A Study of Evaluation for the Management of Diabetes in Bangladesh

Diabetes Mellitus is a leading cause of death in present world. This study was carried out to evaluate the management, control, complication profile and treatment strategies in patients with diabetes and to determine what extent management of diabetes in a tertiary-care diabetic hospital in Bangladesh adhered to current guidelines. Total 140 diabetic pa- tients (type-1 = 3, type-2 = 137) were randomly selected from outpatient department of a tertiary care diabetic hospital in the Rajshahi city, Bangladesh, during the month of August to September. A standard questionnaire was constructed in local language and interview was administrated. The result was expressed as mean ± SD and the age was 53.2 ± 10.5 yr, duration of diabetes was 6.3 ± 5.6 yr and age at the onset of diabetes was 46.9 ± 9.9 yr. The study group comprised of about 43% male and 57% female with varying risk factors including family history (49%), smoking (11%) and both smoking and family history (4%). Results showed deteriorating glycemic control with mean FBG (fasting blood glucose) and PPG (postprandial blood glucose) level was 8.9 ± 3.6 mmol/L and 11.2 ± 4.7 mmol/L respectively. About 25% patients had FBG level 7.8 mmol/L. Of the 51% patients with hypertension, 94% were taking anti-hypertensive medicine and 21% patients with dyslipidemia, 59% were treated with lipid lowering agents. Micro-vascular and Macro-vascular complications were reported in 49% and 11% patients respectively. The rates of diabetic complications were cataract 19%, diabetic retinopathy 14%, neuropathy symptoms 35%, nephropathy 6%, MI 6%, cerebral stroke 4% and history of angina pectoris was 7%. The present study was undertaken to evaluate the analgesic and anti-inflammatory activities of ethanolic extract of Terminalia chebula (commonly known as Haritaki) fruits in experimental animal models. The study was carried out using Swiss Albino mice (20-25 g) and Long Evans rats (100-150 g) of either sex. The analgesic activity of Terminalia chebula was assessed by using hot plate method. For the determination of analgesic effect, doses of ethanolic extract of Terminalia chebula used in the present study were 250 mg/kg and 500 mg/kg body weight (BW). Anti-inflammatory effect was analyzed by carrageenan induced paw edema method with the administration dose of 300 mg/kg BW of animals. The analysis of experimental data was performed by statistical process of ANOVA to determine the variability of sample, while Dunnet’s test was performed for evaluation of comparative analgesic and anti-inflammatory activity of Terminalia chebula with control and standard. The animals were divided into four treatment groups of six animals each and the “Mean ± SEM” is the statistical identifiable value of the data and P values <0.05 was considered statistically significant. Hot plate test showed a significant increase in the mean reaction time to heat stimuli in hot plate method at both 250 mg/kg and 500 mg/kg BW doses throughout the observation period in 30 minutes and 60 minutes after treatment, which was comparable to the standard ketorolac and control group. In carrageenan induced paw edema method, considerable results were found after determining the percentage change in paw volume in extract. In both cases of analgesic and anti-inflammatory study, % inhibition of pain and inflammation were evaluated. Comparing with control, largest inhibition was found in inhibiting inflammation 5 hours after treatment, while the largest inhibition of pain was obtained in 30 minutes and 60 minutes after treatment of doses. Capsicum is a nutritious vegetable and its cultivation in farms is getting popular in Bangladesh. Although many efforts have lain to explore better yielding and nutritionally rich cultivars with suitable modern cultivation techniques but still have to find the desired outcome. Thus, it’s necessary to conduct further research to identify the high-yielding and nutritious capsicum cultivars in Bangladesh. An experiment was conducted from July 2021 to June 2022 at the Bangladesh Institute of Research and Training on Applied Nutrition (BIRTAN) research field with three cultivars of capsicum: B0 = California Wonder, B1 = BARI Misti Morich-1 and B2 = BARI Misti Morich-2 and three mulching: T0 = No mulching, T1 = Water hyacinth, T2 = Poly Mulching in randomized complete block design with three replications to identify better quality capsicum cultivar and suitable mulching material. Among cultivars the BARI Misti Morich-2 (B2) showed increased agronomic parameters like number of branches and effective branches per plant, leaves length and width, consequently yield and yield contributing traits were also enhanced like fruits per plant, fruit length, fruit diameter and yield per plant (25.97%, 4.54%, 3.64% and 21.43%, respectively). Poly Mulching (T2) increased agronomic traits, yield traits and yield (0.61 kg) than BARI Misti Morich-1 (T1). The combined effect of B2T2 increased the number of branches per plant, effective branches per plant, leaves length and breadth by 40%, 90%, 15.57% and 26.22%, respectively, hence resulting in an increased yield of 20%. BARI Misti Morich-2 cultivar showed an increase in Fe, Zn and Vitamin-C content of 26.24% and 23.10%, 8.82% and 5.14%, and 6.03% and 5.74% than B0 and B1 cultivars, respectively. Steganography is a technique that is frequently used to hide hidden information in multimedia artifacts including music, video, and images. In order to protect data saved in the cloud, this paper presents a steganography method for encrypting sound utilizing LSB-based computation. By using the least significant bit (LSB) of a byte to represent a message and then substituting each LSB bit with a binary message and encrypting a significant quantity of data. The proposed system uses the LSB technique of picture steganography, Multi-Level Encryption Algorithm (MLEA) and Two-Level Encryption Algorithm (TLEA) data encryption to give the highest level of cloud security. Compared to other current schemes, the performance of the suggested method is 1.732125% better on average.

Acute Lymphoblastic Leukemia Presenting as Systemic Juvenile Idiopathic Arthritis: Experience from Bangladesh

Background: Acute lymphoblastic leukemia (ALL), the most common paediatric malignancy, is a heterogeneous hematologic disease. ALL patients may present with isolated and persistent osteo-articular complaints, lower incidence of hepatomegaly, splenomegaly or lymphadenopathy without clear laboratory features, and misdiagnosed as systemic juvenile idiopathic arthritis (sJIA). Methods: This was a single center cross sectional study over a period of 4 years. Clinic laboratory profiles of 39 ALL children were compared with 39 age and sex-matched sJIA cases. Result: Among 39 ALL patients 89.7% were initially misdiagnosed as sJIA upon clinical presentation. Majority (66.7%) of ALL patients had oligo-articular joint involvement. In sJIA, small joints of the hands were most commonly involved. The total WBC count was significantly higher in ALL patients (p-value 0.0065). CRP and LDH values between the two groups showed significant differences (p-value 0.00006 and 0.00001 respectively). Conclusion: The presentation of leukemia with arthralgia or arthritis makes the diagnosis difficult for the physicians. The diagnosis of sJIA must be made with caution keeping the possibility of haematological malignancy in mind.

Numerical Study and Optimization of CZTS-Based Thin-Film Solar Cell Structure with Different Novel Buffer-Layer Materials Using SCAPS-1D Software

This study explored the performances of CZTS-based thin-film solar cell with three novel buffer layer materials ZnS, CdS, and CdZnS, as well as with variation in thickness of buffer and absorber-layer, doping concentrations of absorber-layer material and operating temperature. Our aims focused to identify the most optimal thin-film solar cell structure that offers high efficiency and lower toxicity which are desirable for sustainable and eco-friendly energy sources globally. SCAPS-1D, widely used software for modeling and simulating solar cells, has been used and solar cell fundamental performance parameters such as open-circuited voltage (), short-circuited current density (), fill-factor() and efficiency() have been optimized in this study. Based on our simulation results, it was found that CZTS solar cell with Cd0.4Zn0.6S as buffer-layer offers the most optimal combination of high efficiency and lower toxicity in comparison to other structure investigated in our study. Although the efficiency of Cd0.4Zn0.6S, ZnS and CdS are comparable, Cd0.4Zn0.6S is preferable to use as buffer-layer for its non-toxic property. In addition, evaluation of performance as a function of buffer-layer thickness for Cd0.4Zn0.6S, ZnS and CdS showed that optimum buffer-layer thickness for Cd0.4Zn0.6S was in the range from 50 to 150nm while ZnS offered only 50 – 75 nm. Furthermore, the temperature dependence performance parameters evaluation revealed that it is better to operate solar cell at temperature 290K for stable operation with optimum performances. This study would provide valuable insights into design and optimization of nanotechnology-based solar energy technology for minimizing global energy crisis and developing eco-friendly energy sources sustainable and simultaneously.

Relationship between Seed Priming Mediated Seedling Vigor and Yield Performance of Spring Wheat

Seed priming is a pre-germinated technique that can enhance seed germination percentage,faster and synchro-nized germination,better seedling growth,and yield under stress *** ascertain the most effective seed priming method that would ensure the potential yield of wheat in Bangladesh,two experiments were carried out from December 2021 to March 2022 at the Department of Agronomy,Bangladesh Agricultural *** wheat varieties namely BARI Gom-28 and BWMRI Gom-1 were subjected to a range of priming chemicals in both lab and pot *** compounds included the following:control(no priming),hydropriming(distilled water),10000 ppm KNO_(3),15000 ppm KNO_(3),40000 ppm Mannitol,60000 ppm Mannitol,10000 ppm NaCl,20000 ppm NaCl,100 ppm PEG,150 ppm PEG,500 ppm NaOCl,1000 ppm NaOCl,10000 ppm CaCl_(2),20000 ppm CaCl_(2),10000 ppm KCl and 20000 ppm KCl.A complete randomized design(CRD)with three repli-cations was used to set up the *** results showed that BARI Gom-28 and BWMRI Gom-1 responded best to KCl priming in terms of rapid seed germination and strong seedling *** the other hand,the best priming agents for plant growth and productivity turned out to be CaCl_(2) and *** results of this study support the possibility of using seed priming as a technique to improve wheat plant development and output by raising seed emergence and survival rates.

ZnO Incorporated Acrylamide Grafted Chitosan Based Composite Film for Advanced Wound Healing Applications

This study was carried out to prepare ZnO nanoparticles incorporated acrylamide grafted chitosan composite film for possible biomedical application especially drug loading in wound healing. ZnO nanoparticles were prepared by co-precipitation method from zinc acetate di-hydrate and incorporated in acrylamide grafted chitosan. FT-IR and TGA of the prepared composite film confirmed the successful incorporation of ZnO nanoparticles in the acrylamide-grafted polymer matrix. SEM images showed that the ZnO nanoparticles were homogeneously distributed on the porous matrix of the composite film. Water uptake and buffer uptake analysis revealed that the composite film could hold water and buffer sufficiently, which facilitated the absorption of exudate from the wound site. Amoxicillin was loaded in the prepared composite film and the maximum loading efficiency was found to be 67.33% with drug concentration of 300 ppm. In vitro studies showed greater antimicrobial activity of drug-loaded composite film compared to both pure film and standard antibiotic disc. Finally, the In vivo mouse model showed maximum healing efficiency compared to conventional gauge bandages because the loading of antibiotic in the film produced a synergistic effect and healing time was reduced.
