缺氧对HRE-TK/GCV系统杀伤骨肉瘤细胞的增强作用

目的:探讨缺氧条件下,缺氧反应元件启动子-胸苷激酶/丙氧鸟苷(HRE-TK/GCV)系统对骨肉瘤细胞系MG63靶向性杀伤作用。方法:构建由HRE启动子驱动的含潮霉素磷酸转移酶-单纯疱疹病毒胸腺嘧啶激酶(Hygromycinphosphotransferase-thymidinekinase,HyTK)融合基因的真核表达质粒pBI-HRE-HyTK,并将其转染入骨肉瘤细胞系MG63。应用PCR和RT-PCR方法检测TK基因的整合及表达;用MTT法和混合共培养实验分别检测转基因细胞在缺氧或不缺氧状态下对GCV的敏感性以及HRE-TK/GCV系统的“旁观者效应”;流式细胞仪检测细胞周期改变及电镜观察细胞超微结构变化探讨其作用机制。结果:成功构建了真核表达质粒pBI-HRE-HyTK,得到转基因细胞系MG63TK。检测到MG63TK细胞内TK基因的DNA和mRNA。MG63TK细胞在不缺氧状态下对GCV不敏感,当GCV浓度达50μg/ml时,仅30%左右细胞被杀死;而缺氧状态下,GCV浓度1μg/ml时,50%左右细胞被杀死,GCV浓度50μg/ml时,90%以上细胞被杀死。混合共培养实验中,缺氧状态下,HRE-TK/GCV系统旁观者效应明显增强,MG63细胞存活率显著低于不缺氧状态下。缺氧和GCV共同作用使转基因细胞DNA合成抑制,细胞周期阻滞于G0G1期,细胞发生凋亡和坏死。结论:缺氧可以增强HRE-TK/GCV系统对体外培养骨肉瘤细胞系选择性的杀伤作用?

Medium-Term Follow-Up of Children with Unilateral Non or Poorly Functioning Kidney: A Single-Center Experience

Purpose: The aim of the study was to assess the safety of conservative management of nonfunctioning kidney in children. Material and Methods: The study group consisted of 29 children with a unilateral or poorly functioning kidney who were followed at the outpatient clinic of the Urology Unit of a tertiary pediatric medical center for at least two years. Pain, hypertension, urinary tract infection, and renal calculi were assessed regularly during follow up. Results: Mean duration of follow-up was 62 months. The main causes for non-functioning kidney were high grade vesicoureteral reflux (55%) and ureteropelvic junction obstruction (34%). Urinary tract infection (24%) occurred mostly during the first 18 months of follow up. UTI was observed in all children who had bilateral vesicoureteral reflux or an ectopic ureter. None of the children developed pain, hypertension or renal calculi during follow-up period. Conclusions: In children, preserving a nonfunctioning kidney due to causes other than vesicoureteral reflux does not increase the risk of complications on medium term follow up. Those with non-functioning kidney due to high grade vesicoureteral reflux should be offered nephrectomy to decrease the risk of UTI.

Pattern of respiratory-induced changes in fingertip blood volume measured by light transmission

Respiratory-induced fluctuations in heart rate and arterial blood pressure have been intensively investigated, but there is little information on the effect of respiration on peripheral blood volume. In the current study, blood volume changes in the finger, obtained by light transmission measurements, were measured during regular breathing (6 s periods) and long breathing (12 s periods). Respiratory chest-circumference changes were simultaneously measured in order to associate the pattern of tissue blood volume change with the respiratory cycle. Sixteen subjects were studied, and in fourteen finger blood vol ume increased during inspiration and decreased during expiration in the long-breathing periods. In all 14 subjects the start of blood volume decrease was significantly delayed from the start of expiration by mean ± SD 1.00 ± 0.65 s (p < 0.001, range 0 - 2.3 s). The start of blood volume increase was significantly delayed from the end of expiration by 3.45 ± 1.76 s (p < 0.005). In eight, finger blood volume started to increase more than 2 s before the start of inspiration. For the 6 s breathing period, blood volume decreased during inspiration in five examinations, and increased in seven. The increase in peripheral blood volume during inspiration could be attributed to the higher abdominal pressure during inspiration, and to the decrease in sympathetic activity during inspiration and the subsequent vasodilatation. The decrease in peripheral blood volume during inspiration is probably due to the negative thoracic pressure during inspiration and its mechanical effect on thoracic vessels.

Radio Frequency-Microchannels for Transdermal Delivery: Characterization of Skin Recovery and Delivery Window

Transdermal delivery through Radio-Frequency-MicroChannels (RF-MCs) was proven to be a promising delivery method for hydrophilic drugs and macromolecules that must be injected. An important issue in assessing this technology is the life span of the microchannels (MCs). The time window in which the MCs remain open affects the delivery rate and determine the effective delivery duration. The present work focused on the characterization of the ViaDor-MCs recovery and closure process by measurements of transepidermal water loss (TEWL) before and after the formation of MCs, evaluation of the delivery window, and assessment of skin histology. Testosterone-cyclodextrin complex was used as the model drug for evaluation of the transdermal delivery. In-vitro permeation system and in-vivo guinea pig animal model were used in the delivery studies. Our findings demonstrate the recovery process of MCs created by the RF ablation technology. The observed gradual skin recovery affected the transdermal delivery rate. A significant transdermal delivery was shown up to 24 hrs post device application suggesting that an extended delivery of water soluble drugs, including macromolecules, is possible. The histology assessments demonstrated repair and healing of the induced MCs indicating that the RF micro-channeling technology is minimally invasive, transient in nature with no resulting skin trauma.

目的探讨外科手术中经心外膜探查心包内心脏植物神经节丛的位置和微波消融神经节丛的即时效果。方法16例接受心脏手术者同期行高频电刺激心包内植物神经节丛。窦性心律者使用同步高频刺激，房颤者使用连续高频刺激，观察出现迷走效应的位置。探查后对房颤病人使用微波消融，消融和电复律后使用同步电刺激原有迷走效应的植物神经节丛，观察有无迷走效应出现。结果2例病人麻醉后因故不能进行试验被排除。14例在1～5处脂肪垫引出迷走效应。13例病人在右上肺静脉根部脂肪垫有迷走效应，位于心脏右侧的植物神经节丛发生迷走效应的频率明显高于心脏左侧。消融后，原有迷走效应的植物神经节未再引出迷走效应。结论自心外膜使用高频电刺激心脏周围脂肪垫可在1处或多处诱发迷走效应，心脏右侧脂肪垫的迷走效应诱发率高于左侧，右上肺静脉周围脂肪垫诱导率最高。心外膜微波消融可以消除迷走效应。
