Conservative management of spinal pathology with autologous conditioned serum: A systematic review of the literature

BACKGROUNd Chronic inflammatory pain is associated with increased expression of interleukin(IL)-1,an inflammatory cytokine,and activity on its receptor(IL-1R).In response,the body produces IL-1R antagonist(IL-1Ra)to reduce this *** conditioned serum(ACS)is the only biologic therapy for spinal pathologies that enhances the action of endogenous IL-1Ra reserves to improve *** systematic review investigates the effectiveness of ACS in treating pain and dis-ability caused by spinal *** To evaluate the use of ACS as a conservative management option for spinal *** A systematic review of PubMed/Medline was performed to identify studies inve-stigating administration of ACS for treatment of any spinal *** Six articles were included,comprising 684 patients treated with epidural(n=133)or transforaminal(n=551)ACS *** had an average age of 54.0 years with slight female predominance(53.2%).The lumbar spine was most com-monly treated,with 567 patients(82.9%)receiving injections for lumbar radicu-lopathy(n=67),degenerative disc disease(ddd)(n=372),or spinal stenosis(n=128);cervical injections were performed in 109 patients(15.9%).Mean(Sd)follow-up was 21.7(4.8)weeks from first ACS *** studies investigating mecha-nical lumbar and lumbar or cervical radicular pain reported significant pain re-duction at final follow-up compared to *** achieved comparable or su-perior results to lumbar epidural steroid *** events were reported in 21 patients(3.1%),with no serious adverse *** ACS injection is a safe and effective intervention for pain reduction in many spinal pathologies,including cervical and lumbar radiculopathies.

Background Examining the health outcomes of veterans who have completed the United States Veterans Health Administration’s(VHA’s)Traumatic Brain Injury(TBI)Screening and Evaluation Program may aid in the refinement and improvement of clinical care initiatives within the *** study compared self-reported physical functioning,cardiometabolic health conditions,and health care utilization patterns in Million Veteran Program enrollees with TBI Screening and Evaluation Program data(collected between 2007 and 2019),with the goal of enhancing understanding of potentially modifiable health conditions in this *** In this observational cohort study,veterans(n=16,452)were grouped based on the diagnostic outcome of the TBI Screening and Evaluation Program:1)negative TBI screen(Screen^(-));2)positive TBI screen but no confirmed TBI diagnosis[Screen^(+)/Comprehensive TBI Evaluation(CTBIE)^(-)];or 3)positive TBI screen and confirmed TBI diagnosis(Screen^(+)/CTBIE^(+)).Chi-square tests and analysis of covariance were used to explore group differences in physical functioning,cardiometabolic health conditions,and health care utilization patterns,and logistic regressions were used to examine predictors of Screen^(+/-)and CTBIE^(+/-)group *** The results showed that veterans in the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups generally reported poorer levels of physical functioning(P’sdiometabolic health conditions(P’sd increased health care utilization(P’s0.5)compared with the Screen-group;however,health outcomes were generally comparable between the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)***-up analyses confirmed that while physical functioning,hypertension,stroke,healthcare utilization,and prescription medication use reliably distinguished between the Screen-and Screen^(+)groups(P’sdistinguished bet

developing one-dimensional implosions for inertial confinement fusion science

Experiments on the National Ignition Facility show that multi-dimensional effects currently dominate the implosion performance. Low mode implosion symmetry and hydrodynamic instabilities seeded by capsule mounting features appear to be two key limiting factors for implosion performance. One reason these factors have a large impact on the performance of inertial confinement fusion implosions is the high convergence required to achieve high fusion *** tackle these problems, a predictable implosion platform is needed meaning experiments must trade-off high gain for performance. LANL has adopted three main approaches to develop a one-dimensional(1d) implosion platform where 1d means measured yield over the 1d clean calculation. A high adiabat, low convergence platform is being developed using beryllium capsules enabling larger case-to-capsule ratios to improve symmetry. The second approach is liquid fuel layers using wetted foam targets. With liquid fuel layers, the implosion convergence can be controlled via the initial vapor pressure set by the target fielding temperature. The last method is double shell targets. For double shells, the smaller inner shell houses the dT fuel and the convergence of this cavity is relatively small compared to hot spot ignition. However,double shell targets have a different set of trade-off versus advantages. details for each of these approaches are described.

Objective. -To describe the efficacy and tolerability of rapid intravenous valproic acid (VPA) infusions in children with severe migraine headache. Background. -Intravenous VPA is an emerging treatment option for acute migraine headache. Adult data suggests both efficacy and tolerability of rapid VPA infusions as abortive therapy, but little data exist in children. Methods. -We conducted a retrospective chart review of all children who received intravenous VPA at The Children’s Hospital Headache Clinic during an 18-month study period. Baseline intensity of headache pain, time at which maximum relief was attained, pain reduction following therapy, dose and duration of VPA infusion(s), patient’s pulse, blood pressure, respiratory rate, and pulse oximetry were collected. Adverse events were also recorded. Results. -Thirty-one children (age =15 ±2 years; 81%female) requiring 58 clinic visits and 71 VPA infusions were included. Most visits (n=45; 78%) resulted in only one dose of VPA (976 ±85 mg infused over 12 ±4 minutes) for desired pain relief. Percent pain reduction in those children was 39.8%, with time to maximum relief of 63 ±.31 minutes. Some children required a second dose of 500 mg (n=13 visits; 22%), that was infused over 14 ±6 minutes and produced a 57%reduction in pain intensity from baseline. VPA infusions were well tolerated. Adverse events described included cold sensation (1), dizziness (3), nausea (1), possible absence seizure (1), paraesthesia (2), and tachycardia (2). Conclusions. -Rapid infusion of intravenous VPA is generally well tolerated and may play a role in the management of children with acute migraine headache. Prospective, controlled trials to further investigate this treatment in children are warranted.

背景抗Pd-1/Pd-L1单药治疗(肿瘤免疫单药治疗(CIT-mono))或联合铂类化疗(CIT-chemo)目前是转移性NSCLC患者的一线治疗方案。本研究旨在比较CITmono或CIT-chemo治疗对Pd-L1高表达即肿瘤比例评分或肿瘤细胞≥50%的非鳞状(Nsq)-NSCLC患者的疗效。方法基于去识别信息的真实世界数据的回顾性队列研究,纳入2016年10月24日至2019年2月28日期间,接受CITmono或CIT-chemo一线治疗的Pd-L1高表达的转移性Nsq-NSCLC患者,随访数据截至2020年2月28日。采用Kaplan-Meier法比较患者的总生存期(OS)和真实世界无进展生存期(rwPFS)。采用逆概率治疗加权法,根据影响预后的关键基线特征的差异调整风险比(aHR)。结果应用CIT-mono治疗的Pd-L1高表达Nsq-NSCLC患者较应用CITchemo治疗的患者年龄更大,且新发Ⅳ期肺癌更少。CITchemo组患者的中位随访时间为19.9个月,CIT-mono组为23.5个月,两组患者的中位OS和rwPFS无显著差异。对于既往无吸烟史的患者,与CIT-mono相比,CIT-chemo使患者的OS和rwPFS获得显著且有意义的改善,但是样本量较小(n=50;OS:HR=0.25,95%CI:0.07~0.83,交互P=0.02;rwPFS:HR=0.40,95%CI:0.17~0.95,交互P=0.04)。结论除了既往无吸烟史的患者亚组,对于一线CIT治疗方案,联合化疗并不能改善Pd-L1高表达Nsq-NSCLC患者的生存结局。
