目的比较充气式纵隔腔镜与微创McKeown手术治疗食管癌的效果。方法选取108例行微创手术治疗的食管癌患者,其中纵隔腔镜组(n=30)采取充气式纵隔腔镜同步腹腔镜食管癌手术,微创McKeown组(n=78)采取胸腹腔镜联合食管癌手术。比较2组患者的相关临床资料以及术中、术后疗效指标。结果纵隔腔镜组患者的手术时间、首次下床时间、留置胃管时间、术后禁食时间和住院时间均显著短于微创McKeown组(P <0. 05)。纵隔腔镜组切缘阳性率为0%,显著低于微创McKeown组的12. 82%(P <0. 05)。纵隔腔镜组术后疼痛程度显著轻于微创McKeown组(P <0. 05)。纵隔腔镜术后并发症发生率为6. 67%,显著低于微创McKeown组的24. 36%(P <0. 05)。结论充气式纵隔腔镜治疗食管癌的手术创口小,术后疼痛轻,术后并发症较少。

Background Smith-Kingsmore syndrome(SKS)is a rare autosomal dominant disorder caused by de novo mutations of gene MTOR in most cases and germline mosaicism in a few *** first case of SKS was reported in *** incidence of SKS remains *** clinical manifestations of SKS are diverse,and common features are macrocephaly,intellectual disability,and *** patients with SKS have special facial *** presentation The case was a 5-month-old baby girl,who was admitted to the hospital for nystagmus,delayed development for 2 months,and intermittent convulsions for 2 *** patient had a head circumference of 42 cm(+2SD),and showed facial deformity,low limb muscle tension,large areas of pigmentation,as well as mosaic patchy and strip-like pigment loss in her trunk and ***,her development was lagging behind *** examination did not reveal other *** was diagnosed with SKS based on whole-exome sequencing combined with clinical symptoms and *** successively received treatment with adrenocorticotropic hormone,methylprednisolone sodium succinate,topiramate,levetiracetam,and zonisamide to reduce the number of convulsions in a short time,but drug resistance appeared *** combined treatment with multiple antiseizure medications,the patient still had seizures,but the amplitude of limb movement during the seizures was reduced compared to that before *** This case expanded the phenotypic spectrum of SKS for *** also review the related literature to promote the awareness,diagnosis,clinical management,and follow-up of SKS patients with MTOR mutations.

The transformation of CO_(2)into high value-added product is a promising pathway for utilizing CO_(2).However,the process tends to require harsh reaction conditions owing to CO_(2)chemical *** a high efficiency catalytic system with environmentally benign characteristic are important *** this work,protic ionic liquids[TMG][2-OPy]were prepared via one-step neutralization between 1,1,3,3-tetramethylguanidine and 2-hydroxypyridine,applying to the domain of synthesizing quinzoline-2,4(1 H,3H)-diones from CO_(2)and 2-aminobenzontiles without any solvent or metal,achieving the yield of 97%at 90℃for 8 h under atmospheric.A series of substrates with good to acceptable yield were detected,revealing the generality and universality of the ***,the system could be facilely reused for at least six runs,retaining the yield of 94%.A preliminary kinetic equation is calculated with the activation energy of 68 kJ·mol^(-1),and a plausible reaction mechanism was put *** study highlights that the[TMG][2-OPy]enables to activate CO_(2)carboxylation efficiently.

The coronavirus disease 2019(COVID-19)pandemic,caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),has seriously threatened global public health and caused huge economic *** studies of SARS-CoV-2 can help understand the interaction between the virus and host,thereby providing a new perspective in guiding the intervention and treatment of the SARS-CoV-2 *** large amount of SARS-CoV-2 omics data have been accumulated in public databases,this study aimed to identify key host factors involved in SARSCoV-2 infection through systematic integration of transcriptome and interactome *** manually curating published studies,we obtained a comprehensive SARS-CoV-2-human protein-protein interactions(PPIs)network,comprising 3591 human proteins interacting with 31 SARS-CoV-2 viral *** the RobustRankAggregation method,we identified 123 multiple cell line common genes(CLCGs),of which 115 up-regulated CLCGs showed host enhanced innate immunity and chemotactic response *** with network analysis,co-expression and functional enrichment analysis,we discovered four key host factors involved in SARS-CoV-2 infection:IFITM1,SERPINE1,DDX60,and ***,SERPINE1 was found to facilitate SARSCoV-2 replication,and can alleviate the endoplasmic reticulum(ER)stress induced by ORF8 protein through interaction with *** findings highlight the importance of systematic integration analysis in understanding SARS-CoV-2-human interactions and provide valuable insights for future research on potential therapeutic targets against SARS-CoV-2 infection.

Stable and efficient single atom catalysts(SACs)are highly desirable yet challenging in catalyzing acidic oxygen evolution reaction(OER).Herein,we report a novel iridium single atom catalyst structure,with atomic Ir doped in tetragonal PdO matrix(IrSAs-PdO)via a lattice-confined *** optimized IrSAs-PdO-0.10 exhibited remarkable OER activity with an overpotential of 277 mV at 10 mA·cm^(-2) and long-term stability of 1000 h in 0.5 M H_(2)SO_(4).Furthermore,the turnover frequency attains 1.6 s^(-1) at an overpotential of 300 mV with a 24-fold increase in the intrinsic *** high activity originates from isolated iridium sites with low valence states and decreased Ir–O bonding covalency,and the excellent stability is a result of the effective confinement of iridium sites by Ir–O–Pd ***,we demonstrated for the first time that SACs have great potential in realizing ultralow loading of iridium(as low as microgram per square center meter level)in a practical water electrolyzer.

On entering sensory ganglia,herpes simplex viruses 1(HSV-1)establishes a latent infection with the synthesis of a latency associated transcript(LAT)or initiates productive infection with expression of a set of immediate early viral *** precise mechanisms how expression of a genes is suppressed during the latency are *** mechanism that has been proposed is illustrated in the case of ICP0,a key immediate early viral regulatory ***,the 2 kb LAT intron is complementary to the 30 terminal portion of ICP0 m *** test the hypothesis that accumulation of LAT negatively affects the accumulation of ICP0 m RNA,we inserted a DNA fragment encoding two poly(A)sequences into LAT to early terminate LAT transcript without interrupting the complementary sequence of ICP0 transcript(named as SR1603).Comparisons of the parent(SR1601)and mutant(SR1603)HSV-1 viruses showed the following:Neurons harboring latent SR1603 virus accumulated equivalent amounts of viral DNA but higher amounts of ICP0 m RNA and lower amounts of LAT,when compared to neurons harboring the SR1601 *** notable difference between the two viruses is that viral RNA accumulation in explanted ganglia harboring SR1603 virus initiated significantly sooner than that in neurons harboring SR1601 virus,suggesting that ICP0 may act as an activator of viral gene expression in permissive ***,these data suggest that increased ICP0 m RNA by suppressed LAT did not affect the establishment of latency in latently infected murine ganglia.
