Non-alcoholic fatty liver disease(NAFlD)is currently considered the most common cause of liver *** prevalence is increasing in parallel with the obesity and type 2 diabetes mellitus(DM2)epidemics in developed *** recent studies have suggested that NAFlD may be the hepatic manifestation of a systemic inflammatory metabolic disease that also affects other organs,such as intestine,lungs,skin and vascular *** appears that local and systemic proinflammatory/anti-inflammatory cytokine imbalance,together with insulin resistance and changes in the intestinal microbiota,are pathogenic mechanisms shared by NAFlD and other *** is more common in patients with extrahepatic diseases such as inflammatory bowel disease(IBD),obstructive syndrome apnea(OSA)and psoriasis than in the general ***,there is evidence that this association has a negative impact on the severity of liver *** risk characteristics for NAFlD have been identified in populations with IBD(***,obesity,DM2,previous bowel surgery,IBD evolution time,methotrexate treatment),OSA(***,DM2,OSA severity,increased transaminases)and psoriasis(***,metabolic factors,severe psoriasis,arthropathy,elevated transaminases,methotrexate treatment).These specific phenotypes might be used by gastroenterologists,pneumologists and dermatologists to create screening algorithms for *** algorithms should include non-invasive markers of fibrosis used in NAFlD to select subjects for referral to the ***,controlled studies in NAFlD patients with extrahepatic comorbidities are required to demonstrate a causal relationship and also that appropriate multidisciplinary management improves these patients’prognosis and survival.

We develop the theory of multivariate saddlepoint approximations. Our treatment differs from the one in Barndorff-Nielsen and Cox （1979, equation （4.7）） in two aspects： 1） our results are satisfied for random vectors that are not necessarily sums of independent and identically distributed random vectors, and 2） we consider that the sample is taken from any distribution, not necessarily a member of the exponential family of densities. We also show the relationship with the corresponding multivariate Edgeworth approximations whose general treatment was developed by Durbin in 1980, emphasizing that the basic assumptions that support the validity of both approaches are essentially similar.

Background: Increased peak oxygen consumption(VO2 peak) can reduce cardiovascular risks associated with obesity. Our aim was to analyze the effect of a weight loss program on cardiovascular fitness in overweight(W) and obese(O) ***: One hundred and sixty-seven subjects(77 males and 90 females), aged 18–50 years, performed a modified Bruce protocol before(pre)and after(post) a weight loss program of 24 weeks. This program combined physical training(strength, S; endurance, E; combined strength + endurance, SE; or physical activity recommendation, PA) 3 times per week, with a 25%–30% caloric restriction ***: VO2 peak improved in overweight and obese males(pre and post values in l/min, respectively; W = 3.2 ± 0.6 vs. 3.7 ± 0.5, p ll as in overweight females(2.0 ± 0.3 vs. 2.3 ± 0.4, p latory threshold(VT1) increased for all 4 interventions in males(p les, it increased in E(0.9 ± 0.2 vs. 1.4 ± 0.3, p ll subjects except for females in PA group(15.7 ± 0.3 min vs. 15.9 ± 0.3 min, p = 0.495).Conclusion: Our results suggest that all methods, including the recommendation of physical activity, can improve cardiovascular fitness in overweight subjects and obese males.

Background: Gastroesophageal reflux disease (GERD) is a prevalent disease in Western countries. despite effective treatment modalities, in some patients total symptom control is not achieved in clinical practice. A cross-sectional study was designed to assess primary care practitioners’ views on the effectiveness of proton pump inhibitors (PPI) as monotherapy in the control of the most common symptoms of GERD (heartburn and regurgitation), as well as to determine the level of implementation of the “combined therapy” (PPI + alginate-antacids). Methods: A questionnaire on different aspects of the management of GERD was completed by 1491 primary care physicians. The questionnaire was composed of 11 close-ended questions with one-choice answer, with a total of 52 items, covering the main data from patients presenting with GERD. Results: Treatment with PPI alone was mostly considered insufficient for the control of GERD symptoms. The combined treatment of PPI + alginate-antacids was used for 37% and 21% of physicians for treating heart-burn and regurgitation, respectively. A better control of symptoms, an increase in the onset of action and to reduce nocturnal acid breakthrough were the most frequently argued reasons for the use of PPI + alginate-antacids. A high percentage of participants believed that treatment with PPI alone was insufficient for the control of symptoms and 39.8% of physicians reported the persistence of heartburn, 38.6% the persistence of regurgitation and 43.2% the persistence of epigastric discomfort in more than 25% of their patients treated with PPI as monotherapy. The most common schedule for the use of the antacid medication was on demand. Conclusions: Spanish primary care physicians consider that a high proportion of GERD patients continue to suffer from symptoms during PPI treatment alone.
