The Perception of Health Workers and Stakeholders Involved in the Process of Selecting Indigent in Burkina Faso

Background: Indigent selected for their health care is complex and poses enormous challenges. The actors involved have an influence on health actions and their perception is decisive for better care for the indigent. Little evidence exists on these perceptions and this paper has captured this. Methods: A case study involved 163 participants with a questionnaire and an interview guide. The quantitative responses were classified according to a measurement scale, proportions and overall indices of perception (Ip) and satisfaction (CSAT) were calculated. The relationship between variables was investigated using chi-square. Thematic analysis was used with qualitative data. The study met ethical requirements. Results: The participants had a positive perception of the selection: Ip = 0.77, but a minority (1/5) were dissatisfied with the selection. The perception of selection did not differ significantly depending on the experience of the actors. For the vast majority, the definition of indigent was satisfactory (CSAT = 91.4). The selection was done in a top down fashion and steps such as setting up committees, informing stakeholders, had shortcomings which negatively impacted the effectiveness of the selection. The needs of the participants included transparency in the indigent select process, matching the tools for selecting the indigent to the context, strengthening of local action, deconstruction of prejudices in terms of the indigent, power of actors to act, and importance for health services to reach out to the indigent. A Framework for Multi-Hop Ad-Hoc Networking over Wi-Fi Direct with Android Smart Devices

The wide diffusion of mobile devices that natively support ad hoc communication technologies has led to several protocols for enabling and optimizing Mobile Ad Hoc Networks (MANETs). Nevertheless, the actual utilization of MANETs in real life seems limited due to the lack of protocols for the automatic creation and evolution of ad hoc networks. recently, a novel P2P protocol named Wi-Fi Direct has been proposed and standardized by the Wi-Fi Alliance to facilitate nearby devices’ interconnection. Wi-Fi Direct provides high-performance direct communication among devices, includes different energy management mechanisms, and is now available in most Android mobile devices. However, the current implementation of Wi-Fi Direct on Android has several limitations, making the Wi-Fi Direct network only be a one-hop ad-hoc network. This paper aims to develop a new framework for multi-hop ad hoc networking using Wi-Fi Direct in Android smart devices. The framework includes a connection establishment protocol and a group management protocol. Simulations validate the proposed framework on the OMNeT++ simulator. We analyzed the framework by varying transmission range, number of hops, and buffer size. The results indicate that the framework provides an eventual 100% packet delivery for different transmission ranges and hop count values. The buffer size has enough space for all packets. However, as buffer size decreases, the packet delivery decreases proportionally.

Assessment of liver ablation using cone beam computed tomography

AIM: To investigate the feasibility and accuracy of cone beam computed tomography(CBCT) in assessing the ablation zone after liver tumor ***: Twenty-three patients(17 men and 6 women, range: 45-85 years old, mean age 65 years) with malignant liver tumors underwent ultrasoundguided percutaneous tumor ablation [radiofrequency(n = 14), microwave(n = 9)] followed by intravenous contrast-enhanced CBCT. Baseline multidetector computed tomography(MDCT) and peri-procedural CBCT images were compared. CBCT image quality was assessed as poor, good, or excellent. Image fusion was performed to assess tumor coverage, and quality of fusion was rated as bad, good, or excellent. Ablation zone volumes on peri-procedural CBCT and post-procedural MDCT were compared using the nonparametric paired Wilcoxon ***: Rate of primary ablation effectiveness was 100%. There were no complications related to ablation. Local tumor recurrence and new liver tumors were found 3 mo after initial treatment in one patient(4%). The ablation zone was identified in 21/23(91.3%) patients on CBCT. The fusion of baseline MDCT and peri-procedural CBCT images was feasible in all patients and showed satisfactory tumor coverage(at least 5-mm margin). CBCT image quality was poor, good, and excellent in 2(9%), 8(35%), and 13(56%), patients respectively. registration quality between periprocedural CBCT and post-procedural MDCT images was good to excellent in 17/23(74%) patients. The median ablation volume on peri-procedural CBCT and post-procedural MDCT was 30 cm3(range: 4-95 cm3) and 30 cm3(range: 4-124 cm3), respectively(P-value > 0.2). There was a good correlation(r = 0.79) between the volumes of the two techniques. CONCLUSION: Contrast-enhanced CBCT after tumor ablation of the liver allows early assessment of the ablation zone.

Disease control with sunitinib in advanced intrahepatic cholangiocarcinoma resistant to gemcitabine-oxaliplatin chemotherapy

Advanced cholangiocarcinoma is associated with poor prognostic survival and has limited therapeutic options available at present. The importance of angiogenesis and expression of pro-angiogenic factors in intrahepatic forms of cholangiocarcinoma suggest that therapies targeting angiogenesis might be useful for the treatment of this disease. Here we report three cases of patients with advanced intrahepatic cholangiocarcinoma progressive after standard chemotherapy and treated with sunitinib 50 mg/d in 6-wk cycles of 4 wk on treatment followed by 2 wk off treatment(Schedule 4/2). In all three patients, sunitinib treatment was associated with a sustained disease control superior to 4 mo, patients achieving either a partial response or stable disease. A reduction in tumor size and density was observed in all cases, suggesting tumor necrosis as a result of sunitinib treatment in these patients. In addition, sunitinib was generally well tolerated and the occurrence of side effects was managed with standard medical interventions, as required. Our results suggest that sunitinib therapy maybe associated with favorable outcomes and tolerability in patients with advanced cholangiocarcinoma. Those observations contributed to launch a prospective phase Ⅱ multicenter trial investigating sunitinib in advanced intrahepatic cholangiocarcinoma(SUN-CK study; NCT01718327).

Field Study of the Impact of Supplementation with Probiotic Yeast(Saccharomyces cerevisiae Sc47-CNCM I-4407)on reproductive Performance in Dairy Cows

Probiotic yeasts are used widely today in the diets of dairy cows with a high production potential. Various studies have researched the effects of live yeasts, in particular Saccharomyces cerevisiae, on digestion, metabolism and performance in dairy cows. The present study aims at evaluating the beneficial effects of using probiotic yeast in dairy cattle farms as a means of improving the breeding performance of milking cows. A group of 14 dairy cattle farms, with at least 40 dairy cows, was selected for the study, all of them using artificial insemination (AI) and that had not used probiotics in the two milking years preceding the study. On these farms, the lactating cows received probiotic yeast supplementation (5 g/cow/day, Actisaf®, Saccharomyces cerevisiae Sc47-CNCM I-4407, 1.1010 CFU/g, Phileo Lesaffre Animal Care, Marcq-en-Baroeul, France) over 13 months including a 4-week period of adaptation to the product and then the observation period lasting a full year. The analysis was therefore based on 2421 Holstein females inseminated with a total of 4230 doses of conventional Holstein semen at 14 farms over three consecutive years: the period of observation and the two preceding years stated as reference period. Use of daily supplementation with probiotic yeast resulted in a significant improvement of 4 points on average in the success rate of artificial insemination and of 5 points in the success rate of first artificial insemination in multiparous dairy cows. The number of inseminations required to obtain a pregnancy was therefore reduced from 3.1 in the reference period to 2.7 in the probiotic yeast supplementation period (p = 0.007). In parallel, the fat and protein yield by these same animals tended to improve (+5%, p = 0.07) between these two periods, demonstrating that it is possible to combine milk performance and maintenance of an effective reproductive system.
