. The sorafenib didn＇t show significant effect in decreasing neovascularization compared with dexamethsone, DMSO, and PBS group. The sorafenib showed less total lymphangiogenesis than the dexamethasone, DMSO, and PBS group （P=0.011, P〈0.001, P〈0.001, respectively）. The sorafenib group showed reduced expression of VEGF-C, tumor necrosis factor （TNF）-alpha, interleukin （IL）-6, VEGFR-2 and VEGFR-3 compared with DMSO group and PBS group （all P〈0.05）. The sorafenib group didn＇t show difference in the expression of VEGF-A compared with DMSO, neither withPBS. The sorafenib group showed reduced expression of VEGFR-3 compared with dexamethasone （/〉=-0.051）. CONCLUSION： The subconjunctivally administered sorafenib shows significant anti-lymphangiogenic effect, resulting in increased transplant survival in a murine high risk keratoplasty model. We suggest that a close linkage between decreased VEGF-C/VEGFR-2 and -3 signaling and increased corneal graft survival by sorafenib seems to exist.

Centrifugal pumps are being widely used in many industrial and commercial *** of these pumps are being operated at constant speed but could provide energy savings through adjustable speed *** purpose of this study was to get the energy saving rates of the multistage centrifugal pump with variable speed *** this investigation an experimental set up of variable flow and pressure system was made to get energy saving rates and numerical analyses are applied to validate the pump *** energy saving and therefore the cost saving depends on the specific duty cycle of which the machine *** cycle is the proportion of time during which a component,device and system is *** duty cycle segmented into different flow rates and weighting the average value for each segment by the interval *** system was operated at 50%or less of the pump *** input power of the system was carried out by pump characteristics curve of each operating *** energy consumption was done by the product of specific duty cycle and the input power of the system for constant speed and variable speed drive *** total energy consumed for constant speed drive pump was 75,770 k *** and for variable speed drive pump was 31,700 k *** total energy saving of the system was 44,070 k *** or 58.16%***,this paper suggests a method of implementing an energy saving on variable-flow and pressure system of the multistage centrifugal pump.

BACKGROUND Snapping triceps syndrome(STS)is a rare disease,while occurrence of bilateral STS is extremely *** is usually accompanied by dislocation of the ulnar nerve and double snapping is a clinically important ***,to the best of our knowledge,there has been no report of bilateral STS in young active *** SUMMARY A 23-year-old male presented with a complaint of discomfort and snapping on the medial side of both elbows while performing *** physical examination,two distinct snaps that were both palpable and audible were detected on additional clinical *** ultrasonography showed that the ulnar nerve and the medial head of the triceps were dislocated anteriorly over the medial epicondyle of the elbow during flexion ***,he was diagnosed as dislocation of the ulnar nerve and *** anterior subcutaneous transposition of the ulnar nerve combined with partial resection of the snapping portion of the triceps was *** patient’s pain and snapping symptoms were resolved immediately after *** months later,the patient was completely asymptomatic and returned to normal *** STS should be included in the differential diagnosis for active young patients who present with painful snapping on the medial side of the elbow joint,particularly when dislocation of the ulnar nerve is *** sonography is used to assist in accurate diagnosis and differentiation between isolated dislocation of the ulnar nerve and STS.

Optimizing radiotherapy with immune checkpoint blockade in hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is the fifth most common cancer, and its incidence is rapidly increasing in North America and Western Europe as well as South-East Asia. Patients with advanced stage HCC have very poor outcomes;therefore, the discovery of new innovative approaches is urgently needed. Cancer immunotherapy has become a game-changer and revolutionized cancer treatment. A comprehensive understanding of tumor-immune interactions led to the development of immune checkpoint inhibitors (ICIs) as new therapeutic tools, which have been used with great success. Targeting immune checkpoint molecules such as programmed cell death-1 (PD-1) and cytotoxic T lymphocyteassociated protein-4 (CTLA-4) reinvigorates anti-tumor immunity by restoring exhausted T cells. Despite their effectiveness in several types of cancer, of the many immune suppressive mechanisms limit the efficacy of ICI monotherapy. Radiation therapy (RT) is an essential local treatment modality for a broad range of malignancies, and it is currently gaining extensive attention as a promising combination partner with ICIs because of its ability to trigger immunogenic cell death. The efficacy of combination approaches using RT and ICIs has been well documented in numerous preclinical and clinical studies on various types of cancers but not HCC. The application of ICIs has now expanded to HCC, and RT is recognized as a promising modality in HCC. This review will highlight the current roles of PD-1 and CTLA-4 therapies and their combination with RT in the treatment of cancers, including HCC. In addition, this review will discuss the future perspectives of the combination of ICIs and RT in HCC treatment.

The Application of the Korean Dietary Pattern Score;KNHANES (Korean National Health and Nutrition Examination Survey) 2007

The aim of this study was to apply the Korean Dietary Pattern Score (KDPS) to Korean subjects based on traditional Korean-style meals. The KDPS is based on the 3-Chup Bansang (the traditional Korean daily table setting) and considers the intake of 6 major food groups according to the Korean Dietary Reference Intakes (KDRIs). The KDPS consists of 2 parts: the Korean-Style Meal Score (KSMS) and the Food Group Score (FGS). The KDPS was applied dietary data collected during the Korean National Health and Nutrition Examination Survey (KNHANES) in 2007. Socio-demographic status, gender, age, and body mass index are all factors that have been shown to be meaningful predictors for the KDPS. When the effect of the KDPS on risk of disease was evaluated, a higher total KDPS was associated with a decreased risk of elevated systolic blood pressure (p cholesterolemia (p < 0.05). In addition, as the KSMS increased, the risk of central obesity (p < 0.05) and hypertension (p < 0.01) significantly decreased. A higher total KDPS suggests a greater diversity of food intake, and therefore greater dietary diversity appears to lower the risk of disease. The KDPS is relevant because it integrates scores for Korean-style meal patterns and meal evaluation from a nutritional perspective. In conclusion, the KDPS is appropriate for evaluating the quality of diet, adherence to Korean-style meal patterns, and risk of related diseases. These results will be useful for evaluating the nation’s dietary patterns, nutrition, and health status when planning nutrition policies and programs.

Stereotactic body radiotherapy for isolated paraaortic lymph node recurrence from colorectal cancer

AIM： To evaluate the efficacy and complications of stereotactic body radiotherapy in localized paraaortic lymph node recurrence from colorectal cancer. METHODS： From 2003 to 2009, 7 patients with paraaortic lymph node recurrence （1-3 lesions） from colorectal cancer were treated with stereotactic body radiotherapy. Total gross tumor volumes ranged from 4 to 40 mL. The doses were escalated from 36 Gy/patient to 51 Gy/patient and were delivered in 3 fractions. RESULTS： One and 3 year overall survival rates were 100% and 71.4%, respectively, and median survival was 37 mo. Grade IV intestinal obstruction was reported in 1 of 7 patients. This patient received 48 Gy in 3 fractions with a maximum point dose to the intestine of 53 Gy and V45Gy = 3.6 mL. However, 6 patients received an intestinal maximum point dose of 〈 51 Gy and V45Gy of 〈 1 mL, and did not develop any severe complications. CONCLUSION： This pilot study suggests selected paraaortic lymph node recurrence （1-3 closed lesions） that failed to respond to chemotherapy can be potentially salvaged by stereotactic body radiotherapy.

BACKGROUND Gastric cancer is the fifth most diagnosed cancer worldwide and the third most common cause of cancer-related *** recent decades,increasing application of next-generation sequencing has enabled detection of molecular aberrations,including *** cases where tissue is difficult to obtain,cell-free DNA(cfDNA)is used for detecting mutations to identify the molecular profile of ***,we report a rare case of EGFR-SEPT14 fusion detected from cfDNA analysis in a patient with gastric *** SUMMARY A 49-year-old female diagnosed with advanced gastric cancer in July 2019 received capecitabine and then combination chemotherapy of ramucirumab and paclitaxel,but ascites was *** therapy was switched to nivolumab,but disease progression was observed on a positron emission tomography/computed tomography scan in May *** was discontinued,and cfDNA nextgeneration sequencing was immediately *** genomic variants,including fusions,were analyzed from *** following somatic alterations were detected from the patient’s cfDNA:an APC frameshift mutation(NM_000038.5:c.6579del,p.V2194fs)with variant allele frequency of 0.5%,an EGFR amplification with a copy number of 17.3,and an EGFR-SEPT14 fusion with variant allele frequency of 45.3%.The site of the fusion was exon 24 of EGFR fused to exon 10 of *** fusion was in-frame and considered to be protooncogenic. Although the patient refused to continue therapy, we suggest thatEGFR-targeted therapies be tried in such future *** expanded applications of the cfDNA assay may open a new horizon intreatment of patients with advanced gastric cancer.
