The study aims to formulate a solution for identifying the safest route between any two inputted Geographical *** the New York City dataset,which provides us with location tagged crime statistics;we are implementing different clustering algorithms and analysed the results comparatively to discover the best-suited *** results unveil the fact that the K-Means algorithm best suits for our needs and delivered the best ***,a comparative analysis has been performed among various clustering techniques to obtain best *** compared all the achieved results and using the conclusions we have developed a user-friendly application to provide safe route to *** successful implementation would hopefully aid us to curb the ever-increasing crime rates;as it aims to provide the user with a beforehand knowledge of the route they are about to take.A warning that the path is marked high on danger index would convey the basic hint for the user to decide which path to ***,addressing a social problem which needs to be eradicated from our modern era.

Comparison of efficacy of metformin and D-chiro-inositol on clinical biomarkers in patients with polycystic ovarian syndrome:an open label study

Objective:Polycystic ovary syndrome(Pcos)is a pathophysiological disorder affecting reproductive and metabolic indices in *** present study was designed to compare the efficacy of metformin and D-chiroinositol in PcoS ***:In a tertiary care hospital in North India,prospective observational research was undertaken on 1o0 patients with PCOS,which was diagnosed based on European Society of Human Reproduction and Embryology Guidelines and ultrasound of lower *** study involves various clinical characteristics into consideration for the determination of statistical significance(P<0.05)in PCOS ***'s t-test along with the association between PCOS and patients taking metformin and D-chiro-inositol,as well as their impact on various biochemical parameters,were investigated finally using Pearson Correlation ***:This study comprises 50 patients taking metformin and 50 patients taking D-chiro-inositol in women suffering from *** mass index(BMI)and waist-to-hip ratio(WHR)were statistically significant(P<0.05)within the groups of both metformin and *** parameters such as luteinizing hormone(LH),follicle stimulating hormone(FSH),anti-mullerian hormone(AMH)and glycated hemoglobin(HbA1c)were found to be statistically significant(P<0.05)in both ***,FSH and AMH(14.40±0.52;14.28±0.53;1.99±0.10)were comparatively lower in patients taking D-chiro-inositol as compared to metformin group(14.17±0.42;19.88±1.01;2.61±0.04).HbA1c(3.71±0.08)with P<0.05 was found to be decreased more in metformin group as compared to patients taking D-chiro-inositol(4.90±0.09).A positive correlation was found between HbAlc and LH in metformin,&HbAlc and FSH in D-chiro-inositol groups,***:The results indicate that D-chiro-inositol shows better results in reducing clinical variables involved in causing PCOS as compared to metformin whereas metformin has better glycemic control in PCOS patients.

Cervical cancer prevention requires a multipronged approach involving primary, secondary and tertiary prevention. The key element under primary prevention is human papilloma virus(HPV) vaccination. So far, only prophylactic HPV vaccines which prevent HPV infection by one or more subtypes are commercially available. Therapeutic HPV vaccines which aid in clearing established infection are still under trial. Secondary prevention entails early detection of precancerous lesions and its success is determined by the population coverage and the efficacy of the screening technique. A number of techniques are in use, including cytology, visual inspection(using the naked eye, magnivisualizer, acetic acid and Lugol's iodine), HPV testing and a combination of these methods. Updated screening guidelines have been advocated by the American Cancer Society in light of the role of HPV on cervical carcinogenesis. Recent research has also focussed on novel biomarkers that can predict progression to cancer in screen positive women and help to differentiate those who need treatment from those who can be left for follow-up. Last but not the least, effective treatment of precancerous lesions can help to reduce the incidence of invasive cervical cancer and this constitutes tertiary prevention.A combination of these approaches can help to prevent the burden of cervical cancer and its antecedent morbidity and mortality, but all of these are not feasible in all settings due to resource and allocation constraints. Thus, all countries, especially low and middle income ones, have to determine their own cocktail of approaches that work before we can say with certainty that yes, cervical cancer can be prevented.

Prosthetic pollicization following thumb amputation: A case report

The thumb contributes over 50% hand function, which is generally divided into motional, sensory and cosmetic functions. Optimal management of thumb loss necessitates individual consideration of surgical versus prosthetic options, in relation to the needs and circumstances of each patient. This paper presents a case of prosthetic pollicization of an amputated thumb and emphasizes on the fact that the prosthetic replacement has a definite edge over surgical procedures, where amputees refuse to undergo more complicated surgical procedures. The patient was rehabilitated successfully by a noninvasive and cost-effective approach by using high temperature-vulcanized (HTV) silicone material and retaining the prosthesis with medical adhesives. On 3 months recall ap-pointment, no complications were found. The prosthesis was in good shape, without need for any repairs.

Acute Appendicitis Pathways: A Systemic Review

Background: Clinical pathways are effective in reducing healthcare cost and standardizing management protocol for diseases and conditions. However, there are no standardized pathways for appendicitis. This study aims to determine whether implementation of a standardized clinical pathway for managing appendicitis may lead to improved patient care. Materials and Methods: We searched for articles published in PubMed, MEDLINE, and Cochrane library between 1974 and 2015. Thirty-seven papers published in English that met inclusion criteria were included and analyzed in this review. A total of 37 studies met inclusion criteria and were analyzed for the purpose of this study. Results: A clear definition for appendicitis pathway was observed in 30 articles. Appendicitis was diagnosed based on clinical and laboratory findings;however, 34% of the studies included radiological investigations. Sixteen studies provided clear definitions for discharge criteria. Time of follow-up (5 - 28 days) was reported in 10 studies. Operative time was the most commonly used outcome measure. The mean length of stay for non-complicated appendicitis was 1.3 days, and 6.26 days for complicated appendicitis. Most of the studies concluded that the accuracy of a clinical pathway for appendicitis diagnosis can be achieved by assessing the incidence of a normal appendix. The mean cost for patient care was $4874.14 (SAR 18,278.03). Conclusion: There was no standardized definition for appendicitis clinical pathway components. Studies suggested that establishing a clinical pathway for appendicitis improves the outcome and minimizes the cost.

Femoral head fractures without disloca- tion or subluxation are extremely rare injuries. We report a neglected case of isolated comminuted fracture of femoral head without hip dislocation or subluxation of one year duration in a 36-year-old patient who sustained a high en- ergy trauma due to road traffic accident. He presented with painful right hip and inability to bear full weight on right lower limb with Harris hip score of 39. He received cementless total hip replacement. At latest follow-up of 2.3 years, functional outcome was excellent with Harris hip score of 95. Such isolated injuries have been described only once in the literature and have not been classified till now. The purpose of this report is to highlight the extreme rarity, pos- sible mechanism involved and a novel classification system to classify such injuries.

Cephalomedullary fixation for femoral neck/intertrochanteric and ipsilateral shaft fractures： surgical tips and pitfalls

Objective： Surgical management options for femoral shaft fracture and ipsilateral proximal fe- mur fracture vary from single-implant to double-implant fixation. Cephalomedullary fixation in such fractures has rela- tive advantages over other techniques especially because of less soft tissue dissection and immediate postoperative weight beating with accelerated rehabilitation. However, the surgery is technically demanding and there is a paucity of literature describing the surgical techniques for this fixation. The aim of the study was to describe the surgical technique of cephalomedullary fixation for femoral shaft fracture and ipsilateral proximal femur fracture. Methods： Sixteen cases （10 males and 6 females with a mean age of 41.8 years） of ipsilateral proximal femur and shaft fractures were treated by single-stage cephalomedullary fixation at tertiary level trauma center in northern India. The fractures were classified according to AO classification. An intraoperative record of duration of surgery as well as technical challenges unique to each fracture pattern was kept for all the patients. Results： The most common proximal femoral pattern was AO B2.1 observed in 9 of our patients. The AO B2.3 fractures were seen in 4 patients while the AO A1.2 fractures in 3 patients. Four of the AO B2.1 and 2 of the AO B2.3 frac- tures required open reduction with Watson-Jones approach. The mean operative time was around 78 minutes, which tended to decrease as the surgical experience increased. There was only one case of malreduction, which required revision surgery. Conclusion： Combination of ipsilateral femoral shaft fracture and neck/intertrochanteric fracture is a difficult frac- ture pattern for trauma surgeons. Cephalomedullary nail is an excellent implant for such fractures but it requires careful insertion to avoid complications. Surgery is technically demanding with a definite learning curve. Estimation of Hourly Solar Radiation on Horizontal and Inclined Surfaces in Western Himalayas

The hourly solar radiation data required for solar energy system design evaluation and performance studies is gener-ally not available for a number of sites especially in remote locations. As such accurate determination of hourly solar radiation data, is important both at horizontal;surfaces and inclined surfaces. A model to estimate global solar radiation using temperature and sunshine hour data has been developed (Chandel et al. [1]) which is used to calculate the hourly solar radiation Data. The hourly solar radiation has also been calculated using Gueymard [2] daily integration approach from the measured daily solar radiation data. These two predicted hourly solar radiation data values are compared with measured hourly values to test the accuracy of the models. The total solar radiation on the inclined surfaces and vertical surfaces for different orientations, have also been estimated. The estimated values are found to be in close agreement with measured values. The method presented can be used to estimate hourly, global, diffuse solar radiation for horizontal surfaces and total solar radiation on inclined and vertical surfaces at different orientations with greater accuracy for any location.
