AIM:To derive a Malaysia guideline and consensus as part of the Malaysia Retina Group's efforts for diagnosis,treatment,and best practices of diabetic macular edema(DME).The experts'panel suggests that the treatment algorithm to be divided into groups according to involvement the central *** purpose of DME therapy is to improve edema and achieve the best visual results with the least amount of treatment ***:On two different occasions,a panel of 14 retinal specialists from Malaysia,together with an external expert,responded to a questionnaire on management of DME.A consensus was sought by voting after compiling,analyzing and discussion on first-phase replies on the round table discussion.A recommendation was deemed to have attained consensus when 12 out of the 14 panellists(85%)agreed with ***:The terms target response,adequate response,nonresponse,and inadequate response were developed when the DME patients'treatment responses were first *** panelists reached agreement on a number of DME treatment-related issues,including the need to classify patients prior to treatment,firstline treatment options,the right time to switch between treatment modalities,and side effects associated with *** this agreement,recommendations were derived and a treatment algorithm was ***:A detail and comprehensive treatment algorithm by Malaysia Retina Group for the Malaysian population provides guidance for treatment allocation of patients with DME.

Petroleum supplies a large portion of the world’s energy, thereby driving industrial civilization. It remains an important factor in dictating market prices of diverse products and services. Distillation of crude oil is a common fractionation method of petroleum producing desirable products, such as automotive fuel and kerosene. Conventional distillation has been the current fractionation process for decades, producing distillates of interest and is conducted without any involvement of biological materials. The process involves tremendous amount of thermal energy and requiring a long time. This research offers an improved distillation performance and yield. A small amount of rhamnolipid produced from a microbial culture Pseudomonas aeruginosa USM-AR2, is added and mixed thoroughly with the crude oil prior to distillation. This results in a drastic reduction in distillation time without affecting or reducing the quality of the distillate. The pretreatment accelerates the emulsification and biodegradation process by decreasing surface tension and viscosity of oil, which in turn resulted in the shorter distillation times. Reducing the distillation times translates to energy and cost savings in producing petroleum products. Since oil prices affect many other industries, this discovery will have a great impact on the economy.

BackgroundThe COVID-19 pandemic posed a danger to global public health because of the unprecedented physical,mental,social,and environmental impact affecting quality of life(QoL).The study aimed to find the changes in QoL among COVID-19 recovered individuals and explore the determinants of change more than 1 year after recovery in low-resource ***-19 patients from all eight divisions of Bangladesh who were confirmed positive by reverse transcription-polymerase chain reaction from June 2020 to November 2020 and who subsequently recovered were followed up twice,once immediately after recovery and again 1 year after the first *** follow-up study was conducted from November 2021 to January 2022 among 2438 individuals using the World Health Organization Quality of Life Brief Version(WHOQOL-BREF).After excluding 48 deaths,95 were rejected to participate,618 were inaccessible,and there were 45 cases of incomplete *** statistics,paired-sample analyses,generalized estimating equation(GEE)analysis,and multivariable logistic regression analyses were performed to test the mean difference in participants’QoL scores between the two *** participants(n=1710,70.1%)were male,and one-fourth(24.4%)were older than *** average physical domain score decreased significantly from baseline to follow-up,and the average scores in psychological,social,and environmental domains increased significantly at follow-up(P<0.05).By the GEE equation approach,after adjusting for other factors,we found that older age groups(P<0.001),being female(P<0.001),having hospital admission during COVID-19 illness(P<0.001),and having three or more chronic diseases(P<0.001),were significantly associated with lower physical and psychological QoL *** age and female sex[adjusted odd ratio(aOR)=1.3,95%confidence interval(CI)1.0–1.6]were associated with reduced social domain scores on multivariable logistic regression *** or semi-urban peo

Majority of research reports identified moderate reduction in glycated haemoglobin with education interventions regardless of age group. Our study objective was to evaluate the pharmacist interventions in providing patient home care. A 24-week longitudinal quasi-experimental—pre-test/post-test study design was used to assess the effectiveness of a diabetes education program to enhance self-care practices. A double-blinded randomized study design was considered but was not feasible as the investigator was responsible for implementing the intervention and collecting data on outcomes. Since this was a longitudinal study a 25% attrition rate was included in the calculation of sample size. Hence the sample size for the proposed study was 106 subjects with 53 subjects in each group. All analyses were done using SPSS version 18?. The level of significance was set at 0.05. The Research Ethics Committee of hospital and the Malaysian Medical Research and Ethics Committee approved the study. Of the 109 subject who met the study-entry criteria, 3 subjects declined to participate due to lack of time and interest. There was no significant relationship between the demographic and clinical characteristic of participants who completed the study. No significant relationship between the intervention and control groups who completed the study in demographic, clinical and psychosocial contexts. Of the 47 subjects from the intervention group who reported adherent to their daily medication intake after the education intervention, 51 subjects (31.9%) reported taking their medication at the wrong time. The recommended times for oral anti-hyperglycemic medication (OAM) are: sulphonylureas 30 minutes before food, acarbose with food, metformin with or within 30 minutes after food. 目的全氟辛酸(PFOA)和全氟辛烷磺酸(PFOS)是两种环境内分泌干扰物,可通过影响睾丸类固醇激素合成相关基因的表达干扰激素合成,但其影响基因表达的机制尚不明晰。组蛋白修饰可调控基因的转录表达,并与类固醇激素合成密切相关。因此,本研究通过探索组蛋白修饰在PFOA/PFOS干扰睾丸类固醇激素合成中的作用,旨在揭示低剂量PFOA/PFOS暴露导致雄性生殖内分泌紊乱的表观遗传机制。材料与方法雄性大鼠分别暴露于0.015和0.15 mg/kg/d的PFOA和PFOS两个月。采用LC-MS/MS测定大鼠血清、睾丸中的PFOA和PFOS含量;放射免疫法测定血清中的类固醇激素含量。RT-PCR和Western blot分析大鼠睾丸中类固醇激素合成相关基因的表达水平。利用基于LC-MS的蛋白质组学技术分析大鼠睾丸的组蛋白修饰谱,并利用Western blot对组蛋白修饰变化进行验证。染色质免疫沉淀(ChIP)-PCR法分析类固醇激素合成相关基因启动子区域的组蛋白修饰水平。结果暴露后,大鼠血清、睾丸中的PFOA和PFOS浓度均显著升高,表明PFOA/PFOS可穿透血睾屏障并在睾丸中蓄积。此外,低剂量PFOA/PFOS暴露提高了血清中孕酮、睾酮、雌二醇的水平。PFOA暴露后,睾丸中StAR上调,而CYP11A1、3β-HSD、CYP17A1和17β-HSD均下调。8种组蛋白修饰水平均显著降低,Star启动子区域的组蛋白H3K9me1/3水平也下降。PFOS暴露后,睾丸中StAR、CYP11A1、3β-HSD上调,而CYP17A1和17β-HSD下调。Cyp11a1、Hsd3b基因启动子上的H3K18ac、H3K9ac水平均升高。结论低剂量PFOA暴露可通过抑制睾丸组蛋白H3K9甲基化,激活激素合成限速基因Star的表达,最终导致类固醇激素水平升高;而PFOS则通过增强H3K9和H3K18的乙酰化,激活Cyp11a1、Hsd3b基因表达,从而促进睾丸类固醇激素合成。本研究从组蛋白修饰层面揭示了低剂量全氟化合物暴露干扰雄性生殖内分泌的表观遗传机制。

麦瘟病由稻瘟菌小麦致病型（MoT）引起，是一种存在于南美洲和孟加拉国的毁灭性病害。由于MoT通常不会在抽穗期之前使小麦显现染病症状，但在抽穗期其侵染会一直加剧，因此仅根据肉眼观察来使用杀菌剂是无效的。为了开发一种在苗期和营养期准确、灵敏检测MoT以控制病害蔓延的方法，我们对来自巴西的两个MoT分离株的基因组进行了测序，并确定了两个DNA片段：MoT-6098和MoT-6099，它们存在于麦瘟病菌（MoT）基因组中，但不存在于感染稻瘟病菌（MoO）的水稻基因组中。利用聚合酶链反应（PCR），我们在来自南美洲和孟加拉国的53株MoT和MoO分离株中证实了这两对标记引物的特异性。为了测试这两对标记引物的有效性，我们首先建立了一种环介导等温扩增（LAMP）方法。在等温条件下，这种方法在不使用常规PCR仪器的情况下也能检测MoT。随后，我们使用Cas12a蛋白及导向RNA(g RNAs)靶向MoT-6098和MoT-6099序列，两个靶标被识别后，均能激活Cas12a非特异切割单链脱氧核糖核酸酶（ss DNAase）的活性，我们将依赖靶向Cas12a激活ss DNase活性，同时将重组酶聚合酶扩增（RPA）和核酸侧流免疫分析法（NALFIA）相结合，开发出一种准确、灵敏且经济高效地检测受感染小麦植株中MoT特异性DNA序列的方法。这种新技术可应用于田间麦瘟病和其他重要植物病害的快速检测。

The incidence of dengue in Malaysia has shown an increasing trend since the year 2000. Vector control is the primary approach for dengue control in Malaysia. There is an urgent need for new or modified approaches such as the residual spraying on the outer walls that can potentially last long enough to control the Aedes population, particularly in the outbreak-prone areas. In this field study, we conducted outdoor residual spraying (ORS) using a newly formulated polymer-enhanced suspension concentrate (SC-PE) of deltamethrin. The objectives of this study were to evaluate the efficacy of ORS using deltamethrin SC-PE and its effect on wild Aedes populations and to assess its residual bio-efficacy on painted cement walls against the pyrethroid-susceptible strains of laboratory-reared Aedes mosquitoes. Three rounds of spraying in a four-month cycle were conducted between 2014 and 2015 in four residential areas (low-rise and high-rise housing types) in Hulu Langat, Selangor. The bio-efficacy of the insecticide was evaluated by assessing its impact on vector population using ovitrap surveillance. Standard WHO wall deposit bioassay was adapted to determine bio-efficacy of deltamethrin, i.e. post 30 min knockdown and post 24 h mortality after exposure. During the treatment period, we observed significant reductions in the population of Ae. albopictus in the sprayed low-rise housing in both semi-indoor and outdoor environments, while in the high-rise housing, there was also a significant decline in Ae. aegypti population in the semi-indoor environment. The evaluation of the residual bio-efficacy of deltamethrin SC-PE against laboratory-reared Aedes mosquitoes showed that the insecticide lasted longer in the high-rise housing compared to the low-rise housing with >80% mortality achieved continuously for 16 weeks.
