People who have trouble communicating verbally are often dependent on sign language,which can be difficult for most people to understand,making interaction with them a difficult *** Sign Language Recognition(SLR)system takes an input expression from a hearing or speaking-impaired person and outputs it in the form of text or voice to a normal *** existing study related to the Sign Language Recognition system has some drawbacks,such as a lack of large datasets and datasets with a range of backgrounds,skin tones,and *** research efficiently focuses on Sign Language Recognition to overcome previous *** importantly,we use our proposed Convolutional Neural Network(CNN)model,“ConvNeural”,in order to train our ***,we develop our own datasets,“BdSL_OPSA22_STATIC1”and“BdSL_OPSA22_STATIC2”,both of which have ambiguous backgrounds.“BdSL_OPSA22_STATIC1”and“BdSL_OPSA22_STATIC2”both include images of Bangla characters and numerals,a total of 24,615 and 8437 images,***“ConvNeural”model outperforms the pre-trained models with accuracy of 98.38%for“BdSL_OPSA22_STATIC1”and 92.78%for“BdSL_OPSA22_STATIC2”.For“BdSL_OPSA22_STATIC1”dataset,we get precision,recall,F1-score,sensitivity and specificity of 96%,95%,95%,99.31%,and 95.78%***,in case of“BdSL_OPSA22_STATIC2”dataset,we achieve precision,recall,F1-score,sensitivity and specificity of 90%,88%,88%,100%,and 100%respectively.

Background: Following coronary artery bypass grafting (CABG), delirium emerges as a prevalent complication. This study aimed to assess the correlation between elevated serum cortisol levels and the occurrence of postoperative delirium subsequent to off-pump CABG. Methods: Conducted in the Department of Cardiac Surgery at BSMMU from October 2020 to September 2022, this comparative cross-sectional study included a total of 44 participants. Subjects, meeting specific criteria, were purposefully assigned to two groups based on off-pump CABG. Group A (n = 22) consisted of patients with normal serum cortisol levels, while Group B (n = 22) comprised individuals with high serum cortisol levels on the first postoperative day. Delirium onset was assessed at the bedside in the ICU on the 1st, 2nd, 3rd, 4th, and 5th postoperative days using standard tools, namely the Richmond Agitation Sedation score (RASS) and The Confusion Assessment Method (CAM-ICU). Data were collected based on the presence or absence of delirium. Statistical analysis utilized SPSS version 26.0, employing an independent Student’s t-test for continuous data and chi-square and Fischer’s exact test for categorical data. A p-value ≤ 0.05 was considered statistically significant. Results: Group-A had a mean age of 54.50 ± 17.97, and Group-B had a mean age of 55.22 ± 15.45, both with a male predominance (81.81% and 86.36% respectively). The mean serum cortisol level was significantly higher in Group B (829.71 ± vs. 389.98 ± 68.77). Postoperative delirium occurred in 27.3% of Group B patients, statistically significant compared to the 4.5% in Group A. However, patients in Group B who developed delirium experienced significantly longer postoperative ICU and hospital stays (79.29 ± 12.27 vs. 11.44 ± 2.85, p ≤ 0.05). Background: Bangladesh’s tea industry is essential to the country’s economic expansion. Since tea workers in Bangladesh are marginalized within our community, they have limited access to comprehensive eye care services. Productivity and well-being are cornerstones of comprehensive health care strategy. Ocular disorders are influenced by life expectancy, sociodemographic status, and the epidemiological transition. In this context, the state of ocular health and the many eye illnesses remain to be significantly addressed. Purpose: To evaluate the pattern of eye diseases among tea workers in a tea estate of Bangladesh. Methods: This cross-sectional observational study was carried out in Halda Valley Tea Estate, Nazirhat, Fatickchari, Chattogram, Bangladesh, under the supervision of the Department of Community Ophthalmology, BSMMU, following ethical clearance and approval by the IRB board of BSMMU. With informed written consent and approval from the authority of the tea estate, a total of 110 tea workers were recruited. Socio-demographic characteristics, ocular findings, and patterns of eye diseases were determined and recorded. Results: The mean age of the study participants was 39.60 ± 11.63 years. The maximum (58.1%) study participants were 31 - 50 years old, female (64.5%), illiterate (82.7%), and tribal (71.8%) indigenes. Eye diseases were found in 94.5% of workers. Presbyopia (28.2%), cataracts (27.3%), and refractive error (26.4%) were the most common. Tea workers with eye diseases were significantly older than those who did not have any eye diseases (40.20 ± 11.57 vs. 29.17 ± 7.31 years, p Conclusion: A significant number of tea workers had eye diseases, of which presbyopia, cataracts, and refractive error were the most common.

Background: Off-pump coronary artery bypass grafting (OPCAB) is considered a safer alternative to on-pump surgery, especially in patients with left ventricular dysfunction (LVD). Objectives: This study assessed short-term outcomes and functional improvements in LVD patients post-OPCAB. Methods: The study included 200 coronary artery disease patients who underwent isolated off-pump coronary artery bypass grafting (OPCAB) at the National Heart Foundation Hospital and Research Institute between January 2019 and June 2020. Patients were categorized into Group 1, with a left ventricular ejection fraction (LVEF) of 30% - 39%, and Group 2, with an LVEF of 40% or higher. Echocardiographic assessments of left ventricular dimensions and ejection fraction were performed preoperatively, at discharge, and one month postoperatively. Results: In Group 1, preoperative left ventricular internal dimensions during diastole (LVIDd) and systole (LVIDs) were 53.48 ± 4.40 mm and 44.23 ± 3.93 mm, respectively, with a left ventricular ejection fraction (LVEF) of 35.28% ± 2.26%. At discharge, these values improved to 51.58 ± 4.04 mm (LVIDd), 41.23 ± 5.30 mm (LVIDs), and 39.25% ± 3.75% (LVEF). One month postoperatively, further improvements were observed: 46.29 ± 3.76 mm (LVIDd), 37.45 ± 3.68 mm (LVIDs), and 43.22% ± 4.67% (LVEF). Group 2 showed similar positive outcomes, with preoperative values of 47.09 ± 5.06 mm (LVIDd), 35.11 ± 5.25 mm (LVIDs), and 50.13% ± 7.25% (LVEF), improving to 42.37 ± 4.18 mm (LVIDd), 31.05 ± 4.19 mm (LVIDs), and 55.33% ± 7.05% (LVEF) at one month postoperatively. Both groups demonstrated significant improvements in left ventricular function and NYHA class, with most patients moving from class III/IV to I/II. Complications were minimal, and no mortality was observed. Conclusion: OPCAB is safe and effective for patients with LVEF 30% - 39% and LVEF ≥ 40%, providing significant short-term functional improvements without increased risk.

Tricuspid annular plane systolic excursion has been proposed as a simple and reproducible parameter for quantitative assessment of the right ventricular ejection fraction. The prognostic importance of preoperative TAPSE in patients with mitral valve replacement for rheumatic mitral stenosis patients is still under focused. Therefore, the objective of the study was to predict the outcome after MVR in rheumatic mitral stenosis patients in relation to preoperative TAPSE. This comparative cross-sectional study was conducted at the Department of Cardiac Surgery, National Heart Foundation Hospital and Research Institute. A total of 72 patients of rheumatic mitral stenosis patients who underwent mitral valve replacement were included in the study. They were divided into two groups: Group A and B. Group A included 36 patients with TAPSE 0.05) except for the preoperative TAPSE. Mean TAPSE of Group A was 13.17 (±1.40) and Group B was 18.61 (±1.57), the difference was statistically significant (p 0.05). Among the postoperative complications, including postoperative atrial fibrillation was higher in Group A (30.56%) than Group B (11.11%), mean ventilation time was higher in Group A (27.78%) than Group B (5.56%), length of intensive care was higher in Group A (33.33%) than Group B (11.12%), and hospital stay was higher in Group A (25.0%) than Group B (5.56%), (p < 0.05). Higher preoperative TASPE could be used as a prognostic tool for MVR in rheumatic mitral stenosis patients in our settings.

We describe patterns of buttress formation and development in eleven tree species at Lawachara National Park, Bangladesh. Forty-five percent of trees of these 11 species had buttresses. Artocarpus chaplasha Roxb. showed maximum （87%） buttress formation, whereas Alstonia scholaris （L.） R. Br. did not show any buttress. Buttresses were recorded in 20%-40% of trees of six species and 40%-60% of trees in three species. Mean length and height of buttress varied among the species and ranged from 0.37-4.37 m and 0.71-2.13 m, respectively. Buttress height, mean buttress length, total buttress length, and total length plus length of secondaries increased with DBH （diameter at breast height） and tree height. Buttress number did not increase with DBH or tree height. Under-storey and mid-canopy trees produced less developed buttresses than did emergent trees （p〈0.01）. Wood density showed moderate effects on buttress characters （p〈0.05）, while the slope of the land did not. Canopy category was a primary regulating factor for tree buttressing, suggesting that buttresses are mechanical adaptations of trees to counter physical stresses.

In this paper, a hexagonal shape photonic crystal fiber （H-PCF） has been proposed as a gas sensor of which both micro-structured core and cladding are organized by circular air cavities. The reported H-PCF has a single layer circular core which is surrounded by a five-layer hexagonal cladding. The overall pretending process of the H-PCF is completed by using a full vectorial finite element method （FEM） with perfectly matched layer （PML） boundary condition. All geometrical parameters like diameters and pitches of both core and cladding regions have fluctuated with an optimized structure. After completing the numerical analysis, it is clearly visualized that the proposed H-PCF exhibits high sensitivity with low confinement loss. The investigated results reveal the relative sensitivity of 56.65% and confinement loss of 2.31×10^-5 dB/m at the 1.33%tm wavelength. Moreover, effective area, nonlinearity, and V-parameter of the suggested PCF are also briefly described.

This study focuses on comparison between Enzyme Wash and Bleach Wash (Traditional vs. Sustainable Washing Machine) the physical and color fastness to rubbing properties of denim garments. For this experiment indigo dyed cotton denim garments (trousers) were chosen which were 98% cotton and 2% spandex and processed by enzyme with the concentration of 2 g/l, and the temperature of 45°C. In another work, enzyme treated denim garments were used for bleach wash with the concentration of 2 g/l, temperature 45°C and those samples have been processed by traditional and sustainable washing machine. After completing process, the sample properties have been compared like rubbing, tearing and tensile properties.
