Acceptability of an acupuncture intervention for geriatric chronic pain: an open pilot study

OBJECTIVE： This study investigated the acceptability and effectiveness of acupuncture for persistent musculoskeletat pain in the elderly and assessed the conditions for a future controlled trial. METHODS： A total of 60 patients, hospitalized in a geriatric hospital were enrolled. The intervention consisted of eight acupuncture sessions. The main outcome was the patient＇s participation rate. Regarding pain, the evaluation was based on pre- and post-treatment variations. As a high proportion of the patients had cognitive impairment, the behavioral pain scale DOLOPLUS-2 was chosen although self evaluation was used wherever possible. RESULTS： The mean age of the patients was 83 years. The acceptance rate was very high （89.6%） and 90% of the patients completed the entire course of treatment. After five weeks, the mean DOLOPLUS score had decreased significantly （P〈0.01）. The patients reported improved sleep quality and a reduction in their anxiety symptoms. Furthermore, caregivers noticed a decrease in patient aggressiveness making care easier. CONCLUSION： Our results suggest that acupuncture is highly acceptable and could be very useful in the management of chronic pain when performed in very old frail people with chronic physical and mental disability. Acute scrotal pain(AsP)requiring surgical exploration is common in the pediatric population,but little has been reported on this subject with regard to the adult *** aim of this study was to investigate the demographic and clinical characteristics and outcomes of scrotal explorations performed on adult *** over 21 years of age who underwent surgical exploration for ASP with suspected testicular torsion(TT)at 14 French hospitals between January 2005 and December 2019 were included in this *** main outcome measures were demographic characteristics,pathology found during scrotal exploration,and perioperative *** regression was used to perform univariate and multivariate analyses to identify predictors of *** for 1329 men were *** median age was 30(interquartile range[IQR]:25-35;range:21-89)*** the clinical examination,867(65.2%)patients presented with an elevation of the testicle,613(46.1%)patients with scrotal edema or erythema,and 211(15.9%)patients with nausea or *** findings identified TT in only 684(51.5%)patients,epididymo-orchitis in 112(8.4%)patients,a tumor in 16(1.2%)patients,and no causes in 475(35.7%)*** for nonviable testes was required in 101(7.6%)*** multivariate analysis,an elevation of the testicle,erythema/swelling,and the presence of nausea/vomiting were found to be associated with the occurrence of *** torsion is not exclusive to children and adolescents,so must be considered in males of any age with acute scrotal ***,one-third of scrotal explorations in adults did not lead to a diagnosis.

Faecal calprotectin and magnetic resonance imaging in detecting Crohn's disease endoscopic postoperative recurrence

AIM To assess magnetic resonance imaging(MRI) and faecal calprotectin to detect endoscopic postoperative recurrence in patients with Crohn's disease(CD).METHODS From two tertiary centers, all patients with CD who underwent ileocolonic resection were consecutively and prospectively included. All the patients underwent MRI and endoscopy within the first year after surgery or after the restoration of intestinal continuity [median = 6 mo(5.0-9.3)]. The stools were collected the day before the colonoscopy to evaluate faecal calprotectin level. Endoscopic postoperative recurrence(POR) was defined as Rutgeerts' index ≥ i2b. The MRI was analyzed independently by two radiologists blinded from clinical *** Apparent diffusion coefficient(ADC) was lower in patients with endoscopic POR compared to those with no recurrence(2.03 ± 0.32 vs 2.27 ± 0.38 × 10^(-3) mm^2/s, P = 0.032). Clermont score(10.4 ± 5.8 vs 7.4 ± 4.5, P = 0.038) and relative contrast enhancement(RCE)(129.4% ± 62.8% vs 76.4% ± 32.6%, P = 0.007) were significantly associated with endoscopic POR contrary to the magnetic resonance index of activity(Ma RIA)(7.3 ± 4.5 vs 4.8 ± 3.7; P = 0.15) and MR scoring system(P = 0.056). ADC 100%(sensitivity = 0.75, specificity = 0.81, PPV = 0.75, NPV = 0.81) were the best cutoff values to identify endoscopic POR. Clermont score > 6.4(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74), Ma RIA > 3.76(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74) and a MR scoring system ≥ MR1(sensitivity = 0.54, specificity = 0.82, PPV = 0.70, and NPV = 0.70) demonstrated interesting performances to detect endoscopic POR. Aggressive blood pressure treatment of hypertensive intracerebral hemorrhage may lead to global cerebral hypoperfusion:Case report and imaging perspective

Hypoperfusion injury related to blood pressure decrease in acute hypertensive intracerebral hemorrhage continues to be a controversial topic. Aggressive treatment is provided with the intent to stop the ongoing bleeding. However, there may be additional factors, including autoregulation and increased intracranial pressure, that may limit this approach. We present here a case of acute hypertensive intracerebral hemorrhage, in which aggressive blood pressure management to levels within the normal range led to global cerebral ischemia within multiple border zones. Global cerebral ischemia may be of concern in the management of hypertensive hemorrhage in the presence of premorbid poorly controlled blood pressure and increased intracranial pressure.

Direct determination of the oxidative potential in exhaled air(OPEA)as a biological indicator of the redox balance status in airways requires an extremely sensitive sensing approach to counterbalance the high dilution of the exhaled air *** opposite to standard colorimetric assays based on Beer-Lambert Law,the use of simple multiscattering-enhanced absorbance(MEA)strategy results in an improved photonic sensing *** on this strategy and using the ferrous-xylenol orange assay,analytical performances allowing the detection of 3 pmol of H_(2)O_(2) could be *** comparative analysis of photometric configurations–standard vs MEA–highlights the large absorbance gain obtained for tiny H_(2)O_(2) amounts–from 0 to 30 pmol–whereas similar sensing trends are observed at higher concentration *** automated introduction of exhaled air sample into the reaction compartment via needle-impinger coupled with the MEA-based photonic cell enables ultrasensitive determination of OPEA in short time(<5 min).A pilot study conducted on healthy subjects(n=22)and chronic obstructive pulmonary disease(COPD)patients(n=23)volunteers shows a significant increase of the OPEA for COPD versus controls and strong negative correlation between OPEA and spirometry parameters(Tiffeneau index:FEV1/FVC)for ex-/*** perspectives to assess inflammatory episodes in obstructive lung diseases are thus made possible using this sensing approach.
