Pancreatic cancer remains one of the deadliest types of cancer with little or no recent evidence of significant improvement in *** is,in large part,due to the current unavailability of effective screening or even early detection *** tumor cells(CTCs),particularly with the rapidly improving methods for isolation,enrichment,and characterization methods,have emerged as the next best hope to overcome these *** paper reviews the current state-of-the-art of CTC technologies with particular focus on the various isolation and enrichment methods based on liquid biopsy,the current limitations of these technologies and,consequently,the areas of further research and improvements in CTC methods that are needed to translate it to the clinical setting of routine diagnosis,treatment,and management of pancreatic cancer.

A mathematical model is presented for forced convective slip flow of a nanofluid past a radiating stretching/shrinking *** boundary condition is taken into *** nanofluid model involves the Brownian motion and thermophoresis *** group transformation is used to the transport equations as well as the boundary conditions to develop the similarity equations,before being solved numerically using the Runge-Kutta-Fehlberg fourth-fifth order numerical *** show the effects of the controlling parameters on the dimensionless velocity,temperature,nanoparticle volume fraction,skin friction factor,local Nusselt,and local Sherwood numbers,numerical results are presented both in graphical and tabular *** is found that the friction factor decreases with slip and melting parameters for both stretching/shrinking *** is also found that the Nusselt number decreases with the first order slip while it increases with melting and radiation parameters in both ***,the Sherwood number decreases with the melting parameter both for radiating and non-radiating stretching/shrinking *** excellent agreement is found between the present numerical results and published results.

With the advancement of technology and health sciences,health care delivery costs are steadily *** affects both households and ***,the present truth is that health has become an essential but unaffordable *** is very ***,up-to-date,costeffective health care delivery is one of the prime objectives,and focuses on administration and health care *** the per capita spent on health from public/government funds is very poor in developing countries,the responsibility of cost-effective health care delivery falls primarily on the shoulder of the treating *** are becoming an indispensable part of health care delivery,having a diverse role in the emergency,critical care,pain,and perioperative care of *** the population ages,the need for surgical care is also ***,the anesthesiologist can also play a more significant role in delivering cost-effective health care,and minimize the cost without affecting the *** brief narrative review analyzes the current practice of anesthesiologists in two prime areas in the context of cost-savings:Preoperative investigation and low/minimal flow anesthesia.

Pancreatic ductal adenocarcinoma(PDAC)is one of the most lethal cancers in humans,and utilized treatments over the past decades have shown little evidence of improvement in *** lack of progress in PDAC treatment outcomes has largely been attributed to a variety of limitations in all phases of *** limitations most notably include late diagnosis leading to limited treatment options and consequently poorer response to treatments and eventual *** implications regarding the emergence of circulating tumor cells and DNA(ctDNA)have shown promise in augmenting each step in the management of *** paper will review the emergence of ctDNA and its value in detection of common PDAC DNA alterations,potential clinical implications and utility,followed by the current limitations and the next steps that need to be taken to translate its use into a standard of care.

背景早前曾将2-氯普鲁卡因和布比卡因（C／B）联合用于区域麻醉，但因效果不确定而放弃。在这项前瞻性、双盲、随机研究中，我们比较了C／B联合-9利多卡因／布比卡因（L／B）联合的肌间沟阻滞的起效。方法30例拟在肌间沟阻滞下行肩关节镜术患者被分为2组，每组15例。一组（C／B）接受含有碳酸氢钠和肾上腺素的3％2．氯普鲁卡因，随即接受含肾上腺素的0．5％布比卡因；另一组（L／B）接受2％利多卡因而不用3％2．氯普鲁卡因。每15秒评估一次运动和感觉阻滞。主要的观察终点是完全运动阻滞的起效时间。使用时间-事件（生存率）统计分析检验。结果L／B组1例阻滞失败，被排除。C／B和L／B运动阻滞的中位数时间分别为90秒（15—575秒）和180秒（15．3720秒）（P=0．0325），C／B和L／B的感觉阻滞中位数时间分别为90秒（30～600秒）和210秒（30—3900秒）（P=0．0185）。生存分析显示，在第5分钟，C／B组15例患者中13例产生了成功的运动阻滞，而L／B组14例患者中只有7例。在第10分钟，C／B组15例患者全部产生了成功的运动阻滞，而L／B组14例中只有10例。对L／B组阻滞成功或失败的评价需时60分钟。结论本研究证明，肌间沟阻滞C／B比L／B较快达到成功阻滞。

This work was conducted to compare the potentiality between BLRI Napier-3(BN-3)and Pakchong(PK)*** a completely randomized design(CRD)layout,10 plots(5 m′5 m each)were prepared and stem cuttings were *** plants were harvested at 70,80 and 90 days after plantation(DAP)followed by making *** Red Chittagong Cattle(RCC)growing bull calves were selected and equally divided into two treatment groups fed only silages for nutrient utilization and growth *** highest biomass yield(69.3 ton/ha)and plant height(104.6 cm)were observed in PK at 90 *** highest leaf weight(498.6 g/plant)and leaf to stem ratio(LSR)(1.53)were observed in BN-3 at 70 DAP,which was decreased gradually in progressing ***,stem weight was increased with progressing *** matter(DM)(24.71%),total ash(8.35%)and acid detergent fiber(ADF)(61.89%)in PK silage were significantly *** the other hand,crude protein(CP)(9.86%),organic matter(OM)(91.65%)and neutral detergent fiber(NDF)(88.06%)in PK silage did not differ with BN-3 *** matter intake(DMI)and crude protein intake(CPI)from PK silage(2.25 kg/day/animal and 0.22 kg/day/animal)were significantly *** of DM(55.07%),CP(62.35%),OM(57.85%),total ash(30.89%),ADF(73.02%)and NDF(78.19%)for PK silage were significantly higher.N-intake(35.57 g/d)from PK silage was significantly higher,although,N-retention did not differ *** were no significant differences in weight gain of calves fed PK(117 g/d)and BN-3(68 g/d).It can be concluded that PK silage is comparatively better than BN-3 in respect to biomass yield,digestibility and nutrient utilization in growing bull calves.

Determinants of Health Care Seeking Behaviors in Puerperal Sepsis in Rural Sindh, Pakistan: A Qualitative Study

Background: Puerperal sepsis (PS) is one of the major causes of maternal death, contributing to 26,000 deaths per year in developing countries. Early recognition and treatment are essential to managing PS, but numerous social, cultural and technical barriers prevent or delay access to care and necessary medical attention. Through this qualitative study, we identified barriers to care seeking for puerperal sepsis among recently delivered women in Matiari, Pakistan. Methods: We conducted 20 in-depth interviews among recently delivered women with and without sepsis and their family members in September 2012. Key informant interviews were conducted with 14 healthcare providers and traditional birth attendants. The themes used for content analysis were knowledge of danger signs, factors affecting care seeking and local treatment practices for postpartum sepsis. Results: A total of 34 interviews were conducted. Recently delivered women, their family members and traditional birth attendants were unaware of the word PS or the local translated term for PS. However, they were familiar with most of the individual symptoms associated with PS. Healthcare providers were aware of the condition and the associated symptoms. The healthcare providers’ understanding of the seriousness of PS was directly proportional their age and clinical experience. The most common barriers to care seeking was the division of labor within the household, obtaining permission from the primary decision maker, access to transportation, lack of financial resources and support from family members. Conclusion: To improve maternal care seeking behaviors for PS, interventions focusing on increasing knowledge of PS, addressing gender inequality, implementing an affordable community transport service and enhancing TBA’s knowledge and skills to manage PS need to be implemented.
