Following extensive bowel resection, the intestinal tract undergoes a variety of adaptive responses to enhance bowel function. The purpose of this study was to determine the effect of glutamine-supplemented parenteral nutrition on mucosal cellularity and gut function. In addition, enterocyte gene expression of two relevant systems was also characterized and related to the structural and functional changes that occurred. Male Wistar rats underwent a 60% small bowel resection and jugular vein catheterization and were randomized into two groups. The control group (n = 10) received a standard intravenous nutritional solution and the study group (n = 10) received a similar solution but enriched with alanylglutamine dipeptide. After 7 days blood was taken for amino acid analysis, and bowel was harvested to determine mucosal morphology and expression of mucosal cell glutaminase and IGF-I mRNA. Mesentery lymphnodes were cultured to determine the presence of bacteria and thus access bacteria translocation. Serum glutamine concentration and mucosal architecture were maintained in the study group compared to the controls. Anti-microtubule drugs that cause mitotic arrest and subsequent apoptosis of cancer cells are frequently used to treat breast cancer patients with advanced or metastatic diseases. However, patient response rates to this class of chemotherapeutic agents vary significantly. Identification of cellular and genetic factors that are associated with the sensitivity to anti-microtubule drug treatment would be of great clinic implications. Our previous studies have demonstrated that the neuronal protein synuclein-gamma (SNCG) plays oncogenic roles in breast carcinogenesis and is abnormally expressed at high levels in advanced and metastatic breast carcinomas but not expressed in normal or benign breast tissues. In this study, we show that responses of 12 breast cancer cell lines to paclitaxel-induced mitotic arrest and cytotoxicity are highly correlated with SNCG expression status. SNCG-positive cells exhibit a significant higher resistance to paclitaxel-induced mitotic arrest than SNCG-negative cells (p<0.01). Importantly, we demonstrate that down regulation of SNCG expression directly increased the effectiveness of anti-microtubule drug-induced cytotoxicity in breast cancer cells without altering cell responses to doxorubicin. Moreover, we have developed an inhibitor of SNCG that is an EGFP tagged 34 amino acid peptide containing a single ankyrin-like repeat (EGFP-ANK). We demonstrate that expression of EGFP-ANK but not EGFP alone specifically disrupted SNCG-caused resistance to paclitaxel treatment. The strong in vivo binding of ANK peptide to SNCG was demonstrated by co-immunoprecipitation. Taken together, these new findings suggest that SNCG expression in breast carcinomas is likely one causal factor contributing to the poor patient response to paclitaxel treatment The inhibitory effect of ANK peptide on SNCG-induced drug resistance shed new light on future studies to aim SNCG as a therapeutic target

＜正＞Background: The role of surgical resection and thrombectomy for hepatocellular carcinoma （HCC） with portal vein tumor thrombus （PVTT） is controversial. This study aimed to evaluate the effects of the location and extent of PVTT on the long-term outcomes of surgical treatment for HCC. Methods: A total of 438 patients with HCC and PVTT underwent liver resection with or without thrombectomy. These 438 patients were divided into 2 groups: in group A, PVTT was located in the hepatic resection area or protruded into the first branch of the main portal vein beyond the resection line for ＜ 1 cm （286 patients）, and in group B, PVTT extended into the main portal vein （1S2 patients）. Concomitant thrombectomy was performed in 147 patients （51.4%） of group A and in all patients of group B. Results: PVTT recurrence within 6 months after surgery in group B was significantly higher than that in group A: 76.9% vs. 11.3%. Remnant liver recurrence within 1 year after surgery was 43.0% in group A and 78.8% in group B. The cumulative 1-, 2-. 3-, and 5-year overall survival rates were 58.7%, 39.9%, 22.7%, and 18.1% for group A and 39.5%. 20.4%, 5.7%, and 0% for group B, respectively. The overall survivals were significantly better in group A than group B （P ＜ .02）. Conclusions: Liver resection with thrombectomy yielded better outcomes in the HCC patients with PVTT confined to the first or second branch of the main portal vein compared with PVTT extending into the main portal vein.

＜正＞To preserve function and achieve a good cosmetic result,the truncal back suffering from partial thickness burn was used as donor for split ***-seven patients with 62～96%total body surface area（TBSA） burn,including 24～70%full-thickness burn were admitted to our hospital from September 1997 to October *** the patients in this group were suffered partial-thickness burn in the truncal back. During the early treatment,we emphasized the importance to protect and promote the wound healing *** burned area of truncal back was chosen as a donor site to graft the other deep burn parts of body as soon as the back wound was epithelialized,which usually was harvested in day 12 to 17 post *** split thickness grafts were taken and used directly to close excised deep burn,especially to the functional areas. Re-harvesting of grafts from the same truncal back donor site was performed 2 to 3 times after about 10 to 12 days recovery for each operation in some *** aesthetic and functional outcomes were achieved in grafted areas without observable donor site deformity.
