Viral hepatitis and hepatocellular carcinoma:From molecular pathways to the role of clinical surveillance and antiviral treatment

Hepatocellular carcinoma(HCC)is a global health *** to the high prevalence in low-income countries,hepatitis B virus(HBV)and hepatitis C virus infections remain the main risk factors for HCC occurrence,despite the increasing frequencies of non-viral *** addition,hepatitis D virus coinfection increases the oncogenic risk in patients with HBV *** molecular processes underlying HCC development are complex and various,either independent from liver disease etiology or *** reciprocal interlinkage among non-viral and viral risk factors,the damaged cellular microenvironment,the dysregulation of the immune system and the alteration of gutliver-axis are known to participate in liver cancer induction and *** mechanisms and pathways change throughout the natural history of viral hepatitis with the worsening of liver *** high risk of cancer incidence in chronic viral hepatitis infected patients compared to other liver disease etiologies makes it necessary to implement a proper surveillance,both through clinical-biochemical scores and periodic ultrasound *** review aims to outline viral and microenvironmental factors contributing to HCC occurrence in patients with chronic viral hepatitis and to point out the importance of surveillance programs recommended by international guidelines to promote early diagnosis of HCC.

Nanomaterials with unique characteristics exhibit favorable therapeutic and diagnostic properties,implying their enormous potential as biomedical candidates. C60 has been used in gene- and drug-delivery, as imaging agents, and as photosensitizers in cancer therapy. In this study, the influences of a cationic functionalized fullerene on cellular behavior of human colorectal cancer cell line(HT-29) were investigated. Results indicated that HT-29 treated with the studied compound showed a lower sensitivity but a significant impairment in migration and invasion by interfering with the activities of matrix metalloproteinases(MMP-2 and9). The presence of fullerene also altered the capacity of adhesion-related proteins to perform their activity,thereby inducing dramatically adverse effects on the cell physiological functions such as cell adhesion. Thus,our study suggests that this compound is a new potential anti-metastatic effector and a therapeutic component for malignant colorectal cancer.

Inflammatory bowel diseases(IBD)are chronic and relapsing inflammatory conditions of the gut that include Crohn's disease and ulcerative *** pathogenesis of IBD is not completely unraveled,IBD are multi-factorial diseases with reported alterations in the gut microbiota,activation of different immune cell types,changes in the vascular endothelium,and alterations in the tight junctions’structure of the colonic epithelial *** represents a useful tool to enhance our biological understanding and to discover biomarkers in blood and intestinal *** is expected to provide reproducible and quantitative data that can support clinical assessments and help clinicians in the diagnosis and treatment of *** a differential diagnosis of Crohn's disease and ulcerative colitis and the prediction of treatment response can be deducted by finding meaningful *** some non-invasive biomarkers have been described,none can be considered as the“gold standard”for IBD diagnosis,disease activity and therapy *** these reason new studies have proposed an“IBD signature”,which consists in a panel of biomarkers used to assess *** above described approach characterizes“omics”and in this review we will focus on proteomics.

AIM: To study the efficacy of tenofovir disoproxil fumarate (TDF) at low dose in a small open trial of chronic hepatitis B patients with advanced stage disease. METHODS: Eleven patients were treated with TDF 75 mg for a median period of 80 (range, 24-576) wk and then 7 cases were shifted to an adefovir 10 mg treatment group. All patients had been pre-treated with lamivudine: 5 had YMDD resistant mutants and 6 wild- type virus. When TDF was started, 4 patients had low- level viremia and 6 were PCR-negative. RESULTS: During TDF treatment, PCR remained negative in 10 patients, transaminase levels were normal and no significant viral breakthrough was observed. The drug was well tolerated in all cases. When TDF 75 mg was substituted with adefovir 10 mg, 3 out of 7 patients had a persistent viral rebound (2700-130 000 copies/mL), in whom lamivudine had to be reintroduced. CONCLUSION: Low-dose TDF monotherapy can control HBV viremia for an extended period of time without the emergence of resistance and is more potent than adefovir at the standard dosage. The use of a reduced dose of TDF could diminish the cost of therapy in low-income countries, but further studies in a larger population and in HBeAg-positive subjects are needed.

This paper reviews the work done by research- ers at INRS and UBC in the field of integrated microwave photonics （IMWPs） using silicon based waveguide Bragg gratings （WBGs）. The grating design methodology is discussed in detail, including practical device fabrication considerations. On-chip implementations of various fun- darnental photonic signal processing units, including Fourier transformers, Hilbert transformers, ultrafast pulse shapers etc., are reviewed. Recent progress on WBGs- based IMWP subsystems, such as true time delay elements, phase shifters, real time frequency identification systems, is also discussed.

Distinction between Crohn's disease of the colonrectum and ulcerative colitis or inflammatory bowel disease(IBD) type unclassified can be of pivotal importance for a tailored clinical management,as each entity often involves specific therapeutic strategies and ***,no gold standard is available and the uncertainty of diagnosis may frequently lead to misclassification or repeated ***,we have performed a literature search to address the problem of differential diagnosis in IBD colitis,revised current and emerging diagnostic tools and refined disease classification ***,the differential diagnosis is an untangled issue,and the proper diagnosis cannot be reached in up to 10% of patients presenting with IBD *** topic is receiving emerging attention,as medical therapies,surgical approaches and leading prognostic outcomes require more and more disease-specific strategies in IBD *** optimization of standard diagnostic approaches based on clinical features,biomarkers,radiology,endoscopy and histopathology appears to provide only marginal ***,emerging diagnostic techniques in the field of gastrointestinal endoscopy,molecular pathology,genetics,epigenetics,metabolomics and proteomics have already shown promising *** advanced endoscopic imaging techniques and biomarkers can shed new light for the differential diagnosis of IBD,better reflecting diverse disease behaviors based on specific pathogenic pathways.

To the Editor:The risk of de novo cancer increases by 2 to 7-fold after liver transplantation[1].The most common de novo cancers are both solid(non-melanoma skin cancers,colorectal carcinoma,genito-urinary and head-neck cancer)[2,3]as well as hematologic malignancies,in particular the so-called post-transplant lymphoproliferative disorder[4].
