Serial evaluation of myocardial function using the myocardial performance index in Kawasaki disease

Background Kawasaki disease (KD) is known as systemic vasculitis, and more than half of the patients with KD have myo-carditis, which can induce ventricular dysfunction. In this study, we evaluate left ventricular (LV) dysfunction in patients with KD based on the myocardial performance index (MPI) using pulse Doppler (PD) and tissue Doppler imaging (TDI), from the acute to convalescent phases. Methods We retrospectively studied 89 children diagnosed with KD from January 2010 to August 2012. We assessed the presence of coronary artery lesions (CALs) and the LV ejection fraction, PD-MPI, and TDI-MPI at diagnosis, and 2, 14, and 56 days after intravenous immunoglobulin (IVIG) treatment. We enrolled 70 healthy children as a control group. Results The ejection fraction in patients with KD at diagnosis (67.3 ± 0.9%) was lower than that in the control group (69.8 ± 0.8%,P = 0.035), and the LV TDI-MPIs for patients with KD at diagnosis (0.49 ± 0.01) and 2 days after IVIG treatment (0.48 ± 0.01) were higher than those in the control group (0.45 ± 0.01,P = 0.002,P = 0.033, respectively). No signifi cant diff erences were found in the LV dysfunction between the patients with complete and incomplete KD. Septal TDI-MPIs in patients with KD with CAL at diagnosis (0.52 ± 0.02) were higher than those in patients with KD without CAL (0.47 ± 0.01,P = 0.019). Conclusions Transient LV dysfunction occurred in patients with complete and incomplete KD in the acute stage. In patients with KD with CAL at diagnosis, the LV dysfunction was more prominent. The PD-MPI and TDI-MPI are useful parameters for assessing LV function in patients with KD.

Endoscopic resection has been an optimal treatment for selected patients with early gastric cancer(EGC) based on advances in endoscopic instruments and techniques. As endoscopic submucosal dissection(ESD) has been widely used for treatment of EGC along with expanding ESD indication, concerns have been asked to achieve curative resection for EGC while guaranteeing precise prediction of lymph node metastasis(LNM). Recently, new techniques including ESD or endoscopic full-thickness resection combined with sentinel node navigation enable minimal tumor resection and a laparoscopic lymphadenectomy in cases of EGC with high risk of LNM. This review covers the development and challenges of endoscopic treatment for EGC. Moreover, a new microscopic imaging and endoscopic techniques for precise endoscopic diagnosis and minimally invasive treatment of EGC are introduced.

Allergic asthma is a chronic inflammatory disease of the lung and the airway,which is characterized by aberrant type 2 immune responses to otherwise unharmful *** the central role of Th2 cells and type 2 cytokines in the pathogenesis of allergic asthma is well documented,the regulation and plasticity of Th2 cells remain incompletely *** using an animal model of allergic asthma in IL-4-reporter mice,we found that Th2 cells in the lung expressed higher levels of Rora than those in the lymph nodes,and that treatment with an RORa agonist SRI 078 resulted in diminished Th2 cell responses in *** determine the T cell-intrinsic role of RORa in allergic asthma in vivo,we established T cell-specific RORa-deficient^(f/f)fyf(Cd4creRora^(f/f))*** intranasal allergen challenges,Cd4creRora^(f/f)mice exhibited a significantly increased Th2 cells in the lungs and the airway and showed an enhanced eosinophilic inflammation compared to littermate control *** with Foxp3^(YFP-cre)Rora^(f/f)mice and CD^(8+)T cell depletion showed that the increased Th2 cell responses in the Cd4creRora^(f/f)mice were independent of Treg cells and CD^(8+)T *** findings demonstrate a critical regulatory role of RORa in Th2 cells,which suggest that RORa agonists could be effective for the treatment of allergic diseases.

The measurement and prediction of gas pulsations are performed along the discharge pipeline of a reciprocating compressor for a refrigerator. A regression based experimental model of the one-dimensional acoustic field is developed. First, the conventional method for gas pulsation measurement and prediction, which separates the incident and reflected wave of acoustic waves traveling in the frequency domain, is discussed. Then, regression based on our proposed simple model, which is able to predict gas pulsation compared to the conventional method, is introduced for the analysis of a reciprocating compressor(The conventional method requires the value of sound speed in the piping line for the reciprocating compressor). A numerical prediction is made for the regression method. Three power spectrum values along the discharge pipeline are used for analysis, and two values are used for verification. Our results are in a good agreement with the conventional method.

OBJECTIVE:The interleukin(IL)-12 cytokine family is closely related to the development of T helper cells,which are responsible for autoimmune disease enhancement or ***-12 family members are generally heterodimers and share threeα-subunits(p35,p19,and p28)and twoβ-subunits(p40 and EBI3).However,aβ-sheet p40 homodimer has been shown to exist and antagonize IL-12 and IL-23 signaling ***,we assumed the existence of a p40-EBI3 heterodimer in nature and sought to investigate its role in immune ***:The presence of the p40-EBI3 heterodimer was confirmed by ELISA,immunoprecipitation,and western blotting.A p40-EBI3 vector and p40-EBI3-Fc protein were synthesized to confirm the immunological role of this protein in mice with collageninduced arthritis(CIA).The anti-inflammatory effects of p40-EBI3 were analyzed with regard to clinical,histological,and immune cell-regulating features in mice with ***:Clinical arthritis scores and the expression levels of proinflammatory cytokines(e.g.,IL-17,IL-1β,IL-6,and TNF-α)were significantly attenuated in p40-EBI3-overexpressing and p40-EBI3-Fc-treated mice with CIA compared to vehicle-treated mice with *** joint damage and vessel formation-related gene expression were also reduced by p40-EBI3 heterodimer *** vitro,the p40-EBI3-Fc protein significantly suppressed the differentiation of Th17 cells and reciprocally induced CD4+CD25+Foxp3+(regulatory T)cells.p40-EBI3 also inhibited osteoclast formation in a concentrationdependent ***:In this study,p40-EBI3 ameliorated proinflammatory conditions both in vivo and in *** propose that p40-EBI3 is a novel anti-inflammatory cytokine involved in suppressing the immune response through the expansion of Treg cells and suppression of Th17 cells and osteoclastogenesis.

Stereotactic body radiotherapy for isolated paraaortic lymph node recurrence from colorectal cancer

AIM： To evaluate the efficacy and complications of stereotactic body radiotherapy in localized paraaortic lymph node recurrence from colorectal cancer. METHODS： From 2003 to 2009, 7 patients with paraaortic lymph node recurrence （1-3 lesions） from colorectal cancer were treated with stereotactic body radiotherapy. Total gross tumor volumes ranged from 4 to 40 mL. The doses were escalated from 36 Gy/patient to 51 Gy/patient and were delivered in 3 fractions. RESULTS： One and 3 year overall survival rates were 100% and 71.4%, respectively, and median survival was 37 mo. Grade IV intestinal obstruction was reported in 1 of 7 patients. This patient received 48 Gy in 3 fractions with a maximum point dose to the intestine of 53 Gy and V45Gy = 3.6 mL. However, 6 patients received an intestinal maximum point dose of 〈 51 Gy and V45Gy of 〈 1 mL, and did not develop any severe complications. CONCLUSION： This pilot study suggests selected paraaortic lymph node recurrence （1-3 closed lesions） that failed to respond to chemotherapy can be potentially salvaged by stereotactic body radiotherapy.

BACKGROUND Pyogenic infectious spondylitis(PIS)is a rare condition,with an incidence between 0.2 and 2 cases per 100000 per ***’s most common symptom-back or neck pain-occurs in more than 90%of ***,we reported a case of thoracic PIS accompanied by pneumothorax in a 65-year-old male *** SUMMARY A 65-year-old man presented with right chest pain and *** initial erect posteroanterior chest radiography revealed pneumothorax,which was further evaluated by chest computed tomography,revealing pleural effusion in the right lung and a paravertebral abscess with bony destruction of vertebral *** on magnetic resonance imaging,the patient was diagnosed with thoracic infectious spondylitis with an anterior paravertebral *** was prescribed antibiotics and underwent neurosurgery due to aggravated symptoms and neurologic *** examination revealed that the cause of pleural effusion and pneumothorax was Staphylococcus aureus infection contiguously spread to lung *** several surgical treatments with intravenous antibiotic therapy for two months and transition to oral antibiotics(rifampin 600 mg qd and ciprofloxacin 500 mg bid),the patient received physical therapy to recover *** month after discharge,the patient had no chest pain or dyspnea,and exhibited no elevation in inflammatory markers or new thoracic *** To our knowledge,this is the very first report of a case of thoracic PIS with pneumothorax.

Anti-inflammatory Effects of Sanguisorbae Radix on Contact Dermatitis Induced by Dinitrofluorobenzene in mice

Objective:To investigate the anti-inflammatory effects of Sanguisorbae Radix on contact dermatitis(CD).Methods:mice were sensitized by painting 30μL of 1-fluoro-2,4-dinitrofluorobenzene(DNFB)onto each ear for 3 *** days later,mice were challenged by painting with 50μL of DNFB onto the shaved dorsum every 2 *** Radix methanol extract(MESR)was applied onto the shaved dorsum every 2 *** effects of MESR on skin thickness,skin weights,histopathological changes,skin lesions and cytokine production in DNFB-induced CD mice were investigated,as well as its effects on body weights and spleen/body weight ***:Topical application of MESR effectively inhibited enlargement of skin thickness and weight(P<0.05).MESR treatment also inhibited hyperplasia,spongiosis and immune cell infiltration induced by DNFB in inflamed tissues and improved lesions on dorsum skin in CD ***,treatment with MESR suppressed the increase in the levels of tumor necrosis factor α(TNF-α,P<0.01)and interferonγ(IFN-γ,P<0.05),***,MESR had no effect on body weight gain or spleen/body weight ***:These data suggest that MESR acts as an anti-inflammatory agent that decreases the production of TNF-α and IFN-γ,resulting in reductions of skin lesions and histopathological changes in inflamed skin tissues.
