In this review article, various preferences in breast plastic surgery particularly after breast cancer will be discussed in view of the diverse indications for the different construction procedures. The various conditions that necessitate the need for reconstruction are appraised, the important reconstructive procedures are discussed. The most important indications procedures discussed in this review include;Prophylactic subcutaneous mastectomy, Lumpectomy and Radiation, Modified Mastectomy with Axillary Sampling, Nipple reconstruction, and the contralateral breast. These procedures are discussed in view of plastic surgeon practice and patients acceptability in Saudi Arabia. Data from Saudi Arabia in particular was identified through searches of the EMBASE, and MEDLINE database, using the keywords: Saudi Arabia, breast plastic Surgery, breast reconstruction, autologous breast reconstruction, breast augmentation. Advances in prosthetic technologies and modifications in autologous flap techniques, and the development of novel tissue alternatives have allowed for sustained developments in breast reconstruction results. A variety of attitudes has been accessible for addressing the difficulties that endure after resection of breast cancer in Saudi Arabia. Patients should be educated to accept different process in this context.

Split-dose menthol-enhanced PEG vs PEG-ascorbic acid for colonoscopy preparation

AIM:To compare the efficacy and palatability of 4L polyethylene glycol electrolyte(PEG)plus sugar-free menthol candy(PEG+M)vs reduced-volume 2 L ascorbic acid-supplemented PEG(Asc PEG).METHODS:In a randomized controlled trial setting,ambulatory patients scheduled for elective colonoscopy were prospectively *** were randomized to receive either PEG+M or Asc PEG,both splitdosed with minimal dietary *** was assessed on a linear scale of 1 to 5(1=disgusting;5=tasty).Quality of preparation was scored by assignment-blinded endoscopists using the modified Aronchick and Ottawa *** main outcomes were the palatability and efficacy of the *** outcomes included patient willingness to retake the same preparation again in the future and completion of the prescribed ***:Overall,200 patients were enrolled(100patients per arm).PEG+M was more palatable than Asc PEG(76%vs 62%,P=0.03).Completing the preparation was not different between study groups(91%PEG+M vs 86%Asc PEG,P=0.38)but more patients were willing to retake PEG+M(54%vs 40%respectively,P=0.047).There was no significant difference between PEG+M vs Asc PEG in adequate cleansing on both the modified Aronchick(82%vs77%,P=0.31)and the Ottawa scale(85%vs 74%,P=0.054).However,PEG+M was superior in the left colon on the Ottawa subsegmental score(score0-2:94%for PEG+M vs 81%for Asc PEG,P=0.005)and received significantly more excellent ratings than Asc PEG on the modified Aronchick scale(61%vs 43%,P=0.009).Both preparations performed less well in afternoon vs morning examinations(inadequate:29%vs 15.2%,P=0.02).CONCLUSION:4 L PEG plus menthol has better palatability and acceptability than 2 L ascorbic acidPEG and is associated with a higher rate of excellentpreparations;*** identifier:NCT01788709.

Production of Natural Coagulant from Moringa Oleifera Seed for Application in Treatment of Low Turbidity Water

This study focused on developing an efficient and cost effective processing technique for Moringa oleifera seeds to produce natural coagulant for use in drinking water treatment. The produced natural coagulant can be used as an alternative to aluminum sulphate and other coagulants and used worldwide for water treatment. This study investigates processing Moringa oleifera seeds to concentrate the bio-active constituents which have coagulation activity. Moringa oleifera seeds were processed for oil extraction using electro thermal soxhlet. Isolation and purification of bio-active constituents using chromatography technique were used to determine the molecular weight of the bio-active constituents. The molecular weight of bio-active constitu-ents found to be in a low molecular weight range of between 1000 – 6500 Dalton. The proposed method to isolate and purify the bio-active constituents was the cross flow filtration method, which produced the natu-ral coagulant with very simple technique (oil extraction;salt extraction;and microfiltration through 0.45 μm). The turbidity removal was up to 96.23 % using 0.4 mg/L of processed Moringa oleifera seeds to treat low initial turbidity river water between 34-36 Nephelometric Turbidity Units (NTU) without any additives. The microfiltration method is considered to be a practical method which needs no chemicals to be added com-pared to other researchers proposed methods. The natural coagulant produced was used with low dosages to get high turbidity removal which considered to be a breakthrough in this study and recommended to be scaled up for industry level. The product is commercially valuable at the same time it is minimizing the cost of water treatment.

There are many methods to identify and recognize the molecular and behavioural differences between *** of the methods for the detection and identification of unknown organisms as well as intermolecular and intramolecular structural differences is MALDI-TOF mass ***,differentiation of Fomes fomentarius decay capabilities on the chemical properties of the wood cell wall of the tree species Quercus castaneifolia,Juglans regia,and Carpinus betulus were used to determine and characterize the destructive behaviour of *** decay by MALDI-TOF mass *** results showed that the fungus had more significant destructive behaviour on *** than the other *** this evidence,completely removal of xylan hemicellulose fragment+Na+at peak 1227 Da and severe digestion of fragment of glucomannan hemicellulose at peak 1477–1480 Da that it seems that signs of soft-rot patterns were obtained from the decayed sample of ***,while these were incremental and unchanged for *** and ***,***,*** had different peaks of atomic mass than *** and *** wood,*** results showed that this technique could be useful for separating and identifying unknown compounds of the wood cell wall attacked by fungi relying on their biological behaviour.

目的：评估血浆凝集素样氧化型低密度脂蛋白受体-1和脂联素在冠心病合并代谢综合征患者中的水平。方法：收集2009年8月-2010年6月间我院心内科住院的冠心病患者为研究对象,其中平均年龄为66岁的冠心病者30例（CAD组,男13,女7）;平均年龄为6 1岁的冠心病合并代谢综合征者30例（CAD+MS组,13男,17女）;平均年龄为53岁的代谢综合征者30例（MS组,23男,7女）;平均年龄为51岁的来我院体检中心的正常健康对照组30例（22男,8女）。对所有的研究对象进行问卷获取一般资料,并收集外周肘静脉血检测生化指标,采用ELISA法检测LOX-1和脂联素的浓度。结果：血压和BMI在CAD+MS组（142±9mmHg/82±7 mmHg、27.2±1Kg/m2）、MS组（136±6 mmHg/74±7mmHg、25.6±1 Kg/m2）合CAD组（129±10mmHg/77±7mmHg、23.7±1.3 Kg/m2）均高于正常对照组（124±8 mmHg/72±5mmHg、21.1±1.5 Kg/m2）,且差异有统计学意义（均P＜0.05）;血浆甘油三酯、空腹血糖、谷草转氨酶、谷丙转氨酶、肌酐、尿酸和高敏C反应蛋白在CAD+MS组（2.1±1.4mmol/L、6.2±0. mmol/L、23.5±4.8 U/L、21.2±4 U/L、89.3±16 mg/L、314.7±55.1 mmol/L、12.0±8.5 mg/L）均高于正常对照组（0.84±0.3mmol/L、4.5±0.3 mmol/L、11.2±3.6U/L、14.5±4.5U/L、71.0±9.8 mg/L、251.9±42mmol/L、0.7±0.8mg/L）,且差异有统计学意义（均P＜0.05）。尿素氮在CAD组（5.5±0.9mmol/L）均高于CAD+MS组（5.3±1mmol/L）、MS组（4.8±1.4mmol/L）和正常对照组（3.9±0.6mmol/L）、且差异有统计学意义（均P＜0.05）。血浆LOX-1在CAD+MS组（435.9±105.9pg/ml）水平最高,其次是CAD组（196.2±40.4pg/ml）、MS组（140.9±15pg/ml）、正常对照组（87.1±8.5 pg/ml）,且各组间差异有统计学意义（均P＜0.05）。相反,脂联素在CAD+MS组（263.6±26.6μg/L）水平最低,其次是CAD组（369.9±20.7μg/L）、MS组（450.8±25μg/L）、正常对照组（510.9±29.3μtg/L）,且四组间差异有统计学意义（均P＜0.05）。进一步分析发现,血浆LOX-1和脂联素水平不受年龄和性别的影响,且二者的变化呈显著负相关。结论：冠心病患者血浆LOX-1的水平增高,以CAD+MS组增高最为显著;相反,冠心病患者血浆脂联素的水平降低,以CAD+MS组降低最为显著。

Objective:To select best extraction method for the isolated antioxidant compounds from the stems of Adenium ***:Two methods used for the extraction are Soxhlet and maceration *** solvent was used for both extraction *** methanol crude extract was defatted with water and extracted successively with hexane,chloroform,ethyl acetate and butanol *** antioxidant potential for all crude extracts were determined by using 1,1-diphenyl-2-picrylhydrazyl ***:The percentage of extraction yield by Soxhlet method is higher compared to maceration *** antioxidant potential for methanol and its derived fractions by Soxhlet extractor method was highest in ethyl acetate and lowest in hexane crude extracts and found in the order of ethyl acetate>butanol>water>chloroform>methanol>***,the antioxidant potential for methanol and its derived fractions by maceration method was highest in butanol and lowest in hexane followed in the order of butanol>methanol>chloroform>water>ethyl acetate>***:The results showed that isolate antioxidant compounds effected on the extraction method and condition of extraction.

LC-HRMS Based Non-Targeted Metabolomic Profiling of Wheat (<I>Triticum aestivum</I>L.) under Post-Anthesis Drought Stress

Drought stress at the reproductive stage causes severe damage to productivity of wheat. However, little is known about the metabolites associated with drought tolerance. The objectives of this study were to elucidate changes in metabolite levels in wheat under drought, and to identify potential metabolites associated with drought stress through untargeted metabolomic profiling using a liquid chromatography-high resolution mass spectrometry (LC-HRMS)-based technique called Isotopic Ratio Outlier Analysis. Metabolomic analysis was performed on flag leaves of drought-stressed and control (well-watered) plants after 18 days of post-anthesis drought stress at three-hour intervals over a 24-hour period. Out of 723 peaks detected in leaves, 221 were identified as known metabolites. Sixty known metabolites were identified as important metabolites by 3 different methods, PLS-DA, RF and SAM. The most pronounced accumulation due to drought stress was demonstrated by tryptophan, proline, pipecolate and linamarin, whereas the most pronounced decrease was demonstrated by serine, trehalose, N-acetyl-glutamic acid, DIBOA-glucoside etc. Three different patterns of metabolite accumulation were observed over 24-hour period. The increased accumulated metabolites remained higher during all 8 time points in drought stressed leaves. On the contrary, metabolites that showed decreased level remained significantly lower during all or the most time points. However, the levels of some decreased metabolites were lower during the day, but higher during night in drought stressed leaves. Both univariate and multivariate analyses predicted that N-acetyl-glutamic acid, proline, pipecolate, linamarin, tryptophan, and DIBOA-glucoside could be potential metabolite biomarkers, and their levels could serve as indicators of drought tolerance in wheat.

The Prevalence of High Blood Pressure among Marathon Runners during Beirut-Marathon 2014

Objective: To assess the prevalence of high blood pressure (BP) and cardiovascular risk factors among marathon runners during Beirut-Marathon 2014. Methods: A total of 325 marathon runners were divided into 42 km and 10 km groups. They were assessed for cardiovascular risk factors by measuring their BP and answering a questionnaire. The questionnaire composed of 22 questions related to demographic information, risk factors, medical history, family history, medical checkups, use of antihypertensive drugs and definition of hyponatremia. Results: There were 30 runners in the 42 km group and 295 in the 10 km group interviewed. The majority of 42 km runners were males 29 (96.7%) vs. 205 (69.5%) in the 10 km group, (P = 0.001). The 42 km group was older than 10 km group (47 ± 13.8 years vs. 38.5 ± 14.6 years;P = 0.0025). The prevalence of hypertension was 46.7% in the 42 km group as compared to 31.2% in the 10 km group (P = 0.08). Systolic BP (SBP) was higher in 42 km group vs. 10 km group (143 ± 22.4 mm Hg vs. 129.9 ± 17.8 mm Hg;P = 0.0004). The heart rate was lower among 42 km vs. 10 km group (71 ± 11.1 bpm vs. 84 ± 16 bpm;P Conclusion: There is a high prevalence of HTN among marathon runners but minorities were aware that they have hypertension. The 42 km runners tend to be older with higher systolic blood pressure as compared to the 10 km runners.

Malaysian Agriculture Research and Development Institute （MARDI） is the country＇s major custodian of the nation＇s crops genetic resources. It has a global responsibility in the conservation of genetic resources of banana, sweet potato and citrus, and national responsibility for the conservation of crops, such as rice, tropical fruits and some herbs and medicinal plant species. MARDI rice seed genebank at Seberang Prai in the state of Penang was established in 1984 and currently holds 12,770 registered rice accessions, of which 53% are of local origin. In addition, MARDI also conserves genes in the field, mostly for underutilized tropical fruit genetic resources. At present, about 168 species of underutilized, rare and wild tropical fruit species consisting of about 2,000 accessions, the largest in the country, are being conserved. The species include ＂kuini＂（Mangifera odorata）, ＂bacang＂（Mangifera foetida）, ＂binjai＂（Mangifera caesia）, ＂cerapu＂（ Garcinia praniana）, ＂rambai＂（Baccaurea motleyana）, ＂asam gelugor＂（ Garcinia atroviridis）, ＂terap＂（drthocarpus odoratissimus） and ＂pulasan＂（Nephellium rambutan-ake）. The accessions are mostly conserved ex situ in field genebanks located at the various MARDI stations throughout the country. Although the conservation of the various crop species genetic resources is important, the real challenge is enhancing its utilisation. The main objective of the paper was to share and disseminate information related to works conducted and strategies opted by MARDI on the conservation and utilization of crop genetic resources. Information on species of crops that are available in MARDI＇s genebank was also highlighted together with brief information of some of the associated traits of significant importance posses by those genetic resources.
