Background: 70% of all gynecological consultations are for abnormal uterine bleeding. Any approach to optimal management begins with an appropriate diagnosis. 40% of premenopausal women with abnormal uterine bleeding were found to have some intrauterine pathology. This study aims to compare the diagnostic value of transvaginal ultrasonography in comparison to hysteroscopy in detecting uterine abnormalities in patients with abnormal uterine bleeding. Methods: Retrospective observational cross sectional study of 250 women presented with abnormal uterine bleeding. The patients who fulfilled the selection criteria and have been sequentially investigated by transvaginal ultrasound (TVS) and hysteroscopy were included. Results: 90% of patients were from 35 - 49 yrs. 81.2% of patients have body mass index above 25 kg/m2. Transvaginal ultrasound (TVS) compared well with high sensitivity as regards normal endometrium. (TVS) missed 4 patients of endometrial polyps and one patient of sub mucous fibroid. Three patients of adenomyosis were only diagnosed by (TVS);they were reported as being normal by hysteroscopy. Conclusion: (TVS) is considered as an excellent approach to the initial evaluation of uterine pathologies in patients with abnormal uterine bleeding.

A novel 3-D graphical representation of protein sequence has been introduced. A right cone of a unit base and unit height has been selected to represent protein sequences on its surface. The twenty amino acids have been represented by 20 circles and all protein's residues have been represented by n lines on the cone's surface. All the spots which represent the protein's residues have been shown in the cone's top view. The spatial median of all the spots is used as a new descriptor of any protein sequence. This approach was applied on two short segments of protein of yeast Saccharomyces cerevisiae. The examination of the similarities/dissimilarities for the eight ND5 proteins and the six β-globin proteins illustrate the utility of our approach. A linear correlation and significance analysis have been provided to compare our results and the percentage sequence alignment identity.

Objective:To evaluate vascular endothelial growth factor(VEGF)levels in hepatocellular carcinoma patients before and after transcatheter arterial chemoembolization(TACE)and percutaneous ethanol injection(PEI)and its relation to treatment ***:A total of 40 patients with unrespectable hepatocelluar carcinoma were assessed *** patients were suitable to be treated by TACE,while other 20patients were treated with *** VEGF levels were measured before and 1 month after each procedure by *** was assessed after 1 month according to Union Internationale Contre le Cancer evaluation criteria based on change in tumor size as measured by ***:There was no significant difference between TACE and PEI groups with regard to age,sex,tumor size,response to local therapy,or VEGF and alpha-fetoprotein before and after *** levels after TACE were significantly higher than before TACE[(298.1±123.6)pg/m L vs.(205.8±307.3)pg/m L;P=0.001].Also,VEGF levels were significantly higher after PEI than before PEI[(333.8±365.6)pg/m L vs.(245.3±301.8)pg/m L;P=0.000].Non-responders of both groups had significantly high VEGF levels than responder's,both before[(985.0±113.2)pg/m L vs.(117.1±75.3)pg/m L;P<0.001]and after therapy[(1 330.6±495.7)pg/m L vs.(171.0±94.7)pg/m L;P=0.000)].Conclusions:Both TACE and PEI were associated with an increase in serum VEGF in hepatocelluar carcinoma *** levels of VEGF before and after therapy were found in non-responders,suggesting that VEGF is a useful marker in predicting treatment response.

This study aimed at investigating the effect of Facebook on grammar discussion and writing skill in English as a foreign language for university students, The study sample consisted of 60 students at Oklt AI Sqoor College of Science and Arts in Saudi Arabia and was divided into two groups： 30 students for the experimental group and 30 students for the control group. The instrument of the study was an EFL grammar and writing test. Students in both the experimental and the control group were pre-tested using the EFL （English as a Foreign Language） grammar and writing test, After that the experimental group was taught using Facebook while the control group was taught using the traditional method. Finally, students in both groups were post-tested using the EFL grammar and writing test. Results of the study showed that the experimental group outperformed the control group in the post-test of EFL grammar and writing. This difference can be attributed to using Facebook.

This study aimed at investigating the effect of using a Web Quest program on developing some EFL critical reading and writing skills of the first year experimental secondary school students. The participants of the study were 60 students from one of the Experimental Secondary Schools in Egypt. Instruments of the study were EFL critical reading and writing tests. Students in both the experimental and the control groups were pre-tested using the critical reading test and the writing test. Then, students of the experimental group were using Web Quest. Finally, students in both groups were post-tested using the same instruments. The study showed that there was statistically significant difference between the mean scores of the pre and the post critical reading test and writing test of the experimental group in favor of the post tests. This difference can be attributed to using Web Quest.

Aim of the study: To identify all clinico-pathological data, different treatment modalities and the different prognostic factors which affected the locoregional control (LC), disease-free survival (DFS), and overall survival (OS) of Type II endometrial cancer patients. Patients and methods: Data of Type II endometrial carcinoma patients who presented to the Radiation Oncology department, National Cancer Institute, Cairo University during the period from (2000-2012) were retrospectively reviewed. Results: Multivariate analysis identified stage as an independent prognostic factor for OS & DFS, and age was an independent prognostic factor for DFS and LC. Low pretreatment hemoglobin levels significantly affected OS. Conclusion: Large and multicentric clinical trials are required to further study this group of patients and define optimum treatment modalities.

Purpose: Radiotherapy with concurrent temozolomide (TMZ), followed by 6 cycles of adjuvant TMZ, is the standard of care for newly diagnosed Glioblastoma Mulltiforme (GBM). However tumor progression is the role with median survival of almost 14 months. With lack of effective second line chemotherapy, many physicians and some guidelines advocate prolonged use of adjuvant TMZ more than 6 months. We conduct this study to test the efficacy of protracted adjuvant conventional dose TMZ over the standard 6 doses of adjuvant TMZ. Material and Methods: This phase II trial enrolled patients newly diagnosed as GBM, older than age 18 years, with a Karnofsky performance score (KPS) of ≥60, Neurological Performance Scale (NPS) of ≤3. Patients were randomly assigned to the standard concurrent chemoradiotherapy (CCRT) followed by 6 cycles of adjuvant TMZ or the same treatment with more than 6 cycles of adjuvant chemotherapy extended as long as the patient in good performance, with no unacceptable toxicity, no signs of disease progression. The primary end point was OS. Results: A total of 59 patients were recruited in the study and were randomized in two arms. 29 patients joined arm 1 aiming at receiving CCRT followed by adjuvant 6 cycles TMZ (6 cycles arm) and 30 joined arm 2 aiming at receiving the same treatment with more than 6 cycles of TMZ (>6 cycles). 16 patients managed to complete the adjuvant 6 cycles in arm 1. 19 patients in arm 2, completed the 6 cycles with additive more doses with a median of 11 cycles (range: 8 - 23 cycles). Median PFS was 12.1 months for (6 cycles) arm, and 18.8 months for (>6 cycles) arm, HR 0.88 (95% CI: 1.185 - 4.901) (P 0.015);the overall survival for (6 cycles) arm was 18.1 months, versus 24.1 months, HR 0.70 (95% CI: 1.007 - 4.037) (P 0.048). No significant added toxicity was notice and the 4 weekly TMZ was well tolerated. Small non-protein coding micro-RNAs are regularly exported out of cells, both in health and disease. More than ninety percent of extracellular miRNAs are associated with lower-molecular-mass complexes bound to Argonaute 2 (Ago2), nucleophosmin-1 (NPM1) and high density lipoproteins (HDL), whereas the rest (~10%) are membrane-vesicle-encapsulated within exosomes, shedding microvesicles and apoptotic bodies. Regardless of the debate of the nature of circulating miRNA as byproducts of routine cell activities or mediators of cell-cell communication, proper understanding of the molecular behaviors of miRNA in health and disease, is expected to open a new gate for the discovery of new diagnostic tools and possibly therapeutic implementation in the near future.

Changes of MicroRNA-377 in Diabetic Nephropathy

Background: MicroRNAs (miRNAs) play important roles in modulating gene expression. In cultured mesangial cells, MiR-377 exhibited the greatest change on exposure to high glucose and led to increased fibronectin production in in-vitro diabetic nephropathy. Our aim was to identify the pattern of microRNA-377 changes in human diabetic patients with different stages of diabetic nephropathy. Methods: The study included 45 patients divided into 5 groups;patients with stage 1 & 2 diabetic nephropathy (DN), stage 3 & 4 DN, diabetics without DN, stage 1 & 2 chronic kidney disease (CKD), and stage 3 & 4 CKD. The following tests were done to all patients;serum creatinine, estimated glomerular filtration rate (eGFR), albumin to creatinine ratio, blood sugar, urine analysis, glycated hemoglobin, fundus examination. The RNA was extracted from plasma samples, TaqMan microRNA (miRNA) assays from applied Bio systems were used for analysis of MicroRNA-377. Results: This study found that patients with the highest median of the interquartile range (IQR) of miR-377 were those of group 1 with significant difference between them and all other groups. MiR-377 was significantly correlated with glycated hemoglobin but not with eGFR. Conclusion: Plasma miR-377 is highly significantly increased in human diabetic patients with early rather than late diabetic nephropathy, diabetics without nephropathy, or those at any stage of other causes of CKD. Our findings confirm the role of miR-377 as a potentially novel target in the development of diabetic nephropathy in humans.
