Talking with Children about Cancer: A Content Analysis of Text in Children’s Picture Books

Introduction: To tell children that they are diagnosed with cancer is challenging for any healthcare professional and family. Pictures books are one of the communication tools for talking with children about severe diseases. However, little research is available about the contents of picture books about cancer for children. Therefore, this study aimed to examine the contents of children’s picture books about cancer and explore the advantages and disadvantages of using picture books to communicate with children about cancer. Methods: We searched the picture books about cancer written for children aged under ten years old on the *** Web site on 5 July 2019 and hand-searched on the cancer-related institutes’ Web site. We extracted the texts of relevant picture books and conducted a content analysis of them. Results: We identified 2555 picture books and included 30 of them. We identified three main contents, 1) cancer-related knowledge, 2) impacts of cancer, and 3) dealing with cancer. These contents were written with simple words and illustrations. Some of them included the essential contents, which might be misunderstood by children. Conclusions: Picture books might be helpful for children to understand about cancer. However, as these books do not include all contents, it is necessary to select and use multiple books depending on the content which is wanted to tell the child. Further research should evaluate the impacts of these books as a communication tool when talking with children diagnosed with cancer.

Silicosis patients （SILs） and patients who have been exposed to asbestos develop not only respiratory diseases but also certain immunological disorders. In particular, SIL sometimes complicates autoimmune diseases such as systemic scleroderma, rheumatoid arthritis （known as Caplan syndrome）, and systemic lupus erythematoses. In addition, malignant complications such as lung cancer and malignant mesothelioma often occurr in patients exposed to asbestos, and may be involved in the reduction of tumor immunity. Although silica-induced disorders of autoimmunity have been explained as adjuvant-type effects of silica, more precise analyses are needed and should reflect the recent progress in immunomolecular findings. A brief summary of our investigations related to the immunological effects of silica/asbestos is presented. Recent advances in immunomolecular studies led to detailed analyses of the immunological effects of asbestos and silica. Both affect immuno-competent cells and these effects may be associated with the pathophysiological development of complications in silicosis and asbestos-exposed patients such as the occurrence of autoimmune disorders and malignant tumors, respectively. In addition, immunological analyses may lead to the development of new clinical tools for the modification of the pathophysiological aspects of diseases such as the regulation of autoimmunity or tumor immunity using cellmediated therapies, various cytokines, and molecule-targeting therapies. In particular, as the incidence of asbestosrelated malignancies is increasing and such malignancies have been a medical and social problem since the summer of 2005 in Japan, efforts should be focused on developing a cure for these diseases to eliminate nationwide anxiety.

Poor Schedule Management Leads to Discontinuity of Medical Attendance among Patients Infected with HIV

Dropout from medical attendance is a well-recognized issue among patients with human immunodeficiency virus (HIV) infection. We interviewed 23 HIV-positive patients and used text mining methods to analyse the risk factors for dropout. Fifteen patients continued medical attendance and eight patients dropped out of medical attendance. Categories were extracted from the interview data of the patients who continued medical attendance (i.e., the continuity group) and the patients who dropped out of medical care (i.e., the discontinuity group). Categories of the continuity group included needing to take a day off for medical attendance, scheduling each appointment, writing down medical appointments, being grateful for the medical care, and 12 additional categories. Categories of discontinuity group included forgetting the dates of medical appointments, not needing to get a day off for medical attendance, allowing aid for medical care to expire, and 10 additional categories. The discontinuity group had poorer schedule management than the continuity group, which caused them to forget their next medical appointments and delay the renewal of aid for medical care. Thus, medical staff may be able to prevent dropouts by ensuring that patients record the dates of their medical appointments.

It becomes a vitally important issue to understand and use Chinese medicines correctly. The toxicity of Chinese herbal medicines which takes place in recent years, can generalize to 4 aspects: ① improper usage of Chinese medicine properties causes the steady state of body out of balance and leads to the adverse responses of patients beyond the range of indications; ② because of the generalization of “dual modulation effect” of Chinese medicines, as well as the misunderstanding of non-poisonous nature of Chinese herbs, it makes the term of taking medicine unnecessarily long and then causes the hypercorrection or accumulative poisoning; ③ paying much attention to the chemical compositions of patent Chinese medicines while ignoring the impact of processing methods on the property of effective substances in crude drug and the significance of the compatibility of herbal ingredients.

Using a Wavelet-Based and Fine-Tuned Convolutional Neural Network for Classification of Breast Density in Mammographic Images

Classification of breast density is significantly important during the process of breast diagnosis. The purpose of this study was to develop a useful computer-ized tool to help radiologists determine the patient’s breast density category on the mammogram. In this article, we presented a model for automatically classi-fying breast densities by employing a wavelet transform-based and fine-tuned convolutional neural network (CNN). We modified a pre-trained AlexNet model by removing the last two fully connected (FC) layers and appending two newly created layers to the remaining structure. Unlike the common CNN-based methods that use original or pre-processed images as inputs, we adopted the use of redundant wavelet coefficients at level 1 as inputs to the CNN model. Our study mainly focused on discriminating between scattered density and heterogeneously dense which are the two most difficult density cat-egories to differentiate for radiologists. The proposed system achieved 88.3% overall accuracy. In order to demonstrate the effectiveness and usefulness of the proposed method, the results obtained from a conventional fine-tuning CNN model was compared with that from the proposed method. The results demon-strate that the proposed technique is very promising to help radiologists and serve as a second eye for them to classify breast density categories in breast cancer screening.

Patient Safety,Adverse Healthcare Events and Near-Misses in Obstetric Care—A Systematic Literature Review

Systematic development of a patient safety culture is necessary because lack of quality care leads to human suffering. The aim of this review was to identify evidence of obstetric adverse events (AEs) and near-misses in the context of patient safety. We conducted a search of the published literature from Europe, Australia and the USA in the following databases: Cinahl, Cochrane, Maternity and Infant Care, Ovid, Pro-quest and PubMed, guided by PRISMA procedures. A total of 427 studies were screened, 15 full papers retrieved and nine studies included in the final thematic analysis. The selected papers address a broad spectrum of adverse patient safety events in obstetric care. The themes that emerged were: type of AEs, near-misses and their consequences, strategies to support and improve Patient Safety (PS) and domains related to the WHO Patient Safety competence outcomes. The findings of the first theme were grouped into the following categories: healthcare professionals’ perspectives on ethical conflicts, attributing blame and responsibility, and patients’ perspectives on lack of trust and involvement, as well as medication errors. The second theme, strategies to support interventions to improve PS, was based on two sub-themes: communicating effectively and gaining competence by learning from adverse events, while the third theme was domains related to the WHO Patient Safety competence outcomes. In conclusion, few studies have examined strategies for managing AEs despite the existence of programmes that target the implementation of changes, such as improved teamwork training. In addition to exploring strategies to make safety a priority for patients and healthcare professionals, it is of the utmost importance to improve communication with patients and between professionals in order to maintain and enhance safety. Efforts by organizations and individuals to continuously develop knowledge about the risk of AEs and the use of best practice guidelines are also essential

Improvement of decocting method for Kampo medicines using a microwave oven to extend its applications in clinical practice

Decoction of Kampo medicines plays an important role in clinical practice, especially in individualized treatment, while the inconvenience and a long time requirement of the decocting process are impediments to its widespread use in Japan. In this study, we improved the decocting method by using a microwave oven such as those found in most kitchens. To validate the feasibility and safety of this new method, we decocted kakkonto, which is the most widely used formula in clinical treatment in Japan, and keishikabushito, which contains toxic components using a microwave oven. Regarding the contents of 8 characteristic components in the kakkonto decoction and the contents of 6 toxic components in the keishikabushito decoction as indices, and with the extraction and detoxification effects equal to those of the conventional decocting method as targets, we optimized the decocting conditions with Response Surface Methods. With this new method, it took 35 min to obtain almost the same extraction effect for kakkonto as with the conventional decocting method, which takes 40 min;meanwhile, it took only 45 min to detoxify keishikabushito, which takes 60 min using the conventional decocting method. Decocting Kampo medicines with a microwave oven is feasible and as safe as the conventional decocting method. It is a convenient, safe, time-saving method, and may be applied widely in clinical practice. This innovation should allow more patients to benefit from decoction and the individualized treatment it offers.

Effects of Pre-Schooler Lifestyle on the Circadian Rhythm of Secretory Immunoglobulin A

Assuming that secretory immunoglobulin A (s-IgA) in pre-schoolers is influenced by wake-up time, bedtime, sleeping duration and activity, the current study examined the influence of basic pre-schooler lifestyle on the circadian rhythm of s-IgA. We conducted a questionnaire survey on lifestyle and measured steps, heart rate and salivary s-IgA in 44 pre-schoolers aged 4 or 5 years old. Saliva was collected over 2 days at six time points: bedtime, upon waking up, upon arriving at nursery school, before lunch, after lunch and upon going home. To obtain relatively reliable s-IgA values, the s-IgA concentration/total protein concentration ratio (referred to as s-IgA/T.P.) was calculated. Relationship between the s-IgA/T.P. and heart rate, steps, wake-up time, bedtime and sleeping duration was then examined using Pearson’s correlation coefficient. Accordingly, pre-schooler lifestyle had the following effect on circadian rhythm of s-IgA: 1) s-IgA/T.P. values were higher upon waking up than at other measurement time points;2) pre-schoolers with considerable physical activity had high s-IgA/T.P. at bedtime and upon waking up;and 3) pre-schoolers with long sleeping durations and early bedtime had high s-IgA/T.P. upon waking up. Thus, our results confirmed that pre-schoolers’ lifestyle habits, such as bedtime, sleeping duration, physical activity, had an effect on the circadian rhythm of s-IgA.
