BACKGROUND:Biliary cast syndrome(BCS)is an unusual complication of orthotopic liver transplantation (OLTx),and its management is *** success using endoscopic retrograde cholangiopancreatography (ERCP)or open exploration to clear casts has been reported, but failure usually results in *** aimed to review our experience with BCS and highlight a novel combined percutaneous and endoscopic approach for duct clearance.A brief review of the literature is given. Bacterial endocarditis is a rare complication amongst solid organ transplant recipients and is often linked to bacteremia. Majority of these recipients do not have underlying valvular heart disease or congenital valvular abnormalities. Staphylococoocusaureus and Enterococcus species are the most commonly isolated organisms. There are very few reports of gram-negative bacteria causing endocarditis in liver transplant recipients. We report a 51-yearold male, a liver transplant recipient, who developed bacterial endocarditis of the mitral valve due to extended spectrum of betalactamase producing strain of Escherichia coli and was managed successfully with antibiotics.

The dilute sulfuric acid pretreatment of lignocellulosic biomass is a well understood process that significantly enhances the yield of glucose after enzymatic *** goal of this research was to perform a systematic study to evaluate the yield of fermentable sugars during dilute sulfuric acid pretreatment that is co-catalyzed with the transition metal Lewis acid salts:AlCl_(3),FeCl_(2),FeCl_(3),and La(OTf)_(3).All Lewis acids apart from FeCl_(2)reduced the presence of xylo-oligomers by a large margin when compared to the non-co-catalyzed control sample *** presence of these xylo-oligomers acts as inhibitors during enzymatic saccaharification *** Lewis acids AlCl_(3),FeCl_(3),and La(OTf)_(3)were also able to marginally increase the overall enzymatic digestibility specifically for corn stover pretreated at 160℃with 10 mM of Lewis *** hard Lewis acid such as AlCl3 increased the formation inhibitory products such as furfural and 5-hydroxymethylfurfural(HMF).There was good correlation between reduction of xylo-oligomers and increased concentration furfural with increase in Lewis acid hardness.

Porous silica prepared by using an acrylic emulsion has been impregnated with bismuth ion resulting in Bi2Si05 species containing surface. The as-prepared materials have been characterized by X-ray diffraction spectroscopy （XRD）, X-ray photoelectron spectroscopy （XPS）, UV-Vis diffuse reflectance spectroscopy （UV-Vis DRS）, scanning electron microscopy （SEM）, energy dispersive analysis of X-ray （EDAX）, transmission electron microscopy （TEM）, Fourier transform infrared spectroscopy （FTIR） and N2 adsorption/desorption techniques. EDAX analysis confirms the penetration of bismuth ions into the framework of silica to form Bi2SiO5, which is substantiated by XRD. The UV-Vis DRS shows that the catalysts are optically active and XPS confirms the inclusion of bismuth into the framework of silica. FTIR spectra illustrate the formation of Bi-O-Si linkages in the porous silica framework. SEM and TEM show the spherical morphology, whereas N2 adsorption/desorption study confirms the porosity of the prepared materials. The photocatalytic activity of the material is evaluated for the degradation of isoproturon herbicide and it is found that the material is active as compared to the commercial P-25 Degussa TiO2.

Novel en-bloc resection of locally advanced hilar cholangiocarcinoma: the Rex recess approach

Loco-regional recurrence after potentially curative resection remains a problem in hilar cholangiocarcinoma. Hilar dissection risks local spillage of tumor cells leading to suboptimal disease free survival. We have developed a new technique of radical resection for hilar cholangiocarcinoma based on the distinctive anatomy of the Rex recess of the liver, which has been assessed in two patients with locally advanced hilar cholangiocarcinoma. This technique included a right hepatectomy with en-bloc resection of the hepatoduodenal ligament and portal venous reconstruction to the left portal vein at the Rex recess. Both patients had R0 resection and have been disease-free for 26 and 38 months, respectively.

How good is endoscopic ultrasound for TNM staging of gastric cancers? A meta-analysis and systematic review

AIM: To evaluate the accuracy of endoscopic ultrasound (EUS) for staging of gastric cancers. METHODS: Only EUS studies confirmed by surgery were selected. Only studies from which a 2 × 2 table could be constructed for true positive, false negative, false positive and true negative values were included. Articles were searched in Medline, Pubmed, Ovid journals, Cumulative index for nursing & allied health literature, International pharmaceutical abstracts, old Medline, Medline nonindexed citations, and Cochrane control trial registry. Two reviewers independently searched and extracted data. The differences were resolved by mutual agreement. 2 × 2 tables were constructed with the data extracted from each study. Meta-analysis for the accuracy of EUS was analyzed by calculating pooled estimates of sensitivity, specifi city, likelihood ratios, and diagnostic odds ratio. Pooling was conducted by both the Mantel-Haenszel method (fi xed effects model) and DerSimonian Laird method (random effects model). The heterogeneity of studies was tested using Cochran's Q test based upon inverse variance weights. RESULTS: Initial search identified 1620 reference articles and of these, 376 relevant articles were selected and reviewed. Twenty-two studies (n = 1896) which met the inclusion criteria were included in this analysis. Pooled sensitivity of T1 was 88.1% (95% CI: 84.5-91.1) and T2 was 82.3% (95% CI: 78.2-86.0). For T3, pooled sensitivity was 89.7% (95% CI: 87.1-92.0). T4 hada pooled sensitivity of 99.2% (95% CI: 97.1-99.9). For nodal staging, the pooled sensitivity for N1 was 58.2% (95% CI: 53.5-62.8) and N2 was 64.9% (95% CI: 60.8-68.8). Pooled sensitivity to diagnose distant metastasis was 73.2% (95% CI: 63.2-81.7). The P for chi-squared heterogeneity for all the pooled accuracy estimates was > 0.10. CONCLUSION: EUS results are more accurate with advanced disease than early disease. A kinetic study of acid pretreatment was conducted for sorghum non-brown mid rib(SNBMR)(Sorghum bicolor L Moench),sorghum-brown mid rib(SBMR),sunn hemp(Crotalaria juncea L)and kenaf(Gossypiumhirsutum L),focusing on rates of xylose monomer and furfural *** kinetics was investigated using two independent variables,reaction temperature(150°C and 160°C)and acid concentration(1 and 2 wt%),with a constant dry biomass loading of 10 wt%and a treatment time up to 20 min while sampling the mixture every 2 *** experimental data were fitted using a two-step kinetic model based on irreversible pseudo first order kinetics at each *** kinetic orders on the acid concentration,ranging from 0.2 to>3,were observed for both xylose and furfural formation,the values depending on the *** crystallinity index of raw biomass was shown to be a major factor influencing the rate of both xylose and furfural formation.A positive correlation was observed between the activation energy and biomass crystallinity index for xylose formation.
